Thoracotomy of an asymptomatic, functional, posterior mediastinal paraganglioma: A case report

BACKGROUND Paragangliomas in the mediastinum are rare, accounting for only 1%-2% of all paragangliomas and < 0.3% of all mediastinal tumors. Most paragangliomas are nonfunctional, therefore, asymptomatic functional paragangliomas in the left posterior mediastinum are extremely rare. Perioperative management including preoperative preparation, careful intraoperative procedures, and strict postoperative care is important, and one-stage surgical resection should be performed only after appropriate perioperative measures are undertaken. Because those tumors are rare, it is necessary to report known cases to raise awareness regarding them. CASE SUMMARY We report the case of a 47-year-old male who was admitted to our hospital with the chief complaints of intermittent tearing pain on the left side of the chest and back for more than 10 mo. A chest contrast-enhanced computed tomography scan revealed a round, solid mass in the left posterior mediastinum, with low-density cystic lesions in the middle, and no enlarged lymph nodes in the hilum or mediastinum (Figure 1). After the diagnosis of paraganglioma, the patient was preoperatively given an oral adrenoceptor blocking drug (phenoxybenzamine), and intravenous fluid resuscitation for two weeks, subsequently the patient underwent a one-stage resection of lesions via left thoracotomy. The patient’s blood pressure increased to 220/120 mmHg when the tumor was touched, which could be relieved by symptomatic treatment such as accelerating liquid transfusion or other intervention to lower blood pressure. The patient recovered uneventfully after surgery, with no abnormal blood pressure or recurrence during one year of follow-up *** Surgical resection is the preferred treatment for asymptomatic functional paragangliomas.

Chronic liver injury with any etiology can progress to fibrosis and the end-stage diseases cirrhosis and hepatocellular *** progression of liver disease is controlled by a variety of factors,including liver injury,inflammatory cells,inflammatory mediators,cytokines,and the gut *** the current review,we discuss recent data on a large number of cytokines that play important roles in regulating liver injury,inflammation,fibrosis,and regeneration,with a focus on interferons and T helper(Th)1,Th2,Th9,Th17,interleukin(IL)-1 family,IL-6 family,and IL-20 family *** can also produce certain cytokines(such as IL-7,IL-11;and IL-33),and the functions of these cytokines in the liver are briefly *** cytokines have great therapeutic potential,and some are currently being tested as therapeutic targets in clinical trials for the treatment of liver diseases,which are also described.

Kupffer cells(KCs),which are liver-resident macrophages,originate from the fetal yolk sac and represent one of the largest macrophage populations in the ***,the current data on the origin of the cells that restore macrophages during liver injury and regeneration remain ***,we address the question of whether liver macrophage restoration results from circulating monocyte infiltration or local KC proliferation in regenerating livers after partial hepatectomy(PHx)and uncover the underlying *** using several strains of genetically modified mice and performing immunohistochemical analyses,we demonstrated that local KC proliferation mainly contributed to the restoration of liver macrophages after *** KC proliferation was impaired in Il6-knockout(KO)mice and restored after the administration of IL-6 protein,whereas KC proliferation was not affected in Il4-KO or Csf2-KO *** source of IL-6 was identified using hepatocyte-and myeloid-specific Il6-KO mice and the results revealed that both hepatocytes and myeloid cells contribute to IL-6 production after ***,peak KC proliferation was also impaired in myeloid-specific Il6 receptor-KO mice after PHx,suggesting that IL-6 signaling directly promotes KC *** using several inhibitors to block the IL-6 signaling pathway revealed that sirtuin 1(SIRT1)contributed to IL-6-mediated KC proliferation in *** deletion of the Sirt1 gene in myeloid cells,including KCs,impaired KC proliferation after *** conclusion,our data suggest that KC repopulation after PHx is mainly driven by local KC proliferation,which is dependent on IL-6 and SIRT1 activation in KCs.

