INTRODUCTION Gender dysphoria(GD),or gender identity disorder,is defined as persistent distress stemming from the incongruence between one's assigned sex at birth and gender identity.'GD has traditionally been introduced as a rare condition predominant in assigned males at birth(AMABs).2 However,recent studies have shown an upward trend in assigned females at birth(AFABs)with a dramatic reversal of the AMAB:AFAB ratio-.5-The actual AMAB:AFAB ratio varies by age group and study population.4-Questions have been raised concerning the increasing number of youth who seek professional care for GD,especially adolescent AFABs.

Background Depressive symptoms are often seen in schizophrenia. The overlap in presentation makes it difficult to distinguish depressive symptoms from the negative symptoms of schizophrenia. 背景:精神分裂症是一种慢性致残性疾病,其发病及治疗可能与炎性细胞因子有关。目前细胞因子预测抗精神病药物疗效的研究尚少。目的:研究细胞因子对抗精神病药物疗效的预测作用。方法:采用横断面与自然观察随访相结合的方法,(1)对比精神分裂症患者组(n=64)基线期和正常对照组(n=53)血清IL-1β、TNF-α和MCP-1水平;(2)探索基线期细胞因子水平对奥氮平或利培酮单药治疗效果的影响。结果:(1)精神分裂症患者治疗前血清MCP-1水平明显高于健康对照组(t=2.62,p=0.010),IL-1β(t=1.43,p=0.154)、TNF-α(t=0.79,p=0.434)未见变化;(2)精神分裂症患者治疗前MCP-1水平与利培酮单药治疗4周后PANSS量表一般病理评分的减少量呈显著负相关(r=-0.658;p﹤0.001),但在奥氮平组中则未发现(r=-0.031;p=0.855);(3)纳入性别、年龄、受教育年限和BMI后等影响因素后,多元线性回归分析发现,基线血清MCP-1水平可作为利培酮单药治疗效果的独立预测因子(校正R2=0.409,β=-0.658,p﹤0.001)。结论:MCP-1可能参与精神分裂症的发生,治疗前血清MCP-1水平可能是利培酮疗效预测的生物标记物。

背景与目的:目前,经胸乳入路腔镜甲状腺癌手术已广泛开展,但伴颈侧区淋巴结转移的患者能否在腔镜下完成彻底的淋巴结清扫,特别是Ⅵ和Ⅳ区的低位淋巴结清扫,一直存在争议。本研究总结分析笔者团队近年实施的经胸经口联合入路腔镜甲状腺癌的颈侧区淋巴结清扫患者的临床资料,探索采用该术式的必要性及安全性。方法:回顾分析2015年2月—2021年12月于厦门大学附属中山医院63例行腔镜甲状腺切除、中央区及颈侧区淋巴结清扫的甲状腺乳头状癌患者资料。其中,41例行经胸口联合入路手术(联合入路组),包括12例行经胸甲状腺切除、中央区及颈侧区淋巴结清扫后再经口补充清扫Ⅵ和Ⅳ区淋巴结,另29例行经胸和经口手术步骤合理融合,不再单独补充清扫Ⅵ及Ⅳ区淋巴结;22例行单纯经胸入路腔镜甲状腺癌根治并颈侧区淋巴结清扫术(经胸入路组)。比较两组患者的相关临床指标。结果:两组患者的年龄、性别及肿瘤大小差异均无统计学意义(均P>0.05)。联合入路组的12例经口补充清扫颈侧区淋巴结的标本中有8例检出淋巴结,其中2例检出阳性淋巴结。联合入路组与经胸入路组获得的中央区淋巴结总数分别为(8.80±5.78)枚、(8.23±3.53)枚,颈侧区获得的淋巴结总数分别为(31.49±14.90)枚、(29.05±7.80)枚,差异均无统计学意义(P>0.05)。两组的手术时间、住院时间以及术后出血、喉返神经麻痹、甲状旁腺功能减低、副神经损伤、乳糜漏和Horner综合征等并发症发生率差异均无统计学意义(均P>0.05)。术后随访,联合入路组1例颈侧区淋巴结转移癌复发,经胸入路组1例手术清扫侧中央区淋巴结转移癌复发。结论:经胸经口联合入路腔镜甲状腺癌颈侧区淋巴结清扫术安全、有效,与单纯经胸入路疗效相当。但补充清扫中的阳性淋巴结提示,对于淋巴结转移较多的甲状腺癌患者经胸经口联合入路腔镜甲状腺癌颈侧区淋巴结清扫术可能有一定的价值。然而,本研究病例数量较少,随访时间相对较短,有待多中心、大样本量的对照研究及长期随访结果来进一步评估该方法的安全性、有效性和必要性。

