Clinical outcomes of endoscopic resection of superficial nonampullary duodenal epithelial tumors: A 10-year retrospective, single-center study

BACKGROUND Although premalignant duodenal lesions such as adenomas are uncommon,the incidences of these lesions have increased in recent times,and thus,the demand for minimally invasive treatments such as endoscopic resection(ER)has also ***,ER in the duodenum is more challenging than ER in other locations of the gastrointestinal *** To evaluate the safety and efficacy of ER for superficial nonampullary duodenal epithelial tumors(SNADETs)METHODS We performed a retrospective observational study on 56 consecutive patients(58 lesions)diagnosed with SNADETs that underwent ER from January 2011 to December 2020 at Yeungnam University *** demographics,lesion characteristics,and procedural and technical data were collected,and clinical outcomes,including procedure-related complications,completeness of resection,and recurrence were *** Median patient age was 57 years[range,26–77,30(53.6%)men].Endoscopic mucosal resection(EMR)was performed on 57 lesions(98.3%)and snare polypectomy on one(1.7%).Lesions consisted of 52 adenomas with low-grade dysplasia(89.7%),3 adenomas with high-grade dysplasia(5.2%),and 3 intramucosal adenocarcinomas(5.2%).There were 16 cases of intraprocedural bleeding(27.6%)and 1 case of delayed bleeding(1.7%),and all these 17 cases were successfully managed *** perforation or procedure-related death *** lesion size was associated with an increased risk of EMR-related bleeding(P=0.033).During a median follow-up period of 23 mo(range 6–100 mo),no local recurrence occurred,despite the fact one-third of the patients(19 lesions,32.8%)underwent piecemeal resection and 3 patients(3 lesions,5.2%)that underwent en bloc resection had a pathologically determined positive lateral *** patient died from a primary duodenal *** The majority of SNADETs can be safely and curatively resected by EMR,and thus,based on consideration of the high incidence of fatal complications attribut

BACKGROUND The high prevalence of human papillomavirus(HPV)infection in oropharyngeal squamous cell carcinoma(SCC)is well established,and p16 expression is a strong ***-related tumors exhibit unique mechanisms that target p16 and p53 ***,research on HPV prevalence and the combined predictive value of p16 and p53 expression in head and neck cutaneous SCC(HNCSCC),particularly in Asian populations,remains *** retrospective study surveyed 62 patients with HNSCC(2011-2020),excluding those with facial warts or other skin *** To explore the prevalence of HPV and the predictive value of p16 and p53 expression in HNCSCC in Asian *** All patients underwent wide excision and *** staining for HPV,p16,and p53 yielded positive and negative *** relevance of each marker was investigated by categorizing the tumor locations into high-risk and middle-risk zones based on recurrence *** Of the 62 patients,20(32.26%)were male,with an average age of 82.27 years(range 26-103 years).High-risk included 19 cases(30.65%),with the eyelid and lip being the most common sites(five cases,8.06%).Middle-risk included 43 cases(69.35%),with the cheek being the most common(29 cases,46.77%).The p16 expression was detected in 24 patients(38.71%),p53 expression in 42 patients(72.58%),and HPV in five patients(8.06%).No significant association was found between p16 expression and the presence of HPV(P>0.99),with a positive predictive value of 8.33%.CONCLUSION This study revealed that p16,a surrogate HPV marker in oropharyngeal SCC,is not reliable in HNCSCC,providing valuable insights for further research in Asian populations.

The vagus nerve(VN)plays an important role in regulating physiological conditions in the gastrointestinal(GI)tract by communicating via the parasympathetic pathway to the enteric nervous system(ENS).However,the lack of knowledge in the neurophysiology of the VN and GI tract limits the development of advanced treatments for autonomic dysfunctions related to the *** better understand the complicated underlying mechanisms of the VN-GI tract neurophysiology,it is necessary to use an advanced device enabled by microfabrication *** several candidates including intraneural probe array and extraneural cuff electrodes,microchannel electrode array devices can be used to interface with smaller numbers of nerve fibers by securing them in the separate channel *** microchannel electrode array devices to interface teased nerve structures are relatively bulky with thickness around 200µ*** thick design can potentially harm the delicate tissue structures,including the nerve *** this paper,we present a flexible thin film based microchannel electrode array device(thickness:11.5µm)that can interface with one of the subdiaphragmatic nerve branches of the VN in a *** demonstrated recording evoked compound action potentials(ECAP)from a transected nerve ending that has multiple nerve ***,our analysis confirmed that the signals are from C-fibers that are critical in regulating autonomic neurophysiology in the GI tract.

Mononuclear cells from the cord blood and granulocytecolony stimulating factor-mobilized peripheral blood:is there a potential for treatment of cerebral palsy?

To investigate a possible therapeutic mechanism of cell therapy in the field of cerebral palsy using granulocyte-colony stimulating factor（G-CSF）-mobilized peripheral blood mononuclear cells（m PBMCs）,we compared the expression of inflammatory cytokines and neurotrophic factors in PBMCs and m PBMCs from children with cerebral palsy to those from healthy adult donors and to cord blood mononuclear cells donated from healthy *** significant differences in expression of neurotrophic factors were found between PBMCs and m ***,in cerebral palsy children,the expression of interleukin-6 was significantly increased in m PBMCs as compared to PBMCs,and the expression of interleukin-3 was significantly decreased in m PBMCs as compared to *** healthy adults,the expression levels of both interleukin-1βand interleukin-6 were significantly increased in m PBMCs as compared to *** expression of brain-derived neurotrophic factors in m PBMC from cerebral palsy children was significantly higher than that in the cord blood or m PBMCs from healthy *** expression of G-CSF in m PBMCs from cerebral palsy children was comparable to that in the cord blood but significantly higher than that in m PBMCs from healthy *** expression of pro-inflammatory cytokines（interleukin-1β,interleukin-3,and-6）and higher expression of anti-inflammatory cytokines（interleukin-8 and interleukin-9）were observed from the cord blood and m PBMCs from cerebral palsy children rather than from healthy *** findings indicate that m PBMCs from cerebral palsy and cord blood mononuclear cells from healthy newborns have the potential to become seed cells for treatment of cerebral palsy.

