Radiological prognosticators of hepatocellular carcinoma treated by hepatectomy

BACKGROUND: Hepatectomy is the main curative treatment for hepatocellular carcinoma (HCC), but postoperative long- term survival is poor. Preoperative radiological features of HCC displayed by computed tomography or magnetic resonance imaging could serve as additional prognostic factors. This study aimed to identify preoperative radiological features of HCC that may be of prognostic significance in hepatectomy. METHODS: Ninety-two patients who underwent hepatectomy for HCC were included in this study. Preoperative radiological features including tumor number, size, location (peripheral, middle, central), portal vein invasion, hepatic vein invasion, and presence of pseudo-capsule were analyzed in relation to survival. RESULTS: With a median follow-up period of 41.7 months, the 1-, 3- and 5-year overall survival rates were 85%, 65% and 58%, respectively. Univariate analysis showed that portal vein invasion and absence of pseudo-capsule were significant prognostic factors for overall survival, while all the examined radiological features were prognostic factors for disease-free survival. Multivariate analysis for overall survival found no significant factor. On multivariate analysis for disease-free survival, patients who had tumors with portal vein invasion had poorer survival with a hazard ratio of 2.26 (95% CI, 1.05-4.91; P=0.038) and patients with single nodular HCC or pseudo-capsulated HCC had better survival with a hazard ratio of 0.50 (95% CI, 0.27-0.94; P=0.032) and 0.38 (95% CI, 0.14-0.99; P=0.048), respectively. CONCLUSIONS: Demonstrable pseudo-capsule of HCC and solitary HCC on imaging and absence of portal vein invasionare features associated with better disease-free survival after hepatectomy. These features may guide treatment planning for HCC.

Objective:To report the clinical maternal and fetal outcomes of pregnant women with coronavirus disease 2019(COVID-19),along with any associated pregnancy complications,in Hong Kong,China,and to assess the impact of COVID-19 vaccination on these ***:This prospective registry-based observational study included pregnant women who were recruited through convenient sampling and had a laboratory-confirmed severe acute respiratory syndrome coronavirus 2 infection with a cycle threshold(Ct)value result available on admission to eight local hospitals in Hong Kong,*** on clinical symptoms,laboratory results,medical treatments,delivery timing and mode,and pregnancy complications were extracted from the Hospital Authority’s electronic medical record ***,fetal,and pregnancy outcomes were compared between unvaccinated pregnant women with COVID-19 and those who had received at least one dose of COVID-19 vaccine before *** continuous variables and categorical variables were analyzed using the Mann-Whitney U test and the Pearson’s chi-squared test respectively.A P value less than 0.05 was considered statistically ***:A total of 164 pregnant women were included,of whom 78(47.56%)were ***-19 was diagnosed before 28 weeks’gestation in 30(18.29%),while 134(81.71%)were diagnosed at or after 28 weeks’***-two(37.80%)women received at least one dose of COVID-19 *** were no significant differences between vaccinated and unvaccinated groups in the time interval between COVID-19 diagnosis and delivery,the Ct value,and the gestational age at infection onset or delivery(P>0.05).The majority of women were symptomatic at diagnosis regardless of vaccination status 55(88.71%)in vaccinated group vs.78(76.47%)in unvaccinated group(P=0.052).Symptoms did not significantly differ between groups except for cough(62.90%vs.47.06%,P=0.049).The overall rate of severe COVID-19 in pregnant women was *** t

Exact Time-Dependent Wave Functions of a Confined Time-Dependent Harmonic Oscillator with Two Moving Boundaries

By applying the standard analytical techniques of solving partial differential equations, we have obtained the exact solution in terms of the Fourier sine series to the time-dependent Schrodinger equation describing a quantum one-dimensional harmonic oscillator of time-dependent frequency confined in an infinite square well with the two walls moving along some parametric trajectories. Based upon the orthonormal basis of quasi-stationary wave functions, the exact propagator of the system has also been analytically derived. Special eases like （i） a confined free particle, （ii） a confined time-independent harmonic oscillator, and （iii） an aging oscillator are examined, and the corresponding time- dependent wave functions are explicitly determined. Besides, the approach has been extended to solve the case of a confined generalized time-dependent harmonic oscillator for some parametric moving boundaries as well.

Defective lysosomal acidification:a new prognostic marker and therapeutic target for neurodegenerative diseases

Lysosomal acidification dysfunction has been implicated as a key driving factor in the pathogenesis of neurodegenerative diseases,including Alzheimer’s disease and Parkinson’s *** genetic factors have been linked to lysosomal de-acidification through impairing the vacuolar-type ATPase and ion channels on the organelle *** lysosomal abnormalities are also present in sporadic forms of neurodegeneration,although the underlying pathogenic mechanisms are unclear and remain to be ***,recent studies have revealed early occurrence of lysosomal acidification impairment before the onset of neurodegeneration and late-stage ***,there is a lack of methods for organelle pH monitoring in vivo and a dearth of lysosome-acidifying therapeutic ***,we summarize and present evidence for the notion of defective lysosomal acidification as an early indicator of neurodegeneration and urge the critical need for technological advancement in developing tools for lysosomal pH monitoring and detection both in vivo and for clinical *** further discuss current preclinical pharmacological agents that modulate lysosomal acidification,including small molecules and nanomedicine,and their potential clinical translation into lysosome-targeting *** timely detection of lysosomal dysfunction and development of therapeutics that restore lysosomal function represent paradigm shifts in targeting neurodegenerative diseases.

