Actinomycosis is a rare, chronic, spreading, suppurative,granulomatous and fibrosing infection. Actinomyces are normal inhabitants of the oral cavity and gastrointestinal tract. They rarely cause disease and are seldom reported as pathogens. Herein, we reported on a 69-year-old male patient who had undergone Whipple's operation due to ampulla Vater carcinoma, and became infected with actinomycosis at the pancreaticojejunostomy, which mimicked a recurrent malignancy. He was treated with radical resection of the mass at the pancreaticojejunostomy and had an uneventful postoperative course.

Based on the Intensive Field Campaign(IFC-1)data of Boreal Ecosystem-Atmosphere Study (BOREAS).a three-dimensional meso-β scale model is used to simulate the effect of boreal forests on the lower atmosphere.A fine horizontal resolution of 2 km×2 km is used in order to distinguish the vegetative heterogeneity in the boreal region.A total of 20×25 grid points cover the entire sub- modeling area in BOREAS' South Study Area(SSA).The ecosystem types and their coverage in each grid square are extracted from the North American Land Cover Characteristics Data Base (NALCCD)generated by the *** Survey(USgS)and the University of Nebraska- Lincoln(UNL).The topography of the study area is taken from the Digital Elevation Map(DEM) of *** model outputs include the components of the energy balance budget within the canopy and at the *** turbulence parameters in the atmospheric boundary layer and the wind. temperature and humidity profiles extending up to a height of 1500 *** addition to the fine time and spatial step,the unique feature of the present model is the incorporation of both dynamic and biological effects of the Boreal forest into the model parameterization *** model results compare favorably with BOREAS' IFC-1 data in 1994 when the forest was in the luxuriant growing period.

Tensin-1(TNS1)is a 220 kD focal adhesion protein that binds to actin filaments,integrin receptors,small gTPases,tyrosine-phosphorylated proteins,and lipids.1 These binding activities enable TNS1 to link the actin cytoskeleton to integrins and transduce outside-in and inside-out signals at focal adhesion sites,thereby regulating cell attachment,migration,proliferation,and mechanical sensing.1 Lack of TNS1 in kidney epithelial cells results in multiple lumen,instead of single lumen,phenotypes in the three-dimensional(3D)culture.

BACKgROUND Emphysema pyelonephritis(EPN)is a very dangerous type of urinary tract *** is a lethal disease that develops rapidly and causes the patient to deteriorate rapidly,and it can easily lead to systemic infections and even *** incidence is extremely low,and it is prevalent in patients with *** here report a case of EPN in a non-diabetic patient with autosomal dominant polycystic kidney disease(ADPKD).We share the diagnosis and treatment procedure for this extremely rare condition to make this disease easier to identify and address *** SUMMARY A 47-year-old woman presented to the emergency department of our hospital with a high fever and left back pain lasting 4 *** had a history of autosomal dominant polycystic kidney and polycystic *** was diagnosed with left type I EPN and her vital signs deteriorated so quickly that she underwent an emergency operation in which a D-J tube was inserted into her left ureter on the second day after *** months later,she underwent a second-stage flexible ureteroscopy and *** postoperative sepsis,she finally recovered after active symptomatic support treatment and effective anti-infective *** Although EPN is more likely to occur in diabetic patients,for non-diabetic patients with ADPKD and upper urinary tract obstruction,the disease also causes rapid *** and accurate diagnosis and timely removal of the obstruction by invasive means may be able to save the damaged kidney and the patient’s life.

Carbon fixation is the main route of inorganic carbon in the form of CO2 into the *** nature,RuBisCO is the most abundant protein that photosynthetic organisms use to fix CO2 from the atmosphere through the Calvin-Benson-Bassham(CBB)***,the CBB cycle is limited by its low catalytic rate and low energy *** this work,we attempt to integrate the reductive tricarboxylic acid and CBB cycles in silico to further improve carbon fixation *** heterologous enzymes,mostly carboxylating enzymes,are inserted into the Esherichia coli core metabolic network to assimilate CO2 into biomass using hydrogen as energy ***,such a strain shows enhanced growth yield with simultaneous running of dual carbon fixation *** key results include the following.(i)We identified two main growth states:carbon-limited and hydrogenlimited;(ii)we identified a hierarchy of carbon fixation usage when hydrogen supply is limited;and(iii)we identified the alternative sub-optimal growth mode while performing genetic *** results and modeling approach can guide bioengineering projects toward optimal production using such a strain as a microbial cell factory.

