Transcatheter arterial chemoembolization with a fine-powder formulation of cisplatin for hepatocellular carcinoma

AIM:To evaluate the efficacy of transcatheter arterial chemoembolization(TACE) using a suspension of a fine-powder formulation of cisplatin(DDPH) for hepatocellular carcinoma(HCC).METHODS:The study population was comprised of 164 patients who were treated by TACE *** these patients,76 underwent TACE using a suspension of DDPH in lipiodol(LPD)(DDPH group),and the remaining 88 underwent TACE with an emulsion of doxorubicin(ADM) with LPD(ADM group).We compared the DDPH group with the ADM group in terms of the objective early response rate,progression free survival(PFS) and overall survival(OS).RESULTS:The objective early response rate in the DDPH group was significantly higher than that in the ADM group(54% vs 24%,P < 0.001).The PFS rate in the DDPH group was also significantly higher than that in the ADM group(P < 0.001).Moreover,the OS in the DDPH group was significantly longer than that in the ADM group(P = 0.002).Although the incidence rate of nausea or vomiting in the DDPH group was higher than that in the ADM group,the ADM group showed a higher incidence rate of the adverse events of hepatic arterial damage and *** other serious complications were observed in either ***:We conclude that TACE using a suspension of DDPH in LPD could be a useful treatment for HCC.

In this paper, based on a class of multi-group epidemic models of SEIR type with bilinear incidences, we introduce a vaccination compartment, leading to multi-group SVEIR model. We establish that the global dynamics are completely determined by the basic reproduction number R0V which is defined by the spectral radius of the next generation matrix. Our proofs of global stability of the equilibria utilize a graph-theoretical approach to the method of Lyapunov functionals. Mathematical results suggest that vaccination is helpful for disease control by decreasing the basic reproduction number. However, there is a necessary condition for successful elimination of disease. If the time for the vaccines to obtain immunity or the possibility for them to be infected before acquiring immunity is neglected in each group, this condition will be satisfied and the disease can always be eradicated by suitable vaccination strategies. This may lead to over evaluation for the effect of vaccination.

Expression and significance of ICAM-1 and its counter receptors LFA-1 and Mac-1 in experimental acute pancreatitis of rats

AIM： To investigate the role of intercellular adhesion molecule-1 （ICAM-1） and its counter receptors LFA-1 and Mac-1 in acute pancreatitis （AP）. METHODS： SD rats were allocated to AP group and control group randomly （25 rats each）. AP was induced by infusion of 5% chenodeoxycholic acid into the pancreatic duct, followed by ligation of pancreatic duct. The rats were sacrificed at 1, 3, 6, 12 and 24 h after induction of pancreatitis. Five rats were sacrificed at one time point in the two groups before the blood and specimens from pancreas and lung were obtained. Serum amylase and ascitic fluid were measured at each time point. Expression of ICAM-1 at different time points was assessed by immunohistochemistry in pancreas and lung, and the expression of LAF-1 and Mac-1 on neutrophils at different time points was detected by flow cytometer. RESULTS： Induction of AP was confirmed by the serum levels of amylase and histological studies. The expression of ICAM-1 in pancreas increased significantly than that in the control group at all time points （P 〈 0.05 orP 〈 0.01）, as well as the expression in lung except at 1 h. The expression of LFA-1 and Mac-1 on neutrophil in blood increased significantly in AP group than that in control group at several time points （P 〈 0.05 or P 〈 0.01）. The amount of ascitic fluid and serum amylase level of AP group increased significantly than that of control group at all time points （P 〈 0.05 or P 〈 0.01）. Parallel to these results, a significant neutrophil infiltration was found in pancreas and lung tissues of AP group rats. CONCLUSION： Our findings suggest the important role for ICAM-1, LFA-1 and Mac-1 in mediating the development of AP from a local disease to a systemic illness. Upregulation of ICAM-1, LFA-1, Mac-1 and subsequent leukocyte infiltration appear to be significant events of pancreatic and pulmonary injuries in AP.

