Efficacy and safety of clarithromycin for patients with sepsis or septic shock:a systematic review and meta-analysis

Background:Clarithromycin exerts an immunomodulatory role in several human diseases.However,whether this effect improves the prognosis in patients with sepsis remains controversial,and higher levels of clinical evidence are urgently needed.To the best of our knowledge,no meta-analysis to date has reported the clinical efficacy and safety of clarithromycin in sepsis.Methods:A comprehensive literature search of PubMed,EMBASE,and the Cochrane Library was conducted up to December 31,2022.Only randomized controlled trials comparing the clinical efficacy and safety of clarithromycin with controls among patients with sepsis or septic shock were included.Data were pooled by applying a fixed-effects model and a relative risk(RR)estimate with 95%confidence intervals(CIs)using Review Manager(version 5.3;Cochrane Collaboration,Copenhagen,Denmark).Results:Three randomized controlled trials involving a total of 910 patients were included.The pooled results confirmed that clarithromycin had no beneficial effect on progression to multiple organ dysfunction syndrome(RR:1.51;95%CI:1.02-2.25;P=0.04;I^(2)=0%),28-day mortality(RR:1.09;95%CI:0.87-1.36;P=0.46;I^(2)=0%),and 90-day mortality(RR:0.86;95%CI:0.71-1.03;P=0.10;I^(2)=81%)in patients with sepsis or septic shock.Moreover,there was no difference in other serious adverse events between patients who received clarithromycin and those in the control group(RR:1.02;95%CI:0.87-1.19;P=0.83;I^(2)=18%).Conclusion:Our meta-analysis did not reveal an improvement to short-term outcomes in patients with sepsis treated with clarithromycin.However,administration of clarithromycin did not increase the risk of adverse events.

Over the past decades,membrane-based separation processes have found numerous applications in various industries.Membrane contactor is an important part of the separation of dissolved gas in the early stage of gas detection.In this paper,to improve efficiency in the detection of the dissolved gas phase in seawater,a better flat membrane contactor is proposed to achieve efficient degassing,inspired by the way fish breathe underwater and the special structure of fish gills.The bioinspired flow channel structures in the flat membrane contactor are suggested along with the distribution of internal blood vessels in the gill platelet and the feature of the gill platelet surface.Using 3D printing,the special degassing devices are manufactured,and comparative analysis of relevant flow parameters is made using different flow channels,combined with the CFD simulation.The final result showed that the proposed flow channel in the degasser achieves a better degassing effect compared with conventional flow channel when the membrane contact area is limited,which can provide good conditions for subsequent gas detection.

Advances in sepsis-associated liver dysfunction

Recent studies have revealed liver dysfunction as an early event in sepsis. Sepsis-associated liver dysfunction is mainly resulted from systemic or microcirculatory disturbances, spillovers of bacteria and endotoxin (lipopolysaccharide, LPS), and subsequent activa-tion of inflammatory cytokines as well as mediators. Three main cell types of the liver which contribute to the hepatic response in sepsis are Kupffer cells (KCs), hepatocytes and liver sinusoidal endothelial cells (LSECs). In addition, activated neutrophils, which are also recruited to the liver and produce potentially destructive enzymes and oxygen-free radicals, may further enhance acute liver injury. The clinical manifestations of sepsis-associated liver dysfunction can roughly be divided into two categories: Hypoxic hepatitis and jaundice. The latter is much more frequent in the context of sepsis. Hepatic failure is traditionally considered as a late manifestation of sepsis-induced multiple organ dysfunction syndrome. To date, no specific therapeutics for sepsis-associated liver dysfunction are available. Treatment measure is mainly focused on eradication of the underlying infection and management for se-vere sepsis. A better understanding of the pathophysiology of liver response in sepsis may lead to further increase in survival rates.

In 2020 and 2021,severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),a novel coronavirus,caused a global pandemic.Vaccines are expected to reduce the pressure of prevention and control,and have become the most effective strategy to solve the pandemic crisis.SARS-CoV-2 infects the host by binding to the cellular receptor angiotensin converting enzyme 2(ACE2)via the receptor-binding domain(RBD)of the surface spike(S)glycoprotein.In this study,a candidate vaccine based on a RBD recombinant subunit was prepared by means of a novel glycoengineered yeast Pichia pastoris expression system with characteristics of glycosylation modification similar to those of mammalian cells.The candidate vaccine effectively stimulated mice to produce high-titer anti-RBD specific antibody.Furthermore,the specific antibody titer and virus-neutralizing antibody(NAb)titer induced by the vaccine were increased significantly by the combination of the double adjuvants Al(OH)_(3) and CpG.Our results showed that the virus-NAb lasted for more than six months in mice.To summarize,we have obtained a SARS-CoV-2 vaccine based on the RBD of the S glycoprotein expressed in glycoengineered Pichia pastoris,which stimulates neutralizing and protective antibody responses.A technical route for fucose-free complex-type N-glycosylation modified recombinant subunit vaccine preparation has been established.

