Aim: To determined risk of increases Incidence of Thyroid cancer in solitary thyroid nodules so Preoperative distinction between benign and malignant in solitary thyroid nodules is important and helps to avoid unnecessary surgery and its adverse effects, such as hypothyroidism, hypocalcemia, and recurrent nerve injury. methods: Descriptive perspective analyzed data over a period of 6 years April 2015-April 2021 in Saudi Hospital at Hajjah, Yemen. 226 thyroid operations for 207 patients, 135 patient’s diagnosis as Solitary thyroid nodule and 72 patients as multi nodular goiter. Patients with a clinically solitary thyroid nodule were included in the study group. Results: 135 cases of clinically detected solitary thyroid nodules, 126 female and 9 male patients, between 14 - 65 years age, median 41 years and mean 39.76 years, (94, 41) patients respectively Right side thyroid effect more than Left side, Fine needle aspiration cytology (FNAC) sensitivity, specificity and accuracy was (61%, 72%, 64%) respectively. Postoperative histopathology reported 100 (74%) patients as having benign thyroid nodules and 35 patients (26%) as having malignant thyroid nodules. Postoperative transient hypocalcemia was observed in 9 patients (7%), and temporary hoarseness was observed in 3 patients (2%). Conclusion: The incidence of malignancy in solitary thyroid nodules is high. Rapid growth by history and hard fixed nodule by examination and hypoechoic, micro calcification and cervical lymphadenopathy on Thyroid Ultrasound frequently in malignant nodules. male risk factors for thyroid cancer but age, number and size of nodules not included as risk factor. Fine needle aspiration cytology (FNAC) is more helpful for diagnosing if aspiration under Ultrasound guidance and reading by experience histopathologists. Adjuvant chemoradiotherapy versus surgery alone for gastric adenocarcinoma

Objective:Despite resection with curative intent,a majority of patients with gastric cancer will develop disease *** adjuvant chemo-radiotherapy increase the curability of surgery,prevent local recurrence and improve ***:Between December 2005 and February 2010,33 patients were eligible for the study,17 patients were randomly assigned for chemo-radiotherapy (GI) and 16 patients with surgery alone (GII).Patients in GI received chemotherapy (fluorouracil,425 mg/m 2/day,and leucovorin,20 mg/m 2/day,for 5 days) was initiated on day 1 and was followed by chemo-radiotherapy beginning 28 days after the start of the initial cycle of ***-radiotherapy consisted of 4500 cGy of radiation at 180 cGy/day,five days/week for five weeks,with fluorouracil (400 mg/m 2/day) and leucovorin (20 mg/m 2/day) on the first four and the last four days of *** month after the completion of radiotherapy,two five-day cycles of fluorouracil (425 mg/m 2/day) plus leucovorin (20 mg/m 2/day) were given one month ***:Grade 3 gastrointestinal toxicity was more common (23.2%) while grade 3 hematological toxicity were (5.8%).Both 3-year survival (53%) and disease free survival (41%) rates were higher in GI than in GII in which they were 43.7% and 31% *** rate was higher in GII (56.3%) than in GI (35.3%).Conclusion:The present study revealed that chemo-radiotherapy after gastric resection in patients with gastric adenocarcinoma improves survival and relapse rates with manageable ***,studies with larger number of patients are recommended to confirm our results.

L-Arginine Supplementation Alters maternal Blood Biochemical Attributes and milk Composition Relative to Neonatal Traits of <i>Najdi</i>Ewes

In a 2 × 3 factorial arrangement, 3 levels of L-arginine (0, 75 and 150 mg/kg BW per day) were tested at two stages of pregnancy (Early 60 days vs. Late 60 days) on the maternal blood metabolites and postnatal milk composition relative to the newborn traits of Najdi ewes. Thirty adult pregnant Najdi ewes were randomly assigned into 6 treatments (n = 5). Control early (CE) ewes were orally administered 50 mL normal saline (0.9% NaCl) daily during the first 60 days of pregnancy;control late (CL) ewes were similar to CE except saline was given at the last 60 days of pregnancy;low dose of L-arginine at early pregnancy (LE);low dose of L-arginine at late pregnancy (LL);high dose of L-arginine at early (HE) and high dose of L-arginine at late pregnancy (HL). Blood and milk samples were collected on a weekly basis. Data of lambing and newborn traits were considered. There were non-significant differences in litter size due to L-arginine;however lamb birth and weaning weights increased by 35% in LE-ewes. Arginine did not change blood protein and albumin. Low arginine decreased lipid fractions, creatinine, urea, and milk fat;however, it increased AST, ALT, milk lactose and milk ash. Administration of L-arginine at 75 mg/Kg BW per day for 60 days at an early gestation in ewes improved the maternal body health and milk composition for the welfare of the neonates.

