Paradoxical carbon dioxide embolism during laparoscopic hepatectomy without intracardiac shunt:A case report

BACKGROUND Laparoscopic hepatectomy has recently become popular because it results in less bleeding than open hepatectomy.However,CO_(2)embolism occurs more frequently.Most CO_(2)embolisms during laparoscopic surgery are self-resolving and non-symptomatic;however,severe CO_(2)embolism may cause hypotension,cyanosis,arrhythmia,and cardiovascular collapse.In particular,paradoxical CO_(2)embolisms are highly likely to cause neurological deficits.We report a case of paradoxical CO_(2)embolism found on transesophageal echocardiography(TEE)during laparoscopic hepatectomy,although the patient had no intracardiac shunt.CASE SUMMARY A 71-year-old man was admitted for laparoscopic left hemihepatectomy.During left hepatic vein ligation,the inferior vena cava was accidentally torn.We observed a sudden drop in oxygen saturation to 85%,decrease in systolic blood pressure(SBP)below 90 mm Hg,and reduction in end-tidal CO_(2)to 24 mm Hg.A“mill-wheel”murmur was auscultated over the precordium.The fraction of inspired oxygen was increased to 100%with 5 cm H2O of positive end-expiratory pressure(PEEP)and hyperventilation was maintained.Norepinephrine infusion was increased to maintain SBP above 90 mm Hg.A TEE probe was inserted,revealing gas bubbles in the right side of the heart,left atrium,left ventricle,and ascending aorta.The surgeon reduced the pneumoperitoneum pressure from 17 to 14 mm Hg and repaired the damaged vessel laparoscopically.Thereafter,the patient’s hemodynamic status stabilized.The patient was transferred to the intensive care unit,recovering well without complications.CONCLUSION TEE monitoring is important to quickly determine the presence and extent of embolism in patients undergoing laparoscopic hepatectomy.

Transient involuntary movement disorder after spinal anesthesia: A case report

BACKGROUND Spinal anesthesia is commonly used for various surgeries.While many complications occur after induction of spinal anesthesia,involuntary movement is an extremely rare complication.CASE SUMMARY Herein,we report the case of a 54-year-old healthy male patient who experienced involuntary movements after intrathecal injection of local anesthetics.This patient had undergone metal implant removal surgery in both the lower extremities;7 h after intrathecal hyperbaric bupivacaine administration,involuntary raising of the left leg began to occur every 2 min.When the movement disorder appeared,the patient was conscious and cooperative.No other specific symptoms were noted in the physical examination conducted immediately after the involuntary leg raising started;moreover,the patient's motor and sensory assessments were normal.The symptom gradually subsided.Twelve hours after the symptom first occurred,its frequency decreased to approximately once every three hours.Two days postoperatively,the symptoms had completely disappeared without intervention.CONCLUSION Anesthesiologists should be aware that movement disorders can occur after spinal anesthesia and be able to identify the cause,such as electrolyte imbalance or epilepsy,since immediate action may be required for treatment.Furthermore,it is crucial to know that involuntary movement that develop following spinal anesthesia is mostly self-limiting and may not require additional costly examinations.

A new approach to quantifying vehicle induced turbulence for complex traffic scenarios

Traffic-related pollutants adversely affect air quality, especially in regions near major roadways. The vehicleinduced turbulence(VIT) is a significant factor that controls the initial dilution, dispersion, and ultimately the chemical and physical fate of pollutants by altering the conditions in the microenvironment. This study used a computational fluid dynamics(CFD) software FLUENT to model the vehicle-induced turbulence(VIT) generated on roadways, with a focus on impact of vehicle-vehicle interactions, traffic density and vehicle composition on turbulent kinetic energy(TKE). We show, for the first time, that the overall TKE from multiple vehicles traveling in series can be estimated by superimposing the TKE of each vehicle, without considering the distance between them while the distance is greater than one vehicle length. This finding is particularly significant since it enables a new approach to VIT simulations where the overall TKE is calculated as a function of number of vehicles. We found that the interactions between vehicles traveling next to each other in adjacent lanes are insignificant,regardless the directions of the traffic flow. Consequently, simulations of different traffic scenarios can be substantially simplified by treating two-way traffic as one-way traffic, with less than 5% difference in the overall volume-averaged TKE. We also developed equations that allow the estimation of the overall volume-averaged TKE as a function of the number and the type of vehicles.