Can we grow sperm？ A translational perspective on the current animal and human spermatogenesis models

There have been tremendous advances in both the diagnosis and treatment of male factor infertility; however, the mechanisms responsible to recreate spermatogenesis outside of the testicular environment continue to elude andrologists. Having the ability to ＇grow＇ human sperm would be a tremendous advance in reproductive biology with multiple possible clinical applications, such as a treatment option for men with testicular failure and azoospermia of multiple etiologies. To understand the complexities of human spermatogenesis in a research environment, model systems have been designed with the intent to replicate the testicular microenvironment. Currently, there are both in vivoand in vitro model systems. In vivo model systems involve the transplantation of either spermatogonial stem cells or testicular xenographs. In vitro model systems involve the use of pluripotent stem cells and complex coculturing and/or three-dimensional culturing techniques. This review discusses the basic methodologies, possible clinical applications, benefits and limitations of each model system. Although these model systems have greatly improved our understanding of human spermatogenesis, we unfortunately have not been successful in demonstrating complete human spermatogenesis outside of the testicle.

Automatic Mesh Generation on a Regular Background Grid

This paper presents an automatic mesh generation procedure on a 2D domain based on a regular background grid. The idea is to devise a robust mesh generation scheme with equal emphasis on quality and efficiency. Instead of using a traditional regular rectangular grid, a mesh of equilateral triangles is employed to ensure triangular element of the best quality will be preserved in the interior of the domain. As for the boundary, it is to be generated by a node/segment insertion process. Nodes are inserted into the background mesh one by one following the sequence of the domain boundary. The local structure of the mesh is modified based on the Delaunay criterion with the introduction of each node. Those boundary segments, which are not produced in the phase of node insertion, will be recovered through a systematic element swap process. Two theorems will be presented and proved to set up the theoretical basic of the boundary recovery part. Examples will be presented to demonstrate the robustness and the quality of the mesh generated by the proposed technique.

Resource-dependent cities(RCs)are a vulnerable group of urban settlements that often face population shrinkage;however,population changes in RCs in China are not well *** study offers new insight into this matter through a robust analysis that features a longer time scale,a larger sample of RCs,and a finer unit of *** finds new evidence that problems of population shrinkage in RCs are more serious than previous literature has *** 30%of the studied units have experienced either long-term or short-term population shrinkage,and many more are experiencing a slowing down of population *** are especially common among three types of RCs:the resource-depleted RCs,the forestry-based RCs,and RCs in Northeast *** results underscore transition policy inadequacies in addressing population loss,and call for a more comprehensive and diversified population policy that tackles the multifaceted factors that contribute to population shrinkage,including lack of industrial support,maladjustment to market oriented reformation,poor urban environment and natural population decline.

Retrohepatic vena cava deroofing in living donor liver transplantation for caudate hepatocellular carcinoma

The removal of tumor together with the native liver in living donor liver transplantation for hepatocellular carcinoma is challenged by a very close resection margin if the tumor abuts the inferior vena *** is in contrast to typical deceased donor liver transplantation where the entire retrohepatic inferior vena cava is included in total *** we report a case of deroofing the retrohepatic vena cava in living donor liver transplantation for caudate hepatocellular *** order to ensure clear resection margins,the anterior portion of the inferior vena cava was *** right liver graft was inset into a Dacron vascular graft on the back table and the composite graft was then implanted to the recipient inferior vena *** this technique,we observed the no-touch technique in tumor removal,hence minimizing the chance of positive resection margin as well as the chance of shedding of tumor cells during manipulation in operation.

Multi-Factor Duplicate Question Detection in Stack Overflow

Stack Overflow is a popular on-line question and answer site for software developers to share their experience and expertise. Among the numerous questions posted in Stack Overflow, two or more of them may express the same point and thus are duplicates of one another. Duplicate questions make Stack Overflow site maintenance harder, waste resources that could have been used to answer other questions, and cause developers to unnecessarily wait for answers that are already available. To reduce the problem of duplicate questions, Stack Overflow allows questions to be manually marked as duplicates of others. Since there are thousands of questions submitted to Stack Overflow every day, manually identifying duplicate questions is a difficult work. Thus, there is a need for an automated approach that can help in detecting these duplicate questions. To address the above-mentioned need, in this paper, we propose an automated approach named DuPPREDICTOR that takes a new question as input and detects potential duplicates of this question by considering multiple factors. DuPPREDICTOR extracts the title and description of a question and also tags that are attached to the question. These pieces of information （title, description, and a few tags） are mandatory information that a user needs to input when posting a question. DuPPREDICTOR then computes the latent topics of each question by using a topic model. Next, for each pair of questions, it computes four similarity scores by comparing their titles, descriptions, latent topics, and tags. These four similarity scores are finally combined together to result in a new similarity score that comprehensively considers the multiple factors. To examine the benefit of DuPPREDICTOR, we perform an experiment on a Stack Overflow dataset which contains a total of more than two million questions.
