A consensus meeting of national experts from all major national hepatobiliary centres in the country was held on May 26,2023,at the Pakistan Kidney and Liver Institute&Research Centre(PKLI&RC)after initial consultations with the *** Pakistan Society for the Study of Liver Diseases(PSSLD)and PKLI&RC jointly organised this *** effort was based on a comprehensive literature review to establish national practice guidelines for hilar cholangiocarcinoma(hCCA).The consensus was that hCCA is a complex disease and requires a multidisciplinary team approach to best manage these *** coordinated effort can minimise delays and give patients a chance for curative treatment and effective *** diagnostic and staging workup includes high-quality computed tomography,magnetic resonance imaging,and magnetic resonance *** cytology or biopsy utilizing endoscopic retrograde cholangiopancreatography is a mainstay for ***,histopathologic confirmation is not always required before *** ultrasound with fine needle aspiration of regional lymph nodes and positron emission tomography scan are valuable adjuncts for *** only curative treatment is the surgical resection of the biliary tree based on the Bismuth-Corlette *** patients with unresectable hCCA can be considered for liver *** chemotherapy should be offered to patients with a high risk of *** use of preoperative biliary drainage and the need for portal vein embolisation should be based on local multidisciplinary *** with acute cholangitis can be drained with endoscopic or percutaneous biliary *** chemotherapy with cisplatin and gemcitabine has shown improved survival in patients with irresectable and recurrent hCCA.

BACKGROUND Coagulopathy and thromboembolic events are associated with poor outcomes in coronavirus disease 2019(COVID-19)*** is conflicting evidence on the effects of chronic anticoagulation on mortality and severity of COVID-19 *** To summarize the body of evidence on the effects of pre-hospital anticoagulation on outcomes in COVID-19 *** A Literature search was performed on LitCovid PubMed,WHO,and Scopus databases from inception(December 2019)till June 2023 for original studies reporting an association between prior use of anticoagulants and patient outcomes in adults with *** primary outcome was the risk of thromboembolic events in COVID-19 patients taking *** outcomes included COVID-19 disease severity,in terms of intensive care unit admission or invasive mechanical ventilation/intubation requirement in patients hospitalized with COVID-19 infection,and *** random effects models were used to calculate crude and adjusted odds ratios(aORs)with 95%confidence intervals(95%CIs).RESULTS Forty-six observational studies met our inclusion *** unadjusted analysis found no association between prior anticoagulation and thromboembolic event risk[n=43851,9 studies,odds ratio(OR)=0.67(0.22,2.07);P=0.49;I2=95%].The association between prior anticoagulation and disease severity was non-significant[n=186782;22 studies,OR=1.08(0.78,1.49);P=0.64;I2=89%].However,pre-hospital anticoagulation significantly increased all-cause mortality risk[n=207292;35 studies,OR=1.72(1.37,2.17);P<0.00001;I2=93%].Pooling adjusted estimates revealed a statistically non-significant association between pre-hospital anticoagulation and thromboembolic event risk[aOR=0.87(0.42,1.80);P=0.71],mortality[aOR=0.94(0.84,1.05);P=0.31],and disease severity[aOR=0.96(0.72,1.26);P=0.76].CONCLUSION Prehospital anticoagulation was not significantly associated with reduced risk of thromboembolic events,improved survival,and lower disease seve

针对现有软体气动机器人制备成本高、设计灵活性差、功率密度低、控制技术复杂等问题,文章以鞘芯结构的涤纶∕氨纶弹性纱为基材编织针织罗纹组织,以涤纶纱为纬向衬垫纱间隔织入,形成力学各向异性的针织衬纬组织,并将该衬纬组织作为包覆层与气囊内胆装配,实现具有高致动应变、低能量耗散的高性能纺织基气动机器人开发。文章通过力学性能和驱动变形测试,表征分析了衬纬组织织物层的各向异性力学特征和针脚排列方式等因素对软体机器人驱动性能、变形特征、输出力和运动状态的影响规律。结果表明:纺织基软体机器人展现了优良的弯曲应变(弯曲曲率可达0.27 cm^(-1)),高的驱动力输出(13 N∕kg)。此外,开发的纺织基气动机器人具有低成本、设计灵活快捷、操控简单等优势,可通过编织结构设计和针脚方向的配置实现多模式的复杂变形,在可穿戴医疗康复训练设备和软体抓取器件等领域具有可观的应用前景。

