Background: Urosepsis is one of the most common infections that require empirical broad spectrum antibiotics immediately after diagnosis. This has led to development of bacterial resistance by acquiring the capability to destroy the β-lactam ring. methodology: This is a cross-sectional hospital-based study. The study was conducted from 2019 to 2020 at Gezira Hospital for Renal diseases and surgery (GHRDS). A hundred patients were diagnosed clinically with urosepsis and the isolated organisms were Escherichia coli, Staphylococcus aureus, Proteus mirabilis, Klebsiella pneumonia and Pseudomonas aeruginosa. The susceptibility test was conducted by Kirby Bauer disc diffusion technique according to clinical laboratory standard institute (CLSI) guidelines. Seventy eight samples of bacterial genomic DNA were confirmed by 16srRNA and multiplex PCR, were performed for genotypic blaOXA-51 and blaOXA-23 gene characterization of isolated bacteria. Then gel electrophoresis was used to identify the presence or absence of (blaOXA-51 and blaOXA-23) genes. Results: 88.5% (69/78) in 16srRNA detected. Using multiplex PCR, the frequencies of blaOXA-51 and blaOXA-23 genes were 13% and 10.1%, respectively. The percentages of isolates which yielded both blaOXA-51 and blaOXA-23 among P. aeruginosa was 25% (1/4), among K. pneumonia was 17% (1/6), and among E. coli was 8% (3/37). Only blaOXA-51 was detected in P. mirabilis 10% (1/10) and only blaOXA-23 was detected in S. aureus 5% (1/18). Conclusion: In this study, the presence of blaOXA-51 and blaOXA-23 genes was increased in the isolated bacteria.

Optimizing Adjuvant Radiation Planning Outcomes in Patients with Synchronous Bilateral Breast Cancer

Background: Breast cancer is the most common cancer diagnosed worldwide, synchronous bilateral breast cancer accounts for unique entity of the disease, particularly post-operative radiotherapy for Synchronous Bilateral Breast Cancer (SBBC) is challenging with lack of evidence about the best irradiation technique. In this study, we tried to explore the optimum radiotherapy technique regarding the dosimetric parameters. Methods: We recruited 15 SBBC patients in whom post-operative radiotherapy was indicated and we established three plans for each patient using 3DCRT, IMRT and VMAT, and then we compared the three plans as regard target volume coverage parameters and organs at risk (OAR) doses. Results: We found that PTV coverage parameter was superior with IMRT compared with 3DCRT and VMAT in terms of Dmean (p = 0.001), D95% (p = 0.001), Dmax (p = 0.0001), conformity index (p = 0.0001) and HI (p = 0.0001). Extrinsic Risk Factors for Women Breast Cancer in Gaza Strip, Palestine: Associations and Interactions in a Case-Control Study

Background: Worldwide, breast cancer (BC) is the most frequently diagnosed neoplastic disease in women around menopause that is the leading cause of DalYs, because it causes a significant reduction of these women’s ability to function normally in everyday life. methods: The present hospital-based case-control study was carried out between January and August 2018 using a structured questionnaire on 105 breast cancer women and 210 controls who are clinically free from breast cancer. Data about the study cases were collected in the oncology day-care clinics of the two main hospitals in Gaza strip “al-Shifa and European Gaza hospitals”. Univariate logistic regression and multivariate logistic regression analyses were employed to identify the significant factors associated with BC. Results: multivariate logistic regression analyses with adjustment for all confounders revealed that woman with BmI more than or equal 30 kg/m2 are under risk of getting BC 2.9 times greater than those having BmI less than 29 kg/m2 (AOR = 2.895;95% CI: 1.305 - 6.423). Analysis of risk according to reaching menopause showed that the estimated AOR was greater among those reached menopause (3.137, 95% CI 1.824 - 5.395) than among those that did not reach menopause. The risk of developing BC in the case of a history of incidence of BC in the family was more than two times higher (AOR = 2.632, 95% CI 1.528 - 4.535) than in case of the history of a free family of BC. Conclusion: In this context, the above-mentioned risk factors must be taken into consideration in BC management processes in the Gaza strip.

Amyloid goiter (AG) is a rare but well-established disease entity that may occur in a number of conditions. In the following article, we will report two cases of AG. Both patients were young males: 28 & 24 years old, presented with rapidly enlarging thyroid gland manifested with pressure effects (dyspnea and hoarseness of voice). Provisional clinical diagnosis was malignant thyroid neoplasm. One of the patients was markedly hypothyroid while the other was euthyroid. Histopathological evaluation revealed extracellular deposition of deep pink homogenous material that was confirmed as amyloid with congo red. Patient in case 1 was primary localized AG while patient in case 2 was systemic amyloidosis secondary to interstitial pulmonary fibrosis (IPF) that was first manifested by AG. The main aims of the article were to describe histopathological features of amyloidosis of the thyroid gland and to raise awareness of AG to be included in the differential diagnosis in patients presented with rapidly enlarging goiter with mass effects.

men Urethra Strictures: Findings in Urethroplasties Care at the Andrology and Urology Department of Grand Yoff General Hospital in Dakar

