Prenatal glucocorticoids(GCs) have been used to induce maturation of preterm fetal lungs and prevent the development of respiratory distress syndrome of the premature. Pulmonary surfactant induction has been regarded as the most important effect of prenatal GCs. However,report about the prolonged effects of prenatal GCs on the development of rat lung is of limited. In this study, we tried to investigate the acute and chronic modulation effects of prenatal dexamethasone(DEX) to asymmetric dimethylarginine(ADMA)/nitric oxide(NO) signal pathway of lung tissue. Pregnant Sprague Dawley rats at gestational day 14–20 were administered i.p. DEX(0.1 mg kg-1d-1).Acute programming effects of prenatal DEX were assessed at postnatal day 7, and long-term programming effects of offspring were assessed at day 120. We found that repetitive prenatal DEX exposure contributes to DNA oxidative damage and alveolar tissue dysplasia. Prenatal DEXtreatment decreased ADMA and increased i NOS expression. Prenatal DEX treatment also increased TNF-a transcript expression and decreased HDAC2 protein expression at acute stage. In conclusion, repetitive prenatal DEX has prolonged stress damage effects on lung tissue.

A Novel Urine Method for the Diagnosis of Active Tuberculosis by Immunoassay for the Detection of ESAT-6 Using Hydrogel Nanoparticles in HIV Patients

Background: In patients with HIV, conventional tests are of low sensitivity;therefore, a new diagnostic test with hydrogel nanoparticles with reactive blue dye is proposed, which allows the capture, conservation and concentration of ESAT-6 in urine. NIPAs are copolymers that capture low molecular weight proteins and protect against enzymatic degradation. Using an immunoassay, it is possible to detect and quantify ESAT-6 present in urine as a possible marker of active TB. Design/Methods: Study in Lima, Peru, HIV+ participants, ≥18 years with and without tuberculosis (TB). Smear microscopy, culture in solid medium and urine immunoassay were performed. The reference was the diagnosis of TB by radiological or clinical microbiological criteria (indication for TB treatment). There were 2 preanalytical processes: untreated and treated urine (centrifuged, heated), then incubation with NIPAm. After washing, elution, sonication, heat and centrifugation, the final eluate was obtained. This was placed on nitrocellulose membranes, which by means of fixation and incubation processes with anti-ESAT-6 and anti-IgG antibodies, revelation with C-DiGit® Blot Scanner and FluorChem R FR0001. Calibration curves were included in the membranes, density was measured using Image J software. ROC curves, sensitivity and specificity were obtained. Are gap junction gene connexins 26,32 and 43 of prognostic values in hepatocellular carcinoma?A prospective study

AIM: To investigate the prognostic value of the expression of connexin (Cx) 26, 32 and 43 messenger RNA (mRNA) in hepatocellular carcinoma (HCC) tissues. METHODS: Using a reverse-transcriptase polymerase chain reaction (RT-PCR), Cx 26, Cx 32 and Cx 43 mRNAs were determined in the liver tissues of 15 controls and in HCC tissues of 25 patients undergoing curative hepatic resection. The patients were followed up ***: Cx 26 and Cx 32 mRNAs were significantly lower in HCC tissues compared with controls (both P<0.01). By multivariate analysis, a lower level of Cx 26 and Cx 32 mRNA correlated significantly with a risk of HCC recurrence (P = 0.033) and recurrence-related mortality (P = 0.031,P= 0.031). Cx 43 mRNA was higher in HCC tissues compared with controls but did not correlate with postoperative recurrence or recurrence-related mortality. Other significant predictors of HCC recurrence included cellular dedifferentiation (P = 0.033), less encapsulation (P = 0.050), vascular permeation (P= 0.046), and daughter nodules (P= 0.046). Significant variables related to recurrence-related mortality consisted of cell dedifferentiation (P = 0.031), vascular permeation (P = 0.048), and daughter nodules (P = 0.048).The levels of Cx 26 and Cx 32 mRNAs correlated significantly with cell differentiation (P= 0.031). CONCLUSION: A low expression of Cx 26 and Cx 32 mRNAs in HCC tissues is predictive of postoperative recurrence of HCCs.

Topological and evolutional relationships between HCV core protein and hepatic lipid vesicles: Studies in vitro and in conditionally transgenic mice

AIM: To investigate a specific association between hepatic steatosis and hepatitis C virus (HCV) core. METHODS: HeLa cells and primary mouse hepatocytes were transfected with HCV core plasmid, and conditional transgenics in which hepatic over-expression of HCV core is regulated by the tetracycline-off system, were developed. The expression of the HCV core was assessed over one to six months after withdrawal of doxycycline (dox) by immunohistochemistry (IHC) and Western blotting and by sequential liver biopsy. Hepatic steatosis was evaluated using oil red stain. 8-hydroxydeoxyguanosine (8-OHdG) stains and caspase levels were conducted to clarify hepatic oxidative stress and apoptosis rate. Serum aminotransferase was checked. RESULTS: The transfected hepatocytes had globular cores under the lipid vesicles. In transgenic mice on control diet, core expression was robust, localized to the cytoplasmic vesicle membrane and strongly associatedwith microvesicular steatosis, which was gradually replaced by macrovesicular steatosis. However, both steatosis and core positive hepatocytes diminished with time. Increases in aminotransferase, caspase and 8-OHdG were associated with peak core expression. CONCLUSION: The core protein was readily detected and morphologically associated with steatosis in individual hepatocytes both in vitro and in vivo. In vivo, oxidative stress caused by the core potentially reduced the number of core positive hepatocytes and in parallel the level of steatosis. To our knowledge, this is the f irst animal model that directly shows topological relationship between HCV core and hepatic lipid vesicles.

