Surgical Treatment of Chronic Constrictive Pericarditis in a Developing Country

Background: Chronic constrictive pericarditis is a rare but serious clinical entity with a poor prognosis in the absence of surgical treatment. The purpose of this study was to report on our experience of subtotal anterior pericardectomy and analyze our surgical results. Results: We included 74 patients operated on for chronic constrictive pericarditis in our institution during the period from January 1985 to December 2020. There were 29 female and 45 male patients, with an average age of 28 years (range: 8 - 64 years). 36.5% of patients were in NYHA class III or IV. Physical signs were dominated by peripheral signs of right heart failure in 93.2% of cases. The surgical procedure was a subtotal anterior pericardectomy from the left to the right phrenic nerve, freeing the heart chambers and the large vessels. The surgical results were marked by a functional improvement in 82.4% of the cases. The postoperative complications were marked by a low cardiac output in 8.1% of the cases, a atrial fibrillation in 4.1% of the cases, a haemorrhage in 1.4% of the cases, a haemothorax in 2.7% of the cases. Perioperative mortality was 5.4% patients. The causes of death were low cardiac output in 50% of cases, hemorrhage in 25% of cases, and hepatocellular insufficiency in 25% of cases. We observed a mortality of 2.9% after an average follow-up of 5.17 years ± 4.76 years. All other survivors were asymptomatic and no re-intervention for recurrence was performed. Conclusions: Subtotal anterior pericardectomy was the technique we used for the treatment of chronic constrictive pericarditis. It allowed having satisfactory surgical results with a functional improvement and an acceptable morbidity.

Introduction: Despite the advances of interventional catheterization, surgery remains the treatment of choice for some coronary lesions. Objective: To report the indications and results of surgical revascularization of the myocardium at the Abidjan Heart Institute. Patient and Methods: This is a retrospective study of patients with coronary insufficiency who underwent surgical myocardial revascularization between March 2014 and May 2020 in the Cardiovascular Surgery Department of the Abidjan Heart Institute. There were 17 patients, 11 of whom were men (64.7%) and 6 women (35.3%), The mean age of the patients was 57.5 years ± 8.8. All patients were symptomatic with disabling angina in class III of the Canadian Cardiac Society (CCS). This sym- ptomatology had been evolving on average for 5 years and 6 patients had a history of acute coronary syndrome, 2 of whom had undergone prior angioplasty. Coronary angiography revealed mono-truncated (17.6%), bi-truncated (23.5%) and tri-truncated (58.8%) lesions. Results: The patients were operated under cardiopulmonary bypass (CPB) 15 cases (88.2%) and off pump in 2 cases (11.8%). They underwent a single bypass in 23.5% of cases, a double bypass in 47.1% of cases and a triple bypass in 29.4% of cases. We observed 3 cases of complications (17.6%), namely transient acute renal failure, mediastinitis and postoperative bleeding. The operative and hospital mortality was nil. With aim of place at the disposal of the laboratories nonconventional food with food values and which do not have impact on the system haematopoietic of the rats of laboratory in particular the rat of the species Rattus norvegicus, the study of the haematologic parameters of rats nourished with nonconventional food makes containing powder of Moringa oleifera was undertaken. Indeed, analysis of the blood taking away carried out following the administration of five diets container various rates of powder of Moringa oleifera (0% (Control food), 25%, 50%, 75% and 100%) respectively codified L3P, L3P25, L3P50, L3P75 and L3P100. These different food formulations were administered to rats (Ratus norvegicus) during three months. The weight of these animals was daily taken, followed by blood samples every 30 days for the determination of the blood count. The results of the study were shown differences between the various food formulations for each period of blood samples in biological parameters. These differences were more significant for platelets. During the three months of study, the hematocrit, platelets, MCV and outside the control group, were reported a slight progressive increase. The leukocyte parameters of all the rats in the investigation, indicated an increase in the second month. In conclusion, the study shows that during the three months of investigation, the tested foods do not disturb the haematological parameters except for thrombocytes which reveal a modification.

