The Hug-up Test： A New, Sensitive Diagnostic Test for Supraspinatus Tears

Background： The supraspinatus tendon is the most commonly affected tendon in rotator cuff tears. Early detection ofa supraspinatus tear using an accurate physical examination is, therefore, important. However, the currently used physical tests for detecting supraspinatus tears are poor diagnostic indicators and involve a wide range of sensitivity and specificity values. Therefore, the aim of this study was to establish a new physical test for the diagnosis of supraspinatus tears and evaluate its accuracy in comparison with conventional tests. Methods： Between November 2012 and January 2014, 200 consecutive patients undergoing shoulder arthroscopy were prospectively evaluated preoperatively. The hug-up test, empty can （EC） test, full can （FC） test, Neer impingement sign, and Hawkins-Kennedy impingement sign were used and compared statistically tbr their accuracy in terms of supraspinatus tears, with arthroscopic findings as the gold standard. Muscle strength was precisely quantified using an electronic digital tensiometer. Results： The prevalence of supraspinatus tears was 76.5%. The hug-up test demonstrated the highest sensitivity （94.1%）, with a low negative likelihood ratio （NLR, 0.08） and comparable specificity （76.6%） compared with the other four tests. The area under the receiver operating characteristic curve for the hug-up test was 0.854, with no statistical difference compared with the EC test （z = 1.438, P = 0.075） or the FC test （z = 1.498, P = 0.067）. The hug-up test showed no statistical difference in terms of detecting different tear patterns according to the position （X2 = 0.578, P = 0.898） and size （Fisher＇s exact test, P 〉 0.999） compared with the arthroscopic examination. The interobserver reproducibility of the hug-up test was high, with a kappa coefficient of 0.823. Meniscus repair with simultaneous anterior cruciate ligament reconstruction: Clinical outcomes, failure rates and subsequent processing

MicroRNA-548 down-regulates host antiviral response via direct targeting of IFN-λ1

Storage capacity model for cloud download

With the rise and world wide deployment of cloud utilities, the principle of the cloud download is proposed to provide high-quality file content distribution by using dedicated servers as cloud cache to guarantee the data availability and enhance the data transfer rate. As the system scales up to a large population, how to design appropriate storage capacity of cloud cache is a key challenge for cloud download. In this paper, primary elements impacting on storage capacity are explored through deliberating in large-scale commercial cloud download system, i.e. general user usage pattern and available period. And based on statistical analysis of real-world traces, we formulate storage capacity related to these two elements, which is the original contribution different from all previous works. This model gives guidance of potential system policy design. Finally, the effectivity of this model is demonstrated by simulation results compared with empirical data of practical system.

Efficient complementary caching for ISP-aware networks

Internet service providers （ISPs） have taken some measures to reduce intolerable inter-lSP peer-to-peer （P2P） traffic costs, therefore user experiences of various P2P applications have been affected. The recently emerging offiine downloading service seeks to improve user experience by using dedicate servers to cache requested files and provide high-speed uploading. However, with rapid increase in user population, the server-side bandwidth resource of offline downloading system is expected to be insufficient in the near future. We propose a novel complementary caching scheme with the goal of mitigating inter-ISP traffic, alleviating the load on servers of Internet applications and enhancing user experience. Both architecture and caching algorithm are presented in this paper. On the one hand, with full knowledge of P2P file sharing system and offiine downloading service, the infrastructure of complementary caching is designed to conveniently be deployed and work together with existing platforms. The co-operational mechanisms among different major components are also included. On the other hand, with in-depth understanding of traffic characteristics that are relevant to caching, we develop complementary caching algorithm with respect to the density of requests, the redundancy of file and file size. Since such relevant information can be real-time captured in our design, the proposed policy can be implemented to guide the storage and replacement of caching unities, Based on real-world traces over 3 months, we demonstrate that the complementary caching scheme is capable to achieve the ＇three-win＇ objective. That is, for l＇2P downloading, over 50% of traffic is redirected to cache; for offline downloading, the average server-dependence of tasks drops from 0.71 to 0.32; for user experience, the average P2P transfer rate is increased by more than 50 KB/s.

SDE decomposition and A-type stochastic interpretation in nonequilibrium processes

An innovative theoretical framework for stochastic dynamics based on the decomposition of a stochas- tic differential equation （SDE） into a dissipative component, a detailed-balance-breaking component, and a dual-role potential landscape has been developed, which has fruitful applications in physics, engineering, chemistry, and biology. It introduces the A-type stochastic interpretation of the SDE beyond the traditional Ito or Stratonovich interpretation or even the a-type interpretation for multi- dimensional systems. The potential landscape serves as a Hmniltonian-like function in nonequilibrimn processes without detailed balance, which extends this important concept from equilibrium statistical physics to the nonequilibrium region. A question on the uniqueness of the SDE decomposition was recently raised. Our review of both the mathematical and physical aspects shows that uniqueness is guaranteed. The demonstration leads to a better understanding of the robustness of the novel frame- work. In addition, we discuss related issues including the limitations of an approach to obtaining the potential function from a steady-state distribution.