目的探讨应激诱导蛋白Sestrin2(SESN2)对脂多糖(LPS)联合泛天冬氨酸特异性半胱氨酸蛋白酶(caspase)抑制剂zVAD诱导小鼠树突状细胞(DC)坏死性凋亡的影响。方法将培养至第3~10代小鼠骨髓来源树突状细胞系DC2.4给予LPS刺激0、6、12、24 h,并按照刺激时间点进行分组,采用蛋白质免疫印迹试验(Western blotting)检测各组细胞SESN2蛋白表达水平。将过表达空载慢病毒(NC)、SESN2基因过表达RNA序列慢病毒(SESN2 LV-RNA)、小干扰空载慢病毒(NS)以及SESN2基因小干扰RNA序列慢病毒(SESN2 siRNA)分别转染至DC2.4细胞中,转染后72 h,于倒置荧光显微镜下观察细胞荧光表达改变。各转染组细胞给予LPS刺激24 h,同时设立空白对照组给予磷酸盐缓冲液(PBS)并培养24 h,采用Western blotting检测SESN2蛋白表达水平;给予LPS+zVAD刺激24 h,同时设立空白对照组给予PBS并培养24 h,采用Western blotting检测混合谱系激酶结构域样蛋白(MLKL)、磷酸化MLKL(p-MLKL)蛋白表达水平,使用激光共聚焦显微镜观察p-MLKL变化及阳性细胞数量,采用流式细胞术和Hoechst染色检测坏死性凋亡细胞比例。结果与LPS刺激0 h组相比,LPS刺激24 h组细胞SESN2蛋白表达明显升高,故将24 h作为后续实验LPS刺激时间点。成功转染慢病毒后培养24 h,SESN2 siRNA+LPS+zVAD组细胞MLKL磷酸化水平明显高于NS+LPS+zVAD组;SESN2 LV-RNA+LPS+zVAD组细胞MLKL磷酸化水平明显低于NC+LPS+zVAD组;且NS+LPS+zVAD组和NC+LPS+zVAD组MLKL磷酸化水平分别高于NS+PBS组、NC+PBS组。激光共聚焦显微镜下显示,各组细胞p-MLKL蛋白荧光表达数量及荧光强度变化趋势与上述结果一致。流式细胞术和Hoechst染色检测结果均显示,SESN2 siRNA+LPS+zVAD组坏死性凋亡细胞比例明显高于NS+LPS+zVAD组〔流式细胞术:(30.800±1.153)%比(20.800±1.114)%,Hoechst染色:(75.267±0.451)%比(46.267±3.371)%,均P<0.05〕,提示敲减SESN2表达可以进一步加剧细胞坏死性凋亡发生;SESN2 LV-RNA+LPS+zVAD组坏死性凋亡细胞比例明显低于NC+LPS+zVAD组〔流式细胞术:(7.160±0.669)%比(19.240±2.322)%,Hoechst染色:(32.433±3.113)%比(48.567±4.128)%,均P<0.05〕,提示过表达SESN2可以逆转上述改变。结论SESN2对于脓毒症状态下DC坏死性凋亡具有明显抑制效应,干预SESN2有助于调节脓毒症时DC介导免疫反应进程。

Background:The objective of this study was to evaluate the clinical efficacy of thiamine and vitamin C with or without hydrocortisone coadministration on the treatment of sepsis and septic shock.Methods:MEDLINE,EMBASE and CENTRAL databases were searched for randomized controlled trials(RCTs)that made a comparative study between the combination therapy of vitamin C and thiamine with or without hydrocortisone and the administration of placebo in patients with sepsis or septic shock.Two reviewers independently performed study selection,data extraction and quality assessment.Both short-term mortality and change in the sequential organ failure assessment(SOFA)score from baseline(delta SOFA)were set as the primary outcomes.Secondary endpoints included intensive care unit(ICU)mortality,new onset of acute kidney injury,total adverse events,ICU and hospital length of stay,duration of vasopressor usage and ventilator-free days.Meanwhile,trial sequential analysis was conducted for primary outcomes.Results:Eight RCTs with 1428 patients were included in the current study.The results showed no significant reduction of short-term mortality in sepsis and septic shock patients who received combination therapy of vitamin C and thiamine with or without hydrocortisone compared to those with placebo{risk ratio(RR),1.02[95%confidence interval(CI),0.87 to 1.20],p=0.81,I2=0%;risk difference(RD),0[95%CI,−0.04 to 0.05]}.Nevertheless,the combination therapy was associated with significant reduction in SOFA score[mean difference(MD),−0.63,(95%CI,−0.96 to−0.29,p<0.001,I^(2)=0%]and vasopressors duration(MD,−22.11[95%CI,−30.46 to−13.77],p<0.001,I2=6%).Additionally,there were no statistical differences in the pooled estimate for other outcomes.Conclusions:In the current meta-analysis,the combination therapy of vitamin C and thiamine,with or without hydrocortisone had no impact on short-term mortality when compared with placebo,but was associated with significant reduction in SOFA score among patients with sepsis and septic shock.

