Knowledge, Attitudes and Practices of Health Care Staff about Glaucoma in Lomé

Introduction: Glaucoma is a major cause of irreversible blindness worldwide. Early detection and awareness are essential to reduce the impact on eye health. The aim of this study is to assess the knowledge, attitudes and practices of health care staff about glaucoma in Lomé. Materials and Methods: This was a prospective cross-sectional and descriptive study including health care staff in Lomé, from October 1st to December 31st, 2020, for a period of 3 months. After their agreement, the officers responded to questions based on a survey sheet. Results: A total of 169 agents from 3 centers in Lomé, i.e. the Sylvanus Olympio University Teaching Hospital, the Campus University teaching Hospital and the Bè Hospital were involved. 50.9% of the agents were male versus 49.1% female, i.e. a ratio of 1.04. The average age was 33.66 ± 10.45 years. 46.10% were doctors versus 53.90% paramedics. 58.60% had at most 5 years of experience against 41.40% who had more than 5 years of experience. 53.80% defined glaucoma as ocular hypertension while 17.20% defined it as optic nerve disease. 46.20% did not know that there were many types of glaucomas compared to 53.20% who knew that there are many types of glaucomas. 92.30% knew that glaucoma can lead to cause visual loss. Regarding attitudes and practices about glaucoma, 91.70% declared that the follow-up should be done by the ophthalmologist, 81.10% thought that it would be necessary to be checked by the doctor only every 6 months. Finally, 65.08% declared that they had to use eye drops for life for the treatment. Conclusion: Steps should be taken to educate staff more about glaucoma attitudes and practices and also plan for action in the population.

Bilateral Iris Coloboma Revealed by a Decreased Vision: About the First Case in Togo Observed in Kara University Teaching Hospital

Background: Ocular coloboma is the product of an error in the fetal fissure closure, normally occurring between the fifth and sixth weeks of gestation [1]. It may involve the cornea, iris, zonula, ciliary body, choroid, retina and optic nerve. The incidence of this syndrome is 0.7 per 10,000 live-births [1]. The aim of this observation is to present the first case of bilateral coloboma of the iris [1]. This case was associated with an ametropia causing a decrease in visual acuity. Case Presentation: This was a clinical observation concerning a young 12-year-old patient who consulted for blurring of vision which had progressed for approximately 2 years. The ophthalmologic examination revealed an ametropia with a bilateral notch of the pupillary rim suggesting a bilateral coloboma. There was no association with another coloboma such as chorioretinal coloboma which is quite common and is accompanied frequently by visual symptoms. Conclusion: A visual impairment of the child can indicate ametropia. However, other congenital anomalies can be discovered as was the case in this clinical observation.

Investigation of Acoustomagnetoelectric Effect in Bandgap Graphene by the Boltzmann Transport Equation

We study the acoustomagnetoelectric (AME) effect in two-dimensional graphene with an energy bandgap using the semiclassical Boltzmann transport equation within the hypersound regime, (where represents the acoustic wavenumber and is the mean free path of the electron). The Boltzmann transport equation and other relevant equations were solved analytically to obtain an expression for the AME current density, consisting of longitudinal and Hall components. Our numerical results indicate that both components of the AME current densities display oscillatory behaviour. Furthermore, geometric resonances and Weiss oscillations were each defined using the relationship between the current density and Surface Acoustic Wave (SAW) frequency and the inverse of the applied magnetic field, respectively. Our results show that the AME current density of bandgap graphene, which can be controlled to suit a particular electronic device application, is smaller than that of (gapless) graphene and is therefore, more suited for nanophotonic device applications.

