Digested effluent of manure (DEM) produced by biogas-plants contains many macro- and micro-nutrients. In an experiment conducted in southern Kyushu (Japan), forage crops cultivated after receiving DEM and chemical fertilizers exhibited similar dry matter yields. From a logistical standpoint, however, DEM in liquid form is difficult to handle and apply due to the low concentration of nutrients. To overcome this shortcoming, we prepared dehydrated DEM (DDEM) by adding DEM to cattle manure without disturbing the manure fermentation process. The objective of this research was to evaluate the effect of DDEM on dry matter yield and nitrogen recovery rate in annual dual-cropping systems (summer crop of maize or sorghum and winter crop of Italian ryegrass) that are typical of the region and to compare these results to commercial cattle manure alone, in combination with chemical fertilizer (CM or CM + CF), or no fertilizer application (NF). In both cropping systems, the DDEM treatment produced similar dry matter yields (2.6 to 3.02 kg·m-2) and apparent nitrogen recovery rates (43% to 53%) as the CM + CF and CM treatments. This suggests that DDEM can potentially replace chemical fertilizers and commercial cattle manure in the region.

SVO (straight vegetable oil) method means the direct use of vegetable oil as car fuel through installation of a heater unit in the car to decrease vegetable oil viscosity. In this study, the authors carried out performance tests on the direct use of waste cooking oil using a car with a heater unit. Moreover, the authors carried out long run driving on road tests in five years using a public car of Minami-Aizu Town in Fukushima Prefecture and analyzed the case of troubles and clarified the availability and problems of SVO vehicles. As a result, the car with a heater unit shows similar performance in both cases using vegetable oil or diesel fuel as fuel. The tested SVO vehicle of Minami-Aizu Town could be driven 38,127 km mainly by waste vegetable oil with a total driving distance of 52,293 km in long run driving tests in five years, and decreased about 3,813.5 liters of light oil which corresponds to 9.99 t of CO2.

The ocean thermal energy conversion (OTEC) system is a promising solution to provide stable electricity supply. Although the available temperature difference in OTEC systems is small, an ammonia/water mixture as working fluid is expected to decrease irreversible losses in the heat exchangers and to improve system performance. However, in actual heat exchangers, an adequate temperature crossing does not occur in the condenser but in the evaporator. Therefore, clarification of this characteristic is important. To date, the logarithmic temperature difference (LMTD) method is used in performance evaluations of OTEC heat exchangers. This method is of limited use if physical properties of fluids vary. A generalized mean temperature difference (GMTD) method is introduced to perform this evaluation. As changes in fluid property values can be considered in the GMTD method, method dependencies on heat exchanger characteristics, effectiveness, and system characteristics can be studied. In particular, GMTD and LMTD using a pure substance were found to be almost equal. Mean temperature differences using mixtures as working fluid were higher in the evaporator, but lower in the condenser, from the GMTD method than from the LMTD method. For higher ammonia concentrations in ammonia/water mixtures, the mean temperature differences from both methods are different.

Many researchers have worked on the ex- planation of AdaBoost＇s good experimental results in theory. Some work give an upper bound of generaliza- tion error in terms of the margin distribution function, while Breiman gave a sharper generalization error bound based on minimum margin. He also developed the arc- gv algorithm to maximize the minimum margin, then made the minimum margin larger than AdaBoost. How- ever, its empirical results are even worse than AdaBoost. Therefore, is the minimum margin bound not practi- cal？ This paper gives a new concept called Equilibrium margin （Emargin） and proves a new generalization er- ror bound using Emargin, which is always better than minimum margin bound. In addition, we show Emargin is a good indicator of generalization. Then, we conduct experiments showing that the Emargin of AdaBoost is larger than arc-gv, but the generalization error of Ada- Boost is usually better.

A Survey of Learning Needs about Oral Care for Intubated Patients among New Nursing Staff

Our aim was to understand the actual conditions of oral care for intubated patients by new nurses. This study was carried out from December 2017 to March 2018. We applied the theme of the dissipation of the dissociation between nursing skills learned in basic nursing education and nursing skills required in practice. A questionnaire survey was conducted at four facilities among new nursing staff who had acquired their nursing license in the past year. The questionnaire was composed of questions about their experiences of oral care for intubated patients, when they learned these skills, and what they wanted to learn. Answers were obtained from 118 new nurses. Learning about oral care was important for approximately 20% of the nurses, although it was not considered mandatory by all of the nurses. As infection control measures have revealed the acute needs of care for intubated patients, it is important for nursing students to learn about oral care and attitudes towards intubated patients, along with methods to make intubated patients more comfortable.

