Prognostic factors of T4 gastric cancer patients undergoing potentially curative resection

AIM:To investigate the prognostic factors of T4 gastric cancer patients without distant metastasis who could undergo potentially curative resection. METHODS:We retrospectively analyzed the clinical data of 71 consecutive patients diagnosed with T4 gastric cancer and who underwent curative gastrectomy at our *** clinicopathological factors that could be associated with overall survival were *** cumulative survival was determined by the Kaplan-Meier method,and univariate comparisons between the groups were performed using the log-rank *** analysis was performed using the Cox proportional hazard model and a step-wise procedure. RESULTS:The study patients comprised 53 men (74.6%)and 18 women(25.4%)aged 39-89 years (mean,68.9 years).Nineteen patients(26.8%)had postoperative morbidity:pancreatic fistula developed in 6 patients(8.5%)and was the most frequent complication,followed by anastomosis stricture in 5 patients (7.0%).During the follow-up period,28 patients(39.4%)died because of gastric cancer recurrence,and 3(4.2%) died because of another disease or *** all patients,the estimated overall survival was 34.1%at 5 *** analyses identified the following statistically significant prognostic factors in T4 gastric cancer patients who underwent potentially curative resection: peritoneal washing cytology(P<0.01),number of metastatic lymph nodes(P<0.05),and venous invasion(P <0.05).In multivariate analyses,only peritoneal washing cytology was identified as an independent prognostic factor(HR=3.62,95%CI=1.37-9.57)for longterm survival. CONCLUSION:Positive peritoneal washing cytology was the only independent poor prognostic factor for T4 gastric cancer patients who could be treated with potentially curative resection.

Pancreaticobiliary maljunction(PBM)is frequently associated with biliary cancer due to reflux of pancreatic enzymes into the choledochus,and even after surgery to correct the PBM such patients still have a risk of residual bile duct ***,we report the case of a 59-year-old female with carcinoma of the papilla of Vater which developed 2.5 years after choledochoduodenostomy for *** the postoperative follow-up period,computed tomography obtained 2 years after the first operation demonstrated a tumor in the distal end of the choledochus,although she did not have jaundice and laboratory tests showed no abnormalities caused by the previous *** a result,carcinoma of the papilla of Vater was diagnosed at an early stage,followed by surgical *** early detection of periampullary cancer in patients undergoing surgery for PBM,careful long-term follow-up is needed.

Simple procedure for assessing diffuse subarachnoid hemorrhage successfully created using filament perforation method in mice

The murine model of subarachnoid hemorrhage(SAH)is a valuable experimental tool for investigating molecular and cellular mechanisms,and the endovascular filament perforation technique can be used to simulate prominent pathophysiological features observed after human SAH;however,current validation methods for assessing an appropriate SAH model are ***,we introduce a simple procedure for se-lecting a mouse model of diffuse *** was induced in 24 mice using a standard filament perforation *** confirming survival at 24 h,SAH was scored 0-1 based on T2*-weighted images on whole-brain magnetic resonance imaging(MRI)and visual surveillance of the cisterna magna(CM)through the dura *** CM-based SAH grading correlated well with a reference parameter defined by extracted brain(r^(2)=0.53,p<0.0001).The receiver operating characteristic curve revealed a sensi-tivity of 85%and a specificity of 91%for detecting diffuse SAH,with a similar area under the curve(0.89±0.06[standard error of the mean])as the MRI-based grading(0.72±0.10,p=0.12).Our data suggest that confirming an SAH clot in the CM is a valuable way to select a clinically relevant diffuse SAH model that can be used in future experimental studies.

