To Analyze the Sensitivity of RT-PCR Assays Employing S Gene Target Failure with Whole Genome Sequencing Data during Third Wave by SARS-CoV-2 Omicron Variant

Introduction: Omicron is a highly divergent variant of concern (VOCs) of a severe acute respiratory syndrome SARS-CoV-2. It carries a high number of mutations in its spike protein hence;it is more transmissible in the community by immune evasion mechanisms. Due to mutation within S gene, most Omicron variants have reported S gene target failure (SGTF) with some commercially available PCR kits. Such diagnostic features can be used as markers to screen Omicron. However, Whole Genome Sequencing (WGS) is the only gold standard approach to confirm novel microorganisms at genetically level as similar mutations can also be found in other variants that are circulating at low frequencies worldwide. This Retrospective study is aimed to assess RT-PCR sensitivity in the detection of S gene target failure in comparison with whole genome sequencing to detect variants of Omicron. Methods: We have analysed retrospective data of SARS-CoV-2 positive RT-PCR samples for S gene target failure (SGTF) with TaqPath COVID-19 RT-PCR Combo Kit (ThermoFisher) and combined with sequencing technologies to study the emerged pattern of SARS-CoV-2 variants during third wave at the tertiary care centre, Surat. Results: From the first day of December 2021 till the end of February 2022, a total of 321,803 diagnostic RT-PCR tests for SARS-CoV-2 were performed, of which 20,566 positive cases were reported at our tertiary care centre with an average cumulative positivity of 6.39% over a period of three months. In the month of December 21 samples characterized by the SGTF (70/129) were suggestive of being infected by the Omicron variant and identified as Omicron (B.1.1.529 lineage) when sequence. In the month of January, we analysed a subset of samples (n = 618) with SGTF (24%) and without SGTF (76%) with Ct values Conclusions: During the COVID-19 pandemic, it took almost more than 15 days to diagnose infection and identify pathogen by sequencing technology. Analysis of Prevalent Leptospira Serovar in Different Animals of South Gujarat Region during the Year of 2020

Aim of Study: Leptospirosis is a bacterial zoonotic disease transmitted through contact with animals that are harbouring leptospira. Knowledge of prevalent leptospira in a particular animal of a particular geographical area is essential to understand the epizootiology of disease, to understand the linkage between circulating serovars in animals and in humans, and to apply appropriate control measures, etc. Material and Methods: Animal samples from different districts of the south Gujarat region received in the Microbiology department during the year of 2020 for the Microscopic agglutination test (MAT) of leptospirosis were included in the study. Results of MAT which was already performed using 12 different serovars were analysed to prevent serovars in a particular animal. Quantitative data were analysed using frequency and percentage. Result: Out of 1406 animal samples, 151 (11 percent) were positive from animals like cows, buffalos, bullocks and goats. More prevalent serovars in cows were L. ictrohemorrahiae (22%), L. hardjo (19%), L. patoc (17%) and L. pyrogen (16%). In buffalo, L. patoc (58%) and L. hardjo (27%) were found. L. hardjo (50%) in bullock and L. automonalis (50%), L. australis (22%) and L. patoc (14%) in goat were found as prevent serovars. Conclusion: Different prevent serovars has been observed in different animals from the different district south Gujarat region which will be helpful to trace the source of infection in human, to apply control measures, to know the epizootiology of disease, for developing strategies in the future during vaccine development with emphasizing more on the prevalent serovars.

Pure(pristine)anatase and mono–doped and co–doped derivatives of TiO_(2)having nitrogen(N)and cobalt(Co)as dopants with respective fixed doping concentrations of 0.7 mol.%and 1.0 mol.%were synthesized using auto–combustion sol–gel *** doping effects at corresponding non–metal and transition metal sites of TiO_(2)on the basis of the structural,optical and electrical properties have been investigated.X–ray diffraction(XRD)measurement confirms the formation of pure anatase phase of TiO_(2)for all samples having I41/amd space group of tetragonal structure which has been also verified by the Raman spectroscopy *** crystallographic parameters have been calculated by performing Rietveld refinement of XRD data including average crystallite size that has been observed in the range of10–15 *** anatase phase indicates the incorporation of Co^(2+)into TiO_(2)lattice which assists the substitution of N in place of oxygen in co–doped TiO_(2).The band gap tuning towards the visible region from3.2 to 2.1 e V has been achieved with mono–doping and co–doping of the N and Co in TiO_(2)*** can be described in terms of the formation of localized levels of N–2p and Co–3d states in mono–doping and an isolated intermediate band formation in co–doping *** properties have been investigated in details and explained as the synergetic effects of structural and inherent ionic characters of various *** observed band gaps of all doped samples lie within the visible region which makes them pertinent as the solar energy harnessing materials for photocatalytic and photovoltaic applications.

