Flexible ureteroscopic treatment of kidney stones: How do the new laser systems change our concepts?

Objective: Flexible ureteroscopy (fURS) has become a widely accepted and effective technique for treating kidney stones. With the development of new laser systems, the fURS approach has evolved significantly. This literature review aims to examine the current state of knowledge on fURS treatment of kidney stones, with a particular focus on the impact of the latest laser technologies on clinical outcomes and patient ***: We conducted a search of the PubMed/PMC, Web of Science Core Collection, Scopus, Embase (Ovid), and Cochrane Databases for all randomized controlled trial articles on laser lithotripsy in September 2023 without time ***: We found a total of 22 relevant pieces of literature. Holmium laser has been used for intracavitary laser lithotripsy for nearly 30 years and has become the golden standard for the treatment of urinary stones. However, the existing holmium laser cannot completely powder the stone, and the retropulsion of the stone after the laser emission and the thermal damage to the tissue have caused many problems for clinicians. The introduction of thulium fiber laser and Moses technology brings highly efficient dusting lithotripsy effect through laser innovation, limiting pulse energy and broadening pulse ***: While the holmium:yttrium-aluminum-garnet laser remains the primary choice for endoscopic laser lithotripsy, recent technological advancements hint at a potential new gold standard. Parameter range, retropulsion effect, laser fiber adaptability, and overall system performance demand comprehensive attention. The ablation efficacy of high-pulse-frequency devices relies on precise targeting, which may pose practical challenges.

Alkaline water electrolysis is an environmentally friendly and promising approach to produce ***,high cost,low efficiency,and poor stability are roadblocks to commercialization of *** work aims to design and develop a highly efficient,durable,and cost-effective electrocatalyst toward water splitting through modifying metal–organic *** electrocatalytic performance and stability surpass those of noble metal benchmarks at high current density(1–10 A·cm^(−2)).Theoretical calculations and in situ Raman spectra reveal the electronic structure of the synthesized catalyst and the mechanism of the catalytic reaction process,which rationalizes that the high catalytic activity and stability at high current are attributed to the unique electronic structure of cobalt regulated by copper and the protection provided by carbon nanotubes formed in situ,*** addition,this paper proposes that the desorption ability of the catalyst toward the products(H_(2)and O_(2)),rather than the adsorption ability toward the reactants(H^(+)or OH^(−)),is more important to the sustainable and stable electrochemical water splitting progress at high current density,which is a kinetic rather than thermodynamic dominating *** findings provide alternative insights to design and employ high performance catalysts to fuel hydrogen production as a clean energy source to tackle the global energy crisis.

BACKGROUND Lymphangiomas in the gastrointestinal tract are extremely rare in *** a benign lesion,small intestine lymphangiomas often remain asymptomatic and pose challenges for definitive ***,lymphangiomas can give rise to complications such as abdominal pain,bleeding,volvulus,and ***,we report a case of jejunal cavernous lymphangioma that presented with intermittent melena and refractory anemia in a male *** SUMMARY A 66-year-old man presented with intermittent melena,fatigue and refractory anemia nine months *** and colonoscopy were performed many times and revealed no apparent *** management,including transfusion,hemostasis,gastric acid secretion inhibition and symptomatic treatment,was performed,but the lesions tended to recur shortly after ***,the patient underwent capsule endoscopy,which revealed a more than 10 cm lesion accompanied by active *** singleballoon enteroscopy and biopsy,a diagnosis of jejunal cavernous lymphangioma was confirmed,and the patient underwent surgical *** complications or recurrences were observed *** Jejunal cavernous lymphangioma should be considered a cause of obscure gastrointestinal *** endoscopy and single-balloon enteroscopy can facilitate *** resection is an effective management method.

Serum 25-hydroxyvitamin D,genetic susceptibility,and the risk of incident type 2 diabetes:A prospective cohort in East China

Background:The serum vitamin D level varies widely by population,and studies have linked vitamin D levels with the risk of type 2 diabetes mellitus(T2DM).However,the relationship is inconsistent and the impact of vitamin D on T2DM among East Chinese adults is *** study aimed to investigate the association between serum 25-hydroxyvitamin D(25[OH]D)levels and the risk of T2DM and evaluated whether the association is modified by genetic ***:In the Survey on Prevalence in East China for Metabolic Diseases and Risk Factors(SPECT-China)cohort,1862 participants free of T2DM at baseline were included.A weighted genetic risk score was calculated with 28 variants associated with *** logistic models were used to examine the association of serum 25(OH)D and genetic risk with ***:After a 5-year follow-up,132 cases of T2DM were *** observed no significant association between quartiles of serum 25(OH)D and T2DM risk after multivariable adjustment(χ^(2)=0.571,P_(for trend)=0.426).Compared to those in the lowest quartile of 25(OH)D,the odds ratios(ORs)(95%confidence interval[CI])for participants with increased quartiles were 1.29(0.74-2.25),1.35(0.77-2.36),and 1.27(0.72-2.24),*** observed a positive association of glycated hemoglobin(HbA1c)with 25(OH)D at baseline(β=1.752,P=0.001)and after follow-up(β=1.385,P=0.003),and a negative association of ln conversion homeostasis model assessment(HOMA)-βwith 25(OH)D at baseline(β=-0.982,P=0.021).There was no significant interaction between 25(OH)D and diabetes genetic predisposition on the risk of T2DM(χ^(2)=2.710,P_(for interaction)=0.100).The lowest OR(95%CI)of T2DM was among participants with low genetic risk and the highest quartile of 25(OH)D(0.17[0.05-0.62]).Conclusion:Serum 25(OH)D may be irrelevant to the risk of incident T2DM among East Chinese adults regardless of genetic predisposition.

