The Effect of Tramadol Hydrochloride In-traarticular Injection on IL-6 Level in Patients with Tempromandibular Joint Internal Derangement

Purpose: This study was conducted to detect IL-6 in synovial fluid in cases with TmJ internal derangement. Patients & methods: This study was conducted on forty patients ASA class I with TmJ Internal derangement. All patients had been subjected to arthrocentesis. The synovial fluid was collected before wash and lavage was done for the affected joint. Then, the selected patients were divided randomly into two equal groups, group I: consisted of 20 patients where arthrocentesis was performed for the affected joint followed by intraarticular injection of one ml, tramadol hydrochloride. Group II: consisted of 20 patients where arthrocentesis was performed for the affected joint followed by intraarticular injection of one ml. sodium hyaluronate. Another synovial fluid sample was aspirated after one month. The interlieukin-6 receptors in the aspirated synovial fluid were measured using humans IL-6 enzyme-linked immunosorbent assay. Results: IL-6 was detected in the synovial fluid of joints with internal derangement. During follow up assessment of IL-6, the maximum decrease in IL-6 level was in the patients of group I who subjected to arthrocentesis with tramadol injection, as the mean IL-6 was (4.93 ± 1.36) followed by the patients of group II who subjected to arthrocentesis with sodium hyaluronate injection where the mean IL-6 level was (6.88 ± 1.76). There were significant p-value = (P = 0.000). Conclusions: It had been concluded that the detection of IL-6 in the synovial fluid of joints with internal derangement considered an indicator for inflammatory reaction in the joint and also the efficacy of arthrocentesis with tramadol suggested its anti-inflammatory effect.

Percutaneous devices for left atrial appendage occlusion: A contemporary review

Patient with atrial fibrillation(AF) are at risk of developing stroke with the left atrial appendage(LAA) being the most common site for thrombus formation. If left untreated, AF is associated with 4 to 5 folds increase in the risk of ischemic stroke in all age groups. About 5% to 15% of AF patients have atrial thrombi on transesophageal echocardiography, and 91% of those thrombi are located in the LAA in patient with nonrheumatic AF. Although oral anticoagulants are the gold-standard treatment for stroke prevention in patients with non-valvular AF,some patients are at high risk of bleeding and deemed not candidates for anticoagulation. Therefore, LAA occlusion(LAAO) has emerged as alternative approach for stroke prevention in those patients. Surgical LAAO is associated with high rate of unsuccessful closure and recommended only in patients with AF and undergoing cardiac surgery. Percutaneous LAAO uses transvenous access with trans-septal puncture and was first tested using the PLAATO *** is the most common and only Food and Drug Administration(FDA)approved device for LAAO. LAAO using Watchman device is non-inferior to warfarin therapy in preventing ischemic stroke/systemic ***, it is associated with lower rates of hemorrhagic stroke, bleeding and death. Amplatzer is another successful LAAO device that has CE mark and is waiting for FDA approval. Optimal antithrombotic therapy post LAAO is still under debate and highly patient-specific. The aim of this paper is to systematically review the current literature to evaluate the efficacy and safety of different LAAO devices.

A digraph is a graph in which each edge has an orientation. A linear directed path, , is a path whose all edges have the same orientation. A linear simple graph is called directed cordial if it admits 0 - 1 labeling that satisfies certain condition. In this paper, we study the cordiality of directed paths??and their second power . Similar studies are done for ?and the join ?. We show that ,? and ?are directed cordial. Sufficient conditions are given to the join?? to be directed cordial.

