Evaluation of the versatility of perforators-enhanced rhomboid flap technique for ischial sores reconstruction

Background: Prolonged pressure on the skin known as pressure sores, can cause frequent injuries to the skin and underlying tissues. However, several prevention approaches, including non-surgical and surgical management,are available. In flap surgery for pressure sores, a variety of flap types may be used; each offers certain benefits and some incidences of complications and recurrence. This study evaluated the versatility of the perforatorenhanced rhomboid flap technique for ischial sore ***: This prospective study was conducted in the Plastic Surgery Department of Fayoum University *** perforator-enhanced rhomboid flap technique was employed in 20 patients with ischial pressure sores who failed to respond to conservative treatment. A proper history was obtained and a preoperative arterial duplex was performed to determine the site of the prominent perforators for their preservation at the base of the *** follow-ups and evaluations were ***: The mean age of the study group was 43.7 ± 13.1, ranging from 19 to 65 years. Moreover, 80% of the participants were males while 20% were female. The main etiology of the sores was paraplegia in 45% of *** mean surface area of sores was 24.8 ± 6.3 cm2. In 75% of cases, the color match of the flap was rated as excellent. Similarly, 70% of the patients rated flap thickness as excellent. In 55% of the cases, the scar appearance was deemed excellent. Overall satisfaction was reported as excellent by 40% of the patients, while only 5% reported poor satisfaction. Assessment of plant diversity of endemic species of the Saharo-Arabian region in Egypt

Savanna, semi-deserts, and hot deserts characterize the Saharo-Arabian region, which includes Morocco, Mauretania, Algeria, Tunisia, Libya, Egypt, Palestine, Kuwait, Saudi Arabia, Qatar, Bahrain, the United Arab Emirates, Oman, Yemen, southern Jordan, Syria, Iraq, Iran, Afghanistan, Pakistan, and northern India. Its neighboring regions, the Sudano-Zambezian region belonging to the Paleotropical Kingdom and the Mediterranean and Irano-Turanian regions included in the Holarctic Kingdom, share a large portion of their flora with the Saharo-Arabian region. Despite the widespread acknowledgment of the region's global importance for plant diversity, an up to date list of the Saharo-Arabian endemics is still unavailable. The available data are frequently insufficient or out of date at both the whole global and the national scales. Therefore, the present study aims at screening and verifying the Saharo-Arabian endemic plants and determining the phytogeographical distribution of these taxa in the Egyptian flora. Hence, a preliminary list of 429 Saharo-Arabian endemic plants in Egypt was compiled from the available literature. Indeed, by excluding the species that were recorded in any countries or regions outside the Saharo-Arabian region based on different literature, database reviews, and websites, the present study has reduced this number to 126 taxa belonging to 87 genera and 37 families. Regarding the national geographic distribution, South Sinai is the richest region with 83 endemic species compared with other eight phytogeographic regions in Egypt, followed by the Isthmic Desert(the middle of Sinai Peninsula, 53 taxa). Sahara regional subzone(SS1) distributes all the 126 endemic species, Arabian regional subzone(SS2) owns 79 taxa, and Nubo-Sindian subzone(SS3) distributes only 14 endemics. A dynamic analysis of both twisting and regular towers is carried out to determine the results of considering soil-structure interaction(SSI)on high-rise *** addition,the difference between the seismic performance of using twisting towers over regular ones is *** twisting tower is a simulation of the Evolution Tower(Moscow).The towers’skeletons consist of RC elements and rest on a reinforced concrete piled-raft *** soil model is considered as multi-layered with the same soil properties as the zone chosen for the analysis(New Mansoura City,Egypt).The only difference between both towers is their shape in *** whole system is modelled and analyzed in a single step as one full 3D model,which is known as the direct approach in *** analyses are carried out using finite-element software(Midas GTS NX).Dynamic output responses due to three records of seismic loads are proposed and presented in some *** on the results,it is concluded that SSI has a considerable effect on the dynamic response of tall buildings mainly because of the foundation flexibility,as it leads to lengthening the vibration period,increasing the story drift and the base shear for both cases.