Phase II Study of Irinotecan plus S-1 in Treatment of Advanced Gastric Cancer

Objective: The efficacy and safety of irinotecan hydrochloride (CPT-11) plus oral fluoropyrimidine S-1 combination therapy in patients with previously untreated advanced gastric cancer was evaluated. Methods: The regimen comprised CPT-11 plus S-1: CPT-11, 60 mg/m2 (days 1, 15);S-1, 40 - 60 mg/body twice daily (days 1 - 21) followed by a 1-week rest, every 4 weeks. Primary endpoint was response rate. Secondary endpoints were tumor control rate, adverse events, relative dose intensity, and overall survival. Results: Twenty-five patients were enrolled;median age was 66 years. Response rate was 40% (95% confidence interval, 21.1% - 61.3%;complete response in 1;partial response in 9). Tumor control rate was 56.0%, median survival time was 436 days and relative dose intensities were 0.83 for CPT-11 and 0.85 for S-1. Incidence of grade 3 or greater neutropenia, anemia and diarrhea was 16%, 12%, and 12%, ***: The present results indicate that CPT-11 plus S-1 offers lower treatment-related toxicity than regimens including cisplatin and is effective in patients with advanced gastric cancer.

BACKGROUND Lipoprotein(a)[Lp(a)]is a causal risk factor for atherosclerotic cardiovascular diseases;however,its role in acute coronary syndrome(ACS)remains *** To investigate the hypothesis that the Lp(a)levels are altered by various conditions during the acute phase of ACS,resulting in subsequent cardiovascular *** From September 2009 to May 2016,377 patients with ACS who underwent emergent coronary angiography,and 249 who completed≥1000 d of follow-up were ***(a)levels were measured using an isoform-independent assay at each time point from before percutaneous coronary intervention(PCI)to 48 h after *** primary endpoint was the occurrence of major adverse cardiac events(MACE;cardiac death,other vascular death,ACS,and non-cardiac vascular events).RESULTS The mean circulating Lp(a)level decreased significantly from pre-PCI(0 h)to 12 h after(19.0 mg/dL to 17.8 mg/dL,P<0.001),and then increased significantly up to 48 h after(19.3 mg/dL,P<0.001).The changes from 0 to 12 h[Lp(a)Δ0-12]significantly correlated with the basal levels of creatinine[Spearman’s rank correlation coefficient(SRCC):-0.181,P<0.01]and Lp(a)(SRCC:-0.306,P<0.05).Among the tertiles classified according to Lp(a)Δ0-12,MACE was significantly more frequent in the lowest Lp(a)Δ0-12 group than in the remaining two tertile groups(66.2%vs 53.6%,P=0.034).A multivariate analysis revealed that Lp(a)Δ0-12[hazard ratio(HR):0.96,95%confidence interval(95%CI):0.92-0.99]and basal creatinine(HR:1.13,95%CI:1.05-1.22)were independent determinants of subsequent *** Circulating Lp(a)levels in patients with ACS decreased significantly after emergent PCI,and a greater decrease was independently associated with a worse prognosis.

A backpropagation neural network improved by a genetic algorithm for predicting the mean radiant temperature around buildings within the long-term period of the near future

This study aimed to develop a neural network(NN)-based method to predict the long-term mean radiant temperature(MRT)around buildings by using meteorological parameters as training *** MRT dramatically impacts building energy consumption and significantly affects outdoor thermal *** NN-based long-term MRT prediction,two main restrictions must be overcome to achieve precise results:first,the difficulty of preparing numerous training datasets;second,the challenge of developing an accurate NN *** overcome these restrictions,a combination of principal component analysis(PCA)and K-means clustering was employed to reduce the training data while maintaining high prediction ***,three widely used NN models(feedforward NN(FFNN),backpropagation NN(BPNN),and BPNN optimized using a genetic algorithm(GA-BPNN))were compared to identify the NN with the best long-term MRT prediction *** performances of the tested NNs were evaluated using the mean absolute percentage error(MAPE),which was≤3%in each *** findings indicate that the training dataset was reduced effectively by the PCA and *** the three NNs,the GA-BPNN produced the most accurate results,with its MAPE being below 1%.This study will contribute to the development of fast and feasible outdoor thermal environment prediction.