Phase Formation and Magnetic Properties of Nanocomposite Nd-Fe-B Adjusted by Small Amount of Dy and Co

In order to improve and stabilize the magnetic properties of nanocomposite Nd2Fe14B/α-Fe magnetic alloys by a compositional adjustment, small amount of Dy and/or Co was added to Nd9Fe84B7 alloys. DTA analysis on the amorphous of the alloys took place as the soft magnetic phases were crystallized, and then the hard magnetic Nd2Fe14B was precipitated from them. While α-Fe and a metastable 1:7 (TbCu7-type) phase were formed simultaneously in Dy and Co-free alloys, they were crystallized separately at different temperatures after Dy or Co was added. This phase separation occurred more clearly in the Dy-treated alloys and the other soft magnetic phase Fe3B was also stabilized by Dy and/or Co. The 1: 7 phase that was stabilized by Dy and/or Co was not eliminated at 700 ℃, decreasing magnetic properties of the alloys. It was eventually disappeared above 725 ℃, but Fe3B was not eliminated even at 750 ℃ when Dy was added more than 0.5 at% or Co was added more than 2.0 at%. Amount of Nd2Fe14B in the alloys tended to increase as Dy addition increased,whereas Co addition did not lead to any appreciable change in the ratio of α-Fe and Nd2Fe14B. Moreover, Dy addition apparently increased coercivity of an alloy while Co addition had a beneficial effect on remanence. The grains in the Dytreated alloys were usually finer than those in the Co-treated alloys. The grain size of both α-Fe and Nd2Fe14B in the alloys exhibiting mr ≥ 0.72 was in the range of 20 ～ 40 nm or even larger 50 nm, which is larger than the theoretical optimum size ( ～ 10 nm). Typical magnetic properties obtained from a Nd7.5Dy1.5Fe82.5Co1.5B7 alloy annealed for 12 min at 725 ℃were iHc=4.85 kOe, Br= 11.32 kG, (BH)max = 15.73 MGOe, and mr=0.73.

Scalp Acupuncture for Parkinson's Disease:A Systematic Review of Randomized Controlled Trials

Objective： To evaluate the effectiveness of scalp acupuncture （SA）, a modern acupuncture technique specialized to neurological disorders, in managing motor function and symptoms for Parkinson＇s disease （PD） patients. Methods： Two independent reviewers extracted data from all of the randomized clinical trials （RCTs） that assessed the efficacy of SA for PD compared with conventional therapies （CTs）. Sixteen electronic databases were searched. The risk of bias was appraised with the Cochrane Collaboration tool, and the reporting of the included studies was evaluated by the Consolidated Standards of Reporting Trials （CONSORT） 2010 checklist and the revised Standards for Reporting Interventions in Clinical Trials of Acupuncture （STRICTA） guidelines. Results： In total, 4 RCTs met the inclusion criteria. As assessed by the Unified PD Rating Scale （UPDRS）, 2 RCTs showed that SA combined with CTs proved superior to CTs alone [60 cases; weighted mean difference, -3.94; 95% confidence interval （CI）, -6.05 to -1.64, P=0.01; 12=0%]. Based on the Webster scale, however, 3 RCTs showed no superior effect of SA when combined with CTs with high heterogeneity （154 cases; risk ratio, 1.29; 95% CI, 0.79 to 2.12, P=0.30; 12=84%）. The Cochrane risk of bias, adherence to the CONSORT and the STRICTA checklist showed that the quality of all the included RCTs was generally low. Conclusions： The result of our systematic review and meta-analysis suggested that the effectiveness of SA for PD is promising, however, the evidence is not convincing. A sham-controlled RCT design that adheres to the CONSORT and STRICTA guidelines to overcome methodological weakness and that includes a large sample size is strongly recommended to confirm the precise effect of SA on PD.

Predictors of poor outcomes in patients with wild mushroom-induced acute liver injury

To identify early predictive markers of poor outcomes in patients with acute liver injury from wild mushroom *** observational, retrospective record review involved adults aged ≥ 18 years admitted to emergency department with mushroom intoxication from January 2005 to December 2015. The diagnosis of mushroom intoxication was based on the following: (1) a positive history of recent wild mushroom intake (either raw or cooked); (2) the onset of gastrointestinal symptoms, such as watery diarrhea, vomiting, and/or abdominal pain, after ingestion; and (3) the exclusion of other possible causes of acute liver injury. Acute liver injury was defined by a > 5-fold elevation of liver enzymes or moderate coagulopathy [international normalized ratio (INR) > 2.0]. Clinical and laboratory findings were compared in survivors and *** 93 patients with mushroom intoxication, 23, 11 men (47.8%) and 12 women (52.2%), of median age 61 years, developed acute liver injury. The overall in-hospital mortality rate was 43.5% (10/23).
