Efficacy of Intragastric Balloons in the Markers of Metabolic Dysfunction-associated Fatty Liver Disease:Results from Meta-analyses

Background and Aims:Nonalcoholic fatty liver disease,now renamed metabolic dysfunction-associated fatty liver disease(MAFLD),is common in obese *** balloon(IGB),an obesity management tool with low complication risk,might be used in MAFLD treatment but there is still unexplained heterogeneity in results across ***:We conducted a systematic search of 152 citations published up to September ***-analyses,stratified analyses,and meta-regression were performed to evaluate the efficacy of IGB on homeostasis model assessment of insulin resistance(HOMA-IR),alanine aminotransferase(ALT),aspartate aminotransferase(AST),and gamma-glutamyl transpeptidase(GGT),and to identify patients most appropriate for IGB ***:Thirteen observational studies and one randomized controlled trial met the inclusion criteria(624 participants in total).In the overall estimate,IGB therapy significantly improved the serum markers change from baseline to follow-up[HOMA-IR:1.56,95%confidence interval(CI)=1.16–1.95;ALT:11.53 U/L,95%CI=7.10–15.96;AST:6.79 U/L,95%CI=1.69–11.90;GGT:10.54 U/L,95%CI=6.32–14.75].In the stratified analysis,there were trends among participants with advanced age having less change in HOMA-IR(1.07 vs.1.82).The improvement of insulin resistance and liver biochemistries with swallowable IGB therapy was no worse than that with endoscopic *** meta-regression analyses showed that greater HOMA-IR loss was predicted by younger age(p=0.0107).Furthermore,effectiveness on ALT and GGT was predicted by basal ALT(p=0.0004)and GGT(p=0.0026),***:IGB is effective among the serum markers of *** patients had a greater decrease of HOMA-IR after IGB therapy.

Studies on rain-runoff process in the peripheral mountainous area of the Sichuan Basin, which is regarded as a key ecological shelter, will contribute to flood control and environmental protection for the Upper Yang-tze River Basin. In two typical catchments-the Fujiang River Catchment and the Wujiang River Catchment, rainfall simulations have been conducted to study the rain-runoff processes of yellow soil and limestone soil in three types of land use-forestland, farmland and grassland. Results showed that (1) within the same rainfall process, overland flow occurs first on farmland, then on grassland, and finally on forestland; (2) soil surface coverage has a great impact on the occurrence and amount of overland flow. The runoff amount can increase 2-4 times after the coverage is removed;(3) the infiltration before the occurrence of overland flow will decrease because of higher gravel contents of soil, but it takes no effect on infiltration once overland flow becomes stable; (4) the runoff coefficient of the limestone soil forestland is greater than that of the yellow soil forest land, but less than that of the farmland; (5) three empirical infiltration models, including Horton' model, Kostiakov' model, and modified Kostiakov' model, were compared by using the observed results under rainfall simulation. The results showed that the Kostiakov' model performed better than both the Horton' model and modified Kostiakov model. According to the results of this research, the Kostiakov's model can be used to simulate rainfall infiltration when water erosion is modeled in the peripheral mountainous area of the Sichuan Basin.

Impact of Percutaneous Transhepatic Biliary Drainage on Clinical Outcomes of Patients with Malignant Obstructive Jaundice Undergoing Laparoscopic Pancreaticoduodenectomy

Preoperative biliary drainage may increase the morbidity and mortality of *** on percutaneous transhepatic biliary drainage(PTBD)before laparoscopic pancreaticoduodenectomy(LPD),however,are *** aim of this study was to examine the impact of PTBD on clinical outcomes of patients with malignant obstructive jaundice undergoing *** data of 172 patients who had malignant obstructive jaundice and underwent LPD from 2014 to 2017 in our hospital were retrospectively ***,catheterrelated complications,postoperative complications,and oncological outcormes were collected and *** score matching was performed to minimize selection bias associated with the comparison of data between patients who underwent PTBD and then LPD(PTBD group),and those given LPD alone(LPD group).The results showed that,in the PTBD group relative to the LPD group,the operating time was significantly shortened(250.28±69.95 vs.278.58±86.51 min,P=0.0196),the intraopcrative blood loss was markedly reduced(271.96±403.47 vs.429.72±482.47 mL,P=0.022),and overall rates of complications(16.33%vs.36.49%,P=0.0025)including postoperative haemorrhage(2.04%vs.12.16%,P=0.0072)and delayed gastric emptying(4.08% vs.13.51%,P=0.0251)were greatly *** propensity score-matched analysis,with 48 patients enrolled in each group,revealed no statistically significant differences in operating duration(262.71±68.64 vs.280.25±83.52 min,P=0.264),intraoperative blood loss(290.21±407.71 vs.373.75±422.33 mL,P=0.327)and delayed gastric emptying(4.17% vs.12.50%,P=0.1396).PTBD group had lower incidences in overall complications(22.92% vs.39.58%,P=0.0481)and postoperative haemorrhage(2.08% vs.12.50%,P=0.0497)than LPD *** conclusion,patients with malignant obstructive jaundice may benefit from PTBD procedure before LPD in terms of perioperative outcomes.

