Successful resection of metachronous para-aortic, Virchow lymph node and liver metastatic recurrence of rectal cancer

A 66-year-old female presented with the main complaint of defecation trouble and abdominal distention. With diagnosis of rectal cancer, c SS, c N0, c H0, c P0, c M0 c Stage Ⅱ, Hartmann's operation with D3 lymph node dissection was performed and a para-aortic lymph node and a disseminated node near the primary tumor were resected. Histological examination showed moderately differentiated adenocarcinoma, p SS, p N3, p H0, p P1, p M1(para-aortic lymph node, dissemination) f Stage Ⅳ. After the operation, the patient received chemotherapy with FOLFIRI regimen. After 12 cycles of FOLFIRI regimen, computed tomography(CT) detected an 11 mm of liver metastasis in the posteroinferior segment of right hepatic lobe. With diagnosis of liver metastatic recurrence, we performed partial hepatectomy. Histological examination revealed moderately differentiated adenocarcinoma as a metastatic rectal cancer with cut end microscopically positive. After the second operation, the patient received chemotherapy with TS1 alone for 2 years. Ten months after the break, CT detected a 20 mm of paraaortic lymph node metastasis and a 10 mm of lymph node metastasis at the hepato-duodenal ligament. With diagnosis of lymph node metastatic recurrences, we performed lymph node dissection. Histological examination revealed moderately differentiated adenocarcinoma as metastatic rectal cancer in paraaortic and hepato-duodenal ligament areas. After the third operation, we started chemotherapy with modified FOLFOX6 regimen. After 2 cycles of modified FOLFOX6 regimen, due to the onset of neutropenia and liver dysfunction, we switched to capecitabine aloneand continued it for 6 mo and then stopped. Eleven months after the break, CT detected two swelling 12 mm of lymph nodes at the left supraclavicular region. With diagnosis of Virchow lymph node metastatic recurrence, we started chemotherapy with capecitabine plus bevacizumab regimen. Esophageal intramural pseudodiverticulosis of the residual esophagus after esophagectomy for esophageal cancer

A 91-year-old man was referred to our hospital with intermittent dysphagia. He had undergone esophagectomy for esophageal cancer(T3N2M0 Stage Ⅲ) 11 years earlier. Endoscopic examination revealed an anastomotic stricture; signs of inflammation,including redness,erosion,edema,bleeding,friability,and exudate with white plaques; and multiple depressions in the residual esophagus. Radiographical examination revealed numerous fine,gastrografinfilled projections and an anastomotic stricture. Biopsy specimens from the area of the anastomotic stricture revealed inflammatory changes without signs of malignancy. Candida glabrata was detected with a culture test of the biopsy specimens. The stricture was diagnosed as a benign stricture that was caused by esophageal intramural pseudodiverticulosis. Accordingly,endoscopic balloon dilatation was performed and antifungal therapy was started in the hospital. Seven weeks later,endoscopic examination revealed improvement in the mucosal inflammation; only the pseudodiverticulosis remained. Consequently,the patient was discharged. At the latest follow-up,the patient was symptomfree and the pseudodiverticulosis remained in the residual esophagus without any signs of stricture or inflammation.

A new N* resonance as a hadronic molecular state

We report our recent work on a hadronic molecule state of the KKN system with I =1/2 and jR = 1/2＋. We assume that the A（1405） resonance and the scalar mesons, f0（980）, ao（980）, are reproduced as quasi-bound states of KN and KK, respectively. Performing non-relativistic three-body calculations with a variational method for this system, we find a quasibound state of the KIlN system around 1910 MeV below the three-body breakup threshold. This state corresponds to a new baryon resonance of N＊ with JP ： 1/2＋. We find also that this resonance has the cluster structure of the two-body bound states keeping their properties as in the isolated two-particle systems. We also briefly discuss another hadronic molecular state composed by two K and one N, which corresponds to a resonance.

We discuss cluster phenomena in light nuclei. As examples of typical cluster structures, we first review cluster structures of 12C, 16O, and 20Ne, and then introduce some topics of cluster phenomena in light neutron-rich nuclei such as Be and C isotopes. A particular attention is paid on coexistence of cluster and shell-model aspects.

Principle for obtaining high joint quality in dissimilar friction welding of Ti-6Al-4V alloy and SUS316L stainless steel

Ti-6Al-4V alloy(Ti64)and SUS316 L stainless steel rods were dissimilarly friction *** focusing on the detailed observation of interface microstructural evolution during the friction welding(FW),the relationship between the processing conditions,weld interface microstructure,and mechanical properties of the obtained joints were systematically investigated to elucidate the principle for obtaining a high joint quality in the FW of Ti64 and SUS316L.A higher friction pressure produced a lower welding temperature in the FW,hence suppressing the thick intermetallic compound layer ***,hard and brittle Ti64/SUS316L mechanically mixed layers generally formed especially at the weld interface periphery due to the high temperature increasing rate,high rotation linear velocity and high outward flow velocity of the *** harmful layers tended to induce the cracks/voids formation at the weld interfaces hence deteriorating the joints’mechanical *** rotation speed reduction and liquid CO2 cooling during the entire processing decreased the temperature increasing rate,rotation linear velocity and outward flow velocity of the Ti64 at the weld interface ***,they suppressed the formation of the harmful mechanically mixed layers,facilitated the homogeneous and sound interface microstructure generation,and finally produced a high-quality dissimilar joint in the FW of Ti64 and SUS316L.

Effects of silicon addition on optical properties of mould flux with iron oxide and estimation of apparent thermal conductivity of flux

Comparison of the activities of binder-added and binder-free Mo/HZSM-5 catalysts in methane dehydroaromatization at 1073 K in periodic CH_4-H_2 switch operation mode

Three industry-supplied, well-shaped Mo/HZSM-5 catalysts, two binder-added and one binder-free, were tested for the first time in methane dehydroaromatization to benzene at 1073 K and 10000 mL/（g·h） in periodic CH4-H2 switch operation mode, and their catalytic performances were compared with those of three self-prepared, binder-free powder Mo/HZSM-5 catalysts. XRD, 27Al NMR, SEM, BET and NH3-TPD characterizations of all the catalysts show that the zeolites in the two binder-added catalysts are comparable to those in the three binder-free powder catalysts in crystallinity, crystal size, micropore volume and Br{／o}nsted acidity. The test results, on the other hand, show that the catalytic performances of the two binder-added catalysts are worse than those of the four binder-free catalysts on both catalyst mass and zeolite mass bases. Then, TPO and BET measurements of all spent samples were conducted to get a deep insight into the negative effects of binder addition, and the results suggest that the binder additives functioned mainly to enhance the polyaromatization of formed aromatics to coke on their external surfaces and consequently lower the catalysts＇ benzene formation activity and selectivity.

Cystic metastasis from a mucinous adenocarcinoma of duodenum mimicking type Ⅱ choledochal cyst: A case report

A 51-year-old male patient was referred to our hospital because of an incidentally detected cystic mass near the common bile duct(CBD). Imaging studies demonstrated a cystic mass that was suspected to communicate with the CBD. Gastroscopy showed irregular mucosal thickening with hyperemic change in the second portion of the duodenum. A type Ⅱ choledochal cyst combined with duodenal malignancy was suspected. The patient underwent surgical resection and the histological diagnosis was mucinous adenocarcinoma of the duodenum with cystic metastasis. Although its incidence is extremely rare, care should be taken to check for other sites of malignancy when a pericholedochal cystic mass is detected.
