BACKGROUND Biliary stone disease is a highly prevalent condition and a leading cause of hospitalization *** with associated strictures has high residual and recurrence rates after traditional multisession percutaneous transhepatic cholangioscopic lithotripsy(PTCSL).AIM To study one-step PTCSL using the percutaneous transhepatic one-step biliary fistulation(PTOBF)technique guided by three-dimensional(3D)*** This was a retrospective,single-center study analyzing,140 patients who,between October 2016 and October 2023,underwent one-step PTCSL for *** patients were divided into two groups:The 3D-PTOBF group and the PTOBF *** clearance on choledochoscopy,complications,and long-term clearance and recurrence rates were *** Age,total bilirubin,direct bilirubin,Child-Pugh class,and stone location were similar between the 2 groups,but there was a significant difference in bile duct strictures,with biliary strictures more common in the 3D-PTOBF group(P=0.001).The median follow-up time was 55.0(55.0,512.0)*** immediate stone clearance ratio(88.6%vs 27.1%,P=0.000)and stricture resolution ratio(97.1%vs 78.6%,P=0.001)in the 3D-PTOBF group were significantly greater than those in the PTOBF *** complication(8.6%vs 41.4%,P=0.000)and stone recurrence rates(7.1%vs 38.6%,P=0.000)were significantly lower in the 3D-PTOBF *** Three-dimensional visualization helps make one-step PTCSL a safe,effective,and promising treatment for patients with complicated primary *** perioperative and long-term outcomes are satisfactory for patients with complicated primary *** minimally invasive method has the potential to be used as a substitute for hepatobiliary surgery.

Investigation of diagnosis and treatment of hemolysis-elevated liver enzymes-low platelet counts （HELLP） syndrome： clinical analysis of 59 cases

Background Hemolysis-elevated liver enzymes-low platelet counts （HELLP） syndrome is a clinical condition occurring in middle and late stage pregnancy. It is characterized by hemolysis, elevated liver enzymes and low platelet counts. This study involves the analysis of the diagnosis, clinical characteristics and treatment of 59 cases of HELLP syndrome as well as the clinical classification, method of delivery and gestational age at delivery. Methods Clinical data from 59 cases of HELLP syndrome occurring from January 2000 to December 2009 were analyzed retrospectively. Thirty-five cases were classified as complete HELLP syndrome and 24 cases were considered partial HELLP syndrome. Results Twenty-six of the 59 analyzed patients （44%） with complete HELLP syndrome showed rapid onset, severe signs, symptoms, and complications in addition to a poor clinical outcome. Complications included multiple organ dysfunction syndrome （MODS） occurring in 18 cases, eclampsia （3 cases）, placental abruption （3 cases）, and perinatal death （4 cases）. The remaining 33 cases （24 with partial and 9 with complete HELLP） were characterized by less severe signs, symptoms, complications and progression of the condition. Two of these cases were complicated with MODS （6.1%）, and 1 with perinatal death （3.0%）.Twelve non-radical-type cases received conservative treatment. The remaining 4 patients had recurring HELLP syndrome （6.78%）. Conclusions HELLP syndrome is classified as the radical type and non-radical-type according to clinical characteristics and outcome. Classification of HELLP syndrome cases according to clinical features can help in the monitoring and treatment of the disease. Active termination of pregnancy should be considered for radical-type cases. Non-radical-type cases can undergo conservative treatment with close monitoring in an attempt to improve perinatal outcome without increasing maternal morbidity.

The symptoms of HELLP syndrome reappeared and became more severe than before the termination of pregnancy. The patient also had severe and persistent hypoproteinemia, hyponatremia and *** HELLP syndrome is an acute and critical obstetric syndrome which can have heterogeneous presentations and variable prognosis. We should be fully aware of the diverse clinical characteristics of this condition.

Numerical simulations for radon migration and exhalation behavior during measuring radon exhalation rate with closed-loop method

Accurate measurements of the radon exhalation rate help identify and evaluate radon risk regions in the *** these measurement methods,the closed-loop method is frequently ***,traditional experiments are insufficient or cannot analyze the radon migration and exhalation patterns at the gas–solid interface in the accumulation *** CFD-based technique was applied to predict the radon concentration distribution in a limited space,allowing radon accumulation and exhalation inside the chamber intuitively and *** this study,three radon exhalation rates were defined,and two structural ventilation tubes were designed for the *** consistency of the simulated results with the variation in the radon exhalation rate in a previous experiment or analytical solution was *** effects of the vent tube structure and flow rate on the radon uniformity in the chamber;permeability,insertion depth,and flow rate on the radon exhalation rate and the effective diffusion coefficient on back-diffusion were *** on the results,increasing the inser-tion depth from 1 to 5 cm decreased the effective decay constant by 19.55%,whereas the curve-fitted radon exhalation rate decreased(lower than the initial value)as the deviation from the initial value increased by approximately 7%.Increasing the effective diffusion coefficient from 2.77×10^(-7) to 7.77×10^(-6) m^(2) s^(-1) made the deviation expand from 2.14 to 15.96%.The conclusion is that an increased insertion depth helps reduce leakage in the chamber,subject to notable back-diffusion,and that the closed-loop method is reasonably used for porous media with a low effective diffusion coefficient in view of the back-diffusion *** CFD-based simulation is expected to provide guidance for the optimization of the radon exhalation rate measurement method and,thus,the accurate measurement of the radon exhalation rate.