Many variables affect the sealing performance, and their distribution characteristics are difficult to obtain with probabilistic methods owing to the high cost involved. Numerous problems in engineering are similar due to the appearance of small-sample parameters. In this study, the sealing reliability of an aviation seal was defined as the research object, and an interval uncertainty method and multidimensional response surface were proposed to calculate the sealing *** on this, we first analyzed the failure mechanism of the aviation seal and established a leakage rate model. Then, based on the non-probabilistic interval model, an interval uncertainty method was proposed to construct the analytical model. With reference to the limit state equation from the structural reliability theory, the multidimensional response surface was used for fast ***, we chose the single-cylinder gas steering gear used in aircraft as the case study, its sealing reliability in working and non-working statuses were calculated, and the results were verified with the actual maintenance records. By analyzing the sensitivity of some variables, we can improve the sealing reliability of the aviation seal by improving the surface roughness only if the cost ***, we consider that the method proposed in this study realizes the application of smallsample uncertainty analysis in reliability analysis, and could provide a feasible way to solve the similar problems in engineering with multidimensional and small-sample parameters.

This paper presents a new deep-reading logging-while-drilling electromagnetic(EM)logging method to detect bed boundaries ahead of *** all existing EM logging approaches,the new method is based on the scattered electric field radiated by a magnetic dipole *** analyzing the characteristics of electric tensor responses in layered formations,optimal look-ahead electric component is *** selected scattered field contributes to a large portion of the total field and is strongly sensitive to the boundary *** measured voltage from the scattered electric component can be tens of times larger than that from the scattered magnetic fields and it attenuates ***,the detection capability improves significantly.A coaxial open-loop half-circle antenna is then proposed to measure the electric field in logging while drilling environment.A practical tool implementation equipped with two tilted close-loop antennas and two open-loop antennas is further developed for look-ahead *** results demonstrate that the detection depth of the new look-ahead tool can be up to 40 m under favorable *** with current look-ahead logging tools,the new method not only significantly shortens the tool size,but also can recognize the boundary position and azimuth.

Recent studies reveal that weak Lyα emission line may be a ubiquitous feature of nearby BL Laceratae objects(BL Lacs). We present a survey of the Lyα emission lines in BL Lacs, with a focus on data obtained by the Cosmic Origins Spectrograph onboard the Hubble Space Telescope. Among the11 selected targets, seven show a clear detection of the intrinsic Lyα emission line, while one manifests a marginal detection. Two of these intrinsic Lyα lines are newly detected in this work(FBQS J1217+3007 and3 C 66 A). Most selected BL Lacs are either high or intermediate-frequency peaked BL Lacs, with the only exception being one low-frequency peaked BL Lac. A strong anti-correlation between the line equivalent width and the continuum luminosity was found. The possible sources of ionizing photons that are responsible for production of the Lyα emission line are studied based on a widely accepted detailed accretion-jet model. It is found that the clouds of the broad line region are unlikely to be located in the jet cone region with an inclination angle of less than 10 degrees. Contributions from the jet and disk to the emission lines are found to be comparable. Additionally, a possible way to constrain the accretion rate from the emission line is proposed.

Numerical simulation of high-resolution azimuthal resistivity laterolog response in fractured reservoirs

The high-resolution azimuthal resistivity laterolog response in a fractured formation was numerically simulated using a three-dimensional finite element method. Simulation results show that the azimuthal resistivity is determined by fracture dipping as well as dipping direction, while the amplitude differences between deep and shallow laterolog resistivities are mainly controlled by the former. A linear relationship exists between the corrected apparent conductivities and fracture aperture. With the same fracture aperture, the deep and shallow laterolog resistivities present small values with negative separations for low-angle fractures, while azimuthal resistivities have large variations with positive separations for high-angle fractures that intersect the borehole. For dipping fractures, the variation of the azimuthal resistivity becomes larger when the fracture aperture increases. In addition, for high-angle fractures far from the borehole, a negative separation between the deep and shallow resistivities exists when fracture aperture is large as well as high resistivity contrast exists between bedrock and fracture fluid. The decreasing amplitude of dual laterolog resistivity can indicate the aperture of low-angle fractures, and the variation of the deep azimuthal resistivity can give information of the aperture of high-angle fractures and their position relative to the borehole.
