Crohn’s disease(CD)is caused by immune,environmental,and genetic *** can involve the entire gastrointestinal tract,and although its prevalence is rapidly increasing its etiology remains *** biological and small-molecule drugs have advanced the treatment of CD;however,a considerable proportion of patients are non-responsive to all known *** achieve a breakthrough in this field,innovations that could guide the further development of effective therapies are of utmost *** this review,we first propose the innovative concept of pan-lymphatic dysfunction for the general distribution of lymphatic dysfunction in various diseases,and suggest that CD is the intestinal manifestation of pan-lymphatic dysfunction based on basic and clinical preliminary *** supporting evidence is fully summarized,including the existence of lymphatic system dysfunction,recognition of the inside-out model,disorders of immune cells,changes in cell plasticity,partial overlap of the underlying mechanisms,and common gut-derived fatty and bile acid *** benefit of this novel concept is that it proposes adopting the zebrafish model for studying intestinal diseases,especially CD,as this model is good at presenting and mimicking lymphatic *** importantly,the ensuing focus on improving lymphatic function may lead to novel and promising therapeutic strategies for CD.

AIM:To evaluate the efficacy and safety of telbivudine(LDT) in hepatitis B e antigen(HBeAg)-positive chronic hepatitis B(CHB) patients who have high baseline alanine aminotransferase(ALT) levels between 10 and 20 times the upper limit of ***:Forty HBeAg-positive CHB patients with high baseline ALT levels between 10 and 20 times the upper limit of normal were enrolled and received LDT monotherapy for 52 *** forty patients with baseline ALT levels between 2 and 10 times the upper limit of normal were included as *** compared the virological,biochemical,serological and side effect profiles between the two groups at 52 ***:By week 52,the mean decrease in hepatitis B virus(HBV) DNA level compared with baseline was 7.03 log10 copies/mL in the high baseline ALT group and 6.17 log10 copies/mL in the control group,respectively(P < 0.05).The proportion of patients in whom serum HBV DNA levels were undetectable by polymerase chain reaction assay was 72.5% in the high baseline ALT group and 60% in the control group,respectively(P < 0.05).In addition,45.0% of patients in the high baseline ALT group and 27.5% of controls became HBeAg-negative,and 37.5% of those in the high baseline group and 22.5% of controls,respectively,had HBeAg seroconversion(P < 0.05) at week ***,in the high baseline group,4 out of 40 patients(10%) became hepatitis B surface antigen(HBsAg)-negative and 3(7.5%) of them seroconverted(became HBsAg-positive).Only 1 patient in the control group became HBsAg-negative,but had no *** ALT normalization rate,viral breakthrough,genotypic resistance to LDT,and elevations in creatine kinase levels were similar in the two groups over the 52 ***:High baseline ALT level is a strong predictor for optimal results during LDT treatment.

For the purpose of understanding the weathering characteristics of surface layers in purple mudstone sloping fields of the Three Gorges Reservoir area of China, oxide content of major elements, composition of clay minerals, magnetic susceptibility, and difference in weathering characteristics of surface layers under different slope gradients were determined. The results showed that the oxide content of Si, Al, and Fe ranged from 60% to 75% and the weathering coefficient with depth showed no trend along the slope gradient. Also, for gentle (10° and 15°) and intermediate (25° and 40°) slopes the clay relative diffraction peak for kaolinite at the surface between 0-10 cm and 10-20 cm declined with an increase in slope gradient, while the relative diffraction peak for kaolinite in weathered layers on steep slopes (50° and 60°) disappeared altogether. Magnetic susceptibility decreased with increasing depth and, for a given depth layer, decreased with an increase in slope gradient. Analysis of the oxide content, weathering coefficients, clay minerals, and magnetic susceptibility showed that in the Three Gorges Reservoir area, the pedogenesis of the weathering layer in purple mudstone sloping fields was weak with weaker soil formation going from gentle slope to steep slope.

Gene therapy is a new and promising approach which opens a new door to the treatment of human diseases. By direct transfer of genetic materials to the target cells, it could exert functions on the level of genes and molecules. It is hoped to be widely used in the treatment of liver disease, especially hepatic tumors by using different vectors encoding the aim gene for anti-tumor activity by activating primary and adaptive immunity, inhibiting oncogene and angiogenesis. Despite the huge curative potential shown in animal models and some pilot clinical trials, gene therapy has been under fierce discussion since its birth in academia and the public domain because of its unexpected side effects and ethical problems. There are other challenges arising from the technique itself like vector design, administration route test and standard protocol exploration. How well we respond will decide the fate of gene therapy clinical medical practice.

Despite the recent advances in the selective functionalization of C–C bonds in specific substrates,cleavage and functionalization of C–C bonds in acyclic substrates,such as ethane derivatives,remains *** contrast to the well-developed functionalization of one carbon fragment after C–C bond cleavage,herein,we report a novel electro-reductive carboxylation of C(sp^(3))–C(sp^(3))bond in multi-aryl ethanes with carbon dioxide(CO_(2))by utilizing both carbon ***,this reaction exhibits an atom-,step-economic approach for the synthesis of carboxylic acids,fulfilling principal aspirations of organic ***,this method features mild reaction conditions,broad substrate scope,and good functional group *** and asymmetric substrates bearing primary,secondary,or tertiary C(sp^(3))–C(sp^(3))bonds are all amenable to this strategy,enabling one or two structurally different carboxylic acids to be facilely constructed at the same *** investigations indicate that carbanions might be the key intermediates in this reaction,which could be captured by CO_(2)efficiently.

Background Cumulative evidence demonstrates that primary percutaneous coronary intervention（PCI）is a mperfusion strategy for ST-elevation myocardial Infarction（STEMI）.This study was undertaken to evaluate the pre-hospital care-seeking pathway and subsequent care quality in patients with STEMI in the Beijing health care system,which offers patients a choice between seeking care in a small community hospital（SH group）or a large hospital（LH group）.Methods Between January 1 and December 31,2006, a cross-sectional and multicenter survey was conducted in 11 hospitals qualified as tertiary centers in Beijing and included consecutive patients with STEMI admitted within 24 hours after onset of *** Among the 566 patients interviewed,28.3%first arnved at a small community hospitaI and were transferred to large hospitals with the ability to perform primary *** median total pre-hospital delay in the SH group（n=160）was significantly longer than in the LH group（n=406）（225 vs.120 minutes,P〈0.001）.Multivariate analysis showed that interpreting symptoms to non-cardiac origin（OR,1.996;95%CI： 1.264-3.155）,absence of history of myocardial infarction（OR,1.595;95%CI：1.086-3.347）,non-health insuranca coverage（OR,1.931;95%Cl：1.079-3.012）and absence of sense of impending doom （OR,4.367;95%CI：1.279-1 4.925） were independent predictors for choosing small *** adjusting for demographics and medical history,patients in the SH group were 1.698 times（95% CI： 1.1 82-3.661） less likely to receive primary PCI compared with those in the LH group. Conclusions Above one fourth of the STEMI patients in Beijing experienced inter-hospital *** including symptoms interpretation,symptoms,history of myocardial infarcUon,and insurance coverage were associated with the patients＇pre-hospital care-seeking *** patients who were transferred had longer pre-hospital delays and were less Iikely to receive primary PCI.
