Prevalence of Unsatisfactory Pap Smear and Associated Clinical History and Diagnosis in a Tertiary Teaching Hospital in Ghana

Background: A major limitation of cervical cytology is the unsuitability of proportion of smears submitted for analysis and for cytological assessment (unsatisfactory). This study examines the prevalence of unsatisfactory Pap smear, clinical history and diagnosis in the Department of Pathology, Korle-Bu Teaching Hospital (KBTH), Ghana. Materials and Methods: A retrospective review of 15,290 cases spanning 12 years (2005-2016) was carried out at the cytology unit of the Pathology Department of the KBTH. Out of the 15,290 Pap smear records retrieved, 2347 reports were excluded leaving 12,943 for the study. All unsatisfactory smear cases were analyzed and categorized using the Bethesda 2001 System. Results are presented using descriptive statistics. Results: The overall prevalence of unsatisfactory Pap smear was 402 (3.1%). Routine screening smear accounted for 115 (0.9%);reports without clinical history and diagnosis gave 21 (0.2%) and cases with clinical history and diagnosis were 287 (2.2%). The common cause of unsatisfactory Pap smear was scanty cellularity 222 (1.72%). Patient’s history accounted for the least cause of unsatisfactory Pap smear 2 (0.02%). Conclusion: Pap smear results reported as unsatisfactory could harbor cancer malignancy. Samples should be taken by well-trained persons.

Mouse double minute （Mdm） genes span an evolutionary timeframe from the ancient eukaryotic placozoa Trichoplox adhaerens to Homo sapiens, implying a significant and possibly conserved cellular role throughout history. Maintenance of DNA integrity and response to DNA damage involve many key regulatory pathways, including precise control over the tumour suppressor pro- tein p53. In most vertebrates, degradation of p53 through proteasomal targeting is primarily mediated by heterodimers of Mdm2 and the Mdm2-related protein Mdm4 （also known as MdmX）. Both Mdm2 and Mdm4 have p53-binding regions, acidic domains, zinc fingers, and C-terminal RING domains that are conserved throughout evolution. Vertebrates typically have both Mdm2 and Mdm# genes, while analyses of sequenced genomes of invertebrate species have identified single Mdm genes, suggesting that a duplication event occurred prior to emergence of jawless vertebrates about 550-440 million years ago. The functional relationship between Mdm and p53 in T. adhaerens, an organism that has existed for 1 billion years, implies that these two proteins have evolved together to maintain a conserved and regulated function.

A Novel Temperature-Compensated, Intensity-Modulated Fiber Bragg Grating Sensor System

An intensity-modulated, fiber Bragg grating （FBG） sensor system based on radio-frequency （RF） signal measurement is presented. The RF signal is generated at a photodetector by two modulated optical signals reflected from the sensing FBG and a reference one. Bragg wavelength shift of the sensing FBG changes intensity of the RF signal by changing phase difference between the two optical signals, with temperature effect being compensated automatically by the reference FBG, Strain measurement with a maximum sensitivity of -0.34 μV/με has been achieved.

Stability Analysis of Bubonic Plague Model with the Causing Pathogen Yersinia pestis in the Environment

Bubonic plague is a serious bacterial disease, mainly transmitted to human beings and rodents through flea bite. However, the disease may also be transmitted upon the interaction with the infected materials or surfaces in the environment. In this study, a deterministic model for bubonic plague disease with Yersinia pestis in the environment is developed and analyzed. Conditions for existence and stability of the equilibrium points are established. Using Jacobian method disease free equilibrium (DFE) point, E0 was proved to be locally asymptotically stable. The Metzler matrix method was used to prove that the DFE was globally asymptotically stable when R0 0 > 1. Numerical simulations are done to verify the analytical predictions. The results show that bubonic plague can effectively be controlled or even be eradicated if efforts are made to ensure that there are effective and timely control strategies.

