AIM: To evaluate the biopathologic features and clinical significance of nodal micrometastasis(MI) in early gastric cancer(EGC).METHODS: Among 1022 EGC patients who underwent gastrectomy with lymphadenectomy of D1 + β or more from March 2001 to December 2005 at the Korean National Cancer Center, available nodal metastasis was found in 90 p T1N1 patients. Nodal metastasis was confirmed by immunohistochemistry(IHC) with cytokeratin and patients were classified into MI and macrometastasis(MA) groups based on the main tumor burden according to the 6th International Union Against Cancer/American Joint Committee on Cancer staging system; the main tumor burden with a diameter of greater than 0.2 mm but no greater than 2 mm as MI, and greater than 2 mm as MA of the representative metastatic node. Proliferative and apoptotic activities of the primary tumor and the nodal metastasis were measured by IHC with Ki-67 and terminal deoxynucleotidyl transferase d UTP nick end labeling, respectively. Biopathologic and clinical features of the patients were analyzed and compared between MI and MA groups. Patients with recurrence were compared with those without recurrence to identify risk factors for ***: Thirty-seven patients showed MI and the other 53 patients revealed MA in the lymph node; the incidence of patients with MI and MA was 41.1% and 58.9%. The main tumor burden was 0.9 and 4.6 mm in the representative metastatic node, respectively. Japanese N2 stations were more frequently involved in MA group(20.9%) than in MI group(10.3%) butthe difference was not statistically different(P = 0.338). Proliferative and apoptotic activities of MI were decreased than those of MA(26.7% vs 40.5%, P = 0.004 and 1.0% vs 3.0%, P < 0.001, respectively). However, nodal MI in the current study showed a relatively high proliferative activity and an equivalent apoptotic activity compared to other cancers in the previously published studies. Background： Glehnia littoralis has been used for traditional Asian medicine, which has diverse therapeutic activities. However, studies regarding neurogenic effects of G. littoralis have not yet been considered. Therefore, in this study, we examined effects of G. littoralis extract on cell proliferation, neuroblast differentiation, and the maturation of newborn neurons in the hippocampus of adult mice. Methods： A total of 39 male ICR mice （12 weeks old） were randomly assigned to vehicle-treated and 100 and 200 mg/kg G. littoralis extract-treated groups （n = 13 in each group）. Vehicle and G. littoralis extract were orally administrated for 28 days. To examine neurogenic effects ofG. litmralis extract, we performed immunohistochemistry tbr 5-bromo-2-deoxyuridine （BrdU, an indicator for cell proliferation） and doublecortin （DCX, an immature neuronal marker） and double immunofluorescence staining for BrdU and neuronal nuclear antigen （NeuN, a mature neuronal marker）. In addition, we examined expressional changes of brain-derived neurotrophic factor （BDNF） and its major receptor tropomyosin-related kinase B （TrkB） using Western blotting analysis. Results： Treatment with 200 mg/kg, not 100 mg/kg, significantly increased number of BrdU-immunoreactive （＋） and DCX＋ cells （48.0 ±3.1and 72.0 ± 3.8 cells/section, respectively） in the subgranular zone （SGZ） of the dentate gyrus （DG） and BrdU＊/NeuN＋ cells （17.0 ±1.5 cells/section） in the granule cell layer as well as in the SGZ. In addition, protein levels of BDNF and YrkB （about 232% and 244% of the vehicle-treated group, respectively） were significantly increased in the DG of the mice treated with 200 mg/kg ofG. littoralis extract. Conclusion： G. littoralis extract promots cell proliferation, neuroblast differentiation, and neuronal maturation in the hippocampal DG, and neurogenic effects might be closely related to increases ofBDN F and TrkB proteins by G. littoralis extract treatment.

Gastrointestinal stromal tumors(GISTs) are mesenchymal tumors of the gastrointestinal tract that are most commonly found in the stomach. Although GISTs can spread to the liver and peritoneum, metastasis to the skeletal muscle is very rare and only four cases have previously been reported. These cases involved concurrent skeletal metastases of primary GISTs or liver metastases. Here, we report the first case of a distant recurrence in the brachialis muscle after complete remission of an extra-luminal gastric GIST following a wedge resection of the stomach, omental excision, and adjuvant imatinib therapy for one year. Ten months after therapy completion, the patient presented with swelling and tenderness in the left arm. Magnetic resonance imaging revealed a large mass in the brachialis muscle, which showed positivity for c-kit and CD34 upon pathologic examination. This is the first reported case of a solitary distant recurrence of a GIST in the muscle after complete remission had been achieved.

