Can patients with mild non-neoplastic lesions diagnosed at baseline screening be safely exempt from surveillance: evidence from multicenter community-based cohorts

Surveillance recommendations for gastric cancer（GC） in current guidelines focused on advanced precancerous lesions and were based on precise diagnosis of severity/extent of baseline lesions. We aimed to develop a less endoscopy-related equipment-dependent risk-stratification tool, and assessed whether mild-precursor-lesion patients can be safely exempt from surveillance. In the multicenter communitybased cohort, 75,051 participants receiving baseline endoscopy were enrolled during 2015–2017 and followed-up until 2021. Cumulative incidence rates（CIRs） of GC for precancerous-conditions were calculated by Kaplan-Meier method and compared by Log-rank tests. Mixedeffects Cox regression models were used to detect potential factors for progression towards GC. A risk score was calculated as counts of selected factors. An independent cohort, including 26,586 participants was used for external validation. During a median follow-up of 6.25 years, CIRs of GC were 0.302%, 0.436%, and 4.756% for normal group, non-neoplastic（atrophic gastritis/intestinal metaplasia） and neoplastic lesions（low-grade/high-grade dysplasia）, respectively(Ptrend<0.001). Four predictors, including male, ≥60 years, smoking, and limited vegetable consumption, were selected for risk-stratification. High-risk patients（≥3 risk factors） with non-neoplastic lesions showed higher GC risks（adjusted HR=7.73, 95%CI: 4.29–13.92）, and their four-year CIR reached the one-year CIR of neoplastic lesions. Further categorizing non-neoplastic lesions by histological grade, both patients with moderate-to-severe lesions（aHR=3.07, 95%CI: 1.67–5.64） and high-risk patients with mild lesions（aHR=7.29, 95%CI: 3.58–14.86） showed higher risks. Consistent trends were observed in validation cohort. High-risk mild-precursor-lesion patients should receive surveillance within 3–5 years after baseline screening. Our study provides evidence on supplementing current guideline recommendations.

Based on the theory of optimization,we use edges and angles of cells to represent the geometric quality of computational grids,employ the local gradients of the flow variables to describe the variation of flow field,and construct a multi-objective programming *** solution of this optimization problem gives appropriate balance between the geometric quality and adaptation of *** solving the optimization problem,we propose a new grid rezoning method,which not only keeps good geometric quality of grids,but also can track rapid changes in the flow *** particular,it performs well for some complex concave domains with *** also incorporate the rezoningmethod into anArbitrary Lagrangian-Eulerian(ALE)method which is widely used in the simulation of high-speed multi-material *** proposed rezoning and ALE methods of this paper are tested by a number of numerical examples with complex concave domains and compared with some other rezoning *** numerical results validate the robustness of the proposed methods.

Concrete experiences expansive deformation during sulfate attack due to internal expansive stress. Sulfate ions enter concrete pores, react with the pore solution, and produce ettringite. The production of ettringite explains the internal expansive stress that reduces the durability of concrete. In this study, the model of internal expansive stress was achieved through the Eshelby＇s theory, as well as the experimental results for concrete erosion. Numerical simulation indicated that internal expansive stress is not only determined by the water-to-cement ratio of concrete and the concentration of sulfate solution, but also affected by the relaxation time of concrete.

A major barrier to the use of antimicrobial peptides as antibiotics is the toxicity or ability to lyse eukaryotic cells. In this study, a 26-residue amphipathic a-helical antimicrobial peptide A12L/A20L （Ac-KWKSFLKTFKSLK KTVLHTLLKAISS-amide） was used as the framework to design a series of D- and L-diastereomeric peptides and study the relationships of helicity and biological activi- ties of o-helical antimicrobial peptides. Peptide helicity was measured by circular dichroism spectroscopy and demonstrated to correlate with the hydrophobicity of peptides and the numbers of D-amino acid substitu- tions. Therapeutic index was used to evaluate the selectivity of peptides against prokaryotic cells. By introducing D-amino acids to replace the original L-amino acids on the non-polar face or the polar face of the helix, the hemolytic activity of peptide analogs have been significantly reduced. Compared to the parent peptide, the therapeutic indices were improved of 44-fold and 22-fold against Gram-negative and Gram- positive bacteria, respectively. In addition, D- and L-diastereomeric peptides exhibited lower interaction with zwitterionic eukaryotic membrane and showed the significant membrane damaging effect to bacterial cells. Helicity was proved to play a crucial role on peptide specificity and biological activities. By simply replacing the hydrophobic or the hydrophilic amino acid residues on the non-polar or the polar face of these amphipathic derivatives of the parent peptide with D-amino acids, we demonstrated that this method could have excellent potential for the rational design of antimicrobial pep- tides with enhanced specificity.