AIM:To assess the diagnostic ability of endoscopic ultrasonography(EUS)for evaluating causes of dista biliary strictures shown on endoscopic retrograde chol angiopancreatography(ERCP)or magnetic resonance cholangiopancreatography(MRCP),even without iden tifiable mass on computed tomography(CT). METHODS:The diagnostic ability of EUS was retro spectively analyzed and compared with that of routine cytology(RC)and tumor markers in 34 patients with distal biliary strictures detected by ERCP or MRCP a Dokkyo Medical School Hospital from December 2005 to December 2008,without any adjacent mass or ec centric thickening of the bile duct on CT that could cause biliary *** considered as benign strictures on EUS included preservation of the normasonographic layers of the bile duct wall,irrespective of the presence of a mass *** strictures were considered *** diagnosis of underlying diseases was made by pathological examination in 18 cases after surgical removal of the samples,and by clinical follow-up for>10 mo in 16 cases. RESULTS:Seventeen patients(50%)were finally di- agnosed with benign conditions,including 6"normal" subjects,while 17 patients(50%)were diagnosed with malignant *** terms of diagnostic ability,EUS showed 94.1%sensitivity,82.3%specificity,84.2% positive predictive value,93.3%negative predictive value(NPV)and 88.2%accuracy for identifying ma- lignant and benign *** was more sensi- tive than RC(94.1%vs 62.5%,P=0.039).NPV was also better for EUS than for RC(93.3%vs 57.5%,P= 0.035).In addition,EUS provided significantly higher sensitivity than tumor markers using 100 U/mL as the cutoff level of carbohydrate antigen 19-9(94.1%vs 53%,P=0.017).On EUS,biliary stricture that was fi- nally diagnosed as malignant showed as a hypoechoic, irregular mass,with obstruction of the biliary duct and invasion to surrounding tissues. Pancreatic cancer is difficult to diagnose at an early stage and generally has a poor prognosis. Surgical resection is the only potentially curative treatment for pancreatic carcinoma. To improve the prognosis of this disease, it is essential to detect tumors at early stages, when they are resectable. The optimal approach to screening for early pancreatic neoplasia has not been established. The International Cancer of the Pancreas Screening Consortium has recently finalized several recommendations regarding the management of patients who are at an increased risk of familial pancreatic cancer. In addition, there have been notable advances in research on serum markers, tissue markers, gene signatures, and genomic targets of pancreatic cancer. To date, however, no biomarkers have been established in the clinical setting. Advancements in imaging modalities touch all aspects of the clinical management of pancreatic diseases, including the early detection of pancreatic masses, their characterization, and evaluations of tumor resectability. This article reviews strategies for screening high-risk groups, biomarkers, and current advances in imaging modalities for the early detection of resectable pancreatic cancer.

Effects of Fuscoporia obliqua on Postprandial Glucose Excursion and Endothelial Dysfunction in Type 2 Diabetic Patients

Postprandial hyperglycemia has been reported to elicit endothelial dysfunction and provoke future cardiovascular complications. A reduction of postprandial blood glucose levels by the glucosidase inhibitor Fuscoporia obliqua was associated with a risk reduction of cardiovascular complications, but the effects of Fuscoporia obliqua on endothelial function have never been elucidated. This study is aimed to assess the efficacy of Fuscoporia obliqua on postprandial metabolic parameters and endothelial function in type 2 diabetic patients. Postprandial peak glucose (14.47±1.27 vs. 8.50±0.53 mmol/liter), plasma glucose excursion (PPGE), and change in the area under the curve (AUC) glucose after a single loading of test meal (total 450 kcal; protein 15.3%; fat 32.3%; carbohydrate 51.4%) were significantly higher in the diet-treated type 2 diabetic patients (n=14) than the age-and sex-matched controls (n=12). The peak forearm blood flow response and total reactive hyperemic flow (flow debt repayment) during reactive hyperemia, indices of resistance artery endothelial function on strain-gauge plethysmography, were unchanged before and after meal loading in the controls. But those of the diabetics were significantly decreased 120 and 240 min after the test meal. A prior administration of Fuscoporia obliqua decreased postprandial peak glucose, PPGE, and AUC glucose. The peak forearm blood flow and flow debt repayment were inversely well correlated with peak glucose, PPGE, and AUC glucose, but not with AUC insulin or the other lipid parameters. Even a single loading of the test meal was shown to impair the endothelial function in type 2 diabetic patients, and the postprandial endothelial dysfunction was improved by a prior use of Fuscoporia obliqua. Fuscoporia obliqua might reduce macrovascular complication by avoiding endothelial injury in postprandial hyperglycemic status.
