吲哚青绿辅助下玻璃体切除术治疗特发性黄斑裂孔后视网膜神经纤维层的分析研究

Purpose: t. analyze ret.nal nerve fiber layer (RNFL) t.ickness in eyes wit. or wit.out.visual field (VF) defect. aft.r indocyanine green-assist.d vit.ect.my for idiopat.ic macular holes using opt.cal coherence t.mography (OCt. and t. invest.gat. t.e relat.onship bet.een post.perat.ve VF defect. and RNFL damage. Design: Ret.ospect.ve int.rvent.onal case series. Part.cipant.: t.irt.-four eyes of 32 pat.ent. wit. idiopat.ic macular holes t.at.underwent.vit.ect.my bet.een January 2001 and March 2003 were included in t.is st.dy. Eyes were divided int. 3 groups according t. t.e occurrence of post.perat.ve VF defect. and t.e use of indocyanine green for int.rnal limit.ng membrane peeling during surgery: 11 eyes wit. VF defect. aft.r indocyanine green-assist.d vit.ect.my (group 1),9 eyes wit.out.VF defect. despit. t.e use of indocyanine green (group 2),and 14 eyes wit.out.VF defect. t.at.underwent.vit.ect.my wit.out.indocyanine green (group 3). Met.ods: Ret.nal nerve fiber layer t.ickness in each of 4 quadrant. (superior,inferior,nasal,t.mporal)was measured wit. OCt. Main Out.omeMeasure: Ret.nal nerve fiber layer t.ickness around t.e opt.c disc. Result.: t.e mean RNFL t.ickness in 3 of 4 quadrant. (superior,nasal,inferior) in group 1 was significant.y less t.an t.at.in t.e corresponding quadrant.in groups 2 and 3 (P t.e t.mporal quadrant.t.ere was a significant.difference bet.een groups 1 and 3 (P=0.02),but.not.bet.een groups 1 and 2. t.e RNFL t.ickness in group 1 was significant.y less in 3 quadrant. in operat.d eyes t.an in fellow eyes (Pt.e RNFL t.ickness was reduced in eyes wit. VF defect. aft.r indocyanine green-assist.d vit.ect.my for macular holes,suggest.ng t.at.t.e post.perat.veVF defect.may have been caused by RNFL damage relat.ng t. t.e use of indocyanine green.

Met.ods: t.e aut.ors analyzed eight.FAP At.R Y114C pat.ent.. Six pat.ent. showed CNS sympt.ms associat.d wit. lept.meningeal amyloidosis. t. examine t.e funct.on of t.e blood-CSF barrier and blood-brain barrier (BBB), t.e aut.ors performed CSFand MRI st.dies. t.e aut.ors also performed a hist.pat.ologic st.dy of aut.psy specimens t. examine t.e dist.ibut.on of amyloid deposit.on in t.e CNS. Result.: CSF st.dy showed high t.t.l prot.in concent.at.ons and increased albumin CSF/serum concent.at.on quot.ent. (Qalb; an indicat.on of blood-CSF barrier funct.on). MRI wit. gadolinium (Gd) revealed enhancement.from brainst.m t. spinal ord. Serial brain MRI st.dies wit. FLAIR images aft.r Gd administ.at.on showed Gd leakage int. t.e subarachnoid space (t.o pat.ent.). t.ese findings suggest.d t.e blood-CSF barrier and BBB dysfunct.ons. Hist.pat.ologic st.dy revealed int.nse amyloid de posit.on in lept.meninges, vessel walls, and parenchyma in spinal cord and t.e b rain. t.ese dist.ibut.ons of amyloid deposit.on are unique compared t. ot.er t.R relat.d lept.meningeal amyloidosis. Conclusions: Pat.ent. wit. familial amyloid ot.c polyneuropat.y amyloidogenic t.anst.yret.n Y114C had CNS disorders relat.d t. amyloid deposit.on in lept.meninges, vessel walls, and parenchyma in spinal c ord and t.e brain. t.e pat.ogenesis of CNS disorders may reflect.disrupt.on of t.he blood-CSF barrier and blood-brain barrier by amyloid deposit.on.

Met.ods: A 60-year-old Japanese man de veloped endopht.almit.s aft.r t.-assist.d PPV for diabet.c macular edema. Preop erat.ve visual acuit. was 20/200. Four days aft.r surgery, endopht.almit.s assoc iat.d wit. ant.rior chamber hypopyon was not.ced; t.e pat.ent.s vision had det. riorat.d t. hand mot.on. In spit. of severe cell infilt.at.on, t.e ciliary injec t.on and ocular pain were not.significant. Result.: t.e addit.onal PPV wit. irri gat.on of cefazolin (40 μug/ml) and gent.micin (8 μug/ml) was performed. Endop ht.almit.s resolved soon aft.r t.is t.eat.ent. St.phylococcus epidermidis was de t.ct.d in t.e int.avit.eous samples. t.e pat.ent.s visual acuit. improved t. 20 /100. Conclusion: Endopht.almit.s may be a complicat.on of t.-assist.d PPV wit. unique signs and sympt.ms.

Divalent.europium（Ⅱ）-doped mesoporous silicon oxy-nit.ide mat.rials, as a novel t.pe of nanoscopic phot.luminescent.mat.rial, are st.died in which t.e opt.cally act.ve luminescent.cent.rs are isolat.d wit.in t.e pores. Colorless and t.ansparent.blue emit.ing luminous Eu2＋-doped mesoporous silicon oxy-nit.ide mat.rials were prepared from mesoporous silica via impregnat.on, nanocast.ng and nit.ogen-assist.d carbot.ermal reduct.on and nit.idat.on met.od. t.e morphology and porosit. of t.e silica remain unchanged during t.e synt.esis process as revealed by SEM images and by nit.ogen sorpt.on st.dies. Phot.luminescence st.dies reveal a st.ong blue emission band for Eu2＋ cent.red at.425 nm wit. a quant.m efficiency of 47%. t.e luminescent.int.nsit. is great.y influenced by t.e amount.of co-dopant. AI3＋ ions.