Recent data from several laboratories have provided evidence that the newly fertilized oocyte inherits epigenetic signals from the sperm chromatin that are required for proper embryonic development. For the purposes of this review, the term epigenetic is used to describe all types of molecular information that are transmitted from the sperm cell to the embryo. There are at least six different forms of epigenetic information that have already been established as being required for proper embryogenesis in mammals or for which there is evidence that it may do so. These are （i） DNA methylation; （ii） sperm-specific histones, （iii） other chromatin-associated proteins; （iv） the perinuclear theca proteins; （v） sperm-born RNAs and, the focus of this review; and （vi） the DNA loop domain organization by the sperm nuclear matrix. These epigenetic signals should he considered when designing protocols for the manipulation and cryopreservation of spermatozoa for assisted reproductive technology as necessary components for effective fertilization and subsequent embryo development.

Injuries to the nervous system account for the widespread morbidity,mortality,and discomfort *** nerve guidance conduits(NGCs)offer a promising platform for nerve reconstruction,however,they require extracellular matrix(ECM)-like features to better mimic the in vivo ***,this research was aimed to fabricate heparin/growth factors(GFs)-immobilized artificial *** was covalently immobilized onto aligned electrospun polycapro-lactone/gelatin(PCL/Gel)***,basic fibroblast growth factor(bFGF)and nerve growth factor(NGF)were preferentially immobilized on heparinized nanofibers;the immobilization efficiency of GFs was found to be 50%with respect to(w.r.t.)their initial loaded *** in vivo implantation of NGCs in a sciatic nerve defect model revealed the successful retention(~10%w.r.t the initial loaded amount)and bioactivity of NGF for up to 5 *** permeability of bovine serum albumin(BSA)from nanofibrous membranes was further assessed and found to be comparable with the commercialized cel-lulose acetate *** bioactivity of NGCs was assessed in a sciatic nerve defect model in rats for short-term(1 week)and long-term(1-month).The NGCs displayed good structural stability and biocompatibility in *** in vivo evaluation revealed the accumulation of host cells into the transplanted *** together;these heparin/GFs-immobilized artificial NGCs may have broad implications for nerve regeneration and related tissue engineering disciplines.

Chemotherapy as a component of multimodal therapy for gastric carcinoma

Prognosis of locally advanced gastric cancer remains poor, and several multimodality strategies involving surgery, chemotherapy, and radiation have been tested in clinical trials. Phase Ⅲ trial testing the benefit of postoperative adjuvant chemotherapy over treatment with surgery alone have revealed little impact on survival, with the exception of some small trials in Western nations. A large trial from the United States exploring postoperative chemoradiation was the first major success in this category. Results from Japanese trials suggest that moderate chemotherapy with oral fluoropyrimidines may be effective against less-advanced （T2-stage） cancer, although another confirmative trial is needed to prove this point. Investigators have recently turned to neoadjuvant chemotherapy, and some promising results have been reported from phase Ⅱ trials using active drug combinations. In 2005, a large phase Ⅲ trial testing preand postoperative chemotherapy has proven its survival benefit for resectable gastric cancer. Since the rate of pathologic complete response is considered to affect treatment results of this strategy, neoadjuvant chemoradiation that further increases the incidence of pathologic complete response could be a breakthrough, and phase Ⅲ studies testing this strategy may be warranted in the near future.