AIM： Prior Helicobacterpylori （Hpylori infection has often been underestimated. These underestimations have misled physicians attempting to determine the significance between Hpyloriand certain gastrointestinal lesions such as intestinal metaplasia, atrophic gastritis, and gastric cancer. Our study endeavored to debit past Hppylorinfections accurately, easily, and rapidly with the newly developed irnmunoblot kit, Helico Blot 2.1. METHODS： Thirty-three patients, including 25 H pylori infected and 8 uninfected cases, were enrolled in our study. All patients received consecutive gastroendoscopic examinations and ^3C-urea breath test （UBT） tests at 6- or 12-mo intervals for up to 4 years. Serum samples were obtained from each patient at the same time. Intragastric H pylori infection was confirmed in accordance with the gold standard. Twenty-five H pylori-infected patients received triple therapies after initial bacterial confirmation, and were successful in eradicating their infections. Serially obtained sera were tested by means of Helico Blot 2.1. RESULTS： Current infection marker detected by Helico Blot 2.1 was unreliable for representing ongoing Hpylori infection. Only 35 and 37 ku antibodies of H pylorihad significant seroconversion rates 1 year after having been cured. The seroposiUve rates of 116 ku （cytotoxin-associated antigen [CagA]） and Helico Blot 2.1 were nearly 100% during 4-year follow-up period. Both CagA antigen and Helico blot 2.1 could serve as indicators of lo.g-term H pylori infection. CONCLUSION： Helico Blot 2.1 can detect past H pylori infections for up to 4 years, and is the best method to date for detecting previous lo.g-term H pylori infection. 2005 The WJg Press and Elsevier Inc. AIM Capsaicin, a pungent ingredient found in red pepper, has lo.g been used in spices, food additives, and drugs. Cell death induced by the binding of capsaidn was examined in a human gastric adenocarcinoma cell line （AgS cells）. METHODS： By using XTT-based cytotoxicity assay, flow cytometry using the TUNEL method, and quantitation of DNA fragmentation, both cell death and DNA fragmentaUon were detected in AgS cells treated with capsaicin. By using Western blotting methods, capsaicin reduced the expression of Bcl-2, the antiapoptotic protein, in AgS cells in a concentration-dependent manner. RESULTS- After incubation of AgS cells with capsaicin for 24 h, cell viability decreased significantly in a dose-dependent manner. After incubation of AgS cells with capsaicin for 24 h, apoptotic bodies also significantly increased, and were again correlated with the dose of capsaicin. When the concentration of capsaicin was 1 mmol/L, the amount of DNA fragments also increased. Similar results were also in the lower traces. CONCLUSION： These results suggest that capsaicin- induced cell death might be via a Bcl-2 sensitive apoptotic pathway. Therefore, capsaicin might induce protection from gastric cancer.

Patients with multimorbidity are becoming the norm rather than the exception. The management of patients with several chronic diseases is now the most important challenge facing health care systems in developed countries. Based on the actual medical records of ambulatory care visits, this study investigated the prevalence and patterns of multimorbidity in 55 and older population. Among a cohort comprised of 300,000 beneficiaries selected randomly from the National Health Insurance Research Database of Taiwan in 2001, 42,441 were eligible. These were followed lo.gitudinally 10 years. The prevalence of chronic disease rose from 62.3% to 79.8% and multimorbidity rose from 57.4% to 75.7%. Multimorbidity patterns were found fell in-to four clusters: metabolic diseases, cardiac diseases, mental joints and gastrointestinal tract disorders. Hypertension and diabetes mellitus, as well as hyperplasia of the prostate in men, were the most common chronic diseases. The prevalence of chronic disease increased with age, especially high at age 75 - 79. Thought about the health care system for an ageing society is necessary. Applying the concept of customer experience and strengthening people-centered management in an integrated model of health care, enhancing knowledge and skills in the lo.g-term management of chronic disease, revising clinical guidelines and training professionals in caring for the elderly, reinforcing preventive health services, especially in men’s health, modifying the materials for health education, and planning for health manpower resources will provide a better model to ensure the health care for people with multimorbidity.
