The value of prognostic markers for pediatric trauma patients

BACKGROUND: Despite the rapid development of pediatric intensive care medicine, there are still limited data in the literature regarding the follow-up of pediatric trauma patients in pediatric intensive care units(PICUs). In this study, we aim to evaluate our experience with children admitted and followed up with the diagnosis of trauma at our ***: We evaluated the retrospective data of 77 pediatric trauma patients who were admitted to the PICU at Sancaktepe Sehit Prof. Dr. IlhanVarank Training and Research Hospital from August 2020 to December 2022. The demographic data, clinical parameters and laboratory results were recorded. The primary outcome was the mortality in PICU. The performances of markers in predicting mortality were evaluated with receiver operating characteristic(ROC) ***: The median age of the patients was 70(33–157) months, and the median duration of hospitalization in the PICU was 6(2–11) d. Of the 77 patients, 9 died due to trauma(11,1%). Among the clinical parameters, Pediatric Risk of Mortality III(PRISM III) Score, inotrope requirement, extracorporeal treatment requirement, and mechanical ventilator requirement were significantly higher in non-survivors than in survivors. Among the laboratory parameters, procalcitonin(PCT), lactate/albumin ratio(LAR), neutrophil/lymphocyte ratio(NLR), and transfusion requirement were significantly higher in non-survivors than in ***: In pediatric trauma patients, baseline PCT, LAR, and NLR values can be used to identify patients at risk for mortality.

Karapınar region(Konya,Türkiye)is one of the important regions of the world in terms of sinkhole *** research aimed to map the sinkholes in detail,to determine their spatial distribution and geometrical *** this purpose,the long axes,short axes,depths and the proximity to settlements of the sinkholes were measured and their circumferences and areas were *** the studies,the relationship of the sinkholes with lithology,their cross sections,shapes and the related structures were determined and the sinkholes were divided into five main groups as following:Basement rock sinkholes,Obruk Plateau sinkholes,Seyithacı sinkholes,Siyeklik sinkholes and Basin *** d/l ratios of each sinkhole group were separately determined and ***,most of the d/l ratios are smaller than ***,the long axes are higher than the depth and it indicates that the shallow and wide sinkholes are common in the *** has been determined that the fracture systems in the region,as well as the lithology and groundwater factors,are quite effective in the formation and distribution of the sinkholes in the Karapınar *** the distance of the sinkholes to the settlements,Seyithac?sinkholes are the most risky group in the region because they are deep and close to the settlements.

Effect of preoperative immunonutrition and other nutrition models on cellular immune parameters

AIM:To evaluate the effects of preoperative immunonutrition and other nutrition models on the cellular immunity parameters of patients with gastrointestinal tumors before surgical *** addition,effects on postoperative complications were examined. METHODS:Patients with gastrointestinal tumors were randomized into 3 *** immunonutrition group received a combination of arginine,fatty acids and *** second and third group received normal nutrition and standard enteral nutrition,*** protocols were administered for 7 d prior to the *** parameters,in particular prealbumin levels and lymphocyte subpopulations(CD4+,CD8+,CD16+/56+,and CD69 cells)were evaluated before and after the nutrition *** were compared in terms of postoperative complications and duration of hospital stay. RESULTS:Of the 42 patients who completed thestudy,16 received immunonutrition,13 received normal nutrition and 13 received standard enteral nutrition. prealbumin values were low in every group,but this parameter was improved after the nutritional protocol only in the immunonutrition group(13.64±8.83 vs 15.98±8.66,P=0.037).Groups were similar in terms of CD4+,CD16+/56,and CD69+prior to the nutritional protocol;whereas CD8+was higher in the standard nutrition group compared to the immunonutrition *** nutritional protocols,none of the groups had an increase in their lymphocyte ***,groups did not differ in terms of postoperative complications and postoperative durations of hospital stay. CONCLUSION:Preoperative immunonutrition provided a significant increase in prealbumin levels,while it did not significantly alter T lymphocyte subpopulation counts,the rate of postoperative complications and the duration of hospital stay.

High Velocity Forming of Aluminum Cylindrical Cups-Experiments and Numerical Simulations

A new two stage detonation forming machine was developed and cylindrical aluminum cups were formed by using gas detonation forming technology. The forming process was analyzed with the explicit finite element method with various parameters and ANSYS/LS-DYNA software. Defects of wrinkling and rupture were predicted for some forming conditions. The strain and the thickness distribution results were in good agreement with the experimental results. It was seen that thinning and forming mainly take place during the one fourth of the time. The effects of detonation pressure and blank holding force on the deformation of the work pieces were discussed. The numerical results were compared with those obtained in the experiments.

