A brief review on the diagnostic and therapeutic principles of primary urethral cancer

Objective:Primary urethral carcinoma(PUC)is a rare malignant carcinoma but with limited therapeutic options.This review aims to provide an overview of the current strategies on this patient settings.Methods:Recent literature ranging from January 1987 and December 2021 was assessed through PubMed search to assess the diagnostic and therapeutic principles of PUC.Results:A complete of examination including cystoscopy,imaging,and biopsy should be conducted for these patients.Once diagnosed,the clinical decision of PUC should be made according to the tumor location,pathological pattern,and extent of the tumor.For patients with superficial and distal urethral lesions,organ sparing approaches or radical reconstructive procedures can be utilized.While for more advanced disease or nodal involvement,an optimal multimodal treatment strategy consisted of surgery and radiochemotherapy should be adopted.For patients with urothelial carcinoma of the prostate,the management including transurethral resection of the prostate followed by bacille Calmette-Guerin or radical cysto-prostatectomy should depend on the infiltration depth of PUC.Conclusion:A complete of examination is important for the diagnosis of PUC.The management of PUC should be determined by the location,pathological pattern,and extent of the tumor.More multi-institutional collaborations should be held to investigate better treatment modal-ities for PUC.

Impacts of proppant distribution on development of tight oil reservoirs with threshold pressure gradient

Field evidence indicates that proppant distribution and threshold pressure gradient have great impacts on well productivity.Aiming at the development of unconventional oil reservoirs in Triassic Chang-7 Unit,Ordos Basin of China,we presented an integrated workflow to investigate how(1)proppant placement in induced fracture and(2)non-linear flow in reservoir matrix would affect well productivity and fluid flow in the reservoir.Compared with our research before(Yue et al.,2020),here we extended this study into the development of multi-stage fractured horizontal wells(MFHWs)with large-scale complicated fracture geometry.The integrated workflow is based on the finite element method and consists of simulation models for proppant-laden fluid flow,fracture flow,and non-linear seepage flow,respectively.Simulation results indicate that the distribution of proppant inside the induced cracks significantly affects the productivity of the MFHW.When we assign an idealized proppant distribution instead of the real distribution,there will be an overestimation of 44.98%in daily oil rate and 30.63%in cumulative oil production after continuous development of 1000 days.Besides,threshold pressure gradient(TPG)also significantly affects the well performance in tight oil reservoirs.If we simply apply linear Darcy’s law to the reservoir matrix,the overall cumulative oil production can be overrated by 77%after 1000 days of development.In general,this research provides new insights into the development of tight oil reservoirs with TPG and meanwhile reveals the significance of proppant distribution and non-linear fluid flow in the production scenario design.

Single-nucleotide variants account for about half of known pathogenic genetic variants in human.Genome editing strategies by reversing pathogenic point mutations with minimum side effects have great therapeutic potential and are now being actively pursued.The emerge of precise and effcient genome editing strategies such as base editing and prime editing provide powerful tools for nucleotide conversion without inducing double-stranded DNA breaks(DSBs),which have shown great potential for curing genetic disorders.A diverse toolkit of base editors has been devel-oped to improve the editing effciency and accuracy in different context of application.Here,we summarized the evolving of base editors(BEs),their limitations and future perspective of base editing-based therapeutic strategies.

Modeling and simulation of solvent behavior and temperature distribution within long stick propellants with large web thickness undergoing drying