Gamma Irradiation Effect on the Thermal Stability, Optical and Electrical Properties of Acrylic Acid/ methyl methacrylate Copolymer Films

Copolymer films of Acrylic acid/ methyl methacrylate AAc/mmA with comonomer composition of 60/40 was prepared and then irradiated by gamma irradiation. The effect of irradiation on some of the physical properties of the copolymer films was investigated. The dose level ranged from 5 to 200 kGy. The thermo gravimetric analysis (TGA) showed that the thermal stability of the films increased with irradiation dose up to 100 kGy then it started to decrease. The results of the UV-Visible spectroscopy of the films showed a decrease in the values of optical band gap and band tail width with doses starting from 100 kGy. The DC conductivity (?DC) of the films was found to decrease to about three orders of magnitude from its original value with irradiation dose up to 100 kGy then it started to increase with higher doses. moreover, the dielectric constant and dielectric loss values were found to increase with dose up to 100 kGy which may be attributed to the increase in the number of dipoles in the films due to the irradiation of the copolymer films in air. The results indicated that crosslinking dominated over chain scission in the copolymer films with irradiation dose up to 100 kGy then at higher doses, chain scission dominated.

Treatment Outcome of Pharmacokinetics-Based Dosing of Docetaxel and Fluorouracil in Advanced Head and Neck Cancer Patients

Introduction: Docetaxel, Cisplatin and 5-Fluorouracil (DPF) became the standard induction chemotherapy in advanced Head and Neck Cancer (HNC) but associated with high toxicity rate. Several studies reported higher response rates with better tolerability when chemotherapy dose is calculated based on Pharmacokinetics (PK) versus conventional Body Surface Area (BSA). Patients and methods: Thirty nine patients with stage III and IV HNC who received induction DPF were included in the study. Dose of cycle 1 was BSA-based then Docetaxel and 5-FU doses were PK-adjusted starting from cycle 2 whereas Cisplatin dose was BSA-based throughout the study. Results: After median follow up period of 14 months the median overall survival (OS) and progression free survival (PFS) were 15.1 and 10.6 months respectively. Twenty nine patients were available for response assessment. Seven patients (24.1%) achieved complete response while partial response encountered in 19 patients (65.5%) with and Overall response rate of 89.6%. Both treatment related side effects and mortality significantly decreased after the application of PK dose adjustments (p-value 0.007 and 0.01 respectively). Conclusion: PK-guided dose adjustments of 5-FU and Docetaxel in DPF regimen can significantly decrease the treatment related side effects and mortality without compromising the tumor response rate. A randomized clinical trial is needed to compare the PK-guided dose adjustment with the standard BSA based protocol.

Physicochemical Properties of Sewage Sludge from Gaza

This study characterized the physicochemical properties of sewage sludge collected from Gaza wastewater treatment plant. Sludge samples were collected from the drying beds, air-dried, sieved through 2 mm and stored in plastic bags at room temperature. Sludge density, particle size distribution, water holding capacity, void volume, pH, EC, total organic carbon and hydrophobicity were determined. Results showed the bulk density is about 1.18 g/cm3 whereas the real density is 2.12 g/cm3 and void volume is 50%;Particle size distribution showed that the major size of sludge is sand-like size (630 - 200 μm) and the minor size is silt-like size (200 - 20 μm) and clay-like size is less than 20 μm. Sludge has an acidic pH reaction (6.78 ± 0.02) with an electric conductivity equal to (2.49 ± 0.04) mS?cm-1. The hydrophobicity of sludge is very high, water drop penetration time (WDPT) is 114.77 ± 18.78 sec with a radius of 0.44 ± 0.08 cm. In the way around, oil drop penetration time (ODPT) of sludge is 5.05 ± 1.28 sec with a radius of 1.25 ± 0.14 cm. The WDPT/ODPm ration has very high value 22.73 indicating extreme hydrophobicity. High value of hydrophobicity may reduce water filtration in soil when sludge applied for agriculture. These results suggest that sludge application to soil may change the physicochemical properties of soil.

Synthesis of some 1,3,4-thia-, oxa-diazol and 1,2,4 triazole incorporated the biologically active and the pyrazolopyri-dine derivative. molecular modeling and docking of the active compounds into AKR1C3 complexed with its bound inhibitor indomethacin using molsoft ICm 3.4-8C program were performed in order to predict the affinity and orientation of the synthesized compounds at the active site.

