BACKGROUND The prognostic value of coronary collateral circulation(CC)in patients undergoing chronic total occlusion(CTO)percutaneous coronary intervention(PCI)is *** purpose of the study was to assess the prognostic value of current two CC grading systems and their association with long-term outcomes in patients with CTO underwent *** We consecutively enrolled patients with single-vessel CTO underwent PCI between January 2010 and *** patients were categorized into well-developed or poor-developed collaterals group according to angiographic Werner's CC(grade 2 *** 0–1)or Rentrop(grade 3 *** 0–2)grading *** primary endpoint was 5-year cardiac *** Of 2452 enrolled patients,the overall technical success rate was 74.1%.Well-developed collaterals were present in686 patients(28.0%)defined by Werner's CC grade 2,and in 1145 patients(46.7%)by Rentrop grade *** to Werner's CC grading system,patients with well-developed collaterals had a lower rate of 5-year cardiac death compared with those with poordeveloped collaterals(1.6%vs.3.3%,P=0.02),those with suboptimal recanalization was associated with higher rate of 5-year cardiac death compared with optimal recanalization(4.7%vs.0.8%,P=0.01)and failure patients(4.7%vs.1.6%,P=0.12).However,the similar effect was not shown in Rentrop grading *** In patients with the single-vessel CTO underwent PCI,well-developed collaterals by Werner's CC definition were associated with lower rate of 5-year cardiac ***'s CC grading system had a greater prognostic value than Rentrop grading system in patients with CTO underwent PCI.

We systemically investigate optical trapping capability of a kind of tornado waves on Rayleigh *** tornado waves are named as tornado circular Pearcey beams(TCPBs)and produced by combining two circular Pearcey beams with different *** theoretical exploration delves into various aspects,including the propagation dynamics,energy flux,orbital angular momentum,trapping force,and torque characteristics of *** results reveal that the orbital angular momentum,trapping force,and torque of these beams can be finely tuned through the judicious manipulation of their topological charges(l_(1)and l_(2)).Notably,we observe a precise control mechanism wherein the force diminishes with|l_(1)+l_(2)|and|l_(1)-l_(2)|,while the torque exhibits enhancement by decreasing solely with|l_(1)+l_(2)|or increasing with|l_(1)-l_(2)|.These results not only provide quantitative insights into the optical trapping performance of TCPBs but also serve as a valuable reference for the ongoing development of innovative photonic tools.

“自我治理”是福柯晚年的研究议题。他将西方古代自我治理史分为古希腊、希腊化和罗马、基督教三个阶段,其中第二个阶段对于批判当代西方资本主义的过度治理、对其进行“反引导”,有着特殊的借鉴价值。希腊化和罗马时期的自我治理观念以晚期斯多葛学派为典型。“自我治理”者以夫妻关系为载体来实践“自我主体化”,建构独立于外部环境、以相互平等和尊重、互相施爱的“生存忠诚”为根本特征的“婚姻生活模式”。斯多葛学派将夫妻、父母与子女之间的爱理解为“理性之爱”,即将爱、责任建基于“自我”之表征的理性和美德,不为外部条件所动。字面上看,斯多葛学派笔下的“自我”是典型的个人主义者,家庭与朋友、公职被并置为身外之物,但伦理主体以“理性之爱”来履行家庭责任,实则神似原始儒家的“古之学者为己”主张,故而在显性的自我本体之外,不经意地托举出隐性的家本体,实现了“自我”与“家”在根本立场上的和谐。

终身收支视角下长期护理保险的再分配效应——基于性别和参保类型的考察

互助共济是社会保障制度的本质特征,考察其再分配效应是评价一项社会保障制度互助共济功能的有效方法之一。本文从终身收支的视角出发,构建了健康保险精算模型,测算不同性别和参保类型群体的缴费回报率,以比较长期护理保险试点城市制度方案的再分配效应。研究结果表明:首先,由于女性失能持续时间更长、工资水平更低,女性参保者的缴费回报率高于男性,从而呈现出“男性补贴女性”的再分配效应。其次,城居保参保者的缴费回报率高于城职保参保者,呈现出“职工补贴居民”的再分配效应;“比例筹资”模式对不同参保类型群体的再分配效应比“定额筹资”模式更强。第三,相比于城职保参保者,60岁城居保参保者的预期余寿更短,但失能持续时间却更长;60岁男性的预期余寿和失能持续时间均比女性更短。本文建议,提高长护险制度统筹层次,推行比例筹资和待遇均等相结合的模式,形成“量能缴费”和“按需保障”的制度特征;同时,探索和发展农村居民更可及的待遇给付方式,以促进群体间的照护保障公平。