Evaluation of Sensitivity and Specificity of Direct Microscopical Examination of Suspected Mucor Mycosis Samples by Potassium Hydroxide (KOH) during Covid-19 Pandemic Era

Introduction: Mucormycosis is a common fungal infection in India even before the COVID-19 pandemic. The incidence of Mucor mycosis has entered public consciousness in response to an outbreak of cases in India during COVID-19 pandemic. Mortality is unacceptably high, reaching 80% in some case series. Prompt diagnosis is crucial because treatment initiation is time-critical due to the rapid progression of the infection. Direct KOH mount is a simple, rapid, inexpensive point-of-care test, which requires minimum technical aids. Interpretation of KOH smears requires some amount of experience. The present study was to evaluate the sensitivity and specificity of Direct Microscopical examination by KOH preparation of suspected Mucor mycosis samples taking the fungal culture as gold standard. Material and Methods: 230 samples were received in Microbiology Department from May 2021 to August 2021 from the patients suspected of Mucor mycosis. Samples were processed for KOH preparation using different concentrations of KOH like 20% and 40% till all tissues get digested. Samples were cultured also in Sabourauds dextrose agar (SDA) medium. Results of microscopy and culture were statistically analyzed using SPSS software version 23. Result: Sensitivity of KOH preparation was 67%, specificity was 87%. Positive predictive value was 76% and negative predictive value was 81% and accuracy was 80% when compared with the fungal culture. Discussion: Direct microscopic examinations using KOH preparation are the simplest, cheapest method used for the diagnosis of mycotic infections. This study is valuable in establishing a reliable method for early information on diagnosis of Mucor mycosis that may be crucial for determining appropriate therapy for the successful treatment of Mucor mycosis patients. Novel coumarin–benzimidazole derivatives as antioxidants and safer anti-inflammatory agents

Inspired from occurrence of anti-inflammatory activity of 3-substituted coumarins and antiulcer activity of various 2-substituted benzimidazoles,novel compounds have been designed by coupling coumarin derivatives at 3-position directly or through amide linkage with benzimidazole nucleus at *** resultant compounds are expected to exhibit both anti-inflammatory and antioxidant activities along with less gastric toxicity *** series of coumarin–benzimidazole derivatives(4a–e and 5a–e)were synthesized and evaluated for anti-inflammatory activity and antioxidant *** 4c,4d and 5a displayed good anti-inflammatory(45.45%,46.75%and 42.85%inhibition,respectively,versus 54.54% inhibition by indomethacin)and antioxidant(IC_(50) of 19.7,13.9 and 1.2 mmol/L,respectively,versus 23.4 mmol/L for butylatedhydroxytoluene)*** of ulcer index and in vivo biochemical estimations for oxidative stress revealed that compounds 4d and 5a remain safe on gastric mucosa and did not induce oxidative stress in *** of various molecular properties suggests the compounds to be sufficiently bioavailable.

Use of Cost Effective Semi-Automated (Mannual/Micro) MGIT System over BACTEC 960 to Perform First Line Anti-Tuberculosis Drugs Sensitivity Testing

Introduction: Multi-drug resistant tuberculosis (MDR-TB) that is the tuberculosis that is resistant to at least 2 of the first line anti-tuberculosis drugs is fatal infectious disease. Cases of MDR-TB are now increasing with 30,000 cases of MDR-TB reported in 2013 by national TB programme. Rapid diagnosis of MDR-TB is extremely important for rapid treatment of patient and to prevent spread of MDR-TB to other. BACTEC 960 system helps in rapid diagnosis but purchase of expensive instrument for the same is the limitation. However, the same purpose can be solved by use of semi-automated MGIT system. Aims and Objectives: Aim of this study is to do drug sensitivity testing of the first line anti-tuberculosis drugs with the use of semi-automated MGIT systems. 350 newly registered and suspected cases of tuberculosis in tertiary care hospital were included. Samples were processed for digestion and decontamination and inoculated in MGIT tubes and also on LJ medium. Reading was taken using semi-automated MGIT system. Positive tubes were confirmed by rapid test for M. tuberculosis and then drug sensitivity was performed. Result: Out of 350 samples, 62% were sputum;33% were pleural fluid and rest 5% were lymph node, Ascetic fluid, CSF, pus. Average day of positivity by MGIT was 13 - 20 days as compared to 25 - 37 days by solid medium, which was statistically significant with p value Conclusion: Manual MGIT System is a simple, efficient, safe to use diagnostic system. It does not require any expensive/special instrumentation other than the UV lamp for detection of fluorescence. The rapidity by which mycobacteria are detected is the most important advantage of the Manual MGIT. In areas with limited resources where purchase of expensive instruments such as the MGIT960 is out of scope, the use of manual MGIT for rapid susceptibility testing for MDR-TB could be a possibility.

Introduction: Hands of health care workers play an important part in infection transmission in a healthcare setting infections. Even though it is a simple and effective procedure, it is many times ignored by the health care workers (HCWs) due to lack of knowledge and interest, time constraints, skin irritation, understaffing, etc. There are no specific drugs and vaccines available for COVID-19 pandemic management, so as control measures, hand hygiene is an essential measure. It will be helpful even for the second wave preparedness. So this study was planned for analysis of barriers and practices of hand hygiene among health care workers which determine the adherence of hand hygiene in medical practice. Methods: This was a cross-sectional study designed during COVID-19 pandemic management to analyze the barriers and knowledge, attitude and practices in health care workers for hand hygiene. Semi-structured questionnaires based study based on previous experience of seasonal flu outbreak was make to know barriers as well as to analyze the knowledge, attitude and practices of Health care workers. 215 participants were enrolled who were doctors, resident doctors, nurses, lab technicians, ward boys and food distributors. Results: The “Barriers” related questions’ analysis showed that hand hygiene facilities, with the supply of soap, hand rub, basins with adequate as well as clean water, are available at various places in the Hospital. In knowledge related questions, major gaps are found in 5 moments of hand hygiene and regarding aspects of alcohol-based hand-rub.