传统医学非常重要,但它在人类健康和动物保健方面的作用常被人们低估。传统医学在兽医临床的应用有着悠久的历史,它经常用来促进维持机体健康,预防和治疗疾病,尤其用于治疗慢性疾病。在传统中药中,淫羊藿作为一种传统的草药,与其活性成分很久就被用于治疗药物,如心血管疾病治疗和促进发情等,但至今仍未有淫羊藿及其活性成分如何影响雌性小鼠生殖发育的报道。小鼠卵巢是产生卵母细胞的场所,每个卵泡内含有1个卵母细胞。卵泡生长的启动是卵泡发育的重要调控点。卵泡在出生前就已形成此时处于静止状态,原始卵泡阶段:原始卵泡进入生长卵泡池后,便会形成连续不断的卵泡波,直至达到排卵前期的格拉芙氏卵泡阶段。卵泡发育需经过初级卵泡、次级卵泡、腔前卵泡和有腔卵泡阶段。在发生受精时,只有经过充分竞争发育的排卵前卵泡卵母细胞才能支撑胚胎发育至终期。因此,本试验通过检测小鼠体重、卵母细胞体外成熟率(in vitro maturation,IVM)、生殖器官指数和卵巢、子宫组织学形态,评价淫羊藿(epimediu,EP)及淫羊藿多糖(Epimediumpolysaccharide,EPS)和淫羊藿香(icariin,ICA)对小鼠生长和生殖发育的影响。180只健康雌性昆明小鼠随机分为30组,每组6只,分别连续7 d和14 d灌服2、6、10和 14mg/0.1mLEP,2、4、6和8mg/0.1mLEPS,或 0.1、0.3、0.5 和 0.7 mg/0.1mLICA,每个对照组均在相同时间灌服0.1mL生理盐水。所有小鼠在清洁级条件下常规饲养,自由采食与饮水。连续灌服7d或14d后,致死小鼠,取生殖器官(子宫、输卵管和卵巢)并去除脂肪组织,称量生殖器官重计算生殖器官指数(reproductiveindex,RI);部分卵巢用于卵母细胞采集及体外成熟培养(IVM),其余卵巢用于组织形态学分析。本试验为了解淫羊藿及其活性成分如何影响小鼠卵母细胞生长发育提供了基础材料,并确定3种药物的最佳作用剂量。试验结果如下:试验一、淫羊藿对小鼠卵巢和子宫发育的影响生殖器管指数(RI)分析表明,连续7 d灌服10 mg/0.1 mL EP组和连续14 d灌服6 mg/0.1 mL组小鼠RI值显著高于其他EP处理组和对照组。此外,所有7 d和14 dEP处理组的RI值均显着高于对照组。EP处理组卵母细胞体外成熟培养后,pbI排出率均高于对照组(63.30%),但试验组间差异不显著(p<0.05)。7d和14dEP处理组中,6mg/0.1mL组的pbI排出率最高。卵巢组织学观察发现,各试验组间有腔卵泡的数量和大小差异不显著,但所有EP处理组卵巢组织内均有大量次级卵泡,说明EP可以促进卵泡发育。试验二、淫羊藿多糖对小鼠卵巢和子宫发育的作用连续7d和14 d给予不同剂量的EPS,对小鼠体重并无负面影响;EPS处理可增加小鼠体重。14dEP处理组中,4mg/0.1mL组RI显著高于其他EPS组与对照组。卵母细胞体外成熟培养,结果发现所有EPS处理组卵母细胞pbI排出率均高于对照组,但各试验组间差异不显著(p<0.05)。7dEPS处理组中,8mg/0.1 mL组pbI排出率最高,14 d EP处理组中,4 mg/0.1 mL组pbI排出率最高。卵巢组织学观察发现,EPS处理组卵巢组织内次级卵泡数量增加,说明EPS与EP一样,可以促进卵泡发育。试验三、淫羊藿苷对小鼠卵巢和子宫发育的影响ICA 7d和14 d各剂量组对小鼠体重也未发现有负面影响。正如EP和EPS试验一样,ICA处理可增加小鼠体重。在14 d处理组中,0.1、0.3、0.5和0.7 mg/0.1 mL ICA组RI值要显著高于对照组。此外,所有ICA 7 d和14 d处理组的RI值均显著高于对照组。卵母细胞体外成熟培养发现,所有EPS处理组卵母细胞pbI排出率均高于对照组,但试验组间差异不显著(p<0.05)。7 d EPS处理组中,8 mg/0.1 mL组pbI排出率最高;14 dEP处理组中,4 mg/0.1 mL组pbI排出率最高。卵巢组织学观察发现,各ICA处理卵巢组织形态未见明显差异,但卵巢内次级卵泡数量多于对照组,说明ICA可促进卵泡的发育。这些数据表明,一定剂量的EP、EPS和ICA均能促进小鼠卵巢发育;根据IVM和RI值综合分析,在3种药物中ICA促进小鼠卵巢和子宫发育的效果最佳。

