Factors associated with residual gastroesophageal reflux disease symptoms in patients receiving proton pump inhibitor maintenance therapy

AIM To elucidate the factors associated with residual gastroesophageal reflux disease(GERD) symptoms in patients receiving proton pump inhibitor(PPI) maintenance therapy in clinical practice.METHODS The study included 39 GERD patients receiving maintenance PPI therapy. Residual symptoms were assessed using the Frequency Scale for Symptoms of GERD(FSSG) questionnaire and the Gastrointestinal Symptom Rating Scale(GSRS). The relationships between the FSSG score and patient background factors, including the CYP2C19 genotype, were analyzed.RESULTS The FSSG scores ranged from 1 to 28 points(median score: 7.5 points), and 19 patients(48.7%) had a score of 8 points or more. The patients' GSRS scores were significantly correlated with their FSSG scores(correlation coefficient = 0.47, P < 0.005). In erosive esophagitis patients, the FSSG scores of the CYP2C19 rapid metabolizers(RMs) were significantly higher than the scores of the poor metabolizers and intermediate metabolizers(total scores: 16.7 ± 8.6 vs 7.8 ± 5.4, P < 0.05; acid reflux-related symptom scores: 12 ± 1.9 vs 2.5 ± 0.8, P < 0.005). In contrast, the FSSG scores of the CYP2C19 RMs in the non-erosive reflux disease patients were significantly lower than those of the other patients(total scores: 5.5 ± 1.0 vs 11.8 ± 6.3, P < 0.05; dysmotility symptom-related scores: 1.0 ± 0.4 vs 6.0 ± 0.8, P < 0.01). CONCLUSION Approximately half of the GERD patients receiving maintenance PPI therapy had residual symptoms associated with a lower quality of life, and the CYP2C19 genotype appeared to be associated with these residual symptoms.

Multiple sclerosis(MS) is a chronic inflammatory demyelinating disease of the central nervous system.A certain population of patients with MS present with a relapse-remitting disease course(RRMS) during the early phase and eventually advance to a progressive course(PMS).While immune modulation therapies have recently seen tremendous success in RRMS,our limited knowledge of pathogenesis hampers the development of effective treatments for PMS.

Hepatitis C infected patients need vitamin D3 supplementation in the era of direct acting antivirals treatment

It has been reported that the serum level of vitamin D3(Vit D3) could affect the natural course of chronic hepatitis C(CH-C) and the response to treatment with pegylated interferon(Peg-IFN) and ribavirin. Although several mechanisms for the favorable effects of Vit D3 supplementation were reported, the total effect of Vit D3 supplementation remains unclear. Previously, we reported that supplementation with 1(OH)Vit D3 could enhance the Th1 response inducing not only a favorable immune response for viral eradication but also HCC control. Recently, the main treatment of CH-C should be direct acting antivirals(DAAs) without PegIFN. Peg-IFN is a strong immune-modulator. Therefore, an immunological analysis should be carried out to understand the effect of Vit D3 after treatment of DAAs without Peg-IFN. The induction of a favorable immune response by adding Vit D3 might be able to suppress the hepatocarcinogenesis after achieving SVR, especially in children and elderly patients with severe fibrosis lacking sufficient amounts of VitD 3.

Immune checkpoint and inflammation as therapeutic targets in pancreatic carcinoma

Pancreatic adenocarcinoma(PAC) is one of the most deadly malignant neoplasms,and the efficacy of conventional cytotoxic chemotherapy is far from satisfactory. Recent research studies have revealed that immunosuppression and inflammation are associated with oncogenesis,as well as tumor development,invasion,and metastasis in PAC. Thus,immunosuppressionrelated signaling,especially that involving immune checkpoint and inflammation,has emerged as novel treatment targets for PAC. However,PAC is an immuneresistant tumor,and it is still unclear whether immune checkpoint or anti-inflammation therapies would be an ideal strategy. In this article,we will review immune checkpoint and inflammation as potential targets,as well as clinical trials and the prospects for immunotherapy in PAC.

