Dosimetric Comparison of Intensity-modulated Radiotherapy versus Three-Dimensional Conformal Radiotherapy for Patients with Brain Tumors

Purpose: To compare target dose distribution and dose to normal tissue for brain tumors using intensity-modulated and three-dimensional conformal radiotherapy. methods and materials: Twelve patients selected in the present study compared the dosimetry of intensity-modulated radiation therapy (ImRT) and three-dimensional conformal radiation therapy (3DCRT) techniques in patients treated with different diagnosis brain tumor. All patients underwent computed tomography planning in conjunction with magnetic resonance imaging fusion. Prescription dose and normal-tissue constraints were identical for both plans. The tolerance level for maximum dose was 6.0 Gy for lenses and 54.0 Gy for brain stem, and optical nerves. Target coverage was evaluated with the D98%, D95% and D2%. Normal tissue dose was evaluated with the maximum dose to lenses, brain stem, and optical nerves. Results: mean and standard deviation values of PTV 98%, PTV 95% and PTV 2% in ImRT these values were 94.6 ± 2.3, 96.56 ± 1.67 and 106.5 ± 1.87 respectively;for 3DCRT were 92.7 ± 3.09, 95.58 ± 1.24 and 104.45 ± 2.73 respectively. ImRT plan provided reduced dose to brainstem (72.17 ± 29.967) and lenses (2.43 ± 2.39) relative to the 3DCRT plans for brainstem (73.05 ± 34.59) and lenses (7.65 ± 23.24). But optic nerves with ImRT significantly received higher doses (38.86 ± 32.15) than 3D-CRT (27.57 ± 31.45). Conclusion: Dose coverage of the planning target volume (PTV) and organ at risk (OAR) was better with ImRT. If PTV is distant to optical nerves and brainstem 3DCRT technique can be applied, and if the PTV is nearby OAR intensity-modulated treatment technique should be used.

Sorafenib was the first multikinase inhibitor to be approved for use in metastatic renal cell carcinoma. Olmesartan medoxomil used in treatment of hypertension and was reported to inhibit angiogenesis in several models. The present study was designed to assess the safety of a combination of sorafenib plus olmesartan compared to monotherapies in mice bearing Ehrlich’s ascites carcinoma cell line. mice were divided to seven groups, 1) normal mice, 2) Ehrlich’s ascites carcinoma control, 3 - 5) olmesartan (3, 10, 30 mg/kg/day), respectively, 6) sorafenib (30 mg/kg/day) and 7) the combination group: mice received olmesartan (30 mg/kg/day) plus sorafenib. All drug treatments continued for 21 days. At the end of the experiment, a complete blood count was performed and kidney and liver functions were estimated. The combination therapy produced a non-significant change in most of the measurements of complete blood count and liver enzymes when compared to normal animals. On the other hand, the combined therapy significantly increased blood urea nitrogen when compared to normal group but did not change the serum creatinine level. Concomitant administration of olmesartan with sorafenib did not significantly augment the toxicity of the later. Therefore;olmesartan might be a safe candidate with sorafenib in treatment of cancer if clinical data proved the benefit of this combination.

Background: Spinal cord injury (SCI) unsuccessful regeneration was due to glial scar development. It was a major obstacle to axonal restoration. Safe therapeutic intervention by the use of bone marrow derived stem cells (BmmSCs) transplantation applied in the present study could reduce spinal disability. material and methods: Forty male albino rats were divided into four groups: GI: negative control (n = 10 rats);GII: positive control after SCI (n = 10 rats);GIII: SCI + Bm - mSCs intravenous injected and GIV: SCI + Bm - mSCs intra lesion injected (n = 10 rats in each group). The samples were taken from spinal cord tissues around the region of injury and were subjected to histological, immunohistochemical assessment. RNA extraction and real time PCR for detection of nerve regeneration and astrocyte response to the injury were also performed. Results: Clinical improvement occurred by the enhancement in the Basso, Beattie and Bresnahan (BBB) score after SCI. Histological examinations showed positive regenerative responses in GIV compared to GIII. Conclusion: Bm-mSCs transplantation has a promising role in enhancing the microenvironment for nerve regeneration through stumbling the glial scaring formation and inflammatory response after chronic spinal cord injury especially by using intra-lesion route injection.

To paraphrase the Greek philosopher Heraclitus, change is the only constant in construction projects. Changes in scope occur as projects progress from design through practical completion. FIDIC General Conditions Clause 13 [1] is one of the most important terms and implementing. It is also major factor for project success. Once all project parties understand and fairly use this clause, the impact will be minimising the expected disputes by 50% at least. Thus, this paper is considering illustrated variation order clause by flow chart technique to ensure that all the engineers are handling easily those subclasses.

