Objectives The predisposition of intracranial atherosclerotic disease(ICAD)to East Asians over Caucasians infers a genetic basis which,however,remains largely *** prevalence of vascular risk factors(VRFs)in Chinese over Caucasian patients who had a stroke,and shared risk factors of ICAD with other stroke subtypes indicate genes related to VRFs and/or other stroke subtypes may also contribute to *** Unrelated symptomatic patients with ICAD were recruited for genome sequencing(GS,60-fold).Rare and potentially deleterious single-nucleotide variants(SNVs)and small insertions/deletions(InDels)were detected in genome-wide and correlated to genes related to VRFs and/or other stroke *** aneuploidies,copy number variants(CNVs)and chromosomal structural rearrangements were also ***,candidate genes were used for pathway and gene ontology enrichment *** Among 92 patients(mean age at stroke onset 61.0±9.3 years),GS identified likely ICAD-associated rare genomic variants in 54.3%(50/92)of ***-eight patients(52.2%,48/92)had 59 rare SNVs/InDels reported or predicted to be deleterious in genes related to VRFs and/or other stroke *** of the 59 rare variants were identified in local subjects without ICAD(n=126).31 SNVs/InDels were related to conventional VRFs,and 28 were discovered in genes related to other stroke *** study also showed that rare CNVs(n=7)and structural rearrangement(a balanced translocation)were potentially related to ICAD in 8.7%(8/92)of ***,candidate genes were significantly enriched in pathways related to lipoprotein metabolism and cellular lipid catabolic *** Our GS study suggests a role of rare genomic variants with various variant types contributing to the development of ICAD in Chinese patients.

目的探讨伴有前循环和后循环颅内动脉粥样硬化狭窄或闭塞的缺血性卒中患者1年的死亡和卒中复发风险是否存在差异。方法在中国颅内动脉粥样硬化研究（2007年10月至2009年6月）的数据库中，剔除颅外动脉狭窄或闭塞的病例、头核磁弥散加权成像未见急性梗死灶的病例和颅内前后循环联合病变的病例，将患者分为无明显颅内动脉病变组（n=964）、前循环动脉病变组（n=440）、后循环动脉病变组（n=233）。预后评估指标为发病后1年内任何原因所致的死亡、卒中复发。结果在纳入分析的1637例患者中，1年随访结果显示，任何原因所致死亡30例，卒中复发58例。与无明显颅内动脉病变组相比，前循环动脉病变组和后循环动脉病变组校正后的死亡风险比（95％可信区间）分别为1．349（0．311～5．851）、4．542（1．227～16．813）；卒中复发分别为1．663（0．620～4．460）、2．464（0．935—6．493）。结论伴有后循环颅内动脉粥样硬化的缺血性卒中患者1年死亡风险高于前循环者，而前后循环动脉病变卒中复发风险差异有待进一步评估。

目的探讨颅内外动脉粥样硬化的相关因素是否存在差异。方法在中国颅内动脉粥样硬化研究的数据库中,将患者分为4组:无显著颅内外动脉狭窄或闭塞组、单纯颅内动脉病变组、单纯颅外动脉病变组及颅内合并颅外动脉病变组。以无显著颅内外动脉狭窄或闭塞组作为参考,应用多元Logistic回归分析,评估颅内外动脉病变的相关因素。结果共入组2864例缺血性脑血管病患者,其中无显著颅内外动脉狭窄或闭塞组1388例(48.5%)、单纯颅内动脉病变组1074例(37.5%)、单纯颅外动脉病变组141例(4.9%)、颅内合并颅外动脉病变组261例(9.1%)。多因素分析显示,单纯颅内动脉病变的独立相关因素包括:白质病变[比值比(odds ratio,OR)1.359,95%(confidence interval,CI)1.109～1.666,P=0.0031]、白细胞计数(OR 1.045,95%CI 1.007～1.084,P=0.0210)、空腹血糖(OR 1.054,95%CI 1.009～1.101,P=0.0182)、高密度脂蛋白(OR 0.644,95%CI 0.480～0.864,P=0.0034);单纯颅外动脉病变的独立相关因素包括:高龄(OR 1.047,95%CI 1.026～1.069,P<0.0001)、吸烟(OR 1.887,95%CI 1.212～2.937,P=0.0050)、既往缺血性脑血管病史(OR 2.260,95%CI 1.352～3.778,P=0.0019)和白质病变(OR 0.603,95%CI0.394～0.922,P=0.0196)。颅内合并颅外动脉病变组独立相关因素包括:高龄(OR 1.022,95%CI1.006～1.039,P=0.0064)、糖尿病(OR 1.494,95%CI 1.053～2.118,P=0.0244)和卒中家族史(OR1.964,95%CI 1.302～2.961,P=0.013)。高同型半胱氨酸血症是3组共同的独立相关因素。结论颅内外动脉粥样硬化病变的相关因素可能存在差异。白细胞计数、空腹血糖、高密度脂蛋白水平低与颅内动脉粥样硬化病变相关;高龄、吸烟、既往缺血性脑血管病史与颅外动脉粥样硬化相关。高同型半胱氨酸血症是颅内外动脉病变的共同相关因素。
