Influences of Donor and Recipient Gene Polymorphisms on Tacrolimus Dosing and Pharmacokinetics in Asian Liver Transplant Patients

Background: Tacrolimus pharmacokinetics has large inter-individual variability. Objectives: This study aimed to investigate the impact of donor and recipient gene polymorphisms on tacrolimus dosing and pharmacokinetics in Asian liver transplant patients. Methods: Steady-statetacrolimus concentrations at 0, 1, 2, 4 and 6 h were measured. Pharma- cokinetic parameters were estimated with a one-compartment linear model using WinNonlin 6.1 program. DNA from donor liver and recipient blood samples were genotyped for CYP3Aand ABCB1 polymorphisms. Results: A total of 13 donors and 17 recipients were included. Donor genotypes influenced tacrolimus trough concentration-to-dose (C0/D) ratio only at Week 1. Patients with liver grafts from CYP3A5 expressors (*1*1 and *1*3) achieved lower mean C0/D ratio than those with grafts from non-expressors (*3*3) (64.48 versus 129.21 (mcg/L)/(mg/kg/day), P = 0.040). Hence, the dose required to achieve target concentration in patients with donor genotype CYP3A5 expressors was higher than non-expressors (0.12 versus 0.08 mg/kg, P = 0.045). Recipient with ABCB1-C3435T genotype TT demonstrated higher apparent oral clearance of tacrolimus as compared to genotype CC (17.7 versus7.9 L/h, P = 0.033). Conclusions: Donor liver CYP3Apolymorphism could potentially affect tacrolimus C0/D ratio as early as the first week post liver transplant. Genotyping of liver donors may be useful to achieve the optimal drug concentration during this critical period.

An Improved Differential Evolution and Its Industrial Application

In this paper, an improved Differential Evolution (DE) that incorporates double wavelet-based operations is proposed to solve the Economic Load Dispatch (ELD) problem. The double wavelet mutations are applied in order to enhance DE in exploring the solution space more effectively for better solution quality and stability. The first stage of wavelet operation is embedded in the DE mutation operation, in which the scaling factor is governed by a wavelet function. In the second stage, a wavelet-based mutation operation is embedded in the DE crossover operation. The trial population vectors are modified by the wavelet function. A suite of benchmark test functions is employed to evaluate the performance of the proposed DE in different problems. The result shows empirically that the proposed method out-performs signifycantly the conventional methods in terms of convergence speed, solution quality and solution stability. Then the proposed method is applied to the Economic Load Dispatch with Valve-Point Loading (ELD-VPL) problem, which is a process to share the power demand among the online generators in a power system for minimum fuel cost. Two different conditions of the ELD problem have been tested in this paper. It is observed that the proposed method gives satisfactory optimal costs when compared with the other techniques in the literature.

Background:To compare the treatment effectiveness and safety among transarterial infusion chemotherapy(TAI)with FOLFOX regimen,transarterial chemoembolization(TACE),and sorafenib in patients with BCLC stage C hepatocellular carcinoma(HCC).Methods:The data of consecutive patients with BCLC stage C HCC treated with TAI,TACE,or sorafenib from January 2015 to December 2018 at three centers were retrospectively ***-score matched(PSM)analysis was pairwise performed to reduce selection *** effectiveness and safety were evaluated and compared using the Kaplan-Meier method,log-rank test,Cox regression models,andχ2 ***:The median overall survival(OS)in the matched TAI cohort was significantly longer than the sorafenib cohort(19.6 vs.7.5 months,P=0.009),and the TACE cohort(estimated 27.8 vs.6.6 months,P<0.001).The difference in median progression-free survival(PFS)between the matched TAI and sorafenib cohorts was not significant(5.8 vs.2.3 months,P=0.219).The median PFS in the matched TAI cohort was significantly longer than the TACE cohort(6.5 vs.2.8 months,P<0.001).The objective response rate(ORR)in the matched TAI cohort was significantly higher than the sorafenib cohort(36.4%vs.0.0%,P<0.001)and the TACE cohort(48.7%vs.4.7%,P<0.001).The incidences of adverse events(AEs)were similar among these three ***:TAI with FOLFOX regimen was an effective and safe therapy that improved survival of patients with BCLC stage C HCC.

目的本文介绍组织列阵检测技术(tissue microarray technology,TMA)制作组织切片的程序,阐述TMA检测技术的操作经验和改进方法。方法收集147例乳腺癌标本(香港玛丽医院提供),采用TMA技术制备石蜡切片。详述制作TMA石蜡切片的过程并分析和解决制作过程中出现的各种问题。结果 TMA技术可以将数百个组织整齐排列在同一个受体蜡块上,使多种肿瘤基因在一张TMA切片上同时进行检测。乳腺癌组织阵列整齐,微组织块无明显脱失,有小部分折叠,组织形态基本保存完好。结论通过防止和改进在制作过程中出现的问题,可成功制备TMA蜡块,为后期基因表达及功能的检测提供了技术保障。

Sliding inguinal scrotal herniation of the bladder is rare but well-documented in the medical literature. The majority of the patients are asymptomatic and the hernia is usually diagnosed incidentally. Most authors had advocated surgically repair of the hernia with the reduction or resection of the bladder as the definitive management. We present an unusual case of inguinal scrotal herniation of the bladder presented as acute abdominal pain and treated with conservatively with urethral catheterization.

A New Mixed Clustering-Based Method to Analyze the Gait of Children with Cerebral Palsy

Cerebral palsy is a group of persistent central movement and posturaldevelopmental disorders, and restricted activity syndromes. This syndrome iscaused by non-progressive brain damage to the developing fetus or infants. Cerebralpalsy assessment can determine whether the brain is behind or abnormal. If itexists, early intervention and rehabilitation can be carried out as soon as possibleto restore brain function to the greatest extent. The direct external manifestation ofcerebral palsy is abnormal gait. Accurately determining the muscle strengthrelatedreasons that cause this abnormal gait is the primary problem for *** this paper, clustering methods were used to compare and analyze the differencesbetween the abnormal and normal gait parameters of children with andwithout cerebral palsy. Since the collected data contains overlapping data thatmay be mutated, while the centroids are also different, the expected result is *** solve this problem, a mixed clustering method is proposed that combinesthe advantages of K-means and hierarchical clustering, meaning that eachset of data shows a similar trend to specific parameters. Experiment results showthat this method can detect cerebral palsy through the difference between theabnormal gait of children with cerebral palsy and that of normal children.