Parathyroid Hormone as a Marker for the Red Cell Fragility in Different Stages of Chronic Kidney Disease

Objective: The aim of the work is to study the relationship between Red blood cell osmotic fragility and level of parathyroid hormone in patients with different stages of Chronic Kidney Disease including End Stage Renal Disease. Background: Anaemia is one of the common complications associated with Chronic Kidney Disease (CKD) responsible for the increase in the morbidity and mortality in such patients. Several factors have been attributed to causing renal anaemia, amongst which hyperparathyroidism is one of the less recognised reasons. The level of PTH in early stages of chronic kidney disease has not been much studied. The excess amount of Parathyroid Hormone (PTH) secondary to CKD has been suggested to be a causative factor for anaemia. Method: A number of chronic kidney disease patients were studied for the relationship between Red cell osmotic fragility and level of parathyroid hormone. Results: This study was conducted on a number of 111 patients with chronic kidney disease classified into three groups. The study revealed a significant fall in Hb%, along with a rise in Median Osmotic Fragility (MOF) and PTH in the CKD patients. iPTH and MOF were significantly lower in group 3 as compared with cases in group 1. Also, iPTH and MOF were significantly lower in cases in group 2 as compared with cases in group 1. Conclusions

Standardization of meibomian gland dysfunction in an Egyptian population sample using a non-contact meibography technique

AIM:To develop normative data for meibomian gland dysfunction(MGD)parameters,using non-contact meibography technique of Sirius Costruzione Strumenti Oftalmici(CSO)machine,in an Egyptian population ***:Observational,cross-sectional,analytic study,in which 104 Egyptian volunteers were *** upper lids were examined,using“Sirius CSO”*** eyelid was given a degree of meibomian gland loss(MGL),which was calculated by the software of the ***:Mean percentage MGL in right upper lid was of 30.9%±12.6%,and that of left upper lid was 32.6%±11.8%.Thirty-four volunteers(32.7%)had first-degree MGL in their right upper lid,and 67.3%had second-degree *** volunteer(1%)had zero-degree MGL in left upper lid,28(26.9%)had first-degree loss,and 75(72.1%)had second-degree *** of MGL in right upper eyelid was not related to age,but degree of MGL in left upper eyelid increased with *** was statistically significant difference between both genders for degree of MGL in right eye(P=0.036)and in left eye(P=0.027).CONCLUSION:Noncontact meibography is a useful non-invasive tool for diagnosing *** is diagnosed in 100%of apparently normal individuals;26.9%-32.7%of which have first-degree MGL,and 67.3%-72.1%have second-degree MGL.

Memory loss and dementia are major public health concerns with a substantial economic *** stress has been shown to play a crucial role in the pathophysiology of hippocampal damage-induced memory *** investigate whether the antioxidant and anti-inflammatory compound vanillyla cetone(zingerone) can protect against hippocampal damage and memory loss induced by cadmium chloride(CdCl_(2)) administration in rats,we explo red the potential involvement of the nuclear factor erythroid 2-related factor 2(Nrf2) signaling pathway,which is known to modulate oxidative stress and *** healt hy male Wistar rats were divided into five groups:vehicle-treated(control),vanillylacetone,CdCl_(2),vanillylacetone+ CdCl_(2),vanillylacetone+ CdCl_(2)+ brusatol(a selective pharmacological N rf2inhibitor) *** effectively attenuated CdCl_(2)-induced damage in the dental gyrus of the hippocampus and improved the memory function assessed by the Morris Water Maze ***,vanillylacetone markedly decreased the hippocampal tissue levels of inflammatory biomarkers(interleukin-6,tumor necrosis factor-α,intracellular cell adhesive molecules) and apoptosis biomarkers(Bax and cleaved caspase-3).The control and CdCl_(2)-treated groups treated with va nillylacetone showed reduced generation of reactive oxygen species,decreased malondialdehyde levels,and increased superoxide dismutase and glutathione activities,along with significant elevation of nuclear Nrf2 mRNA and protein expression in hippocampal *** the protective effects of vanillylacetone we re substantially blocked by the co-administration of brusatol(a selective N rf2 inhibitor).Va nillylacetone mitigated hippocampal damage and memory loss induced by CdCl_(2),at least in part, by activating the nuclear transcription factor ***,vanillylacetone exerted its potent antioxidant and antiinflammatory actions.