Epidemiological, Diagnostic, Therapeutic and Progressive Aspects of Arterial Hypertension in Children at the University Hospital Center of Bouaké (Côte d’Ivoire)

Introduction: Arterial hypertension (AH) in children is under-diagnosed and often has a poor prognosis. The aim of this study was to describe the epidemiological, diagnostic, therapeutic and evolutionary aspects of hypertension in children at the University Hospital of Bouaké, with a view to improving the prognosis. Methods: This was a cross-sectional, analytical study carried out in the paediatrics department of Bouaké University Hospital. It concerned the medical records of children aged 4 to 15 hospitalised from 1 January 2017 to 31 December 2020 for hypertension. Diagnosis was based on the simplified blood pressure guidelines of the Expert Consensus of the French Society of Hypertension. The variables studied were epidemiological, diagnostic, therapeutic and evolutionary. Quantitative variables were compared at the significance level p ≤ 0.05. Results: The hospital incidence of hypertension was 0.32% (69/21,642). The sex ratio was 0.72. 97.1% of the children were over five years of age. Oedema (49.3%) and breathing difficulties (20.2%) were the main reasons for consultation. Hypertension was classified as threatening (56.5%), confirmed (31.9%) and borderline (11.6%). The cause was renal in 66.7%, dominated by impure nephrotic syndrome (24.6%). Treatment for hypertension consisted mainly of a diuretic (79.7%) and a calcium channel blocker (47.8%). Outcome was favourable in 50.7% of cases. Mortality was 20.3%. No factor was significantly associated with death. Conclusion: Hypertension in children at Bouaké University Hospital is serious. The aetiology is mainly renal. Early diagnosis and management are key to improving prognosis.

Assessment of the Vaccination Status of Children Aged 6 to 30 Months in a Tertiary Level Health Care Center in Côte d’Ivoire

Introduction: Vaccination coverage in Côte d’Ivoire over the period 2011 to 2015 was below the target of 95% for all antigens. The objective of this study was to analyze the vaccination status of children aged 6 to 30 months with a view to improving vaccination coverage. Patients and Methods: This was a descriptive cross-sectional study which took place from June to September 2018 in a tertiary health center, focusing on children aged 6 to 30 months with a correctly completed health record. The parameters studied were sex, age, educational level of mothers, dates of vaccine administration and reason for missed vaccination opportunities. Results: We retained 212 children. The sex ratio was 1.21 and 93% had received the BCG vaccine before the age of 1 month. The average ages of combined and co-administered vaccines for the 1st and 2nd doses were 7.66 ± 3.81 and 12.88 ± 3.95 weeks, respectively. The median was 16.57 weeks for the 3rd dose. The proportion of vaccinated subjects was greater than 90% for the BCG vaccine and the 3 doses of combined vaccines, and 77% for the yellow fever and measles vaccines. The reasons for non-vaccination were attributable to the children’s parents and health facilities. Conclusion: Improving vaccination coverage requires regular supply of vaccines to centers, and the involvement of all health professionals, community and religious leaders in the vaccination awareness process.

From Hai yao, Yang yao to Xi yao: Sinification of Material Medical from the West

In ancient China, Daoist philosophers developed the concepts of qi(energy), Wu Xing(five elements), and yin(feminine, dark, negative) and yang(masculine, bright, positive) opposite forces between 200 and 600 BCE. Based on these philosophies, Zhen Jiu(acupuncture), Ben Cao(materia medica), and the practice of Qi Gong(energy optimization movements) evolved as the three interrelated therapeutic regimens of Chinese medicine(Note 1). Since the time of Zhang Qian, who discovered China's western regions in the 1st century BCE, Hai yao(the exotic elements of materia medica from the maritime Silk Road countries), had been transmitted from the ancient land and maritime routes of the Silk Road to China in the past two millennia(Note 2). Since the late 17th century, the English East India Company, later called the British East India Company, introduced Yang yao(opium) to the Manchu Qing Empire to balance a growing trade deficit for tea export from China to the British Empire. After the First Opium War ended in 1842, enterprising expatriate chemists and druggists in the treaty ports imported Xi yao(modern medicines from the Western world) for sale to the merchant navy and the local market. From the second half of the 19th century onwards, both Hai yao and Xi yao have become a fully integrated part of modern China's armamentarium for the Chinese medicine and Western hospitals and retail pharmacy sectors. This paper articulates the journey of adoption of exotic elements of materia medica from the ancient land and sea routes of the Silk Road, including the western regions and the rest of the world in the past two millennia. Opium traders, ship surgeons, medical and pharmaceutical missionaries, enterprising traders, and policymakers together transformed Ben Cao into Xi yao during the late Manchu Qing dynasty and the early Nationalist Era.
