Background: Epstein Barr Virus infection (EBV) could be associated with cardiovascular disease, including myocarditis. We informed a case of EBV infection presenting initially as myocarditis, followed 5 days later by the typical symptoms of infectious mononucleosis. Case Presentation: A 19-year-old man with persistent retrosternal chest pain was admitted to the emergency department. On physical examination, the patient had sub-febrile fever (37.2°C) and palpable cervical lymphadenopathy, with no hepatosplenomegaly. Initial ECG has repolarization abnormalities in leads II, aVF and III. Laboratory testing revealed elevated cardiac enzymes and liver enzymes (high sensitive troponin I levels 3000 ng/mL, aspartate transaminase 158 U/L, alanine transaminase 100 U/L). Blood white cells were 10,500 μL, platelet level were low 98,000 (thrombocytopenia), and lenfo-monocytosis in complete blood count. We hospitalized the patients the intensive coronary unit (ICU) because of high troponin levels. We recognized hypokinesia of the posterolateral wall of the left ventricle with mild impaired systolic function and increased perimyocardial brightness by echocardiography. Antibody serology tests showed that the anti-EBV capsid antigen IgM (EBV-VCA) was positive, EBV-VCA) IgG was negative. Other etiologies were excluded. Therefore, the patient was referred to cardiac positron emission tomography due to technic problems of magnetic resonance imaging device. We documented in positron emission tomography imaging that increased 18F-FDG uptake on posterior and posterolateral walls of left ventricle (indicating a large jeopardized area). We diagnosed suspected diagnosis of myocarditis without associated pericarditis. We monitored the patient and gave beta blocker, and aldosterone antagonists. Complications in the intensive care unit not occurred. We discharged patients after one week. After three weeks, we re-evaluated the patient. We not observed wall segment motion abnormality

Dear Editor,We read with great interest the recent article by Tapper et al.[1].The authors aimed to determine the clinical predictors of advanced histology in the referral *** multivariate logistic regression analyses,they have demonstrated that body mass index(BMI)30 kg/m2,female gender,and aspartate aminotransferase(AST)>40 IU/L were associated with a non-alcoholic fatty liver disease(nAFLD)activity score>*** have also shown that ASTto-platelet ratio index(APRI)>1 was the most significant predictor of advanced ***,they have suggested that patients with suspected nAFLD should be routinely evaluated for advanced liver disease,including non-invasive indices of fibrosis such as APRI,and serious consideration given to liver *** the value of non-invasive indices of fibrosis such as APRI in patients with nAFLD,we consider this large,retrospective study to be very important;however,we have some questions that should be mentioned as contributory factors.

Purpose: We evaluated biochemical analysis results with the aim of discovering serum levels that have possible effects on the development of retinopathy of prematurity (ROP). Methods: A retrospective study was conducted between January 2017 and January 2018 on a total of 110 infants with 35 or less gestational weeks. The sample included 78 infants who had been diagnosed with different stages of ROP and 32 infants without ROP. Results from routine serum biochemical analyses, performed at birth and at one month after birth, were evaluated. The Independent Sample t-test and Mann-Whitney U test were performed to compare the data. Results: The infants with ROP were born at a mean of 28.0 (±2.1) weeks of gestation, weighing a mean of 1066 (±314) g, and the mean duration of stay in the incubator was 38.2 (±19) days. The infants without ROP were born at a mean of 29.6 (±2.6) weeks of gestation, weighing a mean of 1265 (±372) g. 59 infants (53.6%) were females, and 51 (46.4%) were males. Between infants with and without ROP, differences in the following levels were not statistically significant: glucose, blood urine nitrogen, creatinine, alanine aminotransferase, aspartate aminotransferase, albumin, total protein, direct bilirubin, indirect bilirubin, uric acid, and phosphorus. Serum magnesium (Mg) levels at birth in infants with ROP were significantly higher than infants without ROP (p = 0.014). Conclusions: Serum Mg levels at birth were found to be higher in infants with ROP than in those without ROP. The levels may be dependent on the mothers’ treatment of Mg for different medical reasons.

Eczema herpeticum is a severe infectious eruption caused by Herpes group viruses. The disease usually occurs in the patients with pre existing dermatosis, expecially atopic dermatitis. A three-year-old boy who has high fever and widespread skin eruptions with vesiculopustuler eruptions, erosions and ulcers was brought out to our emergency clinic. The patient had no pre-existing dermatosis, but he had suffered from chicken-pox. As serological, the antibodies of Varicella-IgM and Herpes simplex-IgM were positive. Eczema Herpeticum which occurred on a chicken-pox was diagnosed according to his history, physical examination, laboratory and pathologic findings. Acyclovir and empiric antibiotherapy together with the symptomatic therapy were used for treatment. His skin lesions recovered in one week after these therapies. The aim of this case-report is to emphasize the features of eczema herpeticum which occurred on a pre-existing varicella eruption.