Surgical resection and neoadjuvant therapy in patients with gastric cancer and ovarian metastasis: A real-world study

Actin-depolymerizing factors 8 and 11 promote root hair elongation at high pH

A root hair is a polarly elongated single-celled structure that derives from a root epidermal cell and func-tions in uptake of water and nutrients from the surrounding *** reports have demon-strated that short periods of high pH inhibit root hair extension;but the effects of long-term high-pH treat-ment on root hair growth are still ***,we report that the duration of root hair elongation is signicantly prolonged with increasing external pH,which counteracts the effect of decreasing root hair elongation rate and ultimately produces longer root hairs,whereas loss of actin-depolymerizing factor 8 and 11(ADF8/11)function causes shortening of root hair length at high pH(pH 7.4).Accumulation of ADF8/11 at the tips of root hairs is inhibited by high pH,and increasing environmental pH affects the actinlament(F-actin)meshwork at the root hair *** high pH,the tip-focused F-actin meshwork is absent in root hairs of the adf8/11 mutant,actinlaments are disordered at the adf8/11 root hair tips,and actin turn-over is *** and recycling vesicles do not aggregate in the apical region of adf8/11 root hairs at high ***,our results suggest that,under long-term exposure to high extracellular pH,ADF8/11 may establish and maintain the tip-focused F-actin meshwork to regulate polar trafcking of secretory/recycling vesicles at the root hair tips,thereby promoting root hair elongation.

揭示旅游城镇化与居民幸福水平的适配关系是巩固脱贫攻坚成果和促进乡村振兴的重要议题,也是推进共同富裕的重要环节。通过构建旅游城镇化与居民幸福水平适配理论分析框架,运用数列匹配度、地理探测器、中介效应模型等方法,对2007—2021年湘西地区适配时空演变及机制进行深入探讨。结果表明:①湘西地区旅游城镇化与居民幸福水平适配度均值为0.417,处于勉强适配水平并呈现稳步上升趋势,区域空间差异较大,存在马太效应;②适配度核心极化特征明显,形成以永定区、武陵源区、吉首市、凤凰县四大区县为核心的空间分布格局,并表现“北高南低”“极化领跑、共同发展”“中心—边缘”的空间特征,高适配区县需要发挥其带动作用推动地区均衡发展;③适配度主导驱动因子为旅游资源禀赋;旅游城镇化和居民幸福水平与适配度在高水平与低水平区域呈现空间一致性,同时适配发展地区与旅游城镇化和居民幸福水平均值呈聚集状态;主要适配路径为社会治理,次要路径为分配公平。研究在理论上为提升居民幸福水平提供了旅游地理学的解释,为制定差别化的旅游城镇化发展措施提供科学参考。

Oriented Generation of Novel Thermo-Sensitive Genic Male Sterile Lines with Improved Grain Shape and Outcrossing Rate in Early-Season Rice

Two-line hybrid rice with excellent quality is preferred in the Chinese ***,there is a trade-off between reducing costs for hybrid seed production and lowering the outcrossing rate of the sterile line,which is largely determined by the stigma exsertion rate(SER).In this study,we constructed mutants of male sterility lines with improved grain length(GL)and SER in three elite early-season indica rice varieties through targeted manipulation of the TMS5 and GS3 genes using CRISPR/Cas9-mediated multiplex *** obtained a series of marker-free gs3 single mutants and gs3tms5 double mutants with significantly higher SER,longer grains,and increased 1000-grain weight compared with the wild type(WT).Importantly,the typically thermo-sensitive genic male sterile(TGMS)trait with a higher SER was observed in gs3tms5 mutants,and their F1 hybrids exhibited remarkable improvements in grain shape and yield-related *** findings provided an efficient method to generate new valuable TGMS germplasm with improved SER through the mutagenesis of GS3 and TMS5 synergistically,and demonstrated that GS3 had pleiotropic effects on grain size,SER,and grain quality in early-season indica rice.

Magnetic materials with non-collinear spin orderings provide an outstanding platform to probe spin-tronic phenomena owing to their strong spin-orbit coupling(SOC)and unique Berry *** is thus important to obtain a non-collinear antiferromagnetic(AFM)phase at room temperature(RT).Signifi-cantly,the discovery of novel materials with nearly zero thermal expansion(ZTE)property near RT is required and pursued for avoiding thermal stress and fracture in spintronic ***,the doping of Sn(Ge)at the Ag site in the triangular lattice Mn_(3)Ag_(1-x)Sn(Ge)_(x)N compounds increases effectively the Neel point and makes the interesting non-collinearГ^(5g)AFM phase exist above *** magnetic phase diagrams withГ^(5g)phase up to 498 K were built by the combined analysis of neutron powder diffraction(NPD),magnetic measurements,electronic transport,and differential scanning calorimetry(DSC).The thermal expansion behaviors of Mn_(3)Ag_(1-x)Sn(Ge)_(x)N were modulated,and the nearly ZTE above RT was achieved in Mn_(3)Ag_(0.5)Ge_(0.5)N withinГ^(5g)AFM *** findings offer an effective way to tailor the non-collinear AFM ordering and correlated thermal expansion behavior for potential use in the emerging field of thermal stress-free magnetic chip materials.