Drying is a complicated physical process which involves simultaneous heat and mass transfer in the removal of solvents inside propellants.Inappropriate drying techniques may result in the formation of a hard skin layer near the surface to block the free access of most solvent through for long stick propellants with large web thickness,which lead to lower drying efficiency and worse drying quality.This study aims to gain a comprehensive understanding of drying process and clarify the mechanism of the blocked layer near the propellant surface.A new three-dimensional coupled heat and mass transfer(3D-CHMT)model was successfully developed under transient conditions.The drying experiment results show that the 3DCHMT model could be applied to describe the drying process well since the relative error of the content of solvent between simulation and experiment values is only 5.5%.The solvent behavior simulation demonstrates that the mass transfer process can be divided into super-fast(SF)and subsequent minorfast(MF)stages,and the SF stage is vital to the prevention of the blocked layer against the free access for solvent molecules inside propellant grains.The effective solvent diffusion coefficient(Deff)of the propellant surface initially increases from 3.4×10^(-6)to 5.3×10^(-6)m^(2)/s as the temperature increases,and then decreases to 4.1×10^(-8)m^(2)/s at 60-100 min.The value of Deffof surface between 0-1.4 mm has a unique trend of change compared with other regions,and it is much lower than that of the internal at100 min under simulation conditions.Meanwhile,the temperature of the propellant surface increases rapidly at the SF stage(0-100 min)and then very slowly thereafter.Both the evolution of Deffand temperature distribution demonstrate that the blocked layer near the propellant surface has been formed in the time period of approximately 0-100 min and its thickness is about 1.4 mm.To mitigate the formation of blocked layer and improve its drying quality of finial propellant products effectively,it should be initial

A comparative study to determine the effects of breed and feed restriction on glucose metabolism of chickens

The glucose metabolism of poultry draws wide attention as they have nearly twice the fasting blood glucose than that of mammals.To define the relationship between glucose metabolism and breed of chicken,the outcomes from different growth rate chickens showed that Arbor Acres(AA)broilers,a well-known fast-growing breed,had a lower fasting blood glucose concentration and glucose clearance rate when compared to Silky chickens,a Chinese traditional medicinal chicken with black skin and a slow growth rate.Moreover,AA broilers had a relatively slow rise in blood glucose in response to oral glucose solution than the Silky chickens on 21 and 42 d(P<0.05),which is probably attributed to downregulated expression of pancreatic insulin(INS),and upregulated transcription of phosphoenolpyruvate carboxy kinase 1(PCK1)and glucose transporter 2(GLUT2)in the liver of AA broilers(P<0.05).In response to feeding restriction from 7 to 21 d,both the fasting blood glucose and the response speed of AA broilers to oral glucose were increased on d 21(P<0.05),and the serum glucose concentrations after 3 weeks compensatory growth were improved by early feed restriction in AA broilers.Feed restriction could also upregulate the mRNA level of pancreatic INS on d 21 and 42,as well as decrease the expressions of PCK1,glucose-6-phosphatase catalytic(G6PC),and GLUT2 in the liver on d 21(P<0.05)when compared to the free feeding group.These results revealed that Silky chickens have a stronger capability to regulate glucose homeostasis than AA broilers,and feed restriction could improve the fasting blood glucose and the response to oral glucose of AA broilers.