The perfect combination of renewable energy and desalination technologies is the key to meeting water demands in a cost-effective,efficient and environmentally friendly *** desalination technique by humidificationdehumidification is non-conventional approach suitable for areas with low infrastructure(such as rural and decentralized regions)since it does not require permanent *** this study,this technology is implemented by using solar energy as a source of thermal power.A seawater desalination unit is considered,which consists of a chamber with two evaporators(humidifiers),a wetted porous material made of a corrugated cellulose cardboard and a condenser(dehumidifier).The evaporation system is tested with dry bulb temperature and relative air humidity *** results of numerical simulations indicate that higher inlet air velocities(from 0.75 to 3 m/s)lead to a decrease in theΔT,ΔRH,and *** the air remaining within the evaporator for 30 cm,the temperature differential increases to 5.7°C,accompanied by a 39%rise in relative humidity *** changes result in a significant enhancement in humidification efficiency,achieving a remarkable efficiency level of 78%.However,a wettability value of 630 m^(2)/m^(3)leads to a smaller reduction of these *** the pad thickness,particularly to 0.3 m,improves performance by boostingΔT,ΔRH,and effectiveness,especially for pads with a wettability of 630 m^(2)/m^(3),for which superior performances are predicted by the numerical tests.

The integration of remote sensing and geographic information system(GIS)was employed in this study to delineate the structural lineaments within the eastern section of the Ouarzazate Basin,situated between the southern front of the Central High Atlas and the northern slopes of the Eastern Anti-Atlas(also known as the Saghro Massif).To achieve this objective,Landsat 8 Operational Land Imager(OLI)and Shuttle Radar Topography Mission(SRTM)data were *** Component Analysis(PCA)was computed and a directional filter was applied to the first PCA and the panchromatic band(B8).Additionally,shading was applied to the SRTM data in four directions;N0°,N45°,N90°,N135°.After removing of the non-geological linear structures,the results obtained,using the automatic extraction method,allowed us to produce a synthetic map that included 1251 lineaments with an average length of 1331 m and was dominated by NE-SW,ENE-WSW and E-W directions,***,the high lineament density is clearly noted in the Anti-Atlas(Saghro Massif)and at the level of the northern part,extending from the ait Ibrirne to Arg-Ali Oubourk *** lineament density can always be found around the major faults affecting this *** data collected during the field investigations and from geological maps show that the major direction of the faults and structural accidents range mostly between N45°,N70°and N75°.The correlation of remote sensing results with those collected in the field shows a similarity and coincidence with each *** these results,it is possible to consider the automatic extraction method as a supplementary kind that can serve classical geology by quickly enriching it with additional *** shown in this work,this method provides more information when applied in arid areas where the fields are well outcropped.

Nuclear facility aging is one of the biggest problems encountered in nuclear engineering. Radiation damage is among one of the aging causes. This kind of damage is an important factor of mechanical properties deterioration. The interest of this study is on the Es-Salam research reactor aluminum vessel aging due to neutron radiation. Monte Carlo(MC) simulations were performed by MCNP6 and SRIM codes to estimate the defects created by neutrons in the vessel. MC simulations by MCNP6 have been performed to determine the distribution of neutron fluence and primary knock-on atom(PKA) creation. Considering our boundary conditions of the calculations, the helium and hydrogen gas production in the model at a normalized total neutron flux of 6.62×10^(12) n/cm^2 s were determined to be 2.86 × 10~8 and 1.33 × 10~9 atoms/cm^3 s,respectively. The SRIM code was used for the simulation of defects creation(vacancies, voids) in the aluminum alloy of the Es-Salam vessel(EsAl) by helium and hydrogen with an approximate energy of 11 MeV *** coupling between the two codes is based upon postprocessing of the particle track(PTRAC) output file generated by the MCNP6. A small program based on the Mat Lab language is performed to condition the output file MCNP6 in the format of a SRIM input file. The concentration of silicon was determined for the vessel by the calculation of the total rate of ^(27)Al(n,γ)^(28)Si reaction. The DPA(displacement per atom) was calculated in SRIM according to R.E. Stoller recommendations; the calculated value is 0.02 at a fast neutron fluence 1.89 × 10^(19) n/cm^***-MRx standard for 6061-T6 aluminum was used for the simulation of the evolution of mechanical properties for high fluence. The calculated values of nuclear parameters and DPA obtained were in agreement with the experimental results from the Oak Ridge High Flux Isotope Reactor(HFIR) reported by Farrell and coworkers.