Multiphysics simulation of VVER-1200 fuel performance during normal operating conditions

Nuclear fuel performance modeling and simulation are critical tasks for nuclear fuel design optimization and safety analysis under normal and transient *** performance is a complicated phenomenon that involves thermal,mechanical,and irradiation mechanisms and requires special multiphysics *** this study,a fuel performance model was developed using the COMSOL Multiphysics *** modeling was performed for a 2D axis-symmetric geometry of a UO2fuel pellet in the E110 clad for VVER-1200 *** modeling considers all relevant phenomena,including heat generation and conduction,gap heat transfer,elastic strain,mechanical contact,thermal expansion,grain growth,densification,fission gas generation and release,fission product swelling,gap/plenum pressure,and cladding thermal and irradiation *** model was validated using a code-to-code evaluation of the fuel pellet centerline and surface temperatures in the case of constant power,in addition to validation of fission gas release(FGR)*** prediction proved that the model could perform according to previously published VVER nuclear fuel performance parameters.A sensitivity study was also conducted to assess the effects of uncertainty on some of the model *** model was then used to predict the VVER-1200 fuel performance parameters as a function of burnup,including the temperature profiles,gap width,fission gas release,and plenum pressure.A compilation of related material and thermomechanical models was conducted and included in the modeling to allow the user to investigate different material/performance *** the model was developed for normal operating conditions,it can be modified to include off-normal operating conditions.

Numerical Study of Flow and Gas Diffusion in the Near-Wake behind an Isolated Building

To assist validation of the experimental data of urban pollution dispersion, the effect of an isolated building on the flow and gaseous diffusion in the wake region have been investigated numerically in the neutrally stratified rough-walled turbulent boundary layer. Numerical studies were carried out using Computational Fluid Dynamics （CFD） models. The CFD models used for the simulation were based on the steady-state Reynolds-Average Navier-Stoke equations （RANS） with κ-ε turbulence models; standard κ-ε and RNG κ-ε models. Inlet conditions and boundary conditions were specified numerically to the best information available for each fluid modeling simulation. A gas pollutant was emitted from a point source within the recirculation cavity behind the building model. The accuracy of these simulations was examined by comparing the predicted results with wind tunnel experimental data. It was confirmed that simulation using the model accurately reproduces the velocity and concentration diffusion fields with a fine-mish resolution in the near wake region. Results indicated that there is a good agreement between the numerical simulation and the wind tunnel experiment for both wind flow and concentration diffusion. The results of this work can help to improve the understanding of mechanisms of and simulation of pollutant transport in an urban environment.

Price Prediction of Seasonal Items Using Machine Learning and Statistical Methods

Price prediction of goods is a vital point of research due to how common e-commerce platforms *** are several efforts conducted to forecast the price of items using classicmachine learning algorithms and *** models can predict prices of various financial instruments,e.g.,gold,oil,cryptocurrencies,stocks,and second-hand *** these efforts,the literature has no model for predicting the prices of seasonal goods(e.g.,Christmas gifts).In this context,we framed the task of seasonal goods price prediction as a regression ***,we utilized a real online trailer dataset of Christmas gifts and then we proposed several machine learningbased models and one statistical-based model to predict the prices of these seasonal ***,we utilized a real-life dataset of Christmas gifts for the prediction ***,we proposed support vector regressor(SVR),linear regression,random forest,and ridgemodels as machine learningmodels for price ***,we proposed an autoregressive-integrated-movingaverage(ARIMA)model for the same purpose as a statistical-based ***,we evaluated the performance of the proposed models;the comparison shows that the best performing model was the random forest model,followed by the ARIMA model.
