Background: Obesity is a major risk factor for endometrial carcinoma, and we aim to assess markers of carcinogenesis including PTEN and Ki-67 and hormone receptors profile including ER, PR and AR before and after bariatric surgery to find out its effects in reducing endometrial carcinoma risk in morbid obese females. Patients and methods: The study included 80 females with morbid obesity (BmI > 40 Kg/m2) who underwent bariatric surgery. all were sampled by Pipelle biopsy at baseline and 12 months after operation and examined histopathologically and immunohistochemically for Ki-67, PTEN, ER, PR and AR. Results: Sixty two out of 80 (62/80) females showed no pathological abnormalities;4 had polyps;7 had simple endometrial hyperplasia;4 had atypical endometrial hyperplasia and 3 had endometrial carcinoma. In total, 34 females underwent gastric bypass operation (42.5%) and 46 underwent a sleeve gastrectomy operation (57.5%). There was a statistically significant difference between baseline weight and BmI before and after surgery (p men with simple hyperplasia, resolution occurred in 5 within 7 months of surgery. Three of 4 females with atypical hyperplasia (AH) showed resolution after 9 months. mean Ki-67 score was lower at 12 months (p men with baseline endometrial abnormalities, of whom 5/15 regained glandular PTEN expression as their endometrial abnormalities resolved. Background: Urosepsis is one of the most common infections that require empirical broad spectrum antibiotics immediately after diagnosis. This has led to development of bacterial resistance by acquiring the capability to destroy the β-lactam ring. methodology: This is a cross-sectional hospital-based study. The study was conducted from 2019 to 2020 at Gezira Hospital for Renal diseases and surgery (GHRDS). A hundred patients were diagnosed clinically with urosepsis and the isolated organisms were Escherichia coli, Staphylococcus aureus, Proteus mirabilis, Klebsiella pneumonia and Pseudomonas aeruginosa. The susceptibility test was conducted by Kirby Bauer disc diffusion technique according to clinical laboratory standard institute (CLSI) guidelines. Seventy eight samples of bacterial genomic DNA were confirmed by 16srRNA and multiplex PCR, were performed for genotypic blaOXA-51 and blaOXA-23 gene characterization of isolated bacteria. Then gel electrophoresis was used to identify the presence or absence of (blaOXA-51 and blaOXA-23) genes. Results: 88.5% (69/78) in 16srRNA detected. Using multiplex PCR, the frequencies of blaOXA-51 and blaOXA-23 genes were 13% and 10.1%, respectively. The percentages of isolates which yielded both blaOXA-51 and blaOXA-23 among P. aeruginosa was 25% (1/4), among K. pneumonia was 17% (1/6), and among E. coli was 8% (3/37). Only blaOXA-51 was detected in P. mirabilis 10% (1/10) and only blaOXA-23 was detected in S. aureus 5% (1/18). Conclusion: In this study, the presence of blaOXA-51 and blaOXA-23 genes was increased in the isolated bacteria.

Goal: Contribute to improve treatment of patients suffering from urogenital fistula (UGF). materials and methods: It was a descriptive and prospective study conducted by the United Nations Population Fund (UNFPA) and the Ivorian government, on treatment of fistulas. The caravan took place on 4 periods of 10 days each and has enabled us to recruit 95 patients in the gynecological emergency department of Bouake’s University Hospital Center (UHC) and from the surgical emergencies departments of the regional hospital centers (RHC) of man and Bondoukou, in man from 14th to 23rd of August 2013, then from 24th march to 02nd of April 2014;in Bouaké from 06th to 15th of November 2013 and in Bondoukou from 13th to 22nd of December 2013, respectively. Results: Average age of patients was of 30.8 years. 50 patients were single (52.6%). 44.2% of patients were farmers. In 95.8% of cases it was about obstetrical fistulas of which 52.6% were caused by caesareans. Average duration of delivery labor was of 2.3 days. Vesicovaginal fistulas represented 65.3% of UGF. Suture split were carried out on 90 patients (94.7%). Within 01 month, success rate was of 63.9% for 83 known patients. Conclusion: Incidence of UGF in our country is certainly underestimated. They are mainly of obstetrical origin and treatment relies on sutures split which give satisfactory results.

Objective: Nerve blockade is commonly used in oncosurgical procedures as an alternative to general anesthesia for older patients and those with significant medical problems. We report a single tertiary oncology center experience in performing various lip resections and reconstructive techniques using this technique. Patients and methods: sixty patients with lower lip tumors were enrolled into this study with exclusion of refusing, non-compliant and critically ill patients. A solution of 4 ml 2% Lidocaine and epinephrine was put in a 5 ml syringe. Half the solution (2 ml) was injected into each side with a 23 gauge needle and after 5 minutes the surgical resection was carried out after testing for anesthesia. Results: The mean age was 68 ± 6.2 years. The mean ASA score was 3 ± 0.75. There were 4 cases who expressed painful sensation and their operations were completed with fentanyl increments. mean hospital stay was 1 ± 0.75 days. There were three cases of wound gaping who were treated with secondary closure in an outpatient basis. One patient expressed postoperative acute ischemic heart pain that was managed with anti-ischemic measurements. Conclusion: Bilateral mental nerve block is a safe and effective alternative to general anesthesia in lower lip tumors’ resection especially in older patients and those with poor tolerance for general anesthesia provided that there is no need for cervical nodal dissection.

Retained placenta is a known cause of post-partum haemorrhage and maternal mortality. A recent systemic review has confirmed that the incidence of retained placenta had increased all over the world, which is more common in developed countries. Failure of retroplacental myometrium contraction is the main cause of retained placenta. maternal age greater than 35 years, grandmultipara, preterm labor, history of previous retained placenta, and caesarean section were the risk factors for retained placenta. manual removal of the placenta has been the treatment of choice. Attempts had been made by clinician and researchers to find a safe, effective and reliable method to avoid the need for surgical intervention. The efficacy and safety of prostaglandin, nitroglycerin or acupuncture in the management of retained placenta are yet to be further evaluated. Nonetheless, till date only intraumbilical vein oxytocin has been studied extensively but with varied success. more randomized clinical trials are needed to address this issue. However, if immediate manual placenta removal service is unavailable, a trial of intra-umbilical vein oxytocin 100 IU at a totalvolume of at least 40 m L while preparing for transfer to a tertiary center or theatre may result in spontaneous expulsion of the placenta.

背景:结核病的诊断需要多次就诊。减少诊断过程中的就诊次数,可使服务更具可及性,并大大节约患者的费用。目的:描述患者在肺结核诊断中心就诊时发生的直接费用。方法:在也门和尼泊尔,使用设计好的问卷,对咳嗽时间超过3周的成年人进行面对面调查,并记录他们的费用。结果:共调查了456例成年人。大多数患者就诊时都有人陪同,其中20%是涂阳患者。在尼泊尔,患者男性比例、居住在城市地区比例和平均年龄均高于也门(尼泊尔:123/206[60%],152[74%],平均年龄41岁;也门:120/250[48%],114[36%],平均年龄35岁)。虽然农村地区的患者多数住在亲戚家,但他们的整体费用仍高于城市地区的患者。在2个国家,最高的费用支出均为诊疗费,而且农村患者支付的诊疗费高于城市患者。诊断的费用相当于1周的人均国民收入。结论:患者需要承担相当大的诊断费用,而在医疗机构的诊疗费是这些费用的主要部分。需要接受结核病检查的患者应该能够得到免费的诊断服务。
