Coronary Artery Bypass with Myxedema

Introduction Hypothyroidism increases the risk of cardiovascular complications in patients scheduled for coronary artery bypass grafting. Atrial fibrillation is one of its cardiac complications. Case Report Our 45-year-old male patient was admitted to the Emergency Medicine Clinic of our hospital with chest pain that started in the left arm and extended to the chin 2 days ago. It was noted that the patient had hypothyroidism, but did not have any medication for the disease. On physical examination, dry and pale skin, sparse coarse hair, non-pitting edema were diagnosed with acute coronary artery disease. No signs of ischemia were observed on the ECG at the time of admission, but bradycardic sinus rhythm was recorded. The patient was started to be followed up with the diagnosis of NON-ST myocardial infarction. Nitroglycerin 0.25 - 2 mcgr/h and morphine 2 mg were administered intravenously to the patient whose anginal complaints continued after admission. Anginal complaint continued was operated in 1:1 mode by attaching an intraaortic balloon pump (Maquet Sensation 7Fr 40 cc, Datascope CS300 console) via the left femoral artery. Surgery procedure: The patient was performed with median sternotomi (aortic, two-stage cannulation). While the patient was cooled to 32 degrees and given blood cardioplegia (St. Thomas II) and applied topical cold. After distal anastomoses were performed with saphenous vein graft. Total cross-clamp time was 60 min. Epinephrine was given for bradycardia sinus rhythm. No cardiovascular complications were encountered while being followed in the intensive care unit. The patient was discharged on the 6th postoperative day. Conlucion: Our case, who was taken to emergency CABG operation with myxedema, was discharged without any cardiovascular, respiratory or metabolic complications both in the perioperative and postoperative periods. Effect of pulsed corticosteroids and tocilizumab on hyperinflammation in COVID-19 patients with acute respiratory distress syndrome

Objective:To compare the efficacy of pulsed-dose corticosteroids(≥250 mg methylprednisolone,3 days)and tocilizumab in treating COVID-19-related ***:This prospective observational study included RT-PCR positive COVID-19 patients with acute respiratory distress syndrome,who were admitted to the COVID-19 Adult Intensive Care Unit of Prof *** Dilmener Emergency Hospital(Istanbul,Turkey)between December 1,2020 and February 28,***,laboratory and radiological examinations were used to diagnose COVID-19 associated *** cohort groups were formed:the pulsed-dose corticosteroids group(250 mg methylprednisolone for 3 days),the tocilizumab group(8 mg/day single dose or 400 mg/day for 2 days),and the combined group(pulsed-dose corticosteroid+tocilizumab).The difference in mortality rates among the groups was compared *** most common cause(s)of death was ***,adverse events(secondary infection,acute kidney injury,arrhythmia,gastrointestinal system bleeding)for 28 days were ***:A total of 60 patients were included in this study,with 20 patients in each *** was no statistically significant difference between the 3 groups in mortality rates(55%in the pulsed corticosteroid group,60%in the tocilizumab group,50%in the combined group,χ2=0.404,P=0.817).Infectious causes were found to be the most common cause of mortality in all the three groups,and no difference was found between them(χ2=0.404,P=0.817).There was also no difference in the development of adverse events such as secondary infection,acute kidney injury,arrhythmia,and gastrointestinal bleeding among the groups(P>0.05).Conclusions:Corticosteroids can be used instead of tocilizumab to treat hyperinflammation in COVID-19 patients with acute respiratory distress syndrome.