目的：评估在白内障超声乳化手术时使用适当的手术技巧及相应设备对假性剥脱综合征白内障患者的视力及术中、术后并发症的影响。方法：回顾性研究。选取53例67眼假性剥脱综合征白内障患者接受超声乳化及人工晶状体植入术。平均年龄71.68±9.96（53~89）岁,其中男性24例（45%）,女性29例（55%）,包括核性、皮质性及后囊下和成熟期的白内障。结果：核性白内障较其他类型更为常见（P=0.00）。术前平均最佳矫正视力为0.99±0.30（0.40~1.50）LogM AR,术后平均最佳矫正视力为0.32±0.31（0.00~1.00）LogM AR（P=0.00）。运用虹膜牵拉器12眼（18%）。囊袋张力环植入15眼（22%）,其中按计划植入8眼（12%）,未计划7眼（10%）。后囊破裂4眼（6%）,其中玻璃体脱出2眼（3%）,并接受前段玻璃体切割术;且由于后囊破裂较大,此2眼（3%）转为白内障囊外摘除术。持续性角膜水肿4眼（6%）,前房反应5眼（7%）。人工晶体脱位4眼（6%）,仅1眼（1.5%）需复位。后囊混浊并接受nd：YAG激光后囊切开术13眼（20%）。结论：假性剥脱综合征白内障患者术后视力良好,但术中及术后可能出现的并发症包括后囊破裂,玻璃体脱出,转为白内障囊外摘除术,持续性角膜水肿,前房反应及人工晶状体脱位。为避免并发症的发生,适当的手术技巧和相应设备的使用在手术时是必不可少的。

Level of Knowledge and Attitude of the Patients Older than 65 Years About Pneumococcal Vaccine

Pneumococcal vaccine is a high protectional vaccine that is especially recommended for patients with COPD （chronic obstructive pulmonary disease） over 65 years. It is important for the prevention of many complications secondary to infection. In present study, it is aimed to determine the level of knowledge and attitude of the patients over 65 years, who had admitted to hospital for any reason, about pneumococcal vaccine. Study was performed on December 2013. Thirty-three patients over 65 years, who were admitted to ｜zmir Tepecik Education and Research Hospital for any reason, were chosen randomly. A questionnaire, related with this aim, was performed. Date were evaluated as percentages. Fifteen patients were men （45.6%） and 18 were women （54.4%）. Forty-two percent were housewife （n：14）, 18.4% （n：6） were retired, others were from different professions. Only I patient had an university degree and 14.8% （n：5） were illiterate. number of the patients who heard about pneumonia was 18 （54.6%）. Among them, 94% （n： 17） described pneumonia correctly. Only 1 patient had pneumonia, hospitalized and treated in the past. All of the patients stated that they don＇t have lower respiratory tract infection and influenza infection so often. Sixty-three point five percent （n：21） of the patients has heard about pneumococcal vaccine. Of the 21, 54.5% had heard from their family doctor, 46.5% from social media. none of the patients have an idea about the protective effects of the vaccine. Only 18.4% （n：6） of the patients had pneumococ vaccine, and only 12.3% of them mentioned about its benefits. The percentage of patients who have correct knowledge about vaccination period was 21.7% （n：7）. The percentage of patients who know the side effects of the vaccine was around 33.2% （n：11）. Knowledge level of the patients over 65 years about pneumococvaccination was quite low and only small part of the patients were vaccinated. Family doctors must play an important role on this subject.

Purpose: Retinopathy of prematurity (ROP), an abnormal proliferation of retinal vessels in premature infants with low birth weight, develops due to many factors. This study investigated a possible correlation between haematological parameters and ROP development. Method: This study included 189 infants without ROP and 128 with ROP. All were born at 35 weeks’ gestation or earlier, had a CBC drawn within 72 hours of birth, and had haemogram data for the first month. Haemoglobin (Hb), haematocrit (Hct), mean corpuscular volume (MCV), mean corpuscular haemoglobin (MCH), mean corpuscular haemoglobin concentration (MCHC), red cell distribution width (RDW), platelet counts (PLT), mean platelet volume (MPV) and platelet distribution width (PDW) values were obtained from hospital data and retrospectively analysed. Results: The mean gestational age was 31 weeks and 29 weeks for the control and ROP groups, respectively;the mean birth weights were 1757 g and 1332 g, respectively. The ROP group’s birth Hb, MCV and RDW were significantly lower than the control group (p p p p Conclusion: Premature infants with ROP had lower Hb and increased platelet, MPV and PDW in early postnatal life than infants without ROP. In the first month, when ROP develops, low PLT, PDW and Hb and high MPV could be indicators for ROP diagnosis, follow-up and treatment.