Role of Sphingosine 1-Phosphate Receptor Type 1 in Lymphocyte Egress from Secondary Lymphoid Tissues and Thymus

Circulation of mature lymphocytes between blood and secondary lymphoid tissues plays a central role in the immune system. Homing of lymphocytes from blood into secondary lymphoid tissues beyond high endothelial venules is highly dependent on the interaction between the chemokines CCL19, CCL21, CXCL12, and CXCL13, and their receptors CCR7, CXCR4 and CXCR5. However, the molecular mechanism（s） of lymphocyte egress from secondary lymphoid tissues to lymph remained unclear. We have found a new class of immunomodulator, FTY720 by chemical modification of vegetative wasp-derived natural product, ISP-I （myriocin）. FTY720 has been shown to be highly effective in experimental allograft and autoimmune disease models. A striking feature of FTY720 is the induction of a marked decrease in peripheral blood lymphocytes at doses that show immunomodulating activity in these models. The reduction of circulating lymphocytes by FTY720 is caused by sequestration of lymphocytes into secondary lymphoid tissues and thymus. FTY720 is rapidly converted to （S）-enantiomer of FTY720-phosphate [（S）-FTY720-P] by sphingosine kinase 2 in vivo. （S）-FTY720-P acting as a potent agonist of S1P receptor type 1 （S1P1）, induces long-term down-regulation of S1P1 on lymphocytes, and thereby inhibits the migration of lymphocytes toward S1P. Thus, it is presumed that FTY720-induced lymphocyte sequestration is due to the inhibition of S1P/S1P1-dependent lymphocyte egress from secondary lymphoid tissues and thymus by its active metabolite （S）-FTY720-P. Throughout the analysis of the mechanism of action of FTY720, it is clarified that S1P/S1P1 interaction plays an important role for lymphocyte egress from secondary lymphoid tissues and thymus.

Effects of Vertical Angle of Conical Punch on Stretch Flangeability of High Strength Steel

To clarify the effects of the vertical angle of a conical punch on stretch flangeability, hole expansion forming tests were conducted. Test results showed that the hole expansion ratio becomes larger as the vertical angle *** also showed that the fracture strain at the fracture location on the hole edge was constant and independent of the vertical angle. This is because the hole expansion ratio was controlled not only by the fracture strain, which is independent of the vertical angle, but also by deformation uniformity along the hole edge. From the result of numerical analyses, it was determined that deformation uniformity depends on the gradient of circumferential stress along the radius direction. When the vertical angle is sharp, the circumferential stress showed a steep decline and the deformation localization was suppressed. Consequently, the hole edge deformed more uniformly and the hole expansion ratio became larger. It is concluded that in order to improve stretch flangeability of high strength steel, it is important to uniformly deform the hole edge by applying a conical punch with a sharp vertical angle.

Microstructure and Electrochemical Corrosion Behavior of Fe-Cr System Alloys as Substitutes for Ni-Based Brazing Filler Metal

Two new Fe-Cr system alloys,Fe-20Cr-43Ni-10P（mass%）and Fe-20Cr-20Ni-8P-5Si-2Mo（mass%）,have been developed as substitutes for the expensive Ni-based brazing filler metal used in brazing exhaust gas recirculation *** microstructures and melting properties of the alloys were analyzed by electron probe X-ray microanalyzer and differential scanning *** electrochemical characteristics of the alloys were investigated by potentiodynamic polarization testing in an electrolyte solution made in accordance with the standards of the Automobile Manufacturers Association of ***,the corrosion behaviors of the alloys were investigated by constant-potential polarization testing and surface *** is found that both alloys are composed of solid-solution phases and phosphide *** solid-solution phases serve as the anode,and the phosphide phases serve as the cathode in the corrosion reaction for both ***-20Cr-43Ni-10 P exhibits galvanic corrosion on the entire *** contrast,Fe-20Cr-20Ni-8P-5Si-2Mo is attacked at a few localized areas so that the cavities form on the *** corrosion potential（E_（corr）） is lower than that of Ni-29Cr-6P-4Si（mass%）for both *** means that the Fe-Cr system alloys are more easily corroded than *** corrosion rate and corrosion resistance cannot be investigated by the corrosion current density（i_（corr）） and polarization resistance（R_p）,respectively,because of localized corrosion of the two alloys.