Assessment of large-scale multiple forest disturbance susceptibilities with AutoML framework: an Izmir Regional Forest Directorate case

Disturbances such as forest fires,intense winds,and insect damage exert strong impacts on forest ecosystems by shaping their structure and growth dynamics,with contributions from climate ***,there is a need for reliable and operational methods to monitor and map these disturbances for the development of suitable management *** susceptibility assessment using machine learning methods has increased,most studies have focused on a single ***,there has been limited exploration of the use of“Automated Machine Learning(AutoML)”in the *** this study,susceptibility assessment for multiple forest disturbances(fires,insect damage,and wind damage)was conducted using the PyCaret AutoML framework in the Izmir Regional Forest Directorate(RFD)in *** AutoML framework compared 14 machine learning algorithms and ranked the best models based on AUC(area under the curve)*** extra tree classifier(ET)algorithm was selected for modeling the susceptibility of each disturbance due to its good performance(AUC values>0.98).The study evaluated susceptibilities for both individual and multiple disturbances,creating a total of four susceptibility maps using fifteen driving factors in the *** to the results,82.5%of forested areas in the Izmir RFD are susceptible to multiple disturbances at high and very high ***,a potential forest disturbances map was created,revealing that 15.6%of forested areas in the Izmir RFD may experience no damage from the disturbances considered,while 54.2%could face damage from all three *** SHAP(Shapley Additive exPlanations)methodology was applied to evaluate the importance of features on prediction and the nonlinear relationship between explanatory features and susceptibility to disturbance.

Under optimal process conditions,pyrolysis of polyolefins can yield ca.90 wt%of liquid product,i.e.,combination of light oil fraction and heavier *** this work,the experimental findings reported in a selected group of publications concerning the non-catalytic pyrolysis of polyolefins were collected,reviewed,and compared with the ones obtained in a continuously operated bench-scale pyrolysis *** process parameters were used for the pyrolysis of waste and virgin counterparts of high-density polyethylene,low-density polyethylene,polypropylene and a defined mixture of those(i.e.,25:25:50 wt%,respectively).To mitigate temperature drops and enhance heat transfer,an increased feed intake is employed to create a hot melt plastic *** 1.5 g·min^(-1) feed intake,1.1 L·min^(-1) nitrogen flow rate,and a moderate pyrolysis temperature of 450℃,the formation of light hydrocarbons was favored,while wax formation was limited for polypropylene-rich *** of virgin plastics yielded more liquid(maximum 73.3 wt%)than that of waste plastics(maximum 66 wt%).Blending polyethylenes with polypropylene favored the production of liquids and increased the formation of gasoline-range *** products were mainly composed of C3 hydrocarbons,and no hydrogen production was detected due to moderate pyrolysis temperature.

The Evaluation of the Dietary Habits Influence on the Microhardness of Gingiva-Coloured Composite and Acrylic Denture Base Materials

Purpose: The study investigated the impact of dietary habits, specifically soda, milk kefir, water kefir, almond milk, and distilled water (control) consumption, on the microhardness of gingiva-coloured composite and acrylic denture bases. Methods: Materials included gingiva-coloured composite (Fusion Universal G1), acrylic (Imicryl), and subdivided Procryla group. Subgroups comprised 15 and 30-minute heat polymerized (Pro15, Pro30), and 1 wt% (Pro1Z) and 3 wt% (Pro3Z) zirconium added groups. Immersed in beverages for 1, 7, and 14 days, pH and microhardness were assessed. SEM examined random samples. Statistical analysis used repeated measures ANOVA, and post hoc tests (p Results: The gingiva-coloured composites displayed noteworthy time-associated microhardness changes (p 0.05). Despite variable pH levels in beverages, no substantial group interaction effects were observed (p > 0.05). Initial microhardness rankings shifted after a 14-day immersion. Conclusions: Gingiva-coloured composite exhibited the highest microhardness pre- and post-immersion, followed by Procryla30 and Imicryl groups. .