OBJECTIVE: To investigate blood pressure rhythm(BPR)in Yin deficiency syndrome of hypertension(YDSH)patients and develop a random forest model for predicting YDSH.METHODS: Our study was consistent with technical processes and specification for developing guidelines of Evidence-based Chinese medicine clinical practice(T/CACM 1032-2017). We enrolled 234 patients who had been diagnosed with primary hypertension without antihypertensive medications prior to the enrollment. All participants were divided into Yin deficiency group(YX, n = 74) and non-Yin deficiency group(NYX, n = 160).Participants were professionally grouped by three experienced chief Traditional Chinese Medicine(TCM)physicians according to four examinations(i.e.,inspection, listening and smelling, inquiry and palpation).We collected data on 24 h ambulatory blood pressure monitoring(ABPM) and YDSH rating scale. We divided 24 h of a day into 12 two-hour periods [Chen-Shi(7:00-9:00), Si-Shi(9:00-11:00), Wu-Shi(11:00-13:00), Wei-Shi(13:00-15:00), Shen-Shi(15:00-17:00), You-Shi(17:00-19:00), Xu-Shi(19:00-21:00), Hai-Shi(21:00-23:00), ZiShi(23:00-1:00), Chou-Shi(1:00-3:00), Yin-Shi(3:00-5:00), Mao-Shi(5:00-7:00)] according to the theory of “midnight-midday ebb flow”. We used random forest to build the diagnostic model of YDSH, with whether it was Yin deficiency syndrome as the outcome. RESULTS: Compared with NYX group, YX group had more female participants with older age, lower waist circumference, body mass index(BMI), diastolic blood pressure(DBP), and smoking and drinking rate(all P < 0.05). The YDSH rating scores of YX group [28.5(21.0-36.0)] were significantly higher than NYX group [13.0(8.0-22.0)](P < 0.001), and the typical symptoms of YX group included vexing heat in the chest, palms and soles, dizziness, dry eyes, string-like and fine pulse, soreness and weakness of lumbus and knees, palpitations, reddened cheeks, and tinnitus(all P < 0.05). Background:Escherichia coli(E.coli)infection in humans and animals usually comes with gut dysbiosis,which is potential culprit to skeletal health,it is still unclear to whether diet interfered gut microbiome changes can be a protective strategy to bone loss development.Here,the effects of resistant starch from raw potato starch(RPS),a type of prebiotic,on E.coli-induced bone loss and gut microbial composition in meat ducks were evaluated.Results:The results showed that dietary 12%RPS treatment improved bone quality,depressed bone resorption,and attenuated the pro-inflammatory reaction in both ileum and bone marrow.Meanwhile,the 12%RPS diet also increased the abundance of Firmicutes in E.coli-treated birds,along with higher production of short-chain fatty acids(SCFAs)especially propionate and butyrate.Whereas addition ofβ-acid,an inhibitor of bacterial SCFAs production,to the drinking water of ducks fed 12%RPS diet significantly decreased SCFAs level in cecum content and eliminated RPS-induced tibial mass improvement.Further,treatment with MI-2 to abrogate mucosa-associated lymphoid tissue lymphoma translocation protein 1(Malt1)activity replicated the protective role of dietary 12%RPS in E.coli-induced bone loss including reduced the inhibition on nuclear factorκB(NF-κB)inflammasome activation,decreased bone resorption,and improved bone quality,which were correlated with comparable and higher regulatory T cells(Treg)frequency in MI-2 and 12%RPS group,respectively.Conclusions:These findings suggested that the diet with 12%RPS could alleviate E.coli-induced bone loss in meat ducks by changing the gut microbial composition and promoting concomitant SCFAs production,and consequently inhibiting Malt1/NF-κB inflammasome activation and Treg cells expansion.

NbC ceramic surface-reinforced steel matrix composites were prepared by an in-situ reaction method at different temperatures(1,050℃,1,100℃and 1,150℃)for different times(1 h,2 h and 3 h).The phase constitution,microstructure and fracture morphology of NbC ceramic surface-reinforced steel matrix composites were analyzed by XRD,SEM and EDS,and the effects of the in-situ reaction temperature and time on the mechanical properties were systematically studied.The results indicate that the NbC reinforcement layer is formed through the reaction between Nb atoms and carbon atoms diffused from the steel matrix to the Nb plate.The thickness of this reinforcement layer increases as the reaction time prolongs.Additionally,an increase in reaction temperature results in a thicker reinforcement layer,although the rate of increase gradually decreases.The relationship among the thickness of the Nb C reinforcement layer,the reaction time and temperature was established by data fitting.The optimal tensile performance is achieved at 1,100℃for 1 h,with a tensile strength of 228 MPa.It is also found that the defects between the reinforcement layer and the steel matrix are related to reaction temperature.At 1,100℃,these defects are minimal.Fracture mostly occurs in the NbC reinforced layer of the composites,and the fracture mode is characterized by typical intergranular brittle fracture.

Lymph node status is a key prognostic factor in penile squamous cell carcinoma. Recently, growing evidence indicates a multimodality approach consisting of neoadjuvant chemotherapy followed by consolidation surgery improves the outcome of locally advanced penile cancer. Thus, accurate estimation of survival probability in node-positive penile cancer is critical for treatment decision making, counseling of patients and follow-up scheduling. This article reviewed evolving developments in assessing the risk for cancer progression based on lymph node related variables, such as the number of metastatic lymph nodes, bilateral lymph node metastases, the ratio of positive lymph nodes, extracapsular extension of metastatic lymph nodes, pelvic lymph node metastases, metastatic deposit in sentinel lymph nodes and N stage in TNM classification. Controversial issues surrounding the prognostic value of these nodal related predictors were also discussed.