目的总结腹茧症的临床表现、诊断、治疗及预后。方法回顾性分析2015年1月至2021年12月厦门大学附属中山医院收治的8例腹茧症患者的临床表现、影像学特征、治疗及随访等临床资料。结果本组8例腹茧症患者中1例无腹部症状,7例表现为反复腹痛并伴有完全性肠梗阻或不完全肠梗阻,中位病程时间为6个月(15 d~40年)。6例行腹腔镜腹茧剥离切除+肠粘连松解术,其中1例行腹腔镜手术转开放手术,2例为开腹手术。4例同期行阑尾切除。术后随访3~69个月,有2例并发早期炎症性肠梗阻,1例伤口脂肪液化合并感染,1例术后5个月发生1次肠绞痛,其余患者均恢复良好。结论腹茧症的临床特征不典型,手术是主要的治疗手段,该病的预后总体较好。

目的探讨舌下前庭入路经口腔镜甲状腺手术的临床应用。方法回顾性分析2011年11月至2020年5月厦门大学附属中山医院收治的98例舌下前庭入路经口腔镜甲状腺手术患者的病例资料,其中男3例,女95例,年龄18~57岁。根据开展时间段的不同分成2个阶段,阶段A(2011年11月至2015年11月)患者81例,阶段B(2019年8月至2020年5月)患者17例。分析总结2个阶段患者的一般资料、手术时间以及并发症等。应用SPSS 22.0软件进行统计学处理。结果阶段A患者年龄(34.2±9.4)岁,肿瘤直径为(2.33±0.80)cm,术后病理示良性肿瘤76例,恶性肿瘤5例;良性患者的手术时间为(132.70±47.22)min,其中65例行单侧腺叶次全切除术,6例行峡部切除术,5例行双侧腺叶次全切除术;恶性患者的手术时间为(185.4±31.40)min,术式均为单侧腺叶切除术+单侧中央区淋巴清扫术;术后6例出现颈部感染,均经抗感染、引流等处理后好转;出现1例暂时性声带麻痹,术后2个月恢复;出现2例CO2气体栓塞,术中中转开放手术。阶段B患者年龄(35.1±8.5)岁,肿瘤直径为(1.32±0.67)cm,术后病理示良性肿瘤2例,恶性肿瘤15例;良性患者的手术时间为(153.5±34.64)min,均行单侧腺叶切除术;恶性患者的手术时间为(123.73±14.26)min,术式均为单侧腺叶切除术+单侧中央区淋巴清扫术;未出现术后颈部感染、暂时性声带麻痹、CO2气体栓塞。所有患者术后均有不同程度的下颌部皮肤麻木感,为5 mm操作孔相对应的位置,均于术后1~2周恢复正常。2个阶段均未出现术后二次出血、永久性声带麻痹、甲状旁腺功能减退等并发症。阶段A随访时间中位数为86(57~105)个月,阶段B随访时间中位数为5(3~12)个月;随访期间内患者吞咽、咀嚼、口腔感觉功能及颈部活动无明显异常,且无肿瘤复发、转移。结论舌下前庭入路经口腔镜甲状腺手术是安全可行的,具有良好的美容效果。该术式不会引起患者术后长时间的下颌部皮肤麻木感。