Objectives: To report the experiment conducted at the HOGGY Urology department in the management of urethral stricture by urethroplasty, and to determine the factors that influence the results. material and method: We conducted a descriptive and analytical retrospective study based on the records of patients who underwent urethroplasty in the department, between February 2001 and September 2013. Results: Ninety-one (91) patients were enrolled. Urethroplasties prevalence was 0.83% of the surgical activity of the service. The mean age of patients was 39.83 years. Dysuria (30.77%) followed by pelvic trauma (28.57%) and urinary retentions (25.27%) was the main discovery mode. A periurethral coating was found in 32 patients. The infectious etiology accounted for 44% of cases. In 63% of cases, diagnostic was made by retrograde cystography. The penile urethra was the favorite seat of the UR in 70% of cases. The average length of the urethral stricture (US) was less than 1 cm in 41.17% of cases. The US was unique in more than half of the cases (58.33%). Anastomotic urethroplasty was the best surgical technique with 73.63% of patients. Postoperative morbidity involved 47 patients and was dominated by urinary infections (36 year old). The average duration of follow-up of operated patients was 29 months. After 6 months of follow-up, the best results were obtained with the termino-terminal urethroplasty technique with 62.5%. After a follow-up of 4 years, the success rate was 58.24%. The length of the stenosis and the allocation of gestures on the urethra were the two factors of failure. Conclusion: Stenosis is common in our regions. Treatment results are disappointing. Urethroplasty is the gold standard of surgical treatment and anastomotic urethroplasty gives better results.

AIm:To identify possible maternal risk factors for hepatitis B virus(HBV)acquisition and assess the efficacy of immunoprophylaxis given to infants born to hepatitis B virus surface antigen(HBs Ag)positive ***:Screening of 2000 pregnant females wascarried out using rapid test and confirmed by enzyme immunoassay.A questionnaire consisting of 20 questions about the possible risk factors for acquisition of HBV infection was filled for every pregnant HBs Ag positive female in addition to at least 2 pregnant HBs Ag negative females for each positive *** of HBs Ag positive women were offered passive and active immunoprophylaxis within the 1st 48 h after birth,in addition to 2nd and 3rd doses of HBV vaccine after1 and 6 mo *** were tested for HBs Ag and hepatitis B surface antibodies(HBs Ab)at six months of ***:HBs Ag was confirmed positive in 1.2%of tested pregnant *** factors significantly associated with HBV positivity were;history of injections(OR=5.65),history of seeking medical advice in a clinic(OR=7.02),history of hospitalization(OR=6.82),history of surgery(OR=4)and family history of hepatitis(OR=3.89)(Pmen whose rapid test was not confirmed and could not be reached to provide immunoprophylaxis for thier *** failure was detected in only one newborn(3.7%)who tested positive for HBs Ag at 6 mo of age;and vaccine failure(seronegative to HBs Ab after 4 doses of the vaccine)was detected in another one(3.7%).The success rate of the immunoprophylaxis regimen was 92.6%.CONCLUSION:This pilot study shows that a successful national program for prevention of perinatal transmission of HBV needs to be preceded by an awareness campaign to avoid a high dropout rate.

Experimental Study of Acoustic Noise Correlation Technique for Passive monitoring of Rails

The work presented in this paper aims at investigating the ability of acoustic noise correlation technique for railway infrastructure health monitoring. The principle of this technique is based on impulse responses reconstruction by correlation of random noise propagated in the medium. Since wheel-rail interaction constitutes a source of such noise, correlation technique could be convenient for detection of rail defects using only passive sensors. Experiments have been carried out on a 2 m-long rail sample. Acoustic noise is generated in the sample at several positions. Direct comparison between an active emission-reception response and the estimated noise correlation function has confirmed the validity of the equivalence relation between them. The quality of the reconstruction is shown to be strongly related to the spatial distribution of the noise sources. High sensitivity of the noise-correlation functions to a local defect on the rail is also demonstrated. However, interpretation of the defect signature is more ambiguous than when using classical active responses. Application of a spatiotemporal Fourier transform on data recorded with variable sensor-defect distances has allowed overcoming this ambiguity.

Existing telerehabilitation systems require highly experienced personnel to interpret a patient’s muscle activity in a specialized lab,limiting the biofeedback available at the patient’s *** addition,there is no contact between physiotherapists and patients *** study designed and implemented an interactive teleelectromyography(EmG)biofeedback device for facilitating patients’rehabilitation and giving the physiotherapist the ability to develop and monitor personalized treatment for each patient precisely and *** device comprised a device that acquires EmG signals during muscular activity and two cell phone applications,one for the patient and the other for the physiotherapist;traced muscle activity;and enabled the physiotherapist to monitor and control the patients’sessions through a Google Firebase *** signals were acquired from five subjects,and results were compared to a certified *** evaluation process involved two metrics,correlation coefficient and root mean square value(RmSV).The proposed EmG biofeedback device is an integrated solution with numerous features,such as low cost,handheld size,patient-friendly interface,contributions to rapid healing,and secure *** addition,it provides raw EmG signals for applied research applications.

Background: Most community and hospital acquired infections are due to Staphylococcus aureus, these infections involve both local suppurative and systemic conditions. There is an increase in the incidence rates of acquiring resistant strains of Staphylococcus aureus in hospitals and community in Sudan. This study aims to detect frequency of Staphylococcus aureus isolated from surgical-sites infections (SSIs) and traumatic wounds, and to determine susceptibility to commonly used drugs. Methods: Prospective cross-sectional laboratory based study was followed, 250 patients admitted to Surgery Departments at Wad Madani Teaching Hospitals during 2019 and 2020 were recruited;with clinical manifestation of SSIs and or/traumatic wounds. Swab sample was taken from each participant and incubated aerobically on mannitol salt agar. Biochemical tests and 16rRNA gene used for Staphylococcus aureus identification. Antimicrobial susceptibility was accomplished using Kirby Bauer disc diffusion technique.