From a mixture of α-,β-and y-himachalenes extracted from waste wood of Atlas cedar(Cedrus atlantica),cadalene(1,6-dimethyl-4-isopropylnaphthalene)and iso-cadalene(1,6-dimethyl-3-isopropylnaphthalene)were produced in two steps with up to 71%±5%yield through the ar-himachalene intermediate using I2 and/or AlCl3 as *** selectivity is shown to sharply depend on the operating conditions:while I2/AlCl3 in dichloromethane promotes the formation of cadalene,the formation of iso-cadalene is favored in the presence of AlCl3 in *** bicyclic aromatic compounds were thus obtained through unique rearrangements involving sequential C—C bond cleavage/fo rmation and hydride transfer *** the absence of AlCl3 or I2,dihydrocurcumene was also found to be formed with up to 70%selectivity.A tentative mechanism is proposed and discussed.

Odour Impact Assessment by Means of Dispersion Modeling, Dynamic Olfactometry and Mobile Electronic Nose around Agadir Fishing Port in Morocco

Odorous emissions emitted from various sources including industrial and commercial activities have particular concerns about human health. These malodors emissions are an environmental concern that affects health status and social life of the neighbors. That requires the local authority to set up a management strategy to control this nuisance. The evaluation of odour emissions from fishing port is complex because these emissions depend on several factors such as multiple sources of odor emissions, meteorological conditions, topography and others. That imposes the use of complementary approaches to monitor odours. In this paper, the case of Agadir fishing port is studied, which is adjacent to the tourist area and residential neighborhoods and which hosts a number of points that can generate odors. To assess this odour impact, three methods are used such as dynamic olfactometry, dispersion modeling and mobile electronic nose (e-nose). The use of these three methods in a complementary manner to assess odour impacts around a fishing port allowed both the quantification of the emissions using dynamic olfactometry and the evaluation of their impact on the study area with model dispersion. The results enabled also to identify the most affected areas of the city by odor emissions and to recognize the meteorological parameters maximizing odor impact. The other goal of this work is to compare the results of the odour dispersion modeling and e-nose measurements for one year in terms of frequency of overtaking the set alert thresholds over the same period. Comparison highlights the strengths and weaknesses of both approaches. Modeling can be used predicatively but it does not take into account fugitive emissions reliably in the absence of data on these emissions, modeling based on the hourly average misjudges the odor peaks, while e-nose made it possible to obtain validated data and provides accurate, affordable and real-time odour measurement capability tacking in to account t

Air pollution is a major global issue with widely known harmful effects on human health and the environment. This pollution is a very complex phenomenon given the diversity of pollutants that may be present in the atmosphere. The air quality in urban areas is of a great concern for residents living in cities and represents a current issue that requires an adequate management. So that air quality policy is driven by health concerns. In this paper, we present an overview on the experience of Agadir city to establish the air quality management policy, local authority on the whole have developed a good understanding of air quality in the area. Indeed for several years, efforts have been made to monitor the air quality in this city, this translated by air quality assessment since 2006 using mobile laboratory and fixed station. Our goals in this study were to review the operation of Local Air Quality Management (LAQM) making better use of available resources to improve its outcomes and make recommendations with a view to improving air quality issues. This work highlights the requirement to revise periodically the LAQM for generating priority for air quality issues within local authority and the need to implement the optimizing Air Quality Monitoring Network (AQMN).