Risks of rapid decline renal function in patients with type 2 diabetes

Progressive rising population of diabetes and related nephropathy, namely, diabetic kidney disease and associated end stage renal disease has become a major global public health issue. Results of observational studies indicate that most diabetic kidney disease progresses over decades; however, certain diabetes patients display a rapid decline in renal function, which may lead to renal failure within months. Although the definition of rapid renal function decline remained speculative, in general,it is defined by the decrease of estimated glomerular filtration rate(e GFR) in absolute rate of loss or percent change. Based on the Kidney Disease: Improving Global Outcomes 2012 clinical practice guidelines, a rapid decline in renal function is defined as a sustained declinein e GFR of > 5 m L/min per 1.73 m2 per year. It has been reported that potential factors contributing to a rapid decline in renal function include ethnic/genetic and demographic causes, smoking habits, increased glycated hemoglobin levels, obesity, albuminuria, anemia, low serum magnesium levels, high serum phosphate levels, vitamin D deficiency, elevated systolic blood pressure, pulse pressure, brachial-ankle pulse wave velocity values, retinopathy, and cardiac autonomic neuropathy. This article reviews current literatures in this area and provides insight on the early detection of diabetic subjects who are at risk of a rapid decline in renal function in order to develop a more aggressive approach to renal and cardiovascular protection.

在英文阅读中,文本通常是从左到右水平排列,并且对这种水平排列文本阅读的眼动控制已经有较好的了解。但是,在现实生活中英文文本有时候会以垂直排列的方式呈现。已有的研究表明,垂直呈现条件下文本的阅读速度要显著慢于水平呈现条件,但是这种阅读速度的降低到底是由于词汇识别过程受损还是眼跳目标选择所致目前却还不清楚。因此,本研究通过两个实验,分别控制句子中目标词的词长(长词:10字母词;短词:4字母词)和词频(高频词和低频词),考查不同文本阅读方向对英文阅读眼动控制的影响。实验过程中,要求读者阅读四种呈现方式的文本:正?K匠氏帧⑺呈闭胄呈现、逆时针旋转90°呈现以及每个字母垂直排列呈现,并记录他们阅读中的眼动。与前人的研究结果一致,本研究发现垂直呈现条件下文本的阅读速度慢于水平呈现条件。另外,在所有呈现方式上都发现了稳定的词长效应(对短词的加工快于对长词的加工)以及词频效应(对高频词的加工快于对低频词的加工),并且垂直呈现条件下的词长效应以及词频效应都显著大于水平阅读条件。表明,垂直呈现条件下文本的词汇识别过程受损。而对注视位置的分析发现,文本呈现方向并不影响对目标词的首次注视位置。表明,垂直文本阅读中的眼跳目标选择并为受损。本研究对阅读的眼动控制模型的完善有重要意义。

Purpose: To evaluate the diagnostic power of a novel digital stereoscopic imaging system in the diagnosis of glaucomatous optic neuropathy. Design: Prospective cross-sectional analysis of the diagnostic accuracy of digital stereoscopic optic disc analysis in the diagnosis of glaucomatous optic neuropathy exhibiting mild to moderate field loss. Participants: Fifty-two patients with open-angle glaucoma and 54 normal individuals were recruited. The presence of a reproducible visual field loss characteristic of glaucoma was used as the reference standard for the presence of glaucoma independent of the optic nerve head appearance. Patients were excluded if the optic disc, fundus, or visual field indicated other disease. One eye from each patient and individual was included in the study, the eye with the least field loss and a randomly designated normal eye, respectively. Methods: Simultaneous stereoscopic optic disc photography was performed on each specified eye. Three experienced observers viewed the resultant stereoscopic image of each nerve head using a Z screen, recorded a subjective clinical diagnosis, and undertook digital stereoscopic planimetry. Separate linear regression analysis was performed, post hoc, from the planimetric results for each observer of the logarithm of neuroretinal rim (NRR) against optic disc area derived from each normal eye. Eyes with NRR areas below the 95th prediction interval of the normal cohort were then classified as glaucomatous. Main Outcome Measures: Sensitivity and specificity for the detection of glaucomatous optic neuropathy. Results: With subjective stereoscopic analysis, sensitivity for glaucoma detection among the 3 observers was 80.8%, 76.9%, and 90.4%, with respective specificities of 94.4%, 79.6%, and 79.6%. Regression analysis of the NRR in 30°segments gave sensitivities between 69.2%and 80.8%and specificities between 83.3%and 90.7%.
