For a strong earthquake swarm, the key to estimate the frequency attenuation coefficient h-value of ensuing strong earthquake sequence accurately lies in quantitative calculation and subtraction of the foreshock sequence *** the Jiashi strong earthquake swarm sequence as an example, we analyzed and compared the h-values when foreshock effect has been subtracted or not subtracted. The result shows that the boundary between the two great difference is the M56.6 earthquake on April 11,1997. The h-values with foreshock effect subtracted are all Iess than 1 before the moderately strong earthquakes, but after the earthquake, the h-values are all greater than 1. On the contrary, the h-values with foreshock effect do not show this kind of turning *** test results shows that quantitatively subtracting foreshock effect is beneficial to the judgement of the trend of the activity of the Jiashi strong earthquake swarm. This provides the basis for accurately grasping sequence development

The backreaming operation plays a significant role in safe drilling for horizontal wellbores, while it may cause severe stuck pipe accidents. To lower the risk of the stuck pipe in backreaming operations, the mechanism of cuttings transport needs to be carefully investigated. In this research, a transient cuttings transport with multiple flow patterns model is developed to predict the evolution of cuttings transported in the annulus while backreaming. The established model can provide predictions of the distribution of cuttings bed along the wellbore considering the bulldozer effect caused by large-size drilling tools(LSDTs). The sensitivity analyses of the size of LSDTs, and backreaming operating parameters are conducted in Section 4. And a new theory is proposed to explain the mechanism of cuttings transport in the backreaming operation, in which both the bit and LSDTs have the “cleaning effect” and “plugging effect”.The results demonstrate that the cuttings bed in annuli is in a state of dynamic equilibrium, but the overall trend and the distribution pattern are obvious. First, larger diameters and longer drilling tools could lead to a higher risk of the stuck pipe. Second, we find that it is not the case that the higher flow rate is always better for hole cleaning, so three flow-rate intervals are discussed separately under the given conditions. When the “dangerous flow rate”(35 L/s in Case 4),the higher the flow rate, the better the cleaning effect of cuttings beds. Objective china is among the 30 countries with a high burden of tuberculosis(TB)worldwide,and TB remains a public health *** Prefecture in the southern xinjiang autonomous region is considered as one of the highest TB burden regions in ***,molecular epidemiological studies of Kashgar are *** A population-based retrospective study was conducted using whole-genome sequencing(WGS)to determine the characteristics of drug resistance and the transmission *** A total of 1,668 isolates collected in 2020 were classified into lineages 2(46.0%),3(27.5%),and 4(26.5%).The drug resistance rates revealed by WGS showed that the top three drugs in terms of the resistance rate were isoniazid(7.4%,124/1,668),streptomycin(6.0%,100/1,668),and rifampicin(3.3%,55/1,668).The rate of rifampicin resistance was 1.8%(23/1,290)in the new cases and 9.4%(32/340)in the previously treated *** resistance mutations were detected more frequently in lineage 2 strains than in lineage 3 or 4 strains,respectively:18.6%vs.8.7 or 9%,P<*** estimated proportion of recent transmissions was 25.9%(432/1,668).Multivariate logistic analyses indicated that sex,age,occupation,lineage,and drug resistance were the risk factors for recent *** the low rate of drug resistance,drug-resistant strains had a higher risk of recent transmission than the susceptible strains(adjusted odds ratio,1.414;95%CI,1.023–1.954;P=0.036).Among all patients with drug-resistant tuberculosis(DR-TB),78.4%(171/218)were attributed to the transmission of DR-TB *** Our results suggest that drug-resistant strains are more transmissible than susceptible strains and that transmission is the major driving force of the current DR-TB epidemic in Kashgar.

BACKGROUND Acute decompensation(AD)of cirrhosis is associated with high short-term mortality,mainly due to the development of acute-on-chronic liver failure(ACLF).Thus,there is a need for biomarkers for early and accurate identification of AD patients with high risk of development of ACLF and *** triggering receptor expressed on myeloid cells-1(sTREM-1)is released from activated innate immune cells and correlated with various inflammatory *** To explore the prognostic value of sTREM-1 in patients with AD of *** A multicenter prospective cohort of 442 patients with cirrhosis hospitalized for AD was divided into a study cohort(n=309)and validation cohort(n=133).Demographic and clinical data were collected,and serum sTREM-1 was measured at *** enrolled patients were followed-up for at least 1 *** In patients with AD and cirrhosis,serum sTREM-1 was an independent prognosis predictor for 1-year survival and correlated with liver,coagulation,cerebral and kidney failure.A new prognostic model of AD(P-AD)incorporating sTREM-1,blood urea nitrogen(BUN),total bilirubin(TBil),international normalized ratio(INR)and hepatic encephalopathy grades was established and performed better than the model for end-stage liver disease(MELD),MELD-sodium(MELD-Na),chronic liver failure-consortium(CLIF-C)ACLF and CLIF-C AD ***,sTREM-1 was increased in ACLF and predicted the development of ACLF during first 28-d *** ACLF risk score incorporating serum sTREM-1,BUN,INR,TBil and aspartate aminotransferase levels was established and significantly superior to MELD,MELD-Na,CLIF-C ACLF,CLIF-C AD and P-AD in predicting risk of ACLF *** Serum sTREM-1 is a promising prognostic biomarker for ACLF development and mortality in patients with AD of cirrhosis.

