A Rare Etiology of a Life-Threatening Subdural Hematoma: Case Report and Literature Review

Background: Life-threatening subdural hematoma is commonly related to trauma and rarely revealed by neoplasm. Observation: We report a case of a 53-year-old suffering from mild headache and without a history of trauma, was admitted unconscious due to a subdural hematoma on radiological investigations. Beside the left subdural hematoma, there was also alytic lesion of the sphenoid wing and the temporal bone on the same side. An emergent removal of the subdural hematoma and an excision of the bone lesion were performed. Pathological examination diagnosed a cavernous hemangioma of the skull. The postoperative period was uneventful with a dramatic recovery of the patient. Conclusion: A quick worsening of a chronic headache, acute impairment of an uncommon headache deserve prompt investigation and emergent surgical management in case of intracranial hematoma. Any bone and dural abnormalities at the vicinity of a subdural hematoma require total excision with clean border and pathological examination.

Objective:To assess acute toxicity,the in vitro and in vivo effects of methanol and ethyl acetate extracts(JmE and JEE)of Jatonik polyherbal mixture on some mitochondria-related parameters and their effect on the activity of some liver ***:Acute toxicity of JmE and JEE was determined using Lorke’s *** vitro and in vivo opening of the mitochondrial membrane permeability transition pore(mmPT pore)was spectrophotometrically *** of malondialdehyde(mDA)as an index of lipid peroxidation and the activity of mitochondrial ATPase was evaluated in vitro and in vivo and the effect of JmE and JEE on the activity of liver enzymes such as alkaline phosphatase(ALP),aspartate and alanine aminotransferase(AST and ALT)and gamma-glutamyl transferase(GGT)was also ***:JmE had an LD_(50) of 3808 mg/kg b.w whereas JEE had an LD_(50) greater than 5000 mg/kg *** *** the rats have been fed with both extracts,a photomicrograph of a piece of liver tissue showed no apparent symptoms of *** the in vitro and in vivo studies,both extracts prompted intact mitochondria to open their mmPT *** compared to the control,lipid peroxide product release and ATPase activity were significantly increased(Pmg/kg when treated with JmE,but the activity of AST was considerably enhanced when treated with JEE(PmE and JEE of the Jatonik polyherbal mixture had low toxicity,profound mmPTpore induction,and enhanced ATPase activity,but an increased mDA ***:Jatonik extracts may be a promising target for drug development in diseases where there is dysregulation of apoptosis,however,further studies are needed to better clarify the molecular mechanism involved in these phenomena.

烘干失重法粮食水分测定过程中,干燥箱温度的精准控制是检测结果准确性的重要保障。针对干燥箱温度变化过程非线性、时滞性和温控精度高的特点,提出一种基于粒子群模型参数辨识的变论域模糊PID复合温度控制策略。基于红外辐射干燥机理及能量守恒方程建立温控系统非线性模型,并利用粒子群优化算法完成参数辨识,通过设定二级偏差阈值,将变论域模糊PID算法结合Bang-Bang控制方法构成自适应分级复合控制策略。系统仿真实验表明:相较于常规PID控制和传统模糊PID控制方法,改进后的复合控制策略在超调量、调节时间和控制进度方面明显改善,在不同工作点温度下,控制误差均小于0.4℃,且具有较好的抗干扰能力。

Resistant bacteria can be transmitted to humans through feces or contaminated meat from local chickens. Bacterial strains were isolated from the intestinal contents of 400 local chicken samples from various sales sites. These strains were then characterized using bacteriological and biochemical methods to identify resistant strains. In a study conducted in Ouagadougou, we systematically collected chicken fecal samples from 20 locations across the city, followed by isolation and identification of Salmonella spp. using specific enrichment and culture methods, as well as Escherichia coli. Bacterial strains were characterized using antibiotic resistance profiles were determined through agar diffusion tests, revealing sensitivity or resistance to a range of antibiotics based on established scientific criteria. The results showed that out of the 400 samples collected, 81.25% and 63.5% were contaminated by Escherichia coli and Salmonella spp., respectively. Among these, 86.15% of identified Escherichia coli and 50.78% of Salmonella spp. displayed resistance to at least one tested antibiotic. Among 280 Escherichia coli isolates identified resistant to at least one antibiotic, 31.07% were resistant to cefotaxime (CTX), 20.35% to ceftazidime (CAZ), 21.07% to ceftriaxone (CTR), 75% to amoxicillin clavulanic acid (AmC), 23.57% aztreoname (ATm) and 27.14% were resistant to imipenem (ImP). In the case of the 129 Salmonella spp. isolates resistant to at least one tested antibiotic, 34.88% were resistant to CTX;41.08% to CAZ;35.65% to CTR, 92% to AmC, 39.53% to ATm and finally 47.28% were resistant to ImP. Our study revealed high prevalence of resistance in bacterial strains isolated from local chickens sold outdoors in Ouagadougou. These findings raise significant public health concerns, due to the possible transmission of these resistant strains to humans through the consumption of contaminated meat, thus complicating the treatment of bacterial infections.