●AIM:To identify the differential methylation sites(DMS)and their according genes associated with diabetic retinopathy(DR)development in type 1 diabetes(T1DM)children.●METHODS:This study consists of two surveys.A total of 40 T1DM children was included in the first *** no participant has DR,retina thinning was used as a surrogate indicator for *** lowest 25%participants with the thinnest macular retinal thickness were included into the case group,and the others were *** DNA methylation status was assessed by the Illumina methylation 850K array BeadChip assay,and compared between the case and control *** DMS with a potential role in diabetes were *** second survey included 27 T1DM children,among which four had *** methylation patterns of the four DMS identified by 850K were compared between participants with and without DR by pyrosequencing.●RESULTS:In the first survey,the 850K array revealed 751 sites significantly and differentially methylated in the case group comparing with the controls(|Δβ|>0.1 and Adj.P0.05)between participants with and without DR.●CONCLUSION:The hypermethylation of the TCERG1L gene is a risk factor for DR development in Chinese children with T1DM.

BACKGROUND The complexity of immunoglobulin G4(IgG4)-related diseases and their potential connection to hematologic malignancies remains *** article provided a review of the diagnosis and treatment of a patient with IgG4-related sclerosing cholangitis(SC)and essential thrombocythemia(ET),along with an analysis of relevant literature to enhance comprehension of this *** SUMMARY A 56-year-old male was admitted to two hospitals with deteriorating jaundice and pruritus prior to *** our expectations,the patient was first diagnosed with IgG4-SC and ET with the Janus kinase 2 V617F ***,the administration of acetate prednisone significantly resulted in improvements in both IgG4-SC and *** need to pay attention to immune disorders and inflammation as they contribute to the development of various disease *** When IgG4-SC is suspected without histopathological evidence,diagnostic therapy and long-term regular follow-up can lead to positive treatment *** should be mindful of the potential presence of concurrent hematologic diseases in patients with immune disorders.

背景胃癌是威胁人类生命健康的主要恶性肿瘤之一,人类疱疹病毒(Epstein-Barr virus,EBV)是一种致癌病毒,主要感染上皮细胞、B细胞,与多种肿瘤的发生发展密切相关.本研究尝试探究胃癌患者EBV感染情况及对预后的影响.目的研究胃癌患者EBV感染情况及对预后的影响.方法选取2020-01/2022-12本院300例胃癌患者作为研究组,另选同期同年龄段150例胃部良性肿瘤(胃息肉)患者作为对照组.比较研究组和对照组临床资料、EBV感染阳性率,分析EBV感染与胃癌临床特征的相关性,分析胃癌预后不良的影响因素,根据影响因素构建预测预后不良的列线图模型,分析列线图模型的预测价值.结果研究组EBV感染阳性率(10.67%)高于对照组(1.33%)(P<0.05);Lauren分型混合型胃癌患者EBV感染阳性率(21.05%)高于肠型(7.19%)、弥漫型(7.50%)、未分型(0.00%)患者,程序性死亡分子1(programmed death-1,PD-1)阳性胃癌患者EBV感染阳性率(19.61%)高于PD-1阴性患者(6.06%),程序性死亡配体1(programmed death receptor 1,PD-L1)阳性胃癌患者EBV感染阳性率(21.13%)高于PD-L1阴性患者(7.42%)(P<0.05);研究组预后不良患者年龄、TNM分期Ⅲ-Ⅳ期占比、浸润深度T3+T4占比、淋巴结转移占比、分化程度低分化占比、PD-1阳性率、PD-L1阳性率、EBV感染阳性率高于预后良好患者(P<0.05);年龄、TNM分期Ⅲ-Ⅳ期、浸润深度T3+T4、淋巴结转移、分化程度低分化、PD-1阳性、PD-L1阳性、EBV感染阳性均为胃癌预后不良的独立危险因素(P<0.05);根据影响因素构建预测预后不良的列线图模型,该模型预测胃癌患者预后不良的曲线下面积为0.924(95%CI:0.892-0.956),具有较高预测效能,且预测预后不良的校准度、临床效用良好.结论胃癌患者具有较高EBV感染阳性率,EBV感染与Lauren分型、PD-1/PD-L1信号通路有关,且是胃癌预后不良的影响因素.