The mutagenic effects of discharge plasma and plasma+PAW(plasma and its activated water combined) on Astragalus adsurgens Pall seeds were explored. Needle array-plate dielectric-barrier discharge plasma was used to treat A. adsurgens Pall seeds and PAW was prepared at the same *** deionized water and the obtained PAW were used to cultivate plasma-treated seeds in groups and the survival rate of each group was counted. Results showed that the survival rate of the treated seeds of A. adsurgens Pall when cultured with deionized water was not significantly different from that of the control check(CK). The culture with PAW had an obvious lethal effect and each group reached the half-lethal dose. Reactive oxygen species(ROS) and RNA-Seq analysis of seedlings in the 3 h treatment group showed that the content of ROS in the 3 d post-treatment group was significantly higher than that of the CK. Because expression of the gene with the function of scavenging superoxide free radicals was upregulated, the ROS content of seedlings on the sixth day was significantly lower than that on the third day. Plasma and plasma+PAW treatments changed a large number of gene expressions;particularly, the plasma+PAW group caused plant-growth genes to be significantly upregulated. After treatment, the seedlings of A. adsurgens Pall may grow faster and have higher nutritional value. This research is of great significance to the wider application of isoelectronic bodies and their activated water physicochemical mutagens in biological effects and breeding research.

We report the discovery of a peculiar X-ray transient,EP240408a,by Einstein Probe （EP） and follow-up studies made with EP,Swift,NICER,GROND,ATCA and other ground-based multiwavelength *** new transient was first detected with Wide-field X-ray Telescope （WXT） on board EP on April 8th,2024,manifested in an intense yet brief X-ray flare lasting for *** flare reached a peak flux of 3.9×10-9erg cm-2s-1in 0.5-4 ke V,～300 times brighter than the underlying X-ray emission detected throughout the *** and more precise follow-up observations by EP/FXT,Swift and NICER confirmed the finding of this new *** X-ray spectrum is non-thermal in 0.5-10 ke V,with a power-law photon index varying *** X-ray light curve shows a plateau lasting for ～4 d,followed by a steep decay till becoming undetectable ～10 d after the initial *** on its temporal property and constraints from previous EP observations,an unusual timescale in the range of 7-23 d is found for EP240408a,which is intermediate between the commonly found fast and long-term *** counterparts have been found in optical and near-infrared,with the earliest observation at 17 h after the initial X-ray detection,suggestive of intrinsically weak emission in these *** demonstrate that the remarkable properties of EP240408a are inconsistent with any of the transient types known so far,by comparison with,in particular,jetted tidal disruption events,gamma-ray bursts,X-ray binaries and fast blue optical *** nature of EP240408a thus remains an *** suggest that EP240408a may represent a new type of transients with intermediate timescales of the order of～10 *** detection and follow-ups of more of such objects are essential for revealing their origin.

Post-combustion technology of circulating fluidized bed can largely reduce the emission of nitrogen oxides(NOx)in the process of combustion and succeed in meeting the ultra-low NO_(x)standard for some fuels like Shenmu *** the potential of synergistic control of the emissions of NO_(x)and sulphur dioxide(SO_(2))under post-combustion technology has become a direction that needs further *** experiments were conducted on a 0.1 MW(thermal)circulating fluidized bed(CFB)test platform,composed of a CFB main combustor and post-combustion chamber(PCC).The paper focuses on the effects of air distribution ratio and temperature in CFB and limestone addition on NO_(x)and SO_(2)*** experimental results showed that compared with traditional CFB combustion,post-combustion technology can reduce NO_(x)emission largely,but lead to a slight increase in SO_(2)*** higher SO_(2)emissions at post-combustion can lead to less NO_(x)*** the decrease inλ_(CFB),NO_(x)emission first decreased and then increased;by contrast,SO_(2)emission withλ_(CFB)first increased and then *** post-combustion,whenλ_(CFB)was 0.9,NO_(x)emission was the minimum,while the SO_(2)emission was the *** temperature and limestone addition has less adverse effects on NO_(x)emission under post-combustion,compared with traditional CFB *** injection into the furnace is applicable under post-combustion,and the sulfur removal efficiency under post-combustion is very high,almost equivalent to that under traditional combustion.

Coronavirus disease 2019(COVID-19)is continuing to spread globally and still poses a great threat to human *** its outbreak,it has had catastrophic effects on human society.A visual method of analyzing COVID-19 case information using spatio-temporal objects with multi-granularity is proposed based on the officially provided case *** analysis reveals the spread of the epidemic,from the perspective of spatio-temporal objects,to provide references for related research and the formulation of epidemic prevention and control *** case information is abstracted,descripted,represented,and analyzed in the form of spatio-temporal objects through the construction of spatio-temporal case objects,multi-level visual expressions,and spatial correlation *** rationality of the method is verified through visualization scenarios of case information statistics for China,Henan cases,and cases related to *** results show that the proposed method is helpful in the research and judgment of the development trend of the epidemic,the discovery of the transmission law,and the spatial traceability of the *** has a good portability and good expansion performance,so it can be used for the visual analysis of case information for other regions and can help users quickly discover the potential knowledge this information contains.
