Background: Most community and hospital acquired infections are due to Staphylococcus aureus, these infections involve both local suppurative and systemic conditions. There is an increase in the incidence rates of acquiring resistant strains of Staphylococcus aureus in hospitals and community in Sudan. This study aims to detect frequency of Staphylococcus aureus isolated from surgical-sites infections (SSIs) and traumatic wounds, and to determine susceptibility to commonly used drugs. Methods: Prospective cross-sectional laboratory based study was followed, 250 patients admitted to Surgery Departments at Wad Madani Teaching Hospitals during 2019 and 2020 were recruited;with clinical manifestation of SSIs and or/traumatic wounds. Swab sample was taken from each participant and incubated aerobically on mannitol salt agar. Biochemical tests and 16rRNA gene used for Staphylococcus aureus identification. Antimicrobial susceptibility was accomplished using Kirby Bauer disc diffusion technique. Background: Urosepsis is one of the most common infections that require empirical broad spectrum antibiotics immediately after diagnosis. This has led to development of bacterial resistance by acquiring the capability to destroy the β-lactam ring. Methodology: This is a cross-sectional hospital-based study. The study was conducted from 2019 to 2020 at Gezira Hospital for Renal diseases and surgery (GHRDS). A hundred patients were diagnosed clinically with urosepsis and the isolated organisms were Escherichia coli, Staphylococcus aureus, Proteus mirabilis, Klebsiella pneumonia and Pseudomonas aeruginosa. The susceptibility test was conducted by Kirby Bauer disc diffusion technique according to clinical laboratory standard institute (CLSI) guidelines. Seventy eight samples of bacterial genomic DNA were confirmed by 16srRNA and multiplex PCR, were performed for genotypic blaOXA-51 and blaOXA-23 gene characterization of isolated bacteria. Then gel electrophoresis was used to identify the presence or absence of (blaOXA-51 and blaOXA-23) genes. Results: 88.5% (69/78) in 16srRNA detected. Using multiplex PCR, the frequencies of blaOXA-51 and blaOXA-23 genes were 13% and 10.1%, respectively. The percentages of isolates which yielded both blaOXA-51 and blaOXA-23 among P. aeruginosa was 25% (1/4), among K. pneumonia was 17% (1/6), and among E. coli was 8% (3/37). Only blaOXA-51 was detected in P. mirabilis 10% (1/10) and only blaOXA-23 was detected in S. aureus 5% (1/18). Conclusion: In this study, the presence of blaOXA-51 and blaOXA-23 genes was increased in the isolated bacteria.

Hepatitis C virus(HCV) genotype(GT) 4 represents 12%-15%(15-18 million) of total global HCV infection. It is prevalent in Northern and Equatorial Africa and the Middle East, and is also present in some countries in Europe. GT-4(and subtype 4a in particular) dominates the HCV epidemic in Egypt. In underdeveloped countries, risk factors associated with HCV infection may be due to unsafe medical practices or other factors such as familial transmission, mother's HCV status, or illiteracy. HCV prevention and control programs should include health education, increased community awareness towards the disease, controlling infection distribution in healthcare centers, proper sterilization of medical and dental instruments, and ensuring safe supply of blood and blood-products. Response rates to a 48-wk combined pegylated-interferon(PEG-IFN) and ribavirin(RBV) treatment range from 40%-69%, and HCV-GT-4 has been considered better than GT-1 but worse than GT-2 and GT-3 in treatment with PEG-IFN/RBV. However, with the introduction of the HCV-GT-1 effective protease inhibitors boceprevir and telaprevir in 2011, HCV-GT-4 became the "most difficult(GT) to treat". Recently, the direct-acting antivirals(DAAs) with pan- genotypic activities simeprevir, sofosbuvir, and daclatasvir have been recommended in triple regimens with PEG-IFN/RBV for the treatment of HCV-GT-4. An IFN-free regimen will be available for treatment of all genotypes of HCV in the near future. To date, several DAAs have been developed and are currently being evaluated in various combinations in clinical trials. As new regimens and new agents are being approved by the Food and Drug Administration, we can expect the guidelines for HCV treatment to be changed. The availability of shorter, simpler, and more tolerable treatment regimens can reduce the morbidity and mortality associated with HCV infection. With such a large number of therapeutic agents available, we can end up with a range of choices that we can select from