宁国山核桃种植区环境地质条件复杂,人类活动活跃,降雨充沛,是安徽省滑坡灾害极高易发区之一。选择种植区东部石岭村滑坡为研究对象,采用遥感、地形测量、地质勘查等工作手段,对滑坡的变形特征、形成条件、破坏机制及稳定性等特征进行详细研究,并提出防治建议。研究结果表明:石岭村滑坡为一受基覆界面控制的小型复合式土质滑坡,在工况Ⅱ(自重+持续暴雨)下稳定系数为0.8543(不稳定),处于蠕变状态,复活的可能性较大;其主控因素为软弱的岩土体结构和适宜的地形地貌条件,诱发因素为强降雨和人类工程活动;建议对种植区自然斜坡进行工程治理,同时改变山核桃采摘方式(如采用低空挂网兜拾),恢复植被,减少水土流失。研究成果可为种植区滑坡灾害早期识别和防治等提供重要借鉴。

目的了解贵州省艾滋病患者抗病毒治疗免疫学效果和病毒抑制情况及其影响因素。方法选择艾滋病综合防治信息系统中截止2016年6月正在接受抗病毒治疗且做过CD_4)^(+)T淋巴细胞检测的12210例患者进行分析。结果12210例研究对象中,男性8143例,占66.7%(8143/12210);平均年龄(45.0±13.7)岁,婚姻状况以已婚或同居居多,占57.6%(7039/12210),传播途径以异性传播为主,占63.4%(7739/12210)。基线CD_(4)细胞计数、治疗时长与不同时点CD_(4)细胞水平正相关,女性和同性性传播艾滋病患者治疗9年以后免疫功能恢复更好(P<0.05)。多因素Logistic回归分析结果显示,年龄<50岁[OR=0.664(0.518~0.853)],婚姻状况为已婚或同居[OR=0.776(0.639~0.942)],感染途径为同性性传播[OR=0.319(0.225~0.453)]是病毒抑制成功的保护因素,治疗时长为2~5年组[OR=1.477(1.117~1.954)]是病毒抑制成功的危险因素(P<0.05)。结论贵州省抗病毒治疗人群治疗效果较好,要加强年龄较大、离异或丧偶、吸毒人群、治疗时间<5年等人群的治疗管理。

Predictive value of immune cell counts and neutrophil-to-lymphocyte ratio for 28-day mortality in patients with sepsis caused by intra-abdominal infection

Background:The current study aimed to evaluate the value of immune cell counts and neutrophilto-lymphocyte ratio(NLR)when attempting to predict 28-day mortality.Methods:We conducted an observational retrospective study that included consecutive septic patients.Severity scores on the first day and peripheral circulating immune cell counts(at day 1,day 3,day 5 and day 7 of admission)were collected during each patient’s emergency intensive care unit stay.We assessed the associations of peripheral circulating immune cell counts and NLR with the severity of illness.The relationships between 28-day mortality and peripheral circulating immune cell counts and NLR with were evaluated using Cox proportional cause-specific hazards models.Results:A total of 216 patients diagnosed with sepsis caused by IAI were enrolled.The lymphocyte counts(days 1,3,5 and 7)and monocyte counts(days 3,5 and 7)were significantly lower in nonsurvivors(n=72)than survivors(n=144).The NLR values at each time point were significantly higher in non-survivors.The day 1 lymphocyte counts,as well as the monocyte counts,were significantly lower in the highest-scoring group,when stratified by the Acute Physiology and Chronic Health Evaluation II and Sequential Organ Failure Assessment scores,than in the other groups(p<0.05).The day 1 NLR was significantly higher in the highest-scoring group than in the other groups(p<0.05).The day 5 and day 7 lymphocyte counts,day 3 and day 7 monocyte counts and day 7 NLR were significant predictors of 28-day mortality in the Cox proportional hazards models(day 5 lymphocyte count:hazard ratio,0.123(95%CI,0.055–0.279),p<0.001;day 7 lymphocyte count:hazard ratio,0.115(95%CI,0.052–0.254),p<0.001;day 3 monocyte count:hazard ratio,0.067(95%CI,0.005–0.861),p=0.038;day 7 monocyte count:hazard ratio,0.015(95%CI,0.001–0.158),p<0.001;day 7 NLR:hazard ratio,0.773(95%CI,0.659–0.905),p=0.001).Conclusions The results showed that circulating lymphocytes and monocytes were dramatically decreased within 7 days in non-survivors following