目的构建脓毒症集束化治疗管理与实践方案并探讨其临床应用效果。方法①构建脓毒症集束化治疗管理与实践方案:成立课题小组,负责文献回顾、专家遴选、编制与收发问卷,对专家意见进行整理、统计与分析,并对研究全程质量控制等。于2022年10月至11月开展专家函询,问卷均通过现场填写与微信形式发放和收回。采用李克特量表5级评分法(Likert 5)对各条目评分。②方案临床应用:回顾性选择2022年1月至7月收治于新疆医科大学第一附属医院重症监护病房(ICU)的90例脓毒症患者作为对照组,采用常规的脓毒症集束化治疗与护理策略。前瞻性选择2023年1月至7月收治的90例脓毒症患者作为干预组,在对照组治疗与护理策略的基础上,采用德尔菲函询法构建的脓毒症集束化治疗管理与实践方案。比较两组患者1 h、3 h、6 h集束化治疗(bundle)完成率,治疗1、3、7 d炎症指标变化水平,以及预后指标的差异。结果①构建脓毒症集束化治疗管理与实践方案:最终形成的方案包含4项一级指标、15项二级指标和34项三级指标。共进行两轮专家函询,两轮函询问卷回收率均为100%;专家权威值系数分别为0.948、0.940;各项目的变异系数分别为0~0.287、0~0.187;肯德尔和谐系数(Kendall's W)分别为0.242、0.249,差异均有统计学意义(均P0.05)。结论构建的脓毒症集束化治疗管理与实践方案能够提高集束化治疗完成率,缩短脓毒症患者ICU住院时间,减少ICU住院费用,并有降低28 d病死率的趋势。

BACKGROUND Acute-on-chronic liver disease(AoCLD)accounts for the majority of patients hospitalized in the Department of Hepatology or Infectious *** To explore the characterization of AoCLD to provide theoretical guidance for the accurate diagnosis and prognosis of *** Patients with AoCLD from the Chinese Acute-on-Chronic Liver Failure(ACLF)study cohort were included in this *** clinical characteristics and outcomes,and the 90-d survival rate associated with each clinical type of AoCLD were analyzed,using the Kaplan-Meier method and the log-rank *** A total of 3375 patients with AoCLD were enrolled,including 1679(49.7%)patients with liver cirrhosis acute decompensation(LC-AD),850(25.2%)patients with ACLF,577(17.1%)patients with chronic hepatitis acute exacer-bation(CHAE),and 269(8.0%)patients with liver cirrhosis active phase(LC-A).The most common cause of chronic liver disease(CLD)was HBV infection(71.4%).The most common precipitants of AoCLD was bacterial infection(22.8%).The 90-d mortality rates of each clinical subtype of AoCLD were 43.4%(232/535)for type-C ACLF,36.0%(36/100)for type-B ACLF,27.0%(58/215)for type-A ACLF,9.0%(151/1679)for LC-AD,3.0%(8/269)for LC-A,and 1.2%(7/577)for *** HBV infection is the main cause of CLD,and bacterial infection is the main precipitant of *** most common clinical type of AoCLD is *** diagnosis and timely intervention are needed to reduce the mortality of patients with LC-AD or ACLF.

