Neoadjuvant therapy for resectable pancreatic cancer:a narrative review

The use of neoadjuvant therapy(NAT)for pancreatic ductal adenocarcinoma remains controversial and limited.Therefore,this literature review aimed to assess the feasibility,safety,and efficacy of this treatment.A database search of peer-reviewed articles published in English between January 1990 and June 2021 in PubMed,MEDLINE,and the Web of Science was performed.Original articles,review articles,and meta-analyses relevant to the topic were selected.We found 2 to 4 cycles with FOLFIRINOX,gemcitabine plus nab-paclitaxel,gemcitabine plus S-1,or gemcitabine alone were the most acceptable treatments.Considering the risk of adverse events and cancer progression,NAT is considered safe and tolerable,with a comparable resection rate.Although NAT can result in moderate tumor responses and some extent of local control(improvement of complete resection rate and negative lymph node metastases),no obvious survival benefit is observed.To date,the survival benefits of NAT for resectable pancreatic ductal adenocarcinoma have been very limited.It is too early to say that NAT is the best treatment option for resectable pancreatic cancer.

A 65-year-old female was admitted to our hospital complaining of abdominal pain for the past month.A physical examination revealed mild right upper quadrant tenderness without rebound pain and no palpable abdominal hepatomegaly or mass.Her laboratory results showed mildly impaired liver function:AST 51 U/L(normal range<34 U/L),ALT 49 U/L(normal range<34 U/L),and C-reactive protein 16.5 mg/L(normal range<10 mg/L).

目的分析过氧化物酶体增殖物激活受体γ(PPARγ)在肝细胞癌(HCC)癌前阶段以及癌症发展阶段的作用机制及潜在价值，以期为肝癌预防、实施精准化治疗以及药物探索提供新思路。方法以“hepatocellular carcinoma, PPARγ,precancerous lesions, cancer development, mechanism of action, treatment progress”为英文关键词，以“肝细胞癌，PPARγ,癌前病变，癌症发展，作用机制，治疗进展”为中文关键词检索PubMed与中国知网相关文献，检索时间为2005-01-01-2023-10-01,纳入标准：(1)HCC的诊治现状；(2)PPARγ干预HCC癌前阶段的作用机制研究；(3)PPARγ干预HCC形成后发展阶段的作用机制研究；(4)基于PPARγ治疗肝癌及其癌前病变的药物研究。排除标准：(1)个案、会议及评论性文章；(2)数据不完整的文章；(3)研究机制不明确、存在争议的文章。根据标准共纳入文献57篇。结果 PPARγ在肝癌癌前阶段参与改善肝脏局部炎性微环境、抑制肝脏纤维化与肝癌干细胞增殖与分化，促进肝癌癌前相关抑癌基因的表达。在肝癌形成后的发展阶段，PPARγ促进癌细胞的糖脂代谢，诱导血管形成，降低免疫抑制。以PPARγ为治疗靶点，探索应用于治疗HCC的不同阶段药物，可能是实施肝癌预防与精准化治疗的新方向。结论 PPARγ在肝癌发生发展的不同阶段均起到调控作用，针对PPARγ的进一步研究有助于为防治HCC提供新思路。

The burden of upper motor neuron involvement is correlated with the bilateral limb involvement interval in patients with amyotrophic lateral sclerosis:a retrospective observational study