Effect of dissolved oxygen on methane production from bottom sediment in a eutrophic stratified lake

Clarifying the role of sulfate and dissolved oxygen (DO) in methane production may allow for precise and accurate modeling of methane emissions in eutrophic *** conducted field observations of sulfate,methane,and DO concentrations in Lake Abashiri,a typical brackish and eutrophic lake in a cold region,to develop a DO-based method for quantitively estimating methane production in a eutrophic lake and analyzed the *** found that sulfate concentrations decreased rapidly from 900.0 mg/L in water overlying the sediments to nearly 0.0 mg/L in the bottom *** production was almost uniform across sediment depths of 0.05 to 0.25 m,ranging from 1400 to 1800μmol/m^(2)/***,methane production was found to be a function of DO concentrations in water overlying the bottom and could be modeled by a logistic function:constant production at 1,400μmol/m^(2)/day for DO concentrations of 0.0 to 3.0 mg/L,rapidly decreasing to 0μmol/m^(2)/day for DO concentrations of 3.0 to 6.0 mg/*** methane model was verified using a simple one-dimensional numerical model that showed good agreement with field *** results thus suggest that the proposed methane model reduces uncertainty in estimating methane production in a eutrophic lake.

The development of vehicle integrated photovoltaics-powered electric vehicles (VIPV-EV) significantly reduces CO2 emissions from the transport sector to realize a decarbonized society. Although long-distance driving of VIPV-EV without electricity charging is expected in sunny regions, driving distance of VIPV-EV is affected by climate conditions such as solar irradiation and temperature rise of PV modules. In this paper, detailed analytical results for effects of climate conditions such as solar irradiation and temperature rise of PV modules upon driving distance of the VIPV-EV were presented by using test data for Toyota Prius and Nissan Van demonstration cars installed with high-efficiency InGaP/GaAs/InGaAs 3-junction solar cell modules with a module efficiency of more than 30%. The temperature rise of some PV modules studied in this study was shown to be expressed by some coefficients related to solar irradiation, wind speed and radiative cooling. The potential of VIPV-EV to be deployed in 10 major cities was also analyzed. Although sunshine cities such as Phoenix show the high reduction ratio of driving range with 17% due to temperature rise of VIPV modules, populous cities such as Tokyo show low reduction ratio of 9%. It was also shown in this paper that the difference between the driving distance of VIPV-EV driving in the morning and the afternoon is due to PV modules’ radiative cooling. In addition, the importance of heat dissipation of PV modules and the development of high-efficiency PV modules with better temperature coefficients was suggested in order to expand driving range of VIPV-EV. The effects of air-conditioner usage and partial shading in addition to the effects of temperature rise of VIPV modules were suggested as the other power losses of VIPV-EV.

Temperature evolution from the δ ^(18)O record of Hani peat, Northeast China, in the last 14000 years

From the last deglaciation to the Holocene, the Greenland Ice Core (GISP2) δ 18O records as well as the records of ice-rafted debris on the surface of the North Atlantic have revealed a succession of sudden cooling events on the centennial to millennial scales. However, the temperature proxy records are rarely studied systematically and directly to ensure that this air temperature cooling pattern simultaneously existed in the East Asian Region, in addition to the repeated pattern occurring in the Greater Atlantic Region. A peat cellulose δ 18O temperature proxy record proximately existing for 14000 years was picked up from the Hani peat in Jilin Province, China. It suggests by comparison that the sudden cooling events, such as the Older Dryas, Inter-Allerφd, Younger Dryas, and nine ice-rafted debris events of the North Atlantic, are almost entirely reiterated in the temperature signals of Hani peat cellulose δ 18O. These cooling events show that the repeatedly occurring temperature cooling pattern not only appeared in the North Atlantic Region in the high latitudes, but also in the Northwest Pacific Region in the middle latitudes. The climate change events marking the start of the Holocene Epoch, the Holocene Megathermal, the "8.2 kyr" event, the "4.2 kyr" event, the Medieval Warm Period, and the Little Ice Age are further discussed. The sensitivity response of Hani peat cellulose δ 18O to the land surface temperature and the reason for the age accuracy of peat cellulose 14C are also discussed based on the characteristics of the peat bog environment.
