The accumulation characteristics and exploration potential of oil and gas in the back-arc basin of Japan under the background of high heat flow

The Sea of Japan is located in the southeast margin of Eurasia, in the triangle area of the western Pacific Ocean. Due to the interaction of the Pacific plate, Eurasian plate and Philippine plate, its tectonic environment is complex, forming a typical trench-arc-basin system. At present, 148 oil and gas fields have been discovered in Japan, with an oil and gas resource of 255.78×10^(6) t, showing a good prospect for oil and gas exploration. Based on the previous research and the recently collected geological and geophysical data, the characteristics of tectonic-sedimentary evolution and geothermal field in the basins around the Sea of Japan are analyzed. The results show that the tectonic evolution of the basin is mainly controlled by plate subduction and back-arc oceanic crust expansion, and it mainly undergone four tectonic-sedimentary evolution stages: Subduction period, basin development period, subsidence period and compression deformation period. The overall heat flow value of Japan Sea is high, and it is distributed annularly along Yamato Ridge. The geothermal heat flow value is about 50–130 MW/m^(2), and the average heat flow is75.9±19.8 MW/m^(2), which has a typical “hot basin ”. The high heat flow background provides unique thermal evolution conditions for hydrocarbon generation, which leads to the high temperature and rapid evolution. The authors summarized as “early hydrocarbon generation, rapid maturity and shallow and narrow hydrocarbon generation window”. The type of oil and gas is mainly natural gas, and it mainly distributed in Neogene oil and gas reservoirs. The trap types are mainly structural traps, lithologic traps and composite traps. In addition, the pre-Neogene bedrock oil and gas reservoirs also show a good exploration prospect. The resource prospecting indicates that Niigata Basin, Ulleung Basin and kitakami Basin are the main target areas for future exploration and development.

AIM:To examined the efficacy and safety of treatment with boceprevir,PEGylated-interferon and ribavirin(PR)in hepatitis C virus genotype 1(HCVGT1) PR treatmentfailures in ***:The Boceprevir Named-Patient Program provided boceprevir to HCVGT1 PR *** physicians were invited to contribute data from their patients:baseline characteristics,ontreatment responses,sustained virological response at week 12(SVR12),and safety were collected and *** analysis was performed to determine predictors of ***:150 patients were enrolled from Australia,Malaysia,Singapore and Thailand(Asians = 86,Caucasians = 63).Overall SVR12 was 61%(Asians= 59.3%,Caucasians = 63.5%).SVR12 was higher in relapsers(78%) compared with non-responders(34%).On-treatment responses predicted SVR,with undetectable HCVRNA at week 4,8 and 12 leading to SVR12 s of 100%,87%,and 82%respectively,and detectable HCVRNA at week 4,8 and 12,leading to SVR12 s of 58%,22%and 6%*** patients were similar to Caucasian patients with regards to on-treatment *** with cirrhosis(n= 69) also behaved in the same manner with regards to on-treatment *** with the IL28 B CC genotype(80%) had higher SVRs than those with the CT/TT(56%) genotype(P = 0.010).Multivariate analysis showed that TW8 and TW12 responses were independent predictors of *** adverse events occurred in 18.6%:sepsis(2%),decompensation(2.7%) and blood transfusion(14%).Discontinuations occurred in 30.7%,with 18.6%fulfilling stopping ***:Boceprevir can be used successfully in PR treatment failures with a SVR12 > 80%if they have good on-treatment responses;however,discontinuations occurred in 30%because of virological failure or adverse events.

Objective：This study was to investigate the variables in bone marrow harvesting procedure and individual donor factors which can potentially affect the yield of mesenchymal stromal cells（MSC）.Methods：We determined the yield of MSC from bone marrow under different clinical conditions by comparing the MSC colony numbers from：（1） donors of different ages;（2） healthy donors and patients with leukemia;（3） bone marrow aspirated at different time points during marrow harvesting;（4） bone marrow harvested by different ***：During the process of harvesting,the number of MSC significantly decreased with increase number of aspiration,from 675/ml at the initial decreased to 60/ml after 100 ml bone marrow aspirated,and 50/ml after 200 ml bone marrow *** number of MSC retrieved from leukemia patients（99/ml bone marrow） was significantly lower than that of healthy donors（708/ml bone marrow）.However,there was no significant difference in growth *** was no significant age-related difference of MSC yielded from donors 55 *** there was no significant difference in MSC number between the samples from single end-holed needle and those from multiple-side-hole ***：The optimal bone marrow samples for MSC collection should be obtained earlier in the process of harvesting *** marrow from donors 55 years was equally good as MSC *** autologous MSC from leukemia patients can be utilized for in-vitro MSC expansion.

