We hypothesized that a hip brace may prevent the initial dislocation in the early postoperative period after total hip arthroplasty (THA). We performed a prospective evaluation of the efficacy of a brace in preventing dislocation in 54 primary THAs in 43 patients. All of the patients wore braces for 3 weeks postoperatively. The occurrence of dislocation was evaluated six months postoperatively. There were no cases of dislocation while wearing a brace, while two posterior dislocations occurred 8 and 12 days postoperatively while picking something up from the floor without a brace in so-called provocative positions. The results of this study suggest that a hip brace helps patients to recognize careless provocative positions and prevents the initial hip dislocation in this period instead of usual postoperative management for an average of 6 weeks after discharge, such as a high toilet seat, restricted hip flexion in the activities of daily living, use of a reacher or grabber, an abduction pillow, and a high chair.

Cell competition is a struggle for existence between cells in heterogeneous tissues of multicellular organisms. Loser cells, which die during cell competition, are normally viable when grown only with other loser cells, but when mixed with winner cells, they are at a growth disadvantage and undergo apoptosis. Intriguingly, several recent studies have revealed that cells bearing mutant tumor-suppressor genes, which show overgrowth and tumorigenesis in a homotypic situation, are frequently eliminated, through cell competition, from tissues in which they are surrounded by wild-type cells. Here, we focus on the regulation of cellular competitiveness and the mechanism of cell competition as inferred from two different categories of mutant cells：（1） slower-growing cells and （2） structurally defective cells. We also discuss the possible role of cell competition as an intrinsic homeostasis system through which normal cells sense and remove aberrant cells, such as precancerous cells, to maintain the integrity and normal development of tissues and organs.

AIM: To study the role of gastric mucosal ascorbic acid(AA) in the progression of acute gastric mucosal lesions induced by compound 48/80 (C48/80), a mast cell degranulator, in ***: C48/80 (0.75 mg/kg) was intraperitoneally injected to fasted Wistar rats. Oral administration of AA (10, 50 or 100 mg/kg) was performed 0.5 h after C48/80treatment. Determinations for gastric mucosal lesion severity and blood flow, and assays for gastric mucosal total AA, reduced AA, oxidized AA, vitamin E, thiobarbituric acid reactive substances (TBARS), adherent mucus, nitrite/nitrate (NOx), non-protein SH (NPSH), and myeloperoxidase(MPO), and serum total AA, reduced AA, oxidized AA,and NOx were conducted 0.5 and 3 h after C48/***: Gastric mucosal lesions occurred 0.5 h after C48/80 treatment and progressed at 3 h. Gastric mucosal blood flow decreased 0.5 h after C48/80 treatment but the decrease was recovered at 3 h. Gastric mucosal total AA, reduced AA, vitamin E, and adherent mucus concentrations decreased 3 h after C48/80 *** mucosal oxidized AA concentration remained unchanged after C48/80 treatment. Gastric mucosal NPSH concentration decreased 0.5 h after C48/80 treatment,but the decrease was recovered at 3 h. Gastric mucosal TBARS concentration and MPO activity increased 0.5 h after C48/80 treatment and further increased at 3 *** total AA and reduced AA concentrations increased 0.5 h after C48/80 treatment and further increased at 3 h, while serum oxidized AA concentration increased at 0.5 h. Serum and gastric mucosal NOx concentrations increased 3 h after C48/80 treatment. Right-sided ligamentum teres(RSLT) is a congenital anomaly in which the right umbilical ligament becomes dominant and anomalous ramifications of the hepatic vessels and biliary system are present. A male patient in his 70 s was diagnosed with advanced gallbladder cancer directly infiltrating the right hepatic duct(RHD), together with RSLT. Preoperative three-dimensional simulation of the liver based on multiple detector computed tomography images after cholangiography revealed ramifications of all segmental portal veins from the portal trunk and discordance of the arterial and biliary branching patterns of segment 8. Fusion analysis of the biliary architecture and segmental volumetry showed that the RHD drained segments 1 r, 5, 6, and 7. We successfully performed a modified right-sided hepatectomy sparing segment 8(i.e., resection of the RHD drainage territory), with negative surgical margins. This report is the first to describe major hepatectomy for advanced gallbladder cancer with RSLT.

Gangliocytic paraganglioma(GP) is rare neuroendocrine tumor(NET) with a good prognosis that commonly arising from duodenum. Although the tumor is characterized by its unique triphasic cells(epithelioid, spindle, and ganglion-like cells), the proportions of these three tumor cells vary widely from case to case,and occasionally, morphological and immunohistochemical similarities are found between GP and NET G1(carcinoid tumors). Further, GP accounts for a substantial number of duodenal NETs. Therefore, GP continues to be misdiagnosed, most often as NET G1. However, GP has a better prognosis than NET G1, and it is important to differentiate GP from NET G1. In this article, I wish to provide up-to-date clinicopathological information to help oncologists gain better insight into the diagnosis and clinical management of this tumor.

