BACKGROUND In patients with large stones in the common bile duct(CBD),advanced treatment modalities are generally ***,we present an interesting case of a huge CBD stone treated with electrohydraulic lithotripsy(EHL)by the percutaneous approach and rendezvous endoscopic retrograde cholangiography(ERC)using a nasal *** SUMMARY A 91-year-old woman underwent ERC for a symptomatic large CBD stone with a diameter of 50 *** was referred to our institution after the failure of lithotomy by ERC,and after undergoing percutaneous transhepatic biliary *** attempted to fragment the stone by transhepatic cholangioscopy using ***,the stones were too large and partly soft clay-like for ***,we attempted lithotomy with ERC and cholangioscopy by the rendezvous technique using a nasal endoscope and achieved complete *** complication was observed at the end of this *** Cholangioscopy by rendezvous technique using a nasal endoscope is a feasible and safe endoscopic method for removing huge CBD stones.

Surgical resection for pulmonary recurrence of esophageal cancer after curative esophagectomy

Aim:To clarify the significance of surgical resection for pulmonary recurrence after curative esophagectomy for esophageal ***:Clinical details,such as the recurrence site,timing,and contents of therapies for recurrence,and the prognosis,were examined in 14 patients who underwent surgical resection for pulmonary recurrence that developed after curative ***:The median disease-free interval after esophagectomy was 17.2 *** patients underwent pulmonary resection two times,and in one patient,three *** pulmonary resections were performed when other extra-pulmonary recurrences had been controlled,and R0 resection was *** and/or radiotherapy were additionally performed for pulmonary metastasis in 13 *** median survival time after initial pulmonary resection was 45.5 months,and the 1-,3-,and 5-year overall survival rates were 93%,68%,and 43%,*** 5-year overall survival rate after initial pulmonary resection was 13%in patients with Stage III or IV esophageal cancer and 100%in those with Stage I or II disease(P=0.010).The rate was 56%in patients with tumors<20 mm in size,while all 5 patients with lesions≥20 mm in size died within 3 years(P=0.005).Conclusion:Surgical resection along with systemic therapy is a promising treatment strategy for pulmonary recurrence after curative esophagectomy when it is solitary and *** factors,such as the esophageal cancer stage and the size of the pulmonary metastasis,are useful for deciding on the surgical indication.

This work focused on characterizing and improving the thermal behavior of metal sheet *** decrease the heat transfer from the roof into a building,we investigated the efficiency of four types of phase change materials,with different melting points:PCMІ,PCM II,PCM III and PCM IV,when used in conjunction with a sheet metal *** exterior metal roofing surface temperature was held constant at 50℃,60℃,70℃and 80℃,using a thermal source(halogen lights)for 360 min to investigate and compare the thermal performance of the metal sheet roofing with and without phase change materials for each *** thermal behaviors of the phase change materials were analyzed by differential scanning calorimeter(DSC).The results showed the melting points of PCMІ,PCM II,PCM III and PCM IV were around 45℃,50℃,55℃and 59℃,*** integration of PCM IV into the metal roofing sheet increased the thermal performance by reducing the room temperature up to 2.8%,1.4%,1.0%and 0.7%when compared with the normal metal roof sheet,at the controlled temperatures of 50℃,60℃,70℃and 80℃,*** thermal absorption of the phase change materials also caused a time delay in the model room reaching a steady *** integration of phase change materials with metal roofing sheets resulted in better thermal performance and conservation of electrical energy by reducing the demand for cooling.

This paper describes results of seam welding of relatively high temperature melting materials, AISI 304, C-Mn steels, Ni-based alloys, CP Cu, CP Ni, Ti6Al4V and relatively low temperature melting material, AA6061. It describes the seam welding of multi-layered similar and dissimilar metallic sheets. The method described and involved advancing a rotating non-consumable rod(CP Mo or AISI 304) toward the upper sheet of a metallic stack clamped under pressure. As soon as the distal end of the rod touched the top portion of the upper metallic sheet, an axial force was applied. After an initial dwell time, the metallic stack moved horizontally relative to the stationery non-consumable rod by a desired length, thereby forming a metallurgical bond between the metallic sheets. Multi-track and multi-metal seam welds of high temperature metallic sheets, AISI 304, C-Mn steel,Nickel-based alloys, Cp Cu, Ti6Al4V and low temperature metallic sheets, AA6061 were obtained. Optical and scanning electron microscopy examination and 180 degree U-bend test indicated that defect free seam welds could be obtained with this method. Tensile- shear testing showed that the seam welds of AISI 304, C-Mn steel, Nickel-based alloy were stronger than the starting base metal counterparts while AA6061 was weaker due to softening. The metallurgical bonding at the interface between the metallic sheets was attributed to localized stick and slip at the interface, dynamic recrystallization and diffusion. The method developed can be used as a means of welding, cladding and additive manufacturing.

Non-stoichiometric samarium monosulfide（SmS_x, 0.55≤x≤1.2） was synthesized from Sm_2S_3 and SmH_3 at 1273 K for 3 h under vacuum. The influence of reaction ratio of Sm_2S_3 to SmH_3 on the fabrication of SmS_x was investigated. The fabrication of SmS required the molar ratio of Sm_2S_3 to SmH_3 above 1. Lattice parameter of synthetic SmS_x increased firstly and then decreased to saturate following with the addition of SmH_3 content. SmS_x compact was sintered at 1373 K by spark plasma sintering. Density of synthetic SmS_x was about 99% of theory density. Seebeck coefficient of n-type semiconductor Sm Sx decreased as temperature rose. The absolute value was distributed between 170–280 μV/K. The electrical resistivity of SmS_x（0.86≤x≤1.07） decreased with temperature increasing and showed similar temperature dependence. The surplus Sm which randomly distributed in the SmS_x（0.55≤x≤0.75） matrix led to a remarked reduction of electrical resistivity. The optimized power factor for SmS_（0.6） and Sm S_（0.75） could reach 1500 μW/（K^2·m） at 600 K.

The large eddy simulation (LES) is applied to an unconfined swirling flow of an air surrounding a bluff-body having a central jet of air, and the complicated flowfield that involves the recirculation and vortex breakdown is investigated. The Smagorinsky model is used as the sub-grid scale model. The results of the present numerical simulation are compared with the experimental data of the mean and stochastic root mean square (RMS) variations of two velocity components. Although the inflow conditions are specified in a simple manner, the obtained numerical results are in reasonable agreement with the experiments, except for a part of RMS variation values near downstream of the bluff body. The present numerical calculations can successfully reproduce the two characteristics of the flow, i.e., an upstream recirculation zone established just downstream of the burner plane and the additional recirculation zone established at the more downstream location.