Factors associated with post-pancreaticoduodenectomy hemorrhage： 303 consecutive cases analysis

Background Because of the complexity and severity of the surgery and its associated complications, pancreaticoduodenectomy （PD） is associated with significant morbidity and mortality, especially the hemorrhage post-PD. Exploring the factors associated with post-PD hemorrhage is very important for the patients’ safety.Methods Clinical data from 303 cases of PD between January 1998 and December 2008 were analyzed retrospectively.Results The overall mortality rate was 4.95% （15/303）. However, post-operative bleeding occurred in 25 patients （8.25%） with nine episodes resulting in death （36.00%）. Univariate analysis was performed and identified tumor size, Child’s classification, total pancreatic uncinatic process resection, and pancreatic leakage as significant risk factors for post-PD hemorrhage. In the severe hemorrhage group, incomplete resection of uncinate process of pancreas and pancreatic leakage were the main causes. The multivariate Logistic regression analysis revealed that each of these variables is an independent risk factor.Conclusions Primary prevention of bleeding complications depends on total pancreatic uncinatic process resection and meticulous hemostatic techniques during surgery. In addition, several peri-operative factors were found to contribute to post-PD bleeding.

Antenatal anxiety in the first trimester: Risk factors and effects on anxiety and depression in the third trimester and 6-week postpartum