基于2000-2020年新疆农作物产量、化肥投入、稻田播种面积等统计数据,根据IPCC排放因子法,核算新疆农业源非CO_(2)温室气体(GHG)排放总量,通过对数平均迪氏指数(LMDI)模型对农业源非CO_(2)GHG排放驱动因素进行分解,使用蒙特卡洛模拟结合情景分析法预测其排放趋势。结果表明,研究期内新疆农业源非CO_(2)GHG排放呈现波动上升趋势,增幅达到34.43%,畜牧养殖是主要排放源。驱动因素研究结果表明,2000-2020年,农业经济发展水平和城镇化水平的提高促进了农业源非CO_(2)GHG的排放,贡献排放量分别达4211.74×10^(4)tCO_(2)eq和1016.08×10^(4)tCO_(2)eq。农业非CO_(2)GHG排放强度、乡村人口以及农业结构的降低抑制了农业源非CO_(2) GHG的排放,2000-2020年减排量分别为4163.36×10^(4)tCO_(2)eq、224.84×10^(4)tCO_(2)eq和130.64×10^(4)tCO_(2)eq。农业源非CO_(2)GHG排放在基准和规划情景下均呈上升趋势,但规划情景下的增速快于基准情景,在低碳情景下通过提高农业非CO_(2)GHG排放强度、改善生产结构有效减缓增速,可能在2035年实现负增长。实现农业源非CO_(2)GHG排放达峰需要强化对减排政策的落实,强制约束农业源非CO_(2)GHG排放;降低农业非CO_(2)GHG排放强度,控制因农业生产技术落后所导致的农业源非CO_(2)GHG排放;通过完善减排惩戒激励机制,鼓励技术突破引导新疆农业的低碳转型。

本文分析塔额盆地草原蝗虫的生态位,旨在为当地的蝗灾监控、治理及生态平衡保护提供理论依据。本研究对塔额盆地冲积平原、山前洪积倾斜平原、山地3种地形的草原蝗虫的空间生态位进行分析,统计得出了蝗虫优势度、群落组成差异、Levins生态位宽度和Shannon生态位宽度、Pianka生态位重叠等相关参数。结果表明:3种地形共采集蝗虫6科19种1023头,不同地形优势种、常见种及稀有种均存在差异。山地与山前洪积倾斜平原、冲积平原的蝗虫群落组成差异较大。黑条小车蝗Oedaleus decorus decorus、蓝胫戟纹蝗Dociostaurus tartarus、意大利蝗Calliptamus italicus、红胫戟纹蝗Dociostaurus kraussi kraussi、伪星翅蝗Calliptamus coelesyriensis的生态位较宽且与其他蝗虫存在生态位重叠。塔额盆地草原蝗虫发生种类较多,绝大部分优势种蝗虫对空间资源的利用能力较强、竞争较激烈,对优势危害种加大监测力度,而针对山前洪积倾斜平原中的稀有种类,应加以保护以确保生物多样性。

Psoriasis is a chronic,recurrent,inflammatory systemic disease that is not only characterized by skin manifestations but may also be accompanied by various comorbidities,imposing a heavy burden on patients’physical and mental health and affecting their quality of *** the continuous approval of various biological agents for clinical use has provided more effective treatment options to patients with psoriasis,no consensus on the evaluation of comprehensive treatment goals has yet been *** treat-to-target(T2T)strategy requires consideration of multiple dimensions of treatment outcomes,the development of long-term management goals,and regular assessments of treatment conditions,which are often used in the management of chronic ***,based on the latest consensuses and guidelines,research data,and clinical experience as well as the combination of survey results and expert group discussions,the present consensus focuses on 4 dimensions of short-and long-term integrated management goals for biological agents:alleviating skin lesions,improving quality of life,screening and managing psoriasis comorbidities,and ensuring drug *** implementation methods,evaluation time,treatment monitoring,and program adjustments are also herein described to achieve comprehensive management of psoriasis to the maximum *** consensus provides a reference for clinical practice.

MicroRNA regulatory pattern in spinal cord ischemia-reperfusion injury

After spinal cord injury, dysregulated miRNAs appear and can participate in inflammatory responses, as well as the inhibition of apoptosis and axon regeneration through multiple pathways. However, the functions of miRNAs in spinal cord ischemia-reperfusion injury progression remain unclear. miRCURY LNATM Arrays were used to analyze miRNA expression profiles of rats after 90 minutes of ischemia followed by reperfusion for 24 and 48 hours. Furthermore, subsequent construction of aberrantly expressed miRNA regulatory patterns involved cell survival, proliferation, and apoptosis. Remarkably, the mitogen-activated protein kinase(MAPK) signaling pathway was the most significantly enriched pathway among 24-and 48-hour groups. Bioinformatics analysis and quantitative reverse transcription polymerase chain reaction confirmed the persistent overexpression of miR-22-3 p in both groups. These results suggest that the aberrant miRNA regulatory network is possibly regulated MAPK signaling and continuously affects the physiological and biochemical status of cells, thus participating in the regulation of spinal cord ischemia-reperfusion injury. As such, miR-22-3 p may play sustained regulatory roles in spinal cord ischemia-reperfusion injury.