Amyotrophic lateral sclerosis is a rare neurodegenerative disease characterized by the involvement of both upper and lower motor neurons.Early bilateral limb involvement significantly affects patients'daily lives and may lead them to be confined to bed.However,the effect of upper and lower motor neuron impairment and other risk factors on bilateral limb involvement is unclear.To address this issue,we retrospectively collected data from 586 amyotrophic lateral sclerosis patients with limb onset diagnosed at Peking University Third Hospital between January 2020 and May 2022.A univariate analysis revealed no significant differences in the time intervals of spread in different directions between individuals with upper motor neuron-dominant amyotrophic lateral sclerosis and those with classic amyotrophic lateral sclerosis.We used causal directed acyclic graphs for risk factor determination and Cox proportional hazards models to investigate the association between the duration of bilateral limb involvement and clinical baseline characteristics in amyotrophic lateral sclerosis patients.Multiple factor analyses revealed that higher upper motor neuron scores(hazard ratio[HR]=1.05,95%confidence interval[CI]=1.01–1.09,P=0.018),onset in the left limb(HR=0.72,95%CI=0.58–0.89,P=0.002),and a horizontal pattern of progression(HR=0.46,95%CI=0.37–0.58,P<0.001)were risk factors for a shorter interval until bilateral limb involvement.The results demonstrated that a greater degree of upper motor neuron involvement might cause contralateral limb involvement to progress more quickly in limb-onset amyotrophic lateral sclerosis patients.These findings may improve the management of amyotrophic lateral sclerosis patients with limb onset and the prediction of patient prognosis.

鳖甲煎丸调控miR-140对肝癌干细胞致瘤性及干性的影响

目的探讨鳖甲煎丸对裸鼠皮下成瘤、肝癌干细胞(liver cancer stem cells,LCSCs)干性的抑制作用及其相关机制。方法选取12只BALB/c裸鼠,随机分为4组,即肝癌干细胞皮下成瘤+生理盐水灌胃组(A组)、肝癌干细胞皮下成瘤+鳖甲煎丸灌胃组(B组)、miR-140过表达慢病毒肝癌干细胞皮下成瘤+生理盐水灌胃(C组)、miR-140干扰慢病毒肝癌干细胞皮下成瘤+鳖甲煎丸灌胃(D组),将分选出肝癌干细胞种植于裸鼠皮下形成瘤体,记录瘤体体积大小,并进行多组间对比。苏木精-伊红(HE)染色观察瘤体组织的病理学变化。PCR和Western Bolt分别检测肝癌组织中CD133、CD24、EpCAM、AFP的mRNA表达、蛋白表达以及miR-140的基因表达。结果B组、C组瘤体体积均小于A组,D组瘤体体积大于B组;HE染色结果提示瘤体组织呈恶性后进行PCR与Western Bolt检测,结果提示B组、C组的CD133、CD24、EpCAM、AFP的mRNA以及蛋白表达相对含量均小于A组,D组以上检测指标小于B组;予鳖甲煎丸灌胃后,瘤体中miR-140的表达升高。结论鳖甲煎丸能抑制肝癌干细胞的致瘤能力以及降低肝癌组织中CD133、CD24、EpCAM、AFP,其作用机制可能与促进LCSCs中miR-140的表达有关。

Cobalt doping amount determines dominant reactive species in peroxymonosulfate activation via porous carbon catalysts co-doped by cobalt and nitrogen

Switching the reaction routes in peroxymonosulfate(PMS)-based advanced oxidation processes have attracted much attention but remain challenging.Herein,a series of Co-N/C catalysts with different compositions and structures were prepared by using bimetallic zeolitic imidazolate frameworks based on ZIF-8 and ZIF-67(x Zn/Co-ZIFs).Results show that Co doping amount could mediate the transformation of the activation pathway of PMS over CoN/C.When Co doping amount was less than 10%,the constructed x Co-N/C/PMS system(x≤10%)was singlet oxygen-dominated reaction;however further increasing Co doping amount would lead to the generation and coexistence of sulfate radicals and high-valent cobalt,besides singlet oxygen.Furthermore,the nitrogen-coordinated Co(Co-NX)sites could serve as main catalytically active sites to generate singlet oxygen.While excess Co doping amount caused the formation of Co nanoparticles from which leached Co ions were responsible for the generation of sulfate radicals and high-valent cobalt.Compared to undoped N/C,Co doping could significantly enhance the catalytic performance.The 0.5%Co-N/C could achieve the optimum degradation(0.488 min^(-1))and mineralization abilities(78.4%)of sulfamethoxazole among the investigated Co-N/C catalysts,which was superior to most of previously reported catalysts.In addition,the application prospects of the two systems in different environmental scenarios(pH,inorganic anions and natural organic matter)were assessed and showed different degradation behaviors.This study provides a strategy to regulate the reactive species in PMS-based advanced oxidation process.