Purpose: This study was conducted to detect IL-6 in synovial fluid in cases with TmJ internal derangement. Patients & methods: This study was conducted on forty patients ASA class I with TmJ Internal derangement. All patients had been subjected to arthrocentesis. The synovial fluid was collected before wash and lavage was done for the affected joint. Then, the selected patients were divided randomly into two equal groups, group I: consisted of 20 patients where arthrocentesis was performed for the affected joint followed by intraarticular injection of one ml, tramadol hydrochloride. Group II: consisted of 20 patients where arthrocentesis was performed for the affected joint followed by intraarticular injection of one ml. sodium hyaluronate. Another synovial fluid sample was aspirated after one month. The interlieukin-6 receptors in the aspirated synovial fluid were measured using humans IL-6 enzyme-linked immunosorbent assay. Results: IL-6 was detected in the synovial fluid of joints with internal derangement. During follow up assessment of IL-6, the maximum decrease in IL-6 level was in the patients of group I who subjected to arthrocentesis with tramadol injection, as the mean IL-6 was (4.93 ± 1.36) followed by the patients of group II who subjected to arthrocentesis with sodium hyaluronate injection where the mean IL-6 level was (6.88 ± 1.76). There were significant p-value = (P = 0.000). Conclusions: It had been concluded that the detection of IL-6 in the synovial fluid of joints with internal derangement considered an indicator for inflammatory reaction in the joint and also the efficacy of arthrocentesis with tramadol suggested its anti-inflammatory effect.

Percutaneous devices for left atrial appendage occlusion: A contemporary review

Patient with atrial fibrillation(AF) are at risk of developing stroke with the left atrial appendage(LAA) being the most common site for thrombus formation. If left untreated, AF is associated with 4 to 5 folds increase in the risk of ischemic stroke in all age groups. About 5% to 15% of AF patients have atrial thrombi on transesophageal echocardiography, and 91% of those thrombi are located in the LAA in patient with nonrheumatic AF. Although oral anticoagulants are the gold-standard treatment for stroke prevention in patients with non-valvular AF,some patients are at high risk of bleeding and deemed not candidates for anticoagulation. Therefore, LAA occlusion(LAAO) has emerged as alternative approach for stroke prevention in those patients. Surgical LAAO is associated with high rate of unsuccessful closure and recommended only in patients with AF and undergoing cardiac surgery. Percutaneous LAAO uses transvenous access with trans-septal puncture and was first tested using the PLAATO *** is the most common and only Food and Drug Administration(FDA)approved device for LAAO. LAAO using Watchman device is non-inferior to warfarin therapy in preventing ischemic stroke/systemic ***, it is associated with lower rates of hemorrhagic stroke, bleeding and death. Amplatzer is another successful LAAO device that has CE mark and is waiting for FDA approval. Optimal antithrombotic therapy post LAAO is still under debate and highly patient-specific. The aim of this paper is to systematically review the current literature to evaluate the efficacy and safety of different LAAO devices.

A digraph is a graph in which each edge has an orientation. A linear directed path, , is a path whose all edges have the same orientation. A linear simple graph is called directed cordial if it admits 0 - 1 labeling that satisfies certain condition. In this paper, we study the cordiality of directed paths??and their second power . Similar studies are done for ?and the join ?. We show that ,? and ?are directed cordial. Sufficient conditions are given to the join?? to be directed cordial.

New series of Ag, Cd(II), Ce(III), Co(II), Cr(III), Fe(III), Ni(II) and Pb(II) complexes with (E)-N-(4-(2- hydroxybenzylideneamino) phenylsulfonyl) {S.S} have been synthesized. These compounds have been characterized by different physico-chemical techniques like, melting point, elemental analysis, FT-IR, UV spectroscopy, 1H NmR spectroscopy, conductance measurements, magnetic susceptibility and mass spectral analysis. The metal ions concentrations have been determined using inductively coupled plasma mass spectrometry (ICP-mS). Spectroscopic studies suggest coordination most of complexes in a regular octahedral arrangement in 2L:1m molar ratio by two N azomethine and two OH phenolic from Schiff base {S.S} in the form of [mL2(H2O)2], m metal, L ligand {S.S}. Complexes have been screened for their antibacterial {Gram negative bacteria (Escherichia coli and Pseudomonas aeruginosa)}, {Gram positive bacteria (Bacillus subtilis and Sterptococcus pneumoniae)} and antifungal (Aspergillus fumigates and Candida albicans) showing promising antimicrobial biological activity.

Copolymer films of Acrylic acid/ methyl methacrylate AAc/mmA with comonomer composition of 60/40 was prepared and then irradiated by gamma irradiation. The effect of irradiation on some of the physical properties of the copolymer films was investigated. The dose level ranged from 5 to 200 kGy. The thermo gravimetric analysis (TGA) showed that the thermal stability of the films increased with irradiation dose up to 100 kGy then it started to decrease. The results of the UV-Visible spectroscopy of the films showed a decrease in the values of optical band gap and band tail width with doses starting from 100 kGy. The DC conductivity (?DC) of the films was found to decrease to about three orders of magnitude from its original value with irradiation dose up to 100 kGy then it started to increase with higher doses. moreover, the dielectric constant and dielectric loss values were found to increase with dose up to 100 kGy which may be attributed to the increase in the number of dipoles in the films due to the irradiation of the copolymer films in air. The results indicated that crosslinking dominated over chain scission in the copolymer films with irradiation dose up to 100 kGy then at higher doses, chain scission dominated.
