Comparison of minimally invasive glaucoma surgery with trabecular micro-bypass stent and microhook ab interno trabeculotomy performed in conjunction with cataract surgery

AIM: To evaluate the effectuality and safety of cataract surgery combined with either ab interno trabeculotomy by the microhook(μLOT) or a single i Stent? trabecular bypass implantation(i Stent) in eyes with cataract and mild-tomoderate glaucoma. METHODS: This study enrolled subjects with mild-tomoderate open angle glaucoma with visually significant cataract who used two or more ophthalmic antiglaucoma agents between 60 and 90 y of age. Patients underwent cataract surgery cooperated with either implantation of an i Stent(i Stent-phaco) or excisional goniotomy with the μLOT(μLOT-phaco). Patients underwent μLOT-phaco in the eye with lower the mean deviation, according to the Humphrey field analyzer, while i Stent-phaco was carried out on the other eye. Intraocular pressure(IOP) pre-and post-surgery, alterations in anterior chamber flare(ACF), and corneal endothelial cell density(ECD) were ***: Twenty subjects were enrolled(mean age: 73.6±7.3 y). The mean medicated preoperative IOP was 16.7 mm Hg in the μLOT and 16.2 mm Hg in the i Stent eyes. The mean final IOP at 12 mo was 13.6 mm Hg in the μLOT eyes and 13.6 mm Hg in the i Stent eyes, representing a 17.8% and 17.2% reduction, respectively. The preoperative ACF in the μLOT eyes was 9.5 pc/ms and it returned to normal in 30 d postoperatively, with a value of 11.4 pc/ms. In the i Stent eyes, ACF was 9.6 pc/ms preoperatively and it returned to normal by 7 d postoperatively(11.2 pc/ms atday 7), demonstrating that postoperative inflammation was less in the i Stent eyes. The corneal ECD in both groups was not significantly ***: In this study, i Stent and μLOT are both effective through 12 mo of follow-up. Safety is more favorable in the i Stent eyes, based on early anterior chamber inflammation.

Effective use of the Japan Narrow Band Imaging Expert Team classification based on diagnostic performance and confidence level

Five years have passed since the Japan Narrow Band Imaging Expert Team (JNET) classification was proposed in 2014. However, the diagnostic performance of this classification has not yet been established. We conducted a retrospective study and a systematic search of Medical Literature Analysis and Retrieval System On-Line. There were three retrospective single center studies about the diagnostic performance of this classification. In order to clarify this issue, we reviewed our study and three previous studies. This review revealed the diagnostic performance in regards to three important differentiations.(1) Neoplasia from non-neoplasia;(2) malignant neoplasia from benign neoplasia;and (3) deep submucosal invasive cancer (D-SMC) from other neoplasia. The sensitivity in differentiating neoplasia from non-neoplasia was 98.1%-99.8%. The specificity in differentiating malignant neoplasia from benign neoplasia was 84.7%-98.2% and the specificity in the differentiation D-SMC from other neoplasia was 99.8%-100.0%. This classification would enable endoscopists to identify almost all neoplasia, to appropriately determine whether to perform en bloc resection or not, and to avoid unnecessary surgery. This article is the first review about the diagnostic performance of the JNET classification. Previous reports about the diagnostic performance have all been retrospective single center studies. A large-scale prospective multicenter evaluation study is awaited for the validation.

江西永平铜矿位于华南怀玉山—北武夷山铜铅锌多金属成矿带内,是赣东北地区除了德兴铜矿外的另一个大型铜矿基地。该矿区存在两种类型的花岗质岩石,一种是花岗岩;另一种是英安斑岩。英安斑岩具有典型斑状结构和石英眼结构,而花岗岩则具有单向固结结构。在化学成分上,两者属于高钾的钙碱性系列岩石,英安斑岩贫硅、富Al、Fe、Mg、Ca,以及具有较大的Na_2O/K_2O(0.02～0.64)等特点;而花岗岩富硅、贫Al、Ca,以及富碱和具有较小的Na_2O/K_2O(0.02～0.03)等特点。两种类型的岩石具有一致的REE配分曲线。它们均富集大离子亲石元素(Ba、Rb、K),亏损高场强元素(Th、Nd、Ta、Ti)以及元素Sr和P,显示了与俯冲作用有关的岩浆作用。在结构和化学成分上,花岗岩则类似于美国Climax斑岩钼(铜)矿成矿斑岩的性质(如具有单向固结结构、较高的分异指数、富Si、贫Al、Ca、富Na_2O+K_2O以及K_2O>Na_2O)。与英安斑岩有关的蚀变作用主要有夕卡岩化、黑云母化、白云母化、绿泥石化和萤石化,而与花岗岩有关的蚀变作用主要是白云母化和萤石化;相应地,与英安斑岩有关的成矿作用主要为铜,而与花岗岩有关的成矿作用则主要为钼。2件辉钼矿样品的Re-Os年龄分别为156.7±2.8Ma和155.7±3.6Ma,表明与花岗岩有关的钼成矿作用发生在156Ma左右。本文认为,永平铜钼矿的成矿地球动力学背景应是由挤压向伸展的转换环境。

