Management of hepatocellular carcinoma patients with portal vein tumor thrombosis:A narrative review

Background: Hepatocellular carcinoma(HCC) is one of the main reasons for malignancy-related death. Portal vein tumor thrombosis(PVTT) is the most common form of macrovascular invasion related to HCC occurring in 10%-60% of patients. HCC with PVTT is usually characterized by worsening liver function, vulnerability to blood metastasis, higher incidence of complications associated with portal hypertension, and intolerance to treatment when compared with that without PVTT. If only treated with supportive care, the median survival of HCC with PVTT is about 2.7 months. In the past, sorafenib was the only recommended therapy by guidelines with limited effectiveness. This narrative review aimed to describe the current management options for HCC with PVTT. Data sources: We have reviewed literature from Pub Med on the treatment of HCC with PVTT and compiled evidence-based facts on effective therapies available for different types of PVTT. Results: Sorafenib monotherapy is not much effective, but combining it with other methods can improve survival. Each type of PVTT can beneft from the combination of transarterial chemoembolization and sorafenib than sorafenib monotherapy. The tumor downstaging can be realized possibly after transarterial chemoembolization, but tumor invasion into the main trunk of the portal vein greatly impairs efficacy. Although surgery is a curative approach, it is often not recommended for Vp4 PVTT. Some new methods can broaden the indication, but further explorations are needed. Radiotherapy can decrease the possibility of Vp3 progression to Vp4, but building a forecast model of best radiation dose and response is necessary. Systemic chemotherapy, hepatic arterial infusion chemotherapy, radiofrequency ablation, portal stenting, and traditional Chinese medicine are also benefcial in Vp3-4 PVTT. To investigate the thermal and mechanical behavior of casting wheel,a two-dimensional thermoelastic-plastic finite element model was used to predict the temperature,stress and distortion distribution of the casting wheel during the wheel and belt continuous casting *** effects of grinding thickness and casting speed on the thermal and mechanical behaviors of the center of the hot face of the casting wheel were discussed in *** each rotation,the casting wheel passes through four different spray *** results show that the temperature distribution of the casting wheel in different spray zones is similar,the temperature of the hot face is the highest and the temperature reaches the peak in the spray zoneⅢ.The stress and distortion depend on the temperature distribution,and the maximum stress and distortion of the hot face are 358.2 MPa and 1.82 mm,*** temperature at the center of the hot face decreases with increasing grinding thickness and increases with increasing casting speed.

Daya Bay, a semi-enclosed bay in the South China Sea, is well known for its aquaculture, agriculture, and tourism. In recent years, many environmental problems have emerged, such as the frequent(almost yearly) occurrence of harmful algal blooms and red tides. Therefore,investigations of submarine groundwater discharge(SGD) and associated nutrient inputs to this bay have important theoretical and practical significance to the protection of the ecological system. Such a study was conducted using short-lived radium isotopes ^(223)Ra and ^(224)Ra. The estimated SGD fluxes were 2.89 × 10~7 m^3/d and 3.05 × 10~7 m^3/d based on ^(223)Ra and ^(224)Ra, respectively. The average SGD flux was about 35 times greater than that of all the local rivers. The SGD-associated dissolved inorganic nitrogen(DIN) and dissolved inorganic phosphorus(DIP)fluxes ranged from 1.95 × 10~6 to 2.06 × 10~6 mol/d and from 5.72 × 10~4 to 6.04 × 10~4 mol/d, respectively. The average ratio of DIN to DIP fluxes in SGD was 34, much higher than that in local rivers(about 6.46), and about twice as large as the Redfield ratio(16). Our results indicate that SGD is a significant source of nutrients to the bay and may cause frequent occurrence of harmful algal blooms. This study provides baseline data for evaluating potential environmental effects due to urbanization and economic growth in this region.

To aim at the multimode character of the data from the airplane detecting system, the paper combines Dempster- Shafer evidence theory and subjective Bayesian algorithm and makes to propose a mixed structure multimode data fusion algorithm. The algorithm adopts a prorated algorithm relate to the incertitude evaluation to convert the probability evaluation into the precognition probability in an identity frame, and ensures the adaptability of different data from different source to the mixed system. To guarantee real time fusion, a combination of time domain fusion and space domain fusion is established, this not only assure the fusion of data chain in different time of the same sensor, but also the data fusion from different sensors distributed in different platforms and the data fusion among different modes. The feasibility and practicability are approved through computer simulation.

Effect of TiO_(2) addition on microstructures and properties of Mgo-Cao refractory aggregates

MgO-CaO(40 wt.%CaO)refractory aggregates were prepared using the calcined dolomite and light-burned magnesia fine powder as raw materials and TiO_(2) as *** effect of TiO_(2) on their phase composition,microstructures and properties was investigated by X-ray diffraction and scanning electron *** properties such as bulk density,apparent porosity,relative aggregate tube strength and hydration resistance were also *** results showed that the CaTiO_(3) generated by the reaction between CaO and TiO_(2) was distributed around the CaO grain boundaries and intermittently distributed with MgO,which formed an isolation layer around CaO and greatly improved the hydration ***,the introduction of TiO_(2) promoted sintering and increased the grain size,further improving the strengths and hydration resistance of the *** addition,the most significant enhancement in the hydration resistance and strengths of the samples was achieved when 1.0-2.0 wt.%TiO_(2) was *** this case,the relative strength of aggregate increased from 33.3% to 37.3%-43.1%,and the mass gain after the hydration test decreased from 3.13% to 1.26%-1.45%.