Endoscopic and histopathological evaluation of acute gastric injury in high-dose acetaminophen and nonsteroidal anti-inflammatory drug ingestion with suicidal intent

AIM： To evaluate endoscopic and histopathologic aspects of acute gastric injury due to ingestion of high-dose acetaminophen and nonsteroidal antiinflammatory drugs （NSAIDs） with respect to some risk factors and patient characteristics. METHODS： The study group consists of 50 patients admitted to emergency department with high dose analgesic ingestion （group Ⅰ ） with suicidal intent. Thirty patients with or without mild complaints of dyspepsia （group Ⅱ） were selected as the control group. The study group was stratified according to the use of type and number of analgesics. Endoscopic findings were evaluated according to the Lanza score （LS）, expressing the severity of the gastroduodenal damage and biopsies according to a scoring system based on histopathologic findings of acute erosive gastritis. RESULTS： Gastroduodenal damage was significantly more severe in group Ⅰ compared to group Ⅱ （P 〈 0.01）. The LS was similar in both groups Ⅰ a and Ⅰb. However LS was significantly higher in patients who had ingested multiple NSAIDs （group Ⅰ c） compared to other patients （P 〈 0.01）. The LS was correlated to age （P 〈 0.01） and total amount of drug ingested （P 〈 0.05） in group Ⅰ ; but it was not correlated with Helicobacter pylori （H pylori） infection or duration of exposure （P 〉 0.05）. The biopsy score （BS） was higher in group Ⅰ than group Ⅱ （P 〈 0.01）, and higher in group Ⅰb than group Ⅰa （P 〈 0.05）. CONCLUSION： The histopathologic damage was more severe among NSAID ingesting patients compared to those ingesting only acetaminophen and there is no significant difference in the endoscopic findings between the groups. There is no significant difference in the LS between the groups. This lack of significance is remarkable in terms of the gastric effects of highdose acetaminophen.

Predictive plots for conical pick performance using mechanical and elastoplastic properties of rocks

Conical picks are by far the most widely used drag type cutting tools employed on partial face rock excavation *** cutting force and specific energy are two important design parameters for the conical pick performance,and the rock cutting testing is considered as the promising tool for determining these *** the absence of an instrumented cutting rig,researchers generally rely on empirical predictive *** this,this paper suggests predictive plots for estimating the cutting force and specific energy,in consideration of the cutting depth to define the cuttability with conical *** this context,rock cutting tests were carried out on six volcanic rock samples with varying cutting depths using the unrelieved and relieved cutting *** cutting force and specific energy were correlated with the uniaxial compressive strength,Brazilian tensile strength,elasticity modulus,and plasticity *** plots were proposed for different cutting depths in the unrelieved and relieved cutting modes and exponential relationships were obtained among the cuttability parameters,and mechanical and elastoplastic properties of rocks.

Ocular alignment and results of strabismus surgery in neurologically impaired children

AIM:To evaluate ocular alignment and surgical results of strabismus surgery in neurologically impaired children.· METHODS:Files of 33 neurologically impaired squint children were *** patients had cerebral palsy(CP),4 had CP with mental retardation,therest had mental-motor retardation of unknown *** refractions,type and angle of strabismus,surgeries performed were recorded.· RESULTS:Mean follow-up was 34.0±16.5 ***-three patients had esodeviation,the remaining had *** 19 patients,angle of deviations measured at different visits were highly *** patients with stable angle of deviations or with unstable but high angle deviations had horizontal muscle *** horizontal deviation decreased from 43.75±10,69 D to 12.83±8.38D with surgery.· CONCLUSION:In neurologically impaired strabismic children,surgery is effective in patients with stable deviations or unstable but high angle *** for surgery should be made after a long follow up period.

Prognostic significance of primary tumor localization in stage Ⅱ and Ⅲ colon cancer

AIM To investigate the effects of tumor localization on disease free survival(DFS) and overall survival(OS) in patients with stage Ⅱ-Ⅲ colon *** This retrospective study included 942 patients with stage Ⅱ and Ⅲ colon cancer, which were followed up in our clinics between 1995 and 2017. The tumors from the caecum to splenic flexure were defined as right colon cancer(RCC) and those from splenic flexure to the sigmoid colon as left colon cancer(LCC).RESULTS The median age of the patients was 58 years(range: 19-94 years). Male patients constituted 54.2%. The rates of RCC and LCC were 48.4%(n = 456) and 51.6%(n = 486), respectively. During the median follow-up of 90 mo(range: 6-252 mo), 14.6% of patients developed recurrence and 9.1% of patients died. In patients with stage Ⅱ and Ⅲ disease with or without adjuvant therapy, DFS was similar in terms of primary tumor localization(stage Ⅱ; P = 0.547 and P = 0.481, respectively; stage Ⅲ; P = 0.976 and P = 0.978, respectively). In patients with stage Ⅱ and Ⅲ disease with or without adjuvant therapy, OS was not statistically significant with respect to primary tumor localization(stage Ⅱ; P = 0.381 and P = 0.947, respectively; stage Ⅲ; P = 0.378 and P = 0.904, respectively). The difference between median OS of recurrent RCC(26 ± 6.2 mo) and LCC(34 ± 4.9 mo) cases was eight months(P = 0.092).CONCLUSION Our study showed no association of tumor localization with either DFS or OS in patients with stage Ⅱ or Ⅲ colon cancer managed with or without adjuvant therapy. However, post-recurrence OS appeared to be worse in RCC patients.