CAD Exchange Format in the Field of Public Works Japan

There is a CAD （computer-aided design） data exchange format named SXF （scadec exchange format） in the field of Japanese public works which was developed by a consortium in 1999 to be based on the ISO 10303-202, so that the MLIT （Ministry of Land, Infi＇astructure, Transportation and Tourism） could start e-delivery （e-submit） of CAD drawings. It is one of targets for CALS/EC （continuous acquisition and lifecycle support/electric commerce） program which MLIT are promoting since 1999. Most of local governments have followed the MLIT to start e-delivery, so that SXF has become to be a standard in the public works in Japan with many problems. SXF is an exchange format and so many design companies or contractors would submit the CAD drawings with transferred format before the delivery of e-submit even if they use usually another CAD in their offices. This paper will introduce the standard of CAD exchange format SXF through the activities of Japanese public works.

Single-nucleotide polymorphisms in HORMAD1 may be a risk factor for azoospermia caused by meiotic arrest in Japanese patients

Genetic mechanisms are implicated as a cause of some male infertility, yet are poorly understood. Meiosis is unique to germ cells and essential for reproduction. The synaptonemal complex is a critical component for chromosome pairing, segregation and recombination. Hormadl is essential for mammalian gametogenesis as knockout male mice are infertile. Hormadl-deficient testes exhibit meiotic arrest in the early pachytene stage and synaptonema! complexes cannot be visualized. To analyze the hypothesis that the human HORMAD1 gene defects are associated with human azoospermia caused by meiotic arrest, mutational analysis was performed in all coding regions by direct sequence analysis of 30 Japanese men diagnosed with azoospermia resulting from meiotic arrest. By the sequence analysis, three polymorphism sites, Single Nucleotide Polymorphism 1 （c. 163A〉G）, SNP2 （c. 501T〉G） and SNP3 （c. 918C〉T）, were found in exons 3, 8 and 10. The 30 patients with azoospermia and 80 normal pregnancy-proven, fertile men were analyzed for HORMAD1 polymorphisms. Both SNP1 and SNP2 were associated with human azoospermia caused by complete early meiotic arrest （P〈0.0S）. We suggest that the HORMAD1 has an essential meiotic function in human spermatogenesis.

Effects of Pitavastatin on the Intima-Media Thickness of the Carotid Artery in Patients with Ischemic Stroke: The Pitavastatin Efficacy Study on Surrogate Markers and Imaging for Stroke (PESSMIST)

Pitavastatin is a strong statin that was developed in Japan. The clinical impact of pitavastatin treatment in patients with ischemic stroke has not been reported. We conducted a prospective, open label, clinical case-control study to determine the secondary preventive effects of pitavastatin for patients with cerebral infarction and hyperlipidemia. The pitavastatin group included 20 Japanese patients diagnosed with cerebral infarction and hyperlipidemia without previous statin intake. The control group included 22 patients diagnosed with cerebral infarction without hyperlipidemia. The pitavastatin group of patients received 2 mg of pitavastatin once a day after dinner. The mean age of the patients was 69.3 and 75.5 years for the pitavastatin and control groups, respectively, and the age of the pitavastatin group was significantly younger than that of the control group (P < 0.05). The serum TCho and LDL-C levels significantly decreased two months after the initiation of pitavastatin treatment. The mean and maximum intima-media thickness (IMT) also decreased after the initiation of pitavastatin. The mean and maximum IMT did not show any significant changes in the control group. The change of IMT %/year was less than zero for the pitavastatin group, and was almost zero or higher for the control group. Pitavastatin showed beneficial effects by improving the surrogate makers of stroke. These surrogate makers were effective to evaluate the efficacy of pitavastatin to prevent secondary stroke. Although a prospective randomized study is required to elucidate the long-term effects of pitavastatin, the current study suggests that pitavastatin may be effective to prevent secondary stroke in patients with stroke and hyperlipidemia.

Conversion of Methane to Hydrogen via Pulsed Corona Discharge

Experiments are performed to develop a pulsed corona discharge system for the conversion of methane to hydrogen at atmospheric pressure (≈760 Torr) without using a catalyst. The corona discharge was energized by 10-12 μs wide voltage pulses (≤7 kV) at a repetition rate of about 1.0-1.5 kHz. The residual gases were characterized by mass spectrometry. The conversion of methane is as high as 50.8% producing the 70% yield of hydrogen. The influences of argon on the discharge of methane were studied. This result could be useful for the mass production of hydrogen in both academic and industrial point of view.