Spatial Variation and Trend of Extreme Precipitation in Africa during 1981-2019 and Its Projected Changes at the End of 21^st Century

This study comprehensively examines the patterns and regional variation of severe rainfall across the African continent, employing a suite of eight extreme precipitation indices. The analysis extends to the assessment of projected changes in precipitation extremes using five General Circulation models (GCms) from Coupled model Intercomparison Project Phase 6 (CmIP6) under four Shared Socioeconomic Pathways (SSPs) scenarios at the long-term period (2081-2100) of the 21st century. Furthermore, the study investigates potential mechanisms influencing precipitation extremes by correlating extreme precipitation indices with oceanic system indices, specifically Ni?o 3.4 for El Ni?o-Southern Oscillation (ENSO) and Dipole mode Index (DmI) for the Indian Ocean Dipole (IOD). The findings revealed distinct spatial distributions in mean trends of extreme precipitation indices, indicating a tendency toward decreased extreme precipitation in North Africa, Sahel region, Central Africa and the Western part of South Africa. Conversely, West Africa, East Africa and the Eastern part of South Africa exhibit an inclination toward increased extreme precipitation. The changes in precipitation extreme indices indicate a general rise in both the severity and occurrence of extreme precipitation events under all scenarios by the end of the 21st century. Notably, our analysis projects a decrease in consecutive wet days (CWD) in the far-future. Additionally, correlation analysis highlights significant correlation between above or below threshold rainfall fluctuation in East Africa and South Africa with oceanic systems, particularly ENSO and the IOD. Central Africa abnormal precipitation variability is also linked to ENSO with a significant negative correlation. These insights contribute valuable information for understanding and projecting the dynamics of precipitation extreme in Africa, providing a foundation for climate adaptation and mitigation efforts in the region.

Gamma-ray spectroscopy based on a 100% efficiency hyper-pure germanium detector was used to evaluate the activity concentrations of 226Ra, 232Th, and 40K natural radionuclides in sedimentary, conglomerate, igneous and sedi-ments rock samples collected from four different locations in Eastern desert in Egypt. The obtained activity concentrations are used to evaluate the radi-ological hazards indices, absorbed dose rate, annual effective dose equivalent in air, radium equivalent, external and internal hazard index, radiation level index, annual gonadal dose equivalent, excess lifetime cancer risk and expo-sure rate. The results show that 1) the absorbed dose rate depends on the rock type, 2) the annual effective dose equivalent in air in 71% of sample below 20 mSvy-1 (permissible limit for workers), 3) the conglomerate rocks show low radioactivity level, 4) sedimentary rocks are rich in radium while igneous rocks are rich in thorium and the sediments rocks are rich in both radium and thorium.

Study of Factors Associated with the Practice of Vaccination against COVID-19 among medical Science Students in Dakar (Senegal)

Introduction: Almost a year after the start of the COVID-19 pandemic, Senegal embarked on a vaccination campaign to protect its population. The objective of this study was to identify the factors influencing the COVID-19 vaccination practices within a health training university in Dakar. methodology: This was a cross-sectional, descriptive and analytical study carried out from October 21 2021 to February 21 2022 among students at the Elhadj ibrahima Niass private University in Dakar, Senegal. A 28-question online questionnaire was sent to all students enrolled at the university during this period. Results: Of the 576 students who responded, the average age was 22.60 years, with females predominating (57.81%). medical students accounted for 66.14% of participants, pharmacy 22.40% and dental surgery 11.46%. Of these, 42.01% were bachelor’s students, 29.51% master’s students and 28.47% doctoral students. Vaccination coverage was 50.35%. Students who considered COVID-19 to be very dangerous were more likely to be vaccinated (OR = 5.05 [2.24-11.9]). Those with poor knowledge of vaccines were less likely to be vaccinated (OR = 0.07 [0.03-0.18]), as were those with poor knowledge of contraindications to vaccination (OR = 0.49 [0.28-0.86]). No association was found between vaccination status and socio-demographic or educational characteristics. Conclusion: In view of the importance of knowledge about COVID-19 vaccination, our results suggest that it is important to involve health students in the ministry of Health’s awareness-raising strategies, because their support is necessary for better public awareness.

Epidemiological, Clinical, Therapeutic, and Evolutionary Aspects of Heart Failure in the medical Department at Gao Regional Hospital

Introduction: Heart failure is a major public health pandemic, particularly in Africa, where its prevalence continues to increase. In northern mali, few data exist, hence the interest of this study with the objective of studying the epidemiological, clinical and therapeutic and evolutionary aspects of Heart failure at the regional hospital of Gao. Patients and methods: This was a cross-sectional, descriptive study that took place from July 2022 to June 2023 in the medical department at Gao Hospital. Results: The hospital prevalence of heart failure was 44.1%. The mean age was 47.30 ± 20 years (range: 16-88). Hypertension was the most common with 46.1%, followed by a sedentary lifestyle, and diabetes with 18.2% and 8.3% respectively;NYHA stage III-IV dyspnea was found in 83.9%. Reduced EF heart failure was present in 110 patients (76.9%), seventeen cases with moderately reduced EF (11.9%) and sixteen patients had preserved EF (11.2%). Global heart failure was the dominant (91.6%). The main etiologies of heart failure were dominated by hypertensive heart disease in 46 patients (32.2%), followed by postpartum cardiomyopathy with 43 cases (30.1%), primary dilated cardiomyopathy in 18 patients (12.6%), ischemic heart disease in 16 patients with 11.2%. Seven cases of valvular heart disease, or 4.9%. The evolution was favorable under treatment in 104 patients or 72.7%. In-hospital mortality was 14.7%. Conclusion: Heart failure is a common condition in sub-Saharan Africa, particularly in our country.
