AIM: To clarify the significance of cyclooxygenase-2 (COX-2) expression in human primary hepatocellular carcinoma(HCC) and adjacent nontumorous ***: The COX-2 protein and mRNA were investigatedin 27 HCC tissues with adjacent nontumorous tissues, and 5histologically normal liver tissues, using immunohistochemistryand in situ ***: The well-differentiated HCC expressed COX-2protein (5.68±1.19) more strongly than moderated HCC(3.43± 1.98) and poor differentiated HCC (3.33± 1.50)(P＜0.05 respectively), adjacent nontumorous tissues (4.93±1.05) and normal liver tissues (3.20±1.g2) (P＜0.01respectively); More intensive staining of COX-2 in adjacentnontumorous tissues was observed than that in normal livertissues (P＜0.05). There was no significant difference amongadjacent nontumorous tissues, moderately differentiated HCCand poorly differentiated HCC (P＞0.05). The expression ofCOX-2 mRNA was observed in the cytoplasm of the cells ofHCC and of the hepatocytes in adjacent nontumorous tissuesin which COX-2 protein was ***: The overexpression of COX-2 in well-differentiated HCC suggests that COX-2 may play a role inthe early stages of hepatocarcinogensis.

目的:探讨免疫抑制剂环孢素(CsA)及他克莫司(FK506)对肝癌细胞增殖的影响及可能机制。方法:采用噻唑蓝(MTT)比色法观察CsA及FK506对QGY鄄7701、QGY鄄7703、SMMC鄄7721和BEL鄄7402等4株肝癌细胞增殖的影响。还应用Hoechst33258荧光染料涂片,在荧光显微镜下观察用药组细胞的形态学改变。结果:随着作用时间延长,10μM的CsA对4株肝癌细胞均呈现明显的生长抑制效应(P0.05)。荧光显微镜下CsA用药组肝癌细胞呈现凋亡改变;而FK506用药组与对照组无显著差异。结论:免疫抑制剂CsA非但未促进体外肝癌细胞生长,相反呈明显的抑制效应。FK506尽管未呈现对肝癌细胞增殖的抑制效应,也未显示有生长促进作用。CsA对肝癌细胞的增殖抑制作用与诱导癌细胞发生凋亡有关。

目的:总结我院肝移植术后7例腹内高压病人的临床相关资料,探讨术后腹内高压发生的原因以及其对多器官功能的损害。方法:采用膀胱内压检测法间接测量腹内压(膀胱内压>20mmHg时定为腹内高压)。术后第1天开始,连续3d记录病人的腹内压、血肌酐(Cr)、尿素氮(BNU)水平以及心率、平均动脉压、中心静脉压、肺动脉压,动脉血氧分压/吸入氧浓度比,同时记录病人的机械通气时间。结果:7例腹内高压病人中,5例出现急性肾功能损害,血Cr、BNU水平明显升高,其中3例出现少尿或无尿,1例需作持续性肾替代治疗;5例出现低氧血症,动脉血氧分压/吸入氧浓度比下降,机械通气时间延长;上述病人中1例术后第3天死于多器官功能不全综合征。部分病人心率、中心静脉压、肺动脉压升高。结论:肝移植术后并发的腹内高压对器官功能的损害是多部位的,肾功能与呼吸功能的受损最为常见。加强对腹内高压的重视、术后早期腹内压的严密监测具有较重要的临床意义。

目的:总结我院2002年7月19日施行的辅助性原位尸体肝部分移植治疗Wilson病(肝豆状核变性)病例,探讨该移植技术的可行性和初步临床疗效。方法:通过双灌注快速取肝法获取血型为A型的供肝;在整型台上用CUSA沿肝脏镰状韧带的左侧劈离肝脏,取第Ⅱ、Ⅲ段为重230g的部分移植供肝,仔细结扎断面的管道系统,历时185min。受体女性,22岁,血型A型,因锥体外系症状逐渐加重而具肝移植指征,切除受体病肝左外侧叶(Ⅱ、Ⅲ段),将供肝(Ⅱ、Ⅲ段)原位植入。用5鄄0Prolene缝线分别对供体鄄受体的肝左静脉和门静脉左支行端端连续吻合,均预留“增宽因子”,于受体胃十二指肠动脉水平与供肝的左肝动脉以8鄄0Prolene缝线间断吻合。胆道对合取左肝管鄄空肠Roux鄄en鄄Y式吻合。术中予甲基泼尼松龙静脉注射1000mg,术后采用激素、FK506和骁悉联合免疫抑制治疗。结果:热缺血时间为6min,冷缺血时间15.5h,手术时间6.5h,术中输血1400ml。术后第3天开始进食和离床活动,至今病人已生存16个月,完全恢复正常生活,血清铜和铜蓝蛋白水平恢复正常,移植肝脏体积无明显萎缩,手足震颤明显减轻。结论:辅助性原位尸体部分肝移植是治疗Wilson病的良好方法,同时可避免活体部分肝移植给供体带来的风险。

基于肿瘤抗原的免疫治疗的应用受到验证免疫原性肽稀缺性的阻碍。本研究旨在研究环状RNA(circRNA)在肝胆肿瘤类器官中作为肿瘤抗原肽新来源的潜力。使用RNA测序(RNA-seq)和基于算法的评分工具,预测3950个翻译的肿瘤特异性环状RNA在27个类器官中产生18971个抗原肽。从抗原格局来看,11个氨基酸长度(mer)肽和人白细胞抗原(HLA)-A结合肽具有最高的免疫原性相关评分。在分析的3/5类器官中,有13个预测抗原肽通过质谱(MS)免疫肽组学被直接确认为HLA-A、HLA-B和HLA-C(HLA-ABC)结合肽。在流式细胞术和酶联免疫吸附试验(ELISA)中,由HLA-ABC分子呈递的circRNA衍生的肿瘤特异性肽刺激CD8(CD8)T细胞,显示CD107a干扰素γ(IFNγ)共表达和IFNγ分泌增加。免疫原性环状RNA衍生肽诱导的靶向类器官的细胞毒性T细胞活性在杀伤实验中得到验证。值得注意的是,来自circTBC1D15的抗原肽YGFNEILKK不仅被认为是类器官的HLA-ABC呈递肽,而且还显著降低了肿瘤类器官的存活率。本研究的发现强调了产生肿瘤抗原的一个关键亚群,这对靶向肿瘤特异性circRNA具有重要意义。