在石油工业中对于井壁稳定性问题受到越来越多的关注,并且随着生产在更具挑战性和更恶劣的环境—如超深水以及高压和高温场,合理配制钻井泥浆密度预防井壁坍塌变得更加迫切。因此,在钻探油气井期间,必须控制钻井的方向,保持地应力、岩石强度、孔隙压力和钻井液处于适当的平衡。本文分析了HM盆地气页岩不稳定机理。研究发现,当钻井液与钻井液接触时,地层和与钻井液接触的页岩的膨胀性对井壁稳定的影响特较大,同时井壁稳定会受到页岩的层理的影响。本文通过SEM分析,分析HM页岩矿物组分、孔隙及裂缝结构。HM盆地页岩形成中的粘土矿物含量较高,主要是伊利石、伊利石夹层和高岭石以及蒙脱石混合层。同时使用水基泥浆进行测试,得到12.6%至16.5%的膨胀率,表明页岩水合膨胀能力较高,阳离子交换能力在11649mmol/kg之间;与水接触时,回收率大于60%,表明页岩水化分散能力较弱。本文进一步讨论和比较了Mohr-Coulomb,Mogi-coulomb,Drucker-Prager,Hoek-Brown和Modified Lade优缺点及适用性,采用Mohr-Coulomb准则来判别沿层理面是否发生剪切失效和优选Mogi-Coulomb准则来判别岩石本体是否发生剪切破坏。基于该模型,编制计算机程序求解,预测井壁的坍塌当量泥浆密度。结果表明,页岩井壁坍塌压力与层理面、井壁的井眼井斜角及井眼方位角有关,在不同产状层理面条件下,维持井壁稳定所需钻井液密度值区别较大,钻井时需要根据区块层理产状来确定合适的泥浆密度密度。本文通过实验及数学建模分析,形成了一套重点考虑页岩层理的井壁稳定预测方法,为目标区块地区钻井工程井壁稳定设计提供了参考。

加纳在国内建立了国家健康保险计划(NHIS),以期提高全体民众的基本医疗卫生服务质量。加纳国家健康保险计划(NHIS)采取了拨出大量财政资金、保证弱势群体等措施来扩大了整体覆盖范围,其融资结构也日趋合理,但加纳国家健康保险计划(NHIS)参保情况整体却不乐观。本研究旨在深入了解加纳国家健康保险计划(NHIS)的特征,探究影响加纳国家健康保险计划(NHIS)参保率的主要因素。本研究运用决策理论、文献分析、实证分析方法。结合加纳国家健康保险计划(NHIS)的相关政策,建立参保影响因素假设模型。本研究以Kwabre East Municipality的18岁及以上人群为研究对象,采用便利抽样法,发放调查问卷700份,有效问卷共600份,调查问卷信度较好,信度系数为0.93。本研究运用统计软件SPSS 24.0进行相关分析以及多元回归分析,以探究各要素变量之间的关系。多元回归分析结果显示,风险感知、健康需求、医疗服务体系、收入水平和医疗卫生服务可得性等是影响加纳国家健康保险计划(NHIS)参保率的主要因素。即,风险感知、健康需求、医疗服务体系、收入水平和医疗卫生服务可得性,是促进民众参加国家健康保险计划(NHIS)的正向影响因素。当加纳国家健康保险计划(NHIS)的特征作为中间变量时,总体R从71%增加到73%,即加纳国家健康保险计划(NHIS)的特征加强了这些参保影响因素和加纳国家健康保险计划(NHIS)参保率之间的关系。同时,研究显示,受访者对国家健康保险计划(NHIS)、国家医疗保险局(NHIA)相关职能及作用知之甚少,这说明国家医疗保险局(NHIA)与民众联系不够密切,民众对该计划的看法没有向上反馈,这都不利于国家健康保险计划(NHIS)、国家医疗保险局(NHIA)的成长和发展。本研究建议加纳国家医疗保险局(NHIA)及其分支机构加强对国家健康保险计划(NHIS)教育和宣传等手段,充分了解民众的心声。其次,本研究建议在Kwabre East Municipality的社区内设立卫生保健设施配备齐全、服务质量可靠的卫生中心,这也是鼓励民众积极参加加纳国家健康保险计划(NHIS)的重要手段。

The Equilibrium Moisture Content of Five Lesser Utilized Species of Ghana Contrasted with Three European Species

Equilibrium moisture contents (EMC) of wood species are very necessary in the utilization of these in service. This study investigated the EMC of five lesser utilized species of Ghana and compared it with that of three European species. Sixteen randomly sampled specimens of each of the eight species (heartwood and sapwood) with dimensions 3 cm × 3 cm × 3 cm were exposed at various relative humidity conditions of 30%, 45%, 60%, 75% and 90% in a temperature and humidity-controlled climate chamber at a temperature of 25°C in accordance to German standard DIN 52182. The species are Albies alba, Fagus sylvatica and Picea abies which are European species and Amphimas pterocarpoides, Antiaris toxicaria, Canarium schweinfurthii, Celtis zenkeri and Cola gigantea are wood species from Ghana. Internal wood temperature and humidity were measured with datalogger. Samples were considered to have reached equilibrium at any given humidity when the daily weight changes were less than 0.1 mg according to German standard DIN 52183. After the last measurements of the weight changes, the samples were dried at 103°C until there was a constant weight. The five tropical hardwoods had low sorption values recorded and high sorption values for the European species and this could be attributed to differences in the microstructure of these woods.