Intravitreal conbercept injection with panretinal photocoagulation for high-risk proliferative diabetic retinopathy with vitreous hemorrhage

AIM:To assess the clinical efficacy and safety of combining panretinal photocoagulation(PRP)with intravitreal conbercept(IVC)injections for patients with high-risk proliferative diabetic retinopathy(HR-PDR)complicated by mild or moderate vitreous hemorrhage(VH),with or without diabetic macular edema(DME).METHODS:Patients diagnosed with VH with/without DME secondary to HR-PDR and received PRP combined with IVC injections were recruited in this retrospective *** establishing the patient’s diagnosis,an initial IVC was performed,followed by prompt administration of *** cases who significant bleeding persisted and impeded the laser operation,IVC was sustained before supplementing with *** the completion of PRP,patients were meticulously monitored for a minimum of six *** therapy and IVC injections were judiciously adjusted based on fundus fluorescein angiography(FFA)*** effect and the incidence of adverse events were ***:Out of 42 patients(74 eyes),29 were male and 13 were female,with a mean age of 59.17±12.74y(33-84y).The diabetic history was between 1wk and 26y,and the interval between the onset of visual symptoms and diagnosis of HR-PDR was *** affected eye received 2.59±1.87(1-10)IVC injections and underwent 5.5±1.02(4-8)sessions of *** these,68 eyes received PRP following 1 IVC injection,5 eyes after 2 IVC injections,and 1 eye after 3 IVC *** absorption of VH was observed in all 74 eyes 5-50wk after initial treatment,with resolution of DME in 51 eyes 3-48wk after initial treatment.A newly developed epiretinal membrane was noted in one *** acuity significantly improved in 25 *** complications such as glaucoma,retinal detachment,or endophthalmitis were ***:The study suggests that the combination of PRP with IVC injections is an effective and safe modality for treating diabetic VH in patients with HR-PDR.

Unveiling the Re,Cr,and I diffusion in saturated compacted bentonite using machine-learning methods

The safety assessment of high-level radioactive waste repositories requires a high predictive accuracy for radionuclide diffusion and a comprehensive understanding of the diffusion *** this study,a through-diffusion method and six machine-learning methods were employed to investigate the diffusion of ReO_(4)^(−),HCrO_(4)^(−),and I−in saturated compacted bentonite under different salinities and compacted dry *** machine-learning models were trained using two *** dataset contained six input features and 293 instances obtained from the diffusion database system of the Japan Atomic Energy Agency(JAEA-DDB)and 15 *** other dataset,comprising 15,000 pseudo-instances,was produced using a multi-porosity model and contained eight input *** results indicate that the former dataset yielded a higher predictive accuracy than the *** gradient-boosting exhibited a higher prediction accuracy(R2=0.92)and lower error(MSE=0.01)than the other machine-learning *** addition,Shapley Additive Explanations,Feature Importance,and Partial Dependence Plot analysis results indicate that the rock capacity factor and compacted dry density had the two most significant effects on predicting the effective diffusion coefficient,thereby offering valuable insights.

Distributed Nash Equilibrium Seeking Strategies Under Quantized Communication

This paper is concerned with distributed Nash equi librium seeking strategies under quantized communication. In the proposed seeking strategy, a projection operator is synthesized with a gradient search method to achieve the optimization o players' objective functions while restricting their actions within required non-empty, convex and compact domains. In addition, a leader-following consensus protocol, in which quantized informa tion flows are utilized, is employed for information sharing among players. More specifically, logarithmic quantizers and uniform quantizers are investigated under both undirected and connected communication graphs and strongly connected digraphs, respec tively. Through Lyapunov stability analysis, it is shown that play ers' actions can be steered to a neighborhood of the Nash equilib rium with logarithmic and uniform quantizers, and the quanti fied convergence error depends on the parameter of the quan tizer for both undirected and directed cases. A numerical exam ple is given to verify the theoretical results.

Monocyte to High-density Lipoprotein Cholesterol Ratio as a Predictor of Nonalcoholic Fatty Liver Disease in Childhood Obesity

Objective:Inflammation is involved in the development and progression of nonalcoholic fatty liver disease(NAFLD).The monocyte to high-density lipoprotein cholesterol ratio(MHR)has emerged as a marker for various inflammation-related *** aim of the present study was to investigate the association between the MHR and NAFLD in a population with childhood ***:Based on hepatic ultrasound,a total of 504 children with obesity(357 with NAFLD and 147 without NAFLD)were included in the *** correlation between the MHR and NAFLD risk factors was assessed by Pearson's and Spearman's *** stepwise logistic regression analyses were conducted to explore the association between the MHR and the risk of ***:The MHR in patients with NAFLD was significantly greater than that in patients without NAFLD[0.52(0.44-0.67)versus 0.44(0.34-0.57),P＜0.001].Multivariate stepwise logistic regression analysis demonstrated that the MHR[odds ratio(OR):1.033,95％confidence interval(CI):1.015-1.051;P＜0.001]was an independent predictor of NAFLD in childhood obesity patients,as were age(OR:1.205,95％CI:1.059-1.371;P=0.005],waist circumference[OR:1.037,95％CI:1.008-1.067;P=0.012],and alanine transaminase[OR:1.067,95％CI:1.045-1.089;P＜0.001].Additionally,MHR quartiles showed a significant positive association with the incidence of NAFLD after adjusting for potential confounding ***:The present study showed that the MHR may serve as an available and useful indicator of NAFLD in individuals with childhood obesity.