AIm: To analyze experiences to identify treatment outcomes and prognostic factors in a Saudi ***: medical records of patients with brainstem gliomas treated from July 2001 to December 2012 were reviewed to identify treatment outcomes of surgery, radiation therapy and chemotherapy and associated prognostic factors in a Saudi ***: We analyzed 49 brain stem glioma(BSG) patients from July 2001 to December 2012; 31 of them were males(63.3%) with a median age of 12.6 years(range: 8-64 mo). Twenty-two patients(44.9%) had diffuse intrinsic pontine gliomas(DIPG) and 15(30.6%) presented with focal/tectal BSG. Histopathology was available in 30 patients(61.2%). median survival time for the whole cohort was 1.5 years. One and two year OS rates were 51.1% and 41.9% respectively. Two year OS rates for focal/tectal, dorsally exophytic, cervicomedullary and DIPG tumors were 60%, 33.3%, 33.3% and 13.6% respectively(P 18 years) P = 0.01, KPS mptoms(maximal resection) P = 0.002, and concurrent chemotherapy with radiation therapy in DIPG(better if given) P = ***: BSG, especially the DIPG subgroup, had a dismal prognosis, needing more aggressive neurosurgical, radiation and chemotherapy techniques, while focal and tectal tumors were found to have a better prognosis.

AIm: To assess the diagnostic accuracy,of aminotransferase-to-platelet ratio index(APRI) alone and with antischistosomal antibody(Ab) in patients with hepatitis C virus(HCV) and schistosomiasis coinfection. mETHODS: This retrospective study included medical records of three hundred and eighty three Egyptianmen patients who had undergone percutaneous liver biopsy between January 2006 to April 2014 in tertiary care hospital in Qatar for diagnosis or monitoring purpose were selected. Data of patients > 18 years of age were included in the study. The values of HCV RNA titer and antischistosomal antibody titer were also taken into consideration. Patients were excluded from the study if they had any other concomitant chronic liver disease,including; history of previous antiviral or interferon therapy,immunosuppressive,therapy,chronic hepatitis B infection,human immunodeficiency virus co-infection,autoimmune hepatitis,decompensated liver disease,hepatocellular carcinoma,prior liver transplantation,and if no data about the liver biopsy present. RESULTS: median age of patients was 46 years. About 7.1% had no fibrosis,whereas 30.4%,37.5%,20.4%,and 4.6% had fibrosis of stage Ⅰ,Ⅱ,Ⅲ,and Ⅳ respectively. In bivariate analysis,APRI score,levels of AST,platelet count and age of patient showed statistically significant association with liver fibrosis(P mal antibody titer(P = 0.52) and HCV RNA titer(P = 0.79) failed to show a significant association. The respective AUC values for no fibrosis,significant fibrosis,severe fibrosis and cirrhosis of APRI score were 63%,73.2%,81.1% and 88.9% respectively. This showed good sensitivity and specificity of APRI alone for grading of liver fibrosis. But the inclusion of anti-Schistosoma antibody did not improve the prediction of fibrosis stage. CONCLUSION: The study results suggest that noninvasive biochemical markers like APRI are sensitive and specific in diagnosing the degree of fibrosis and cirrhosis in patient

After decades of research and development, the WSR-88 D(NEXRAD) network in the United States was upgraded with dual-polarization capability, providing polarimetric radar data(PRD) that have the potential to improve weather observations,quantification, forecasting, and warnings. The weather radar networks in China and other countries are also being upgraded with dual-polarization capability. Now, with radar polarimetry technology having matured, and PRD available both nationally and globally, it is important to understand the current status and future challenges and opportunities. The potential impact of PRD has been limited by their oftentimes subjective and empirical use. more importantly, the community has not begun to regularly derive from PRD the state parameters, such as water mixing ratios and number concentrations, used in numerical weather prediction(NWP) *** this review, we summarize the current status of weather radar polarimetry, discuss the issues and limitations of PRD usage, and explore potential approaches to more efficiently use PRD for quantitative precipitation estimation and forecasting based on statistical retrieval with physical constraints where prior information is used and observation error is included. This approach aligns the observation-based retrievals favored by the radar meteorology community with the model-based analysis of the NWP community. We also examine the challenges and opportunities of polarimetric phased array radar research and development for future weather observation.

Systematic CR-39 bulk etching experiments were conducted over a wide range of concentrations(2–30 N)of NaOH-based *** analysis and a deep discussion of the results are presented.A comprehensive nuclear track chemical etching data bank was *** regimes of CR-39 bulk etching were *** I spans etchant concentrations from 2 to 12 *** II spans concentrations from 12 to 25 *** call this the dynamic bulk etching *** III is for concentrations greater than 25 *** this regime,the bulk etch rate is saturated with respect to the etchant *** classification is discussed and *** role of ethanol in NaOH-based etchants is explored and discussed.A parameter called the “reduced bulk etch rate” is defined here,which helps in analyzing the dependence of bulk etching on the amount of ethanol in the *** bulk etch rate shows a natural logarithmic dependence on the density of ethanol in the etchant.

Generalizing detection performance expressions by incorporating new fading models such as Weibul and generalized Gamma (α-μ) can be accomplished at the expense of mathematical complexity. Non-integer powers combined with exponential or special functions represent a real challenge in evaluating the necessary integrals. In this paper the authors highlight the divergence problems associated with solving complicated integrals by series expansions focusing on detection problems and illustrating the common mistakes encountered when carrying term by term integration. Additionally, the authors collected different mathematical theories to consolidate the sufficient conditions to achieve convergent results.