Gaussian Kernel Based SVR Model for Short-Term Photovoltaic MPP Power Prediction

Predicting the power obtained at the output of the photovoltaic(PV)system is fundamental for the optimum use of the PV ***,it varies at different times of the day depending on intermittent and nonlinear environmen-tal conditions including solar irradiation,temperature and the wind speed,Short-term power prediction is vital in PV systems to reconcile generation and demand in terms of the cost and capacity of the *** this study,a Gaussian kernel based Support Vector Regression(SVR)prediction model using multiple input variables is proposed for estimating the maximum power obtained from using per-turb observation method in the different irradiation and the different temperatures for a short-term in the DC-DC boost converter at the PV *** performance of the kernel-based prediction model depends on the availability of a suitable ker-nel function that matches the learning objective,since an unsuitable kernel func-tion or hyper parameter tuning results in signiﬁcantly poor *** this study for theﬁrst time in the literature both maximum power is obtained at max-imum power point and short-term maximum power estimation is *** evaluating the performance of the suggested model,the PV power data simulated at variable irradiations and variable temperatures for one day in the PV system simulated in MATLAB were *** maximum power obtained from the simu-lated system at maximum irradiance was 852.6 *** accuracy and the perfor-mance evaluation of suggested forecasting model were identiﬁed utilizing the computing error statistics such as root mean square error(RMSE)and mean square error(MSE)*** and RMSE rates which obtained were 4.5566*10-04 and 0.0213 using ANN *** and RMSE rates which obtained were 13.0000*10-04 and 0.0362 using SWD-FFNN *** SVR model,1.1548*10-05 MSE and 0.0034 RMSE rates were *** the short-term maximum power prediction,SVR gave higher prediction performance according to ANN and SWD-FFNN.

Removal of heavy metals from aqueous solution by sawdust adsorption

The adsorption of lead, cadmium and nicel from aqueous solution by sawdust of walnut was investigated. The effect of contact time, initial metal ion concentration and temperature on metal ions removal has been studied. The equilibrium time was found to be of the order of 60 min. Kinetics fit pseudo first-order, second-order and intraparticle diffusion models, hence adsorption rate constants were calculated. The adsorption data of metal ions at temperatures of 25, 45 and 60~C have been described by the Freundlich and Langmuir isotherm models. The thermodynamic parameters such as energy, entropy and enthalpy changes for the adsorption of heavy metal ions have also been computed and discussed. Ion exchange is probably one of the major adsorption mechanisms for binding divalent metal ions to the walnut sawdust. The selectivity order of the adsorbent is Pb（I1）~Cd（II）〉Ni（I1）. From these results, it can be concluded that the sawdust of walnut could be a good adsorbent for the metal ions from aqueous solutions.

Performance of Lung Cancer Prediction Methods Using Different Classification Algorithms

In 2018,1.76 million people worldwide died of lung *** of these deaths are due to late diagnosis,and early-stage diagnosis significantly increases the likelihood of a successful treatment for lung *** learning is a branch of artificial intelligence that allows computers to quickly identify patterns within complex and large datasets by learning from existing ***-learning techniques have been improving rapidly and are increasingly used by medical professionals for the successful classification and diagnosis of early-stage *** are widely used in cancer *** particular,machine learning has been used in the diagnosis of lung cancer due to the benefits it offers doctors and *** this context,we performed a study on machine-learning techniques to increase the classification accuracy of lung cancer with 32×56 sized numerical data from the Machine Learning Repository web site of the University of California,*** this study,the precision of the classification model was increased by the effective employment of pre-processing methods instead of direct use of classification *** datasets were derived with pre-processing methods and six machine-learning classification methods were used to achieve this *** study results suggest that the accuracy of the k-nearest neighbors algorithm is superior to random forest,naïve Bayes,logistic regression,decision tree,and support vector *** performance of pre-processing methods was assessed on the lung cancer *** most successful preprocessing methods were Z-score(83%accuracy)for normalization methods,principal component analysis(87%accuracy)for dimensionality reduction methods,and information gain(71%accuracy)for feature selection methods.

A Computational Investigation of the Thermohydraulics of an EGS Project

A numerical analysis of the thermohydraulics of an enhanced geothermal system project is presented. The rock structures are modelled as porous medium, based on the computationally obtained hydraulic fracturing data of other authors. The influence of the domain size, grid resolution, temporal resolution and the discretization scheme is assessed to obtain a highly accurate numerical solution under the prevailing modelling assumptions. Based on the numerical model, different production scenarios are investigated and discussed. The relative positioning of the injection and production ports is also analyzed. It is shown that there is a considerable potential for optimizing the production rate and the port configuration to obtain the most favorable results for the production temperature, investment costs and operation costs.