Acute lower extremity arterial thrombosis after intraocular foreign body removal under general anesthesia: A case report and review of literature

BACKGROUND Surgery,which is a major risk factor for venous thrombosis,has rarely been considered a risk factor for arterial thrombosis.Recent studies have suggested that venous and arterial thromboses share common risk factors and have a bidirectional relationship.Accordingly,there is a growing interest in the risk of arterial thrombosis after surgery.We report a case of acute bilateral lower extremity arterial thromboses that developed after a prolonged surgery.CASE SUMMARY A 59-year-old man was hospitalized for intraocular foreign body removal surgery.He was a heavy-drinking smoker and had untreated hypertension and varicose veins in both legs.The operation was unexpectedly prolonged,lasting 4 h and 45 min.Immediately after emergence from general anesthesia,the patient complained of extreme pain in both legs.After the surgical drape was removed,cyanosis was evident in both feet of the patient.The pulse was not palpable,and continuous-wave Doppler signals were inaudible in the bilateral dorsalis pedis and posterior tibial arteries.Computed tomography angiography confirmed acute bilateral thrombotic occlusion of the popliteal arteries,proximal anterior tibial arteries,and tibioperoneal trunks.Arterial pulse returned in both lower limbs after 6 h of heparin initiation.The patient was discharged on postoperative day 26 without any sequelae.CONCLUSION Acute lower extremity arterial thrombosis can occur after surgery.Anesthesiologists should pay particular attention to patients with risk factors for thrombosis.

Hyperreflective photonic crystals created by shearing colloidal dispersions at ultrahigh volume fraction

Colloidal crystallization serves as one of the most economic and scalable production methods for photonic crystals.However,insufficient optical performance,nonuniformity and low reproducibility remain challenges for advanced high-value applications.In this study,we optimally formulate a photocurable dispersion of silica particles and apply shear flow to unify the orientation of the colloidal crystals,ensuring high optical performance and uniformity.The silica particles experience strong repulsion at ultrahigh volume fractions of 50%but demonstrate low mobility,leading to polycrystalline structures.Applying shear flow to the dispersions allows the silica particles to rearrange into larger crystalline domains with a unidirectional orientation along the flow.This shear-induced structural change produces absolute reflectivity at the stopband as high as 90%and a high transparency of 90%at off-resonant wavelengths with minimal diffusive scattering.Furthermore,the strong interparticle repulsion ensures a uniform volume fraction of particles throughout the dispersion,reducing deviations in the optical properties.We intricately micropattern the photocurable dispersions using photolithography.Additionally,the photonic films and patterns can be stacked to form multiple layers,displaying mixed structural colors and multiple reflectance peaks without sacrificing reflectivity.These superior photonic materials hold promise for various optical applications,including optical components and anticounterfeiting patches.

Current lithium-ion batteries(LIBs)rely on organic liquid electrolytes that pose significant risks due to their flammability and toxicity.The potential for environmental pollution and explosions resulting from battery damage or fracture is a critical concern.Water-based(aqueous)electrolytes have been receiving attention as an alternative to organic electrolytes.However,a narrow electrochemicalstability window,water decomposition,and the consequent low battery operating voltage and energy density hinder the practical use of aqueous electrolytes.Therefore,developing novel aqueous electrolytes for sustainable,safe,high-performance LIBs remains challenging.This Review first commences by summarizing the roles and requirements of electrolytes–separators and then delineates the progression of aqueous electrolytes for LIBs,encompassing aqueous liquid and gel electrolyte development trends along with detailed principles of the electrolytes.These aqueous electrolytes are progressed based on strategies using superconcentrated salts,concentrated diluents,polymer additives,polymer networks,and artificial passivation layers,which are used for suppressing water decomposition and widening the electrochemical stability window of water of the electrolytes.In addition,this Review discusses potential strategies for the implementation of aqueous Li-metal batteries with improved electrolyte–electrode interfaces.A comprehensive understanding of each strategy in the aqueous system will assist in the design of an aqueous electrolyte and the development of sustainable and safe high-performance batteries.

Targeting the RAS/RAF/MAPK pathway for cancer therapy: from mechanism to clinical studies

Metastatic dissemination of solid tumors,a leading cause of cancer-related mortality,underscores the urgent need for enhanced insights into the molecular and cellular mechanisms underlying metastasis,chemoresistance,and the mechanistic backgrounds of individuals whose cancers are prone to migration.The most prevalent signaling cascade governed by multi-kinase inhibitors is the mitogen-activated protein kinase(MAPK)pathway,encompassing the RAS–RAF–MAPK kinase(MEK)–extracellular signal-related kinase(ERK)pathway.RAF kinase is a primary mediator of the MAPK pathway,responsible for the sequential activation of downstream targets,such as MEK and the transcription factor ERK,which control numerous cellular and physiological processes,including organism development,cell cycle control,cell proliferation and differentiation,cell survival,and death.Defects in this signaling cascade are associated with diseases such as cancer.RAF inhibitors(RAFi)combined with MEK blockers represent an FDAapproved therapeutic strategy for numerous RAF-mutant cancers,including melanoma,non-small cell lung carcinoma,and thyroid cancer.However,the development of therapy resistance by cancer cells remains an important barrier.Autophagy,an intracellular lysosome-dependent catabolic recycling process,plays a critical role in the development of RAFi resistance in cancer.Thus,targeting RAF and autophagy could be novel treatment strategies for RAF-mutant cancers.In this review,we delve deeper into the mechanistic insights surrounding RAF kinase signaling in tumorigenesis and RAFi-resistance.Furthermore,we explore and discuss the ongoing development of next-generation RAF inhibitors with enhanced therapeutic profiles.Additionally,this review sheds light on the functional interplay between RAF-targeted therapies and autophagy in cancer.