Synthesis and Physicochemical Studies for Novel Bioactive metal Complexes of macrolide Antibiotic Tylosin

Tylosin is a well-established antibiotic that has been widely employed in human and veterinary medicines. It can act as a potential ligand binding metal ions due to various donor atoms in the structure. Our study on the complexation of various metal ions with tylosin ligand revealed that they preferably coordinate with mycaminose fragment to establish Novel trends complexes. Tylosin ligand (TYS) behaves as bidentate for complexation with different metal ions such as Cr(III), mn(II), Fe(III), Co(II), Ni(II), Cu(II) and Zn(II). Various essential metal complexes of tylosin were synthesized and characterized by techniques such as UV, IR, Elemental analysis, magnetic susceptibility and ESR spectra of Cu(II) complex. These techniques are used to know their geometries and mode of bonding, with stoichiometry, 2:2 (m:L). Thermal analysis (TGA and DTA) of ligands and their metal complexes were carried out to distinguish between the coordinate and hydrate solvents and to estimate the stability ranges, peak temperatures. The thermodynamic parameters, such as activation energy (ΔE*), the enthalpy of activation (ΔH*), entropy of activation (ΔS*) and Gibbs free energy (ΔG*) are calculated and discussed. Some tylosin complexes show higher activity than tylosin for some bacterial and fungal strains. Low concentration value of minimum inhibitory concentration (mIC) results is 15.625 μg/ml for both complexes [Zn2(TYS)2Cl2(H2O)4]·25H2O and [Cu2(TYS)2Cl2(H2O)4]·25H2O with B. cereus genus maybe a valuable data used to produce novel therapeutic agent. This study constitutes several essential aspects for future research on tylosin metal complexes as antibacterial assessment and as potential medicinal agents.

Over 11% of all pregnancies in the US result in preterm birth, greatly contributing to perinatal morbidity and mortality （Goldenberg and Rouse, 1998）. Preterm birth etiologies remain largely unknown, and effective prevention methods have yet to be developed. The use of biofluid （e.g., serum or urine） for the analysis of the naturally occurring peptidome （mW 〈 4000） as a source of biomarkers has been reported for different diseases （Villanueva et al., 2006; Ling et al., 2010a, 2010b, 2010c, 2011）. mass spectrometry-based profiling of naturally occurring peptides can provide an extensive in- ventory of serum peptides derived from either high-abundant endogenous circulating proteins or cell and tissue proteins （Liotta and Petricoin, 2006）.

Acidity, Electric Conductivity, Dissolved Oxygen Total Dissolved Solid and Salinity Profiles of marine Water in Gaza: Influence of Wastewater Discharge

Effects of wastewater discharge on the coastal area in Gaza strip Palestine are not fully investigated. This study investigated the effect wastewater discharge of the physical and chemical properties of marine water in the coastal area over a period of 2 year. Sea water and sediment samples were collected from about 20 different sites. The water and the sediments were collected wastewater/sea water mixing zone at depth 0, 2.5 and 5 m water column depth at a direct wastewater discharging pipe line from different location and similarly from other locations. Water temperature T, electric conductivity (EC), dissolved oxygen (DO), pH, total dissolved solid (TDS) and salinity were determined for water samples using field equipment such as pH-meter, DO meter, TDS-/ and EC-meter. On the other hand, NO3-, SO42-, PO43- were determined in the laboratory using chemical methods, details are described in materials and method section. Results showed average and standard deviation of T, EC, DO, pH, TDS and salinity were 22.02°C ± 4.1°C, EC: 58.41 ± 4.8 ms/cm;DO: 6.96 ± 1.8 mg/L;pH: 7.69 ± 0.37;TDS: 30.51 ± 3.29 and salinity 4.39 ± 0.12 (%);whereas, average and standard deviation of NO3-: were 299.8 ± 204.1 mg/L;SO42-, 5736.9 ± 817.1;and PO43-: 164.35 ± 120.7 mg/L. The measured values indicate significant differences due to high value of standard deviation of some measured parameters. This indicates the influence of wastewater discharge in sea water as shown inside the manuscript in the appropriate section. The study recommends efficient treatment of wastewater and reuses it for agricultural purposes instead of discharging it in sea.