The unfolded protein response(UPR)is a cellular stress response pathway activated when the endoplasmic reticulum,a crucial organelle for protein folding and modification,encounters an accumulation of unfolded or misfolded proteins.The UPR aims to restore endoplasmic reticulum homeostasis by enhancing protein folding capacity,reducing protein biosynthesis,and promoting protein degradation.It also plays a pivotal role in coordinating signaling cascades to determine cell fate and function in response to endoplasmic reticulum stress.Recent research has highlighted the significance of the UPR not only in maintaining endoplasmic reticulum homeostasis but also in influencing various physiological processes in the nervous system.Here,we provide an overview of recent findings that underscore the UPR’s involvement in preserving the function and viability of neuronal and myelinating cells under physiological conditions,and highlight the critical role of the UPR in brain development,memory storage,retinal cone development,myelination,and maintenance of myelin thickness.

LONP1 ameliorates liver injury and improves gluconeogenesis dysfunction in acute-on-chronic liver failure

Background:Acute-on-chronic liver failure(ACLF)is a severe liver disease with complex pathogenesis.Clinical hypoglycemia is common in patients with ACLF and often predicts a worse prognosis.Accumulating evidence suggests that glucose metabolic disturbance,especially gluconeogenesis dysfunction,plays a critical role in the disease progression of ACLF.Lon protease-1(LONP1)is a novel mediator of energy and glucose metabolism.However,whether gluconeogenesis is a potential mechanism through which LONP1 modulates ACLF remains unknown.Methods:In this study,we collected liver tissues from ACLF patients,established an ACLF mouse model with carbon tetrachloride(CCl 4),lipopolysaccharide(LPS),and D-galactose(D-gal),and constructed an in vitro hypoxia and hyperammonemia-triggered hepatocyte injury model.LONP1 overexpression and knockdown adenovirus were used to assess the protective effect of LONP1 on liver injury and gluconeogenesis regulation.Liver histopathology,biochemical index,mitochondrial morphology,cell viability and apoptosis,and the expression and activity of key gluconeogenic enzymes were detected to explore the underlying protective mechanisms of LONP1 in ACLF.Results:We found that LONP1 and the expressions of gluconeogenic enzymes were downregulated in clinical ACLF liver tissues.Furthermore,LONP1 overexpression remarkably attenuated liver injury,which was characterized by improved liver histopathological lesions and decreased serum alanine aminotransferase(ALT)and aspartate aminotransferase(AST)in ACLF mice.Moreover,mitochondrial morphology was improved upon overexpression of LONP1.Meanwhile,the expression and activity of the key gluconeogenic enzymes were restored by LONP1 overexpression.Similarly,the hepatoprotective effect was also observed in the hepatocyte injury model,as evidenced by improved cell viability,reduced cell apoptosis,and improved gluconeogenesis level and activity,while LONP1 knockdown worsened liver injury and gluconeogenesis disorders.Conclusion:We demonstrated that gluconeogenesis dysfunc

Ballistic performances of the hourglass lattice sandwich structures under high-velocity fragments

In this paper,the numerical simulation method is used to study the ballistic performances of hourglass lattice sandwich structures with the same mass under the vertical incidence of fragments.Attention is paid to elucidating the influences of rod cross-section dimensions,structure height,structure layer,and rod inclination angle on the deformation mode,ballistic performances,and ability to change the ballistic direction of fragments.The results show that the ballistic performances of hourglass lattice sandwich structures are mainly affected by their structural parameters.In this respect,structural parameters optimization of the hourglass lattice sandwich structures enable one to effectively improve their ballistic limit velocity and,consequently,ballistic performances.
