Monkeypox(mpox),is a disease from the Poxviridae family that can cause several serious medical issues.This mini-review sought to analyze the existing literature regarding the current mpox outbreak with a focus on the prevalence,diagnostics,and containment measures.Mpox cases have been reported to World Health Organization(WHO)from 85 Member States in all six WHO regions during the period of January 1,2022,through August 3,2022.Standardized or optimized guidelines for the clinical care of patients with mpox are limited,particularly in low-resource settings.In an effort to achieve guidance and meet standards,special attention should be paid to this outbreak in order to eradicate such a rare infectious disease by analyzing prevention and control measures.Patient outcomes may also be poor,and their illnesses may last for a long time.The spectrum of clinical symptoms,including complications and sequelae,as well as aspects of the illness may be indicators of sickness severity and complications;therefore,its clinical presentation must be better understood to improve containment measures.In addition,it is important to create and evaluate a standard of care that takes a variety of parameters into account,including antiviral,immune therapies,and clinical metrics that are particular to mpox.The global emergence of mpox has presented new challenges for public health and has called for further investigation into its epidemiological profile across international contexts.

Diabetes is one of the deadliest diseases.Due to its effects on the lives of people,it has attracted a lot of attention recently.The causes of the various forms of diabetes,including type 1 and type 2,were discussed along with how they affect those who have the disease.Younger people are more prone to type 1 diabetes than older people,who are more likely to develop type 2.The treatment options and strategies for the two forms of diabetes were also discussed in addition to how the disease affects the quality of life of people.Among several factors that were explained,it has been shown that people from low and middle-income countries are more prone to having diabetes.Additionally,the condition is more likely to affect some races more than others.It is associated with obesity.According to statistics,those who are poor are more severely affected by the disease.The progression of the disease over time has been associated with an increase in disability and mortality.

There have been numerous concerns about the disease and how it affects the human body since the severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)pandemic began in December 2019.The impact of SARS-CoV-2 on the liver is being carefully investigated due to an increase in individuals with hepatitis and other liver illnesses,such as alcoholic liver disease.Additionally,the liver is involved in the metabolism of numerous drugs used to treat comorbidities and coronavirus disease 2019(COVID-19).Determining how SARS-CoV-2 affects the liver and what factors place individuals with COVID-19 at a higher risk of developing liver problems are the two main objectives of this study.This evaluation of the literature included research from three major scientific databases.To provide an update on the current impact of COVID-19 on the liver,data was collected and relevant information was incorporated into the review.With more knowledge about the effect of the disease on the liver,better management and therapeutics can be developed,and education can ultimately save lives and reduce the long-term impact of the pandemic on our population.

A framework for dynamic modelling of railway track switches considering the switch blades,actuators and control systems

The main contribution of this paper is the development and demonstration of a novel methodology that can be followed to develop a simulation twin of a railway track switch system to test the functionality in a digital environment.This is important because,globally,railway track switches are used to allow trains to change routes;they are a key part of all railway networks.However,because track switches are single points of failure and safety-critical,their inability to operate correctly can cause significant delays and concomitant costs.In order to better understand the dynamic behaviour of switches during operation,this paper has developed a full simulation twin of a complete track switch system.The approach fuses finite element for the rail bending and motion,with physics-based models of the electromechanical actuator system and the control system.Hence,it provides researchers and engineers the opportunity to explore and understand the design space around the dynamic operation of new switches and switch machines before they are built.This is useful for looking at the modification or monitoring of existing switches,and it becomes even more important when new switch concepts are being considered and evaluated.The simulation is capable of running in real time or faster meaning designs can be iterated and checked interactively.The paper describes the modelling approach,demonstrates the methodology by developing the system model for a novel“REPOINT”switch system,and evaluates the system level performance against the dynamic performance requirements for the switch.In the context of that case study,it is found that the proposed new actuation system as designed can meet(and exceed)the system performance requirements,and that the fault tolerance built into the actuation ensures continued operation after a single actuator failure.