Objective: Nerve blockade is commonly used in oncosurgical procedures as an alternative to general anesthesia for older patients and those with significant medical problems. We report a single tertiary oncology center experience in performing various lip resections and reconstructive techniques using this technique. Patients and methods: sixty patients with lower lip tumors were enrolled into this study with exclusion of refusing, non-compliant and critically ill patients. A solution of 4 ml 2% Lidocaine and epinephrine was put in a 5 ml syringe. Half the solution (2 ml) was injected into each side with a 23 gauge needle and after 5 minutes the surgical resection was carried out after testing for anesthesia. Results: The mean age was 68 ± 6.2 years. The mean ASA score was 3 ± 0.75. There were 4 cases who expressed painful sensation and their operations were completed with fentanyl increments. Mean hospital stay was 1 ± 0.75 days. There were three cases of wound gaping who were treated with secondary closure in an outpatient basis. One patient expressed postoperative acute ischemic heart pain that was managed with anti-ischemic measurements. Conclusion: Bilateral mental nerve block is a safe and effective alternative to general anesthesia in lower lip tumors’ resection especially in older patients and those with poor tolerance for general anesthesia provided that there is no need for cervical nodal dissection.

Urogenital Fistula in Cote d’Ivoire: Epidemiological, Clinical and Therapeutical Aspects

Goal: Contribute to improve treatment of patients suffering from urogenital fistula (UGF). Materials and methods: It was a descriptive and prospective study conducted by the United Nations Population Fund (UNFPA) and the Ivorian government, on treatment of fistulas. The caravan took place on 4 periods of 10 days each and has enabled us to recruit 95 patients in the gynecological emergency department of Bouake’s University Hospital Center (UHC) and from the surgical emergencies departments of the regional hospital centers (RHC) of Man and Bondoukou, in Man from 14th to 23rd of August 2013, then from 24th March to 02nd of April 2014;in Bouaké from 06th to 15th of November 2013 and in Bondoukou from 13th to 22nd of December 2013, respectively. Results: Average age of patients was of 30.8 years. 50 patients were single (52.6%). 44.2% of patients were farmers. In 95.8% of cases it was about obstetrical fistulas of which 52.6% were caused by caesareans. Average duration of delivery labor was of 2.3 days. Vesicovaginal fistulas represented 65.3% of UGF. Suture split were carried out on 90 patients (94.7%). Within 01 month, success rate was of 63.9% for 83 known patients. Conclusion: Incidence of UGF in our country is certainly underestimated. They are mainly of obstetrical origin and treatment relies on sutures split which give satisfactory results.

背景:结核病的诊断需要多次就诊。减少诊断过程中的就诊次数,可使服务更具可及性,并大大节约患者的费用。目的:描述患者在肺结核诊断中心就诊时发生的直接费用。方法:在也门和尼泊尔,使用设计好的问卷,对咳嗽时间超过3周的成年人进行面对面调查,并记录他们的费用。结果:共调查了456例成年人。大多数患者就诊时都有人陪同,其中20%是涂阳患者。在尼泊尔,患者男性比例、居住在城市地区比例和平均年龄均高于也门(尼泊尔:123/206[60%],152[74%],平均年龄41岁;也门:120/250[48%],114[36%],平均年龄35岁)。虽然农村地区的患者多数住在亲戚家,但他们的整体费用仍高于城市地区的患者。在2个国家,最高的费用支出均为诊疗费,而且农村患者支付的诊疗费高于城市患者。诊断的费用相当于1周的人均国民收入。结论:患者需要承担相当大的诊断费用,而在医疗机构的诊疗费是这些费用的主要部分。需要接受结核病检查的患者应该能够得到免费的诊断服务。