比较了产生于室温固态钢以及高温液态钢上激光诱导等离子体的特征、原子铁的激发温度以及电子密度。通过在386～400nm波长范围的中性铁原子发射谱线,由波尔兹曼作图法确定了铁原子的激发温度。通过测量Al I394.4nm的谱线宽度,估算了电子密度。对固体钢来说,铁元素的激发温度从延迟时间为10μs时的10 800K下降到延迟时间为80μs时的7 300K。当延迟时间分别为10μs和70μs时,产生于固态钢和液态钢上等离子体间的激发温度并没有显著差别。在铁元素和铝元素大部分的中性原子线中,可以观察到液态钢的谱线宽度比固态钢的谱线宽度更窄。当激光脉冲的观察延迟时间均为10μs时,产生于液态钢上等离子体的电子密度大约为(0.99±0.15)×1017/cm3,这相当于产生在固态钢上等离子体电子密度的46%。

Parkin in cancer:Mitophagy-related/unrelated tasks

Dysfunctional mitochondria may produce excessive reactive oxygen species, thus inducing DNA damage, which may be oncogenic if not repaired. As a major role of the PINK1-Parkin pathway involves selective autophagic clearance of damaged mitochondria via a process termed mitophagy, Parkin-mediated mitophagy may be a tumorsuppressive mechanism. As an alternative mechanism for tumor inhibition beyond mitophagy, Parkin has been reported to have other oncosuppressive functions such as DNA repair, negative regulation of cell proliferation and stimulation of p53 tumor suppressor function. The authors recently reported that acute ethanol-induced mitophagy in hepatocytes was associated with Parkin mitochondrial translocation and colocalization with accumulated 8-OHd G(a marker of DNA damage and mutagenicity). This finding suggests:(1) the possibility of Parkin-mediated repair of damaged mitochondrial DNA in hepatocytes of ethanol-treated rats(ETRs) as an oncosuppressive mechanism; and(2) potential induction of cytoprotective mitophagy in ETR hepatocytes if mitochondrial damage is too severe to be repaired. Below is a summary of the various roles Parkin plays in tumor suppression, which may or may not be related to mitophagy. A proper understanding of the various tasks performed by Parkin in tumorigenesis may help in cancer therapy by allowing the PINK1-Parkin pathway to be targeted.

To retrieve the fuel debris in Fukushima Daiichi Nuclear Power Plants(1F),it is essential to infer the fuel debris distribution.In particular,the molten metal spreading behavior is one of the vital phenomena in nuclear severe accidents because it determines the initial condition for further accident scenarios such as molten core concrete interaction(MCCI).In this study,the fundamental molten metal spreading experiments were performed with different outlet diameters and sample amounts to investigate the effect of the outlet for spreading-solidification behavior.In the numerical analysis,the moving particle full-implicit method(MPFI),which is one of the particle methods,was applied to simulate the spreading experiments.In the MPFI framework,the melting-solidification model including heat transfer,radiation heat loss,phase change,and solid fraction-dependent viscosity was developed and implemented.In addition,the difference in the spreading and solidification behavior due to the outlet diameters was reproduced in the calculation.The simulation results reveal the detailed solidification procedure during the molten metal spreading.It is found that the viscosity change and the solid fraction change during the spreading are key factors for the free surface condition and solidified materials.Overall,it is suggested that the MPFI method has the potential to simulate the actual nuclear melt-down phenomena in the future.

Case of pediatric traditional serrated adenoma resected via endoscopic submucosal dissection

Traditional serrated adenoma(TSA)is a type of serrated polyp of the colorectum and is thought to be a precancerous lesion.There are three types of serrated polyps,namely,hyperplastic polyps,sessile serrated adenomas/polyps,and TSAs.TSA is the least common of the three types and accounts for about 5% of serrated polyps.Here we report a pediatric case of TSA that was successfully resected by endoscopic submucosal dissection(ESD).This rare case report describes a pediatric patient with no family history of colonic polyp who was admitted to our hospital with hematochezia.On colonoscopy,we found a polypoid lesion measuring 10 mm in diameter in the lower rectum.We selected ESD as a surgical option for en bloc resection,and histopathological examination revealed TSA.The findings in this case suggest that TSA with precancerous potential can occur in children,and that ESD is useful for treating this lesion.