Solid-state sodium metal batteries utilizing inorganic solid electrolytes(SEs) hold immense potentials such as intrinsical safety,high energy density,and environmental ***,the interfacial inhomogeneity/instability at the anode-SE interface usually triggers the penetration of sodium dendrites into the electrolyte,leading to short circuit and battery ***,confronting with the original nonuniform and high-resistance solid electrolyte interphase(SEI) at the Na-Na3Zr2Si2PO12interface,an oxygen-regulated SEI innovative approach is proposed to enhance the cycling stability of anode-SEs interface,through a spontaneous reaction between the metallic sodium(containing trace amounts of oxygen)and the *** oxygen-regulated spontaneous SEI is thin,uniform,and kinetically stable to facilitate homogenous interfacial Na+transportation,Benefitting from the optimized SEI,the assembled symmetric cell exhibits an ultra-stable sodium plating/stripping cycle for over 6600 h under a practical capacity of 3 mAh *** si-sol id-state batteries with Na3V2(PO4)3cathode deliver excellent cyclability over 500 cycles at a rate of 0.5 C(1 C=117 mA cm-2) with a high capacity retention of95.4%.This oxygen-regulated SEI strategy may offer a potential avenue for the future development of high-energy-density solid-state metal batteries.

Artificial neural network based modeling of liquid membranes for separation of dysprosium

In recent years,the liquid membrane process has been widely investigated to remove rare earth ***,transport modeling of this process requires the accurate values of several parameters,which are difficult to ***,the accurate simulation of this process is a challenging *** this study,the artificial neural network(ANN)based approach is used to model the liquid membrane process for removing *** results from a previous study were used to train the ***,the number of neurons in the hidden layer was *** minimum mean squared error between experimental results and model predictions is found with ten *** predictions were successfully validated with experimental results with correlation factor(R)of 0.9987,which confirms the authenticity of the trained *** ANN was then used to study the effects of different operating parameters on transport *** higher volume ratio of membrane solution to feed solution(3-4)with 50-60 min of operation,higher feed pH(5),HCl concentration in stripping solution of 2 mol/L,and moderate concentration of carrier species(0.5 mol/L)with 0.5×10^(-4) mol/L dysprosium initial concentration are found to be optimum values of operating conditions for maximizing the transport rate.

Tree ring derived summer temperature variability over the past millennium in the western Himalayas of northern Pakistan

Long-term high resolution climate proxies are essential for understanding climate variability particularly,in regions such as the western Himalayas of northern Pakistan,where few long-term climate records are *** standard dendrochronological methods,an 1132-year(882 to 2013 C.E.)tree-ring chronology of Juniperus excelsa *** was established from the western Himalayas,northern Pakistan(WHNP).Tree growth was negatively and significantly(r=-0.65)correlated with the growing season(June-July)mean temperature,and positively and weakly(r=0.22)associated with *** inverse relationship of tree radial growth with temperature and positive association with precipitation demonstrated that forest growth is sensitive to high temperature related *** a reliable STD chronology and robust reconstruction model,a 928-year(1086 to 2013 C.E.)mean temperature reconstruction was developed for the WHNP using the substantial negative correlation between the summer temperature and standard tree ring-width *** to statistical validation,the reconstruction accounted for 41.6% of the climatic variation for the period of 1956-2013 *** *** extreme-warm periods occurred in 1093 C.E.(29.42℃)and extreme cold periods in 1088 C.E.(26.99℃)observed during the past 928 *** reconstruction's multi-taper method(MTM)spectral analysis reveals significant(p<0.05)2-3-year and 63.8-year *** the 2-3-year cycle occurred within the range of ENSO variation,which indicates that ENSO had an impact on the regional temperature in our studied area.

In this research work,we constructed the optical soliton solutions of nonlinear complex Kundu-Eckhaus(KE)equation with the help of modified mathematical *** obtained the solutions in the form of dark solitons,bright solitons and combined dark-bright solitons,travelling wave and periodic wave solutions with general *** our knowledge earlier reported results of the KE equation with specific *** obtained solutions are more useful in the development of optical fibers,dynamics of solitons,dynamics of adiabatic parameters,dynamics of fluid,problems of biomedical,industrial phenomena and many other *** calculations show that this technique is more powerful,effective,straightforward,and fruitfulness to study analytically other higher-order nonlinear complex PDEs involves in mathematical physics,quantum physics,Geo physics,fluid mechanics,hydrodynamics,mathematical biology,field of engineering and many other physical sciences.