Objective: Anxiety is common among pregnant women. However, research attention in the area of reproductive mental health has mainly focused on postpartum depression in past decades. Given adverse outcomes of antenatal anxiety, there is an urgent need to fill the research gaps. The objectives of the present study were to determine the prevalence of antenatal anxiety symptoms and examine the risk factors and effects of anxiety symptoms in early pregnancy on anxiety and depressive symptoms in later pregnancy and early postpartum period. Methodology: A prospective longitudinal design with quantitative approach was adopted. A consecutive sample of 1470 Chinese pregnant women from hospitals in Hong Kong was invited to participate in the study and was assessed using standardized instruments on 3 time points including first and third trimesters of pregnancy and 6-week postpartum. Results: The results showed that 17.7% of pregnant women manifested anxiety symptoms in the first trimester of pregnancy. Single mothers, younger mothers, mothers who smoked before pregnancy and mothers who received low education level reported significantly higher levels of anxiety symptoms in the first trimester. Unwanted pregnancy, low self esteem, low marital satisfaction and perceived low social support were significant psychosocial risk factors for anxiety symptoms in the first trimester. Anxiety symptoms in the first trimester were independent predictors for anxiety symptoms in the third trimester ( QUOTE β = 0.26, t = 5.74, p < 0.001), however anxiety symptoms in the first trimester no longer significantly predicted anxiety and depressive symptoms in 6 weeks postpartum after adjusting for the effects of potential confounders. Discussions: The present study points to the need for greater research and clinical attention to antenatal anxiety given that antenatal anxiety is a prevalent problem and has serious impacts on maternal well-being. 目的:探索现代医家治疗急性缺血性中风和急性出血性中风的通用中药处方特点,挖掘潜在诊疗思路和临床用药规律,为今后中西医结合治疗急性中风病提供参考,最终达到改善患者临床预后、提高患者生活质量的意义。方法:采用文献挖掘的方法,搜集当代医家治疗急性缺血性中风和急性出血性中风的临床研究文献,应用万方数据库、中文科技期刊数据库和中国知网期刊全文数据库作为数据源,汇集符合既定纳入和排除标准的文献。在Microsoft Office Excel 2019软件对符合纳入标准医案的用药建立Microsoft Office Excel2019数据库进行频数分析。通过统计药物使用频数,选取累计使用频数超过总用药频数≥10的高频药物做有关功效、性味、归经的频数分析。然后,将上述中风病医案用药Excel数据库导入IBM SPSS Modeler18软件,筛选并根据各药物功效、性味、归经等进行聚类分析。使用SPSS15.0对高频药物进行关联分析,并进行聚类图可视化,得出治疗急性缺血性中风和急性出血性中风的组方规律和处方特点。结果:1.按照纳入标准和排除标准,检索并纳入文献共138篇。其中治疗急性缺血性中风文献69篇,急性出血性中风文献69篇。文献涉及的中药共194味。急性缺血性中风使用频次≥10有24味药,使用频次达532次,其中丹参,石菖蒲,胆南星,川芎,大黄为治疗急性缺血性中风的排名前五高频用药;急性出血性中风使用频次≥10有32味药,使用频次达679次,其中大黄,石菖蒲,胆南星,三七,赤芍为治疗急性出血性中风的排名前五高频用药。2.对高频药物进行功效分类及统计。其中急性缺血性中风治疗药物包括补虚药,发散解表药,活血化瘀药,止痉药等14类,其中补虚药最多,占高频药物使用频次21.39%;而治疗急性出血性中风的药物分为补虚药,活血化瘀药,清热药,化痰止咳平喘药,平肝熄风药等15类,其中补虚药使用最多,占使用频次16.75%。3.对高频药物进行属性统计。治疗急性缺血性中风以温性、寒性药物使用最多,药味使用以甘、苦、辛味最多,归经方面以归肝、肺、脾经为主;而急性出血性中风则以寒性、温性和平性药物使用为主,药味则以甘、苦、辛味最多,归经方面以归肝、肺、心经为主。4.选择Lance距离类型,使用最长距离法,通过聚类分析发现急性缺血性中风药物的聚五类分别为活血化瘀药、安神开窍药、滋肾养阴药、燥湿消痰药、清热凉血药等五类;而急性出血性中药的聚五类分别为为安神开窍药、活血化瘀药、燥湿消痰药、滋肾养阴药和清热凉血药。5.中药-中药分析:设置支持度大于等于0.2,置信度大于等于0.8,通过关联分析技术对中药与中药进行研究,针对急性缺血性中风药物方剂共找出42组中药-中药关系,共发现19组核心药物配伍,分别为黄芪-茯苓-大黄、黄芪-水蛭-地龙、黄芪-水蛭-胆南星、黄芪-水蛭-丹参、黄芪-水蛭-大黄、天麻-钩藤-大黄、黄芪-地龙-赤芍、黄芪-地龙-天麻、黄芪-地龙-川芎、黄芪-地龙-黄芩、黄芪-地龙-石菖蒲、黄芪-地龙-胆南星、黄芪-地龙-丹参、黄芪-地龙-红花、黄芪-地龙-大黄、黄芪-赤芍-黄芪、黄芪-天麻-胆南星、黄芪-黄芩-丹参、黄芪-丹参-红花。其核心药物组合为黄芪-地龙-石菖蒲-胆南星-天麻-钩藤-赤芍-红花-黄芩-丹参;针对急性出血性中风药物方剂共找出11组中药-中药关系,共发现7组核心药物配伍,分别为大黄-厚朴-芒硝、大黄-牛膝-石菖蒲、大黄-枳实-石菖蒲、大黄-枳实-胆南星、大黄-芒硝-石菖蒲、大黄-芒硝-胆南星、大黄-三七-石菖蒲,其核心药物组合为大黄-芒硝-三七-枳实-胆南星-石菖蒲。该药组可视为大黄-芒硝、三七-枳实、胆南星-石菖蒲的组合。结论:本研究结果揭示了急性缺血性中风和急性出血性中风的高频用药以及主要用药类别,总结了其通用中药处方特点。1.治疗急性缺血性中风的核心用药为黄芪,高频用药有:丹参、石菖蒲、胆南星等,以活血化瘀药、补虚药、平肝熄风药使用最多,最常用药对为黄芪-红花、黄芪-大黄、黄芪-丹参。2.治疗急性出血性中风的核心用药为大黄,高频用药有:大黄,石菖蒲,胆南星等,以补虚药,活血化瘀药,清热药使用最多。最常用药对为大黄-胆南星、大黄-石菖蒲、桃仁-红花、大黄-枳实、大黄-芒硝。3.治疗急性缺血性中风及急性出血性中风的通用高频药物为丹参、川芎、红花、赤芍、水蛭、石菖蒲、胆南星,通用的处方特点为善用活血化瘀药、开窍药和化痰止咳定喘药。本研究初步提取的中药包括功效、性味、归经以及中药配伍应用集合等,可为中医药针对中风病临床遣方用药配伍提供参考,为中医在脑血管意外领域的持续发展提供理论依据。