BACKGROUND Pancreaticoduodenectomy(PD)is a technically complex operation,with a re-latively high risk for complications.The ability to rescue patients from post-PD complications is as a recognized quality measure.Tailored protocols were instituted at our low volume facility in the year 2013.AIM To document the rate of rescue from post-PD complications with tailored protocols in place as a measure of quality.METHODS A retrospective audit was performed to collect data from patients who experienced major post-PD complications at a low volume pancreatic surgery unit in Trinidad and Tobago between January 1,2013 and June 30,2023.Stan-dardized definitions from the International Study Group of Pancreatic Surgery were used to define post-PD complications,and the modified Clavien-Dindo classification was used to classify post-PD complications.RESULTS Over the study period,113 patients at a mean age of 57.5 years(standard deviation[SD]±9.23;range:30-90;median:56)underwent PDs at this facility.Major complications were recorded in 33(29.2%)patients at a mean age of 53.8 years(SD:±7.9).Twenty-nine(87.9%)patients who experienced major morbidity were salvaged after aggre-ssive treatment of their complication.Four(3.5%)died from bleeding pseudoaneurysm(1),septic shock secondary to a bile leak(1),anastomotic leak(1),and myocardial infarction(1).There was a significantly greater salvage rate in patients with American Society of Anesthesiologists scores≤2(93.3%vs 25%;P=0.0024).CONCLUSION This paper adds to the growing body of evidence that volume alone should not be used as a marker of quality for patients requiring PD.Despite low volumes at our facility,we demonstrated that 87.9%of patients were rescued from major complications.We attributed this to several factors including development of rescue protocols,the competence of the pancreatic surgery teams and continuous,and adaptive learning by the entire institution,cul-minating in the development of tailored peri-pancreatectomy protocols.

Background The effect of transdermal glyceryl trinitrate(GTN,a nitrovasodilator)on clinical outcome when administered before hospital admission in suspected stroke patients is unclear.Here,we assess the safety and efficacy of GTN in the prespecified subgroup of patients who had an ischaemic stroke within the Rapid Intervention with Glyceryl trinitrate in Hypertensive stroke Trial-2(RIGHT-2).Methods RIGHT-2 was an ambulance-based multicentre sham-controlled blinded-endpoint study with patients randomised within 4hours of onset.The primary outcome was a shift in scores on the modified Rankin scale(mRS)at day 90.Secondary outcomes included death;a global analysis(Wei-Lachin test)containing Barthel Index,EuroQol-5D,mRS,telephone interview for cognitive status-modified and Zung depression scale;and neuroimaging-determined‘brain frailty’markers.Data were reported as n(%),mean(SD),median[IQR],adjusted common OR(acOR),mean difference or Mann-Whitney difference(MWD)with 95%CI.Results 597 of 1149(52%)patients had a final diagnosis of ischaemic stroke;age 75(12)years,premorbid mRS>2107(18%),Glasgow Coma Scale 14(2)and time from onset to randomisation 67[45,108]min.Neuroimaging‘brain frailty’was common:median score 2[2,3](range 0–3).At day 90,GTN did not influence the primary outcome(acOR for increased disability 1.15,95%CI 0.85 to 1.54),death or global analysis(MWD 0.00,95%CI-0.10 to 0.09).In subgroup analyses,there were non-significant interactions suggesting GTN may be associated with more death and dependency in participants randomised within 1hour of symptom onset and in those with more severe stroke.Conclusions In patients who had an ischaemic stroke,ultra-acute administration of transdermal GTN in the ambulance did not improve clinical outcomes in a population with more clinical and radiological frailty than seen in previous in-hospital trials.WHAT IS ALREADY KNOWN ON THIS TOPIC⇒Transdermal glyceryl trinitrate(GTN)was associat-ed with less death and dependency in those with acute stroke treated within 6hours of stroke

Nonplanar dust acoustic solitary and rogue waves in an ion beam plasma with superthermal electrons and ions