Metal halide perovskites have emerged as promising light-emitting materials for next-generation displays owing to their remarkable material characteristics including broad color tunability,pure color emission with remarkably narrow bandwidths,high quantum yield,and solution processability.Despite recent advances have pushed the luminance efficiency of monochromic perovskite light-emitting diodes(PeLEDs)to their theoretical limits,their current fabrication using the spincoating process poses limitations for fabrication of full-color displays.To integrate PeLEDs into full-color display panels,it is crucial to pattern red–green–blue(RGB)perovskite pixels,while mitigating issues such as cross-contamination and reductions in luminous efficiency.Herein,we present state-of-the-art patterning technologies for the development of full-color PeLEDs.First,we highlight recent advances in the development of efficient PeLEDs.Second,we discuss various patterning techniques of MPHs(i.e.,photolithography,inkjet printing,electron beam lithography and laserassisted lithography,electrohydrodynamic jet printing,thermal evaporation,and transfer printing)for fabrication of RGB pixelated displays.These patterning techniques can be classified into two distinct approaches:in situ crystallization patterning using perovskite precursors and patterning of colloidal perovskite nanocrystals.This review highlights advancements and limitations in patterning techniques for PeLEDs,paving the way for integrating PeLEDs into full-color panels.

Programmable directional color dynamics using plasmonics

Adaptive multicolor filters have emerged as key components for ensuring color accuracy and resolution in outdoor visual devices.However,the current state of this technology is still in its infancy and largely reliant on liquid crystal devices that require high voltage and bulky structural designs.Here,we present a multicolor nanofilter consisting of multilayered‘active’plasmonic nanocomposites,wherein metallic nanoparticles are embedded within a conductive polymer nanofilm.These nanocomposites are fabricated with a total thickness below 100 nm using a‘lithography-free’method at the wafer level,and they inherently exhibit three prominent optical modes,accompanying scattering phenomena that produce distinct dichroic reflection and transmission colors.Here,a pivotal achievement is that all these colors are electrically manipulated with an applied external voltage of less than 1 V with 3.5 s of switching speed,encompassing the entire visible spectrum.Furthermore,this electrically programmable multicolor function enables the effective and dynamic modulation of the color temperature of white light across the warm-to-cool spectrum(3250 K-6250 K).This transformative capability is exceptionally valuable for enhancing the performance of outdoor optical devices that are independent of factors such as the sun’s elevation and prevailing weather conditions.

Dragonfly Interaction Algorithm for Optimization of Queuing Delay in Industrial Wireless Networks

In industrial wireless networks,data transmitted from source to destination are highly repetitive.This often leads to the queuing of the data,and poor management of the queued data results in excessive delays,increased energy consumption,and packet loss.Therefore,a nature-inspired-based Dragonfly Interaction Optimization Algorithm(DMOA)is proposed for optimization of the queue delay in industrial wireless networks.The term“interaction”herein used is the characterization of the“flying movement”of the dragonfly towards damselflies(female dragonflies)for mating.As a result,interaction is represented as the flow of transmitted data packets,or traffic,from the source to the base station.This includes each and every feature of dragonfly movement as well as awareness of the rival dragonflies,predators,and damselflies for the desired optimization of the queue delay.These features are juxtaposed as noise and interference,which are further used in the calculation of industrial wireless metrics:latency,error rate(reliability),throughput,energy efficiency,and fairness for the optimization of the queue delay.Statistical analysis,convergence analysis,the Wilcoxon test,the Friedman test,and the classical as well as the 2014 IEEE Congress of Evolutionary Computation(CEC)on the benchmark functions are also used for the evaluation of DMOA in terms of its robustness and efficiency.The results demonstrate the robustness of the proposed algorithm for both classical and benchmarking functions of the IEEE CEC 2014.Furthermore,the accuracy and efficacy of DMOA were demonstrated by means of the convergence rate,Wilcoxon testing,and ANOVA.Moreover,fairness using Jain’s index in queue delay optimization in terms of throughput and latency,along with computational complexity,is also evaluated and compared with other algorithms.Simulation results show that DMOA exceeds other bio-inspired optimization algorithms in terms of fairness in queue delay management and average packet loss.The proposed algorithm is also evaluated for the conflicting o