This study aimed to investigate the characteristics of the coupling of the center of pressure (COP) movement and toes in older adults during one-legged standing (OLS). Participants were community dwelling older and younger adults. Toe vertical force during the OLS was recorded using a plantar-pressure distribution system, and the vertical forces in the hallux, little toe, and the first and fifth metatarsophalangeal (MP) joints were analyzed. A cross-correlation function was performed to examine the coordination between toes and between COP movement and four regions of the foot. The cross-correlation functions showed no significant difference in coefficients between toes. The OLS duration in the older adults was significantly correlated with the coefficient between the medial-lateral component of the COP and the first MP region. This study suggests that it is important to press the first MP region appropriately relative to COP movement to maintain the OLS.

Long-Term Outcomes of Systemic-to-Pulmonary Artery Shunt in Patients with Functional Single Ventricle and Heterotaxy Syndrome

Objectives: We sought to determine the long-term outcomes and mortality-associated factors after systemic-topulmonaryartery shunt (SPS) in patients with heterotaxy syndrome. Methods: We retrospectively analyzed allpatients with a functional single ventricle and heterotaxy syndrome who underwent SPS at our center from January2001 to April 2022. Results: This study involved 84 patients. Restrictive pulmonary blood flow requiringearly modulation was presented in 34 (40%) patients. Compared with patients without restrictive pulmonaryblood flow (N = 50 [60%]), the postnatal survival of these 34 patients was significantly lower at 10 years (log rank:p = 0.04), but the statistical significance disappeared at 20 years (log rank: p = 0.18). Among 31 patients whounderwent SPS, 11 (35%) had extracardiac total anomalous pulmonary venous connection (TAPVC). The survivalrate after SPS was 80% at 10 years. Cox regression analysis showed that extracardiac TAPVC (hazard ratio6.44, 95% confidence interval 1.23–33.7, p = 0.03) and pulmonary venous obstruction (PVO) at TAPVC repair(hazard ratio 11.2, 95% confidence interval 2.13–58.5, p = 0.004) were significantly associated with death. In25 patients who underwent bidirectional cavopulmonary shunt (BCPS), surgical interventions on the pulmonaryartery (PA) were performed after SPS in 7 of 9 patients with PA coarctation, 3 of 4 with non-confluent PAs, and4 of 12 with normal PAs. At SPS, primary central PA plasty was performed in three patients with PA coarctationand 2 with non-confluent PAs. There was no significant difference in the PA index before BCPS between patientswith and without primary central PA plasty (p = 0.49). Among 20 patients who underwent total cavopulmonaryconnection (TCPC), adverse events occurred in 7 (35%) patients, including death in 1 (5%), intervention for pulmonaryarteriovenous malformation (PAVM) in 3 (15%), and surgical intervention for PVO in 3 (15%). Although observational motor learning is one method of skill acquisition, this type of motor learning is not equally effective for all individuals. To clarify factors associated with the effectiveness of motor learning, we examined the association between model-observational skill acquisition and the Autism-Spectrum Quotient (AQ), which is reportedly associated with motor learning via visual information. Twenty healthy adults performed the Kendama task. The participants practiced under three conditions: using their own methods (self), following observation of model actions (model-observation), and following observation of their own actions (self-observation). Measurement trials were performed 20 times prior to self-practice sessions and after each practice session. Success ratios were calculated for each measurement trial. All participants completed the AQ. The difference in success ratios for measurement sessions following practices between the self and model-observation conditions was significantly negatively correlated with AQ scores. Individuals with low AQ values can more easily acquire skills via model-observational motor learning than those with relatively higher AQ values.

Joint Coordination Organizes to Form the Task-Dependent Trajectory of the Body Center of Mass

As the central nervous system controls whole-body motion, which involves multi-joint movement, certain problems with regard to the number of variables controlled by the central nervous system arise (i.e., the “degree of freedom problem”). The central nervous system solves these problems not by controlling joint movements, but rather by controlling only the task-dependent center of mass (COM) position of the whole body. Although uncontrolled joint movement should be organized in a coordinate manner to form the task-dependent COM position, it is unclear what kind of law joint coordination is organized by. Hence, in the present study, we aim to clarify the shape of joint coordination by elucidating the mutual relationship between the COM trajectory and joint movement during whole-body motion. Downward squatting motions with five trunk angles are recorded by using a 3-D motion analysis system in 8 healthy males. The COM trajectory shows a task-dependent path in all trunk conditions. The shank angle decreases with an increase in the trunk angle to produce the task-dependent COM trajectory, whereas the thigh showsd a constant angle. These findings demonstrate that the COM trajectory is constrained by biomechanical dynamics and minimum muscle torques, and that the joints are organized into a lawful coordinative structure to form the COM trajectory.