The propagation characteristics of dust acoustic solitary and rogue waves are investigated in an unmagnetized ion beam plasma with electrons and ions following kappa-type distribution in nonplanar geometry. The reductive perturbation method （RPM） is employed to derive the cylindrical/spherical Korteweg-de Vries （KdV） equation, which is further transformed into standard KdV equation by neglecting the geometrical effects. Using new stretching coordinates, nonlinear Schrrdinger equation （NLSE） has been derived from the standard KdV equation to study the different order rational solutions of dust acoustic rogue waves （DARWs）. The impact of various physical parameters on the characteristics of dust acoustic solitary waves （DASWs） is elaborated specifically in nonplanar geometry. Further, the effects of ion beam and superthermality of electrons/ions on the characteristics of DARWs are studied. The results obtained in the present investigation may be useful in comprehending a variety of phenomena in Earth＇s magnetosphere polar cap region where the presence of positive ion beam has been detected and also in other regions of space/astrophysical environments where dust along with superthermal electrons and ions exists.

Dust acoustic shock waves in magnetized dusty plasma

We have presented a theoretical study of the dust acoustic （DA） shock structures in a magnetized, electron depleted dusty plasma in the presence of two temperature superthermal ions. By deriving a Korteweg-de Vries-Burgers equation and studying its shock solution, we aim to highlight the effects of magnetic field and obliqueness on various properties of the DA shock structures in the presence of kappa-distributed two temperature ion population. The present model is motivated by the observations of Geotail spacecraft in the Earth＇s magnetotail and it is seen that the different physical parameters such as superthermality of the cold and hot ions, the cold to hot ion temperature ratio, the magnetic field strength, obliqueness and the dust kinematic viscosity greatly influence the dynamics of the shock structures so formed. The results suggest that the variation of superthermalities of the cold and hot ions have contrasting effects on both positive and negative polarity shock structures. Moreover, it is noted that the presence of the ambient magnetic field affects the dispersive properties of the medium and tends to make the shock structures less wide and more abrupt. The findings of present investigation may be useful in understanding the dynamics of shock waves in dusty plasma environments containing two temperature ions where the electrons are significantly depleted.

Optimization of SnS active layer thickness for solar cell application

This work presents a comparative study of n-SnS and p-SnS active layers for increased solar cell efficiency. Tin sulphide thin films of various thicknesses having p-type and n-type conductivity were fabricated by thermal evaporation. Both type of films had the same（113） orientation of the crystal planes with a constant tensile strain of ~ 0.003 and ~ 0.011, respectively. The persistent photocurrent was observed in all n-SnS and p-SnS samples with the current＇s time decay constant decreasing with increasing film thickness. Hole mobility of thicker p-SnS films was found to be greater than the electron mobility in n-SnS samples, with mobility（both hole and electron） showing an increasing trend with film thickness. The optimum absorber layer thickness for both p-and n-SnS layers should have a high value of diffusion length for a given absorption coefficient and band-gap.

Screening Landraces for Additional Sources of Field Resistance to Cassava Mosaic Disease and Green Mite for Integration into the Cassava Improvement Program

Twelve cassava landraces were evaluated for sources of resistance genes to diseases and pests of major economic importance in Africa. The objective was to assess their levels of field resistance to mosaic disease （ACMD）, bacterial blight （CBB）, anthracnose （CAD）, and green mite （CGM）, compared to TMS30572, an elite cultivar widely adopted in Africa. Considerable genotypic variation was observed among cultivars for resistance to ACMD and CGM but not for CBB and CAD. The lowest mean incidence of 12% and severity of 1.8 on a scale of 1-5 for ACMD was recorded for Atu, a landrace with farmer acceptable qualities. In comparison, the improved cultivar, TMS 30572, had a mean disease incidence of 72% and a severity score of 2.8. Another landrace, MS-20 had the lowest CGM damage score （2.1） while TMS 30572 emerged as one of the susceptible cultivars with a damage score of 3. Additional sources of resistance to ACMD and CGM that may possibly be better than the popular improved cultivar, TMS 30572, were identified in this study. These could serve as novel sources of additional genes to complement existing resources for elite cassava breeding in Africa.