AIM: To evaluate the long-term efficacy adefovir(ADV)-based combination therapies in entecavir(ETV)-resistant chronic hepatitis B(CHB) patients. METHODS: F i fty CHB pat ient s wi t h genotypic resistance to ETV at 13 medical centers in South Korea were included for the analysis. All the patients received rescue therapy with the combination of ADV plus ETV(ADV/ETV,n = 23) or ADV plus lamivudine(LMV)(ADV/LMV,n = 27) for more than 12 mo. Patients were monitored at least every 3-4 mo during ADV-based combination therapy by clinical examination as well as biochemical and virological assessments. Hepatitis B virus(HBV) DNA levels were measured by realtime PCR and logarithmically transformed for analysis. Cumulative rates of virologic response(VR; HBV DNA < 20 IU/m L) were calculated using the Kaplan-Meier method,and the difference was determined by a logrank test. Multivariate logistic regression and Cox proportional hazards models were used to identify independent risk factors significantly associated with short-term and long-term VR,***: Baseline median HBV DNA levels were 5.53(2.81-7.63) log10 IU/m L. The most commonly observed ETV genotypic mutation sites were rt184 and rt202. Patients were treated for a median of 27(12-45) mo. Overall,cumulative VR rates at 6,12,24,and 36 mo were 26%,36%,45%,and 68%,respectively. Patients treated with the ADV/ETV combination showed higher cumulative VR rates(35%,43%,65%,and 76%,respectively) than those with the ADV/LAM combination(18%,30%,30%,and 62%,respectively; P = 0.048). In the multivariate analysis,low baseline HBV DNA levels(< 5.2 log10 IU/m L) and initial virologic response at 3 mo(IVR-3; HBV DNA < 3.3 log10 IU/m L after 3 mo) were independent predictive factors for VR. Patients with favorable predictors achieved cumulative VR rates up to 90% at 36 mo. 目的:本研究旨在证实朝鲜白头翁、人参和甘草的复合水提物(water extract ofPulsatillakoreana(Yabe ex Nakai)Nakai ex ***.,*** andGlycyrrhizauralensisFisch,WEPPG)的抗血管生成作用。方法:使用纤维母细胞生长因子致血管生成的人类脐静脉内皮细胞模型衡量细胞的增殖、黏附及迁移,同时进行细管形成实验及纤维母细胞生长因子致鸡胚绒毛尿囊膜血管生成实验检测WEPPG的抗血管生成作用。结果:WEPPG能够显著抑制纤维母细胞生长因子所致血管生成的人类脐静脉内皮细胞的增殖、黏附及迁移。信号蛋白分析显示多种蛋白表达变化,如细胞周期素A、p63、KIP2的上调及nibrin蛋白和黏着斑激酶的下调。与对照组相比,WEPPG显著减少了鸡胚绒毛尿囊膜血管生成。结论:本研究的结果证实了WEPPG的抗血管生成作用,这可能是这种药物具有抗癌功效的原因之一。

目的 (18F-氟戊基)三苯基季磷盐(18F-FPTP)是一种新的有应用前景的心肌PET显像剂,其在心肌细胞内积聚量多且肝脏清除快。该研究小组在小鼠体内做了18F-FPTP PET显像的代谢区室分析和两项线性分析,即用于生成参数图的线性最小二乘法（LLS）和基函数法（BFM）分析。此外,还研究了动态分析的最小动态显像时间,并进行了计算机模拟。方法对心肌梗死（MI）小鼠模型（n=12）进行18F-FPTP动态PET显像（18 min）和CT显像。在左心室、正常心肌和梗死区勾画感兴趣区（ROI）。比较两室模型（速率常数k1和k2;2室2参数）和不可逆摄取的三室模型（速率常数k1-k3;3室3参数）的拟合优度,并进行部分容积和溢出校正（Vα和α=1-Va）。LLS和BFM用于基于ROI和基于体素的动力学参数估算,估算结果与相应代谢区室模型标准的基于ROI的非线性最小二乘法（NLS）结果进行比较。通过计算机模拟研究估算方法的统计学特点。结果 2室2参数模型最适于描述18F-FPTP的动力学表现。正常心肌的平均k1、k2和Va值分别是6.8ml·min-1·g-1、1.1/min和0.44,而梗死组织的分别是1.4 ml·min-1·g-1、1.1/min和0.32。动态显像10 min的数据即足够用于估算各项参数。LLS和BFM的估算结果与ROI水平的NLS的结果有很好的相关性（k1:1.22x-0.30,r2=0.90;y=1.26x+0.00,r2=0.92）。动态参数图形（αk1,k1,k2,Va）的局部分布具有生理相关性。在计算机模拟中,LLS和BFM比NLS的特征性更强。结论快速的动力学特征和心肌较高的特异性摄取使18F-FPTP能在显像开始的最初10 min动态显像内,利用2室2参数模型进行定量分析。LLS和BFM能在体素水平进行参数估算,这2种方法可用于定量评价18F-FPTP在心肌中的分布,进而研究其在不同条件、不同疾病模型和病种中进行心肌显像的表现。

Application of the Prague C and M criteria for endoscopic description of columnar-lined esophagus in South Korea

AIM:To ascertain whether the Prague circumferential(C) length and maximal(M) length criteria for grading the extent of Barrett's esophagus can be applied prior to its widespread application in South ***:Two hundred and thirteen consecutive cases with endoscopic columnar-lined esophagus(CLE) were included and classified according to the Prague C and M ***:Of 213 cases with CLE, the distribution of maximum CLE lengths was: 0.5-0.9 cm in 99 cases(46.5%); 1.0-1.4 cm in 63 cases(29.6%); 1.5-1.9 cm in 15 cases(7.0%); 2.0-2.4 cm in 14 cases(6.6%); 2.5-2.9 cm in 1 case(0.5%); and 7.0 cm in 1 case(0.5%). Twenty cases(9.4%) had columnar islands alone. Two hundred and eight cases(97.7%) lacked the circumferential CLE component(C0Mx). Columnar islands were found in 70 cases(32.9%), of which 20 cases(9.4%) had columnar islands ***:In regions where most CLE patients display short or ultrashort tongue-like appearance, more detailed descriptions of CLE's in < 1.0 cm lengths and columnar islands, as well as avoidance of repeating the prefix "C0" need to be considered in parallel with the widespread application of the Prague system in South Korea.