Optimized Adsorption of Small and Medium Molecules by a Biosorbent Based on Hevea Hulls

In the context of the recovery of agricultural waste, many researches have focused on the preparation of adsorbents from natural waste from fruit trees, egg shells, palm waste or sawdust. This work aims to optimize the preparation of a biosorbent from rubber hulls by studying its ability to adsorb small and medium molecules. The influence of parameters such as drying temperature (X1), particle size (X2), stirring time (X3) and sodium hypochloride mass (X4) was studied. The results indicate that the model used for biosorbent optimization on methylene blue and iodine index is significant. In addition, this model has greater adsorption capabilities on small molecules than with large molecules. Statistical analysis of the data shows that temperature is the most influential factor in the adsorption of small molecules. On the other hand, particle size has a significant influence on the adsorption of large molecules. The optimum biosorbent preparation values are 1.0 for drying temperature (X1), −1.0 for biosorbent grain size (X2), 1.0 for stirring time (X3) and 1.0 for sodium hypochloride mass (X4).

The transformation of uterine fibroids is common in relation to their development. Giant forms of cystic degeneration are rare. They raise diagnostic difficulties with other pelvic tumors, such as ovarian tumors and leiomyosarcomas. Magnetic resonance imaging specifies the original organ, the volume and the main relationships of fibromyoma with adjacent structures. The diagnosis of certainty is based on laparotomy coupled with histology. The authors illustrate these difficulties by observing a giant cystic degenerative fibroma in a 26-year-old G1P1 woman in the postpartum period.

Attitudes and Practices of Health Care Providers Regarding the Management of Uncomplicated Malaria in Abidjan, Cote d＇lvoire

Malaria is a major public health problem in the Cote d＇Ivoire where it is the leading cause of morbidity and mortality. The objective of this study was to assess the knowledge, attitudes and practices of health care providers on the diagnosis and therapeutic procedures used to ensure the management of uncomplicated malaria in the health district of Abidjan. A cross-sectional study was conducted from November 2008 to December 2008 among 169 caregivers who treated 1,691 cases of uncomplicated malaria. Results： The knowledge of the caregivers of the national guidelines regarding malaria was sufficient （89%）. A diagnosis of uncomplicated malaria was established only on clinical arguments （70%）. The percentage of patients who were prescribed artemisinin-based combination therapy （ACT） was 86%. However, 25% of the antimalarials drugs that were prescribed to patients （oral quinine, artemisinin derivatives monotherapy, and herbal medicine） did not meet the recommendations on first choice treatment as suggested by the national program against malaria. These results indicate sufficient knowledge of the national guidelines for the treatment of malaria by health care providers as evidenced by acceptable prescriptions for ACTs despite an incomplete knowledge of the guidelines.

Desorption of Methylene Blue Adsorbed on Activated Carbon from Cocoa Pod Shell

Environmental protection has become a concern for the world. For this reason, the objective of this work is to remove methylene blue adsorbed on activated carbon. The coal used comes from cocoa pod shells. Before pyrolysis, the shells were ground, sieved and impregnated with orthophosphoric acid. Before desorption, the activated carbons were initially saturated with MB. These saturated coals were brought into contact with a sodium chloride (NaCl) solution and then stirred. The evolution of the resorbed MB concentration was monitored by spectrophotometry. The desorption tests showed a remarkable elimination from the first 10 minutes. The desorption kinetics comprises two phases: a rapid kinetics between 0 and 30 minutes and a slow kinetics between 30 and 60 minutes. The desorption of the dye reaches a concentration aqual to 0.84 mg/l at pH = 4 at temperature = 80°C. For modeling, the coefficient of the Langmuir II model is greater than or equal to O.9893. The model of Langmuir III is less than or equal to 0.9373. The Freundlich model coefficient is 0.9842 or less. The desorption is thefore carried out on energy-homogeneous adsorption sites and without any interaction between the adsorbed cations of the dye. Experimental parameters such as pH, temperature and concentration of sodium chloride (NaCl) solution influence the desorption of MB. And the model of Langmuir II describes well the process of desorption of the MB.
